[Chloroquine and pyrimethamine/sulfadoxine resistant malaria tropica in a child with diabetes mellitus].

A 16-year old girl with insulin-dependent diabetes mellitus (8 years' duration) developed tropic malaria 7 weeks after her return from Kenya despite a longtime prophylaxis using pyrimethamine and sulfadoxine (Fansidar). The disease was detected during an episode of ketoacidosis which proved exceptionally difficult to manage. Adequate chloroquine therapy resulted in temporary recovery. A recurrence of malaria four weeks later was successfully treated with quinine and doxycycline. Intraleucocytary parasites were found during both these episodes. Already prior to antimalarial drug therapy the girls' preexisting retinopathy was found to have deteriorated.

[Immunologic findings in homosexual males with generalized lymphadenopathy. Prodromal state of acquired immunodeficiency syndrome?].

Peripheral blood of 6 homosexual men with lymphadenopathy associated with fever, weight loss and night sweat, 13 homosexuals with lymphadenopathy without further clinical symptoms, 13 healthy homosexuals and 20 healthy hetero-sexual blood donors was evaluated for different lymphocyte subpopulation, mitogen induced T-cell proliferation and Gamma-interferon production. In the six patients with lymphadenopathy lymphocyte subpopulations were also analysed in lymph node-biopsies. delayed type hypersensitivity was established by skin test's using different recall antigens. Sera of all homosexual men were screened for immunoglobulin levels, complement activity, presence of immunocomplexes and autoantibodies. Most of the homosexuals with lymphadenopathy but without clinical symptoms and all patients with lymphadenopathy associated with clinical symptoms had a significantly decreased helper-(OKT 4) suppressor- (OKT 8) cell ratio. In contrast, the OKT 4/OKT 8 ratio was found to be normal in lymphocyte suspension from lymph node-biopsy from the 6 patients with lymphadenopathy and clinical symptoms. Within the group of healthy homosexuals this ratio exhibited a rather large variation, in three of the tested persons the ratio was below 1. No functional abnormalities of T-cell immunity were observed in the homosexual men as compared to the control group, with the exception of a significantly increased LAG- and PWM-induced lymphocyte proliferation in the group of healthy homosexual men. Likewise no significant differences were obtained with regard to the further analysed immunological parameters. It is concluded that the obtained immunological results do not provide the possibility to define persons at immediate risk for the development of AIDS.

The distribution and interaction of haemoglobin variants and the beta thalassaemia gene in Liberia.

In a population survey in Liberia, West Africa, 12 major tribes were examined for the prevalence of Hb S, Hb C, and the beta thalassaemia (beta Thal) gene. Hb C is rare; Hb S and beta Thal occur in polymorphic frequencies. The distribution of both genes shows an inverse correlation. The beta Thal trait was diagnosed by quantitation of Hb A2 on DE 52-microchromatography. This method proved to be reliable and useful for mass screening.

Sickle cell-beta +-thalassaemia: a haematological and clinical study in Liberia.

Clinical and haematological features of 20 patients of several Liberian ethnic groups with sickle cell-beta +-thalassaemia are reported. Haemoglobin analysis showed increased Hb A2 values, high Hb A levels (median 25%), variable amounts of Hb F and a slight imbalance of non alpha/alpha globin chain synthesis ratios. The clinical and other haematological findings varied but the disease seems to run a relatively mild course in the majority of the patients.

Flubendazole versus mebendazole in intestinal helminthic infections.

In a double-blind study the vermicidal effect of flubendazole, a new benzimidazole derivative, was evaluated and compared to mebendazole. Both drugs were administered in a single dose of 600 mg. While in the treatment of Ascaris and hookworm infestations flubendazole and mebendazole showed a similar efficacy, mebendazole seemed to be slightly superior in the treatment of trichuriasis. Both drugs were well tolerated and no side effects were observed even in patients with a heavy worm load.

Malaria-induced thrombocytopenia.

Platelet counts were investigated in 26 patients with P. falciparum malaria and 39 patients with P. vivax malaria before and after treatment. Before schizontocidal treatment 22 of 26 (85%) patients with P. falciparum malaria and 30 of 39 (72%) patients with P. vivax malaria had depressed platelet counts below 150,000/microliters blood. There was a correlation between low platelet counts and high counts of malarial plasmodia (parasitized red blood cells) in P. falciparum and P. vivax infections (p less than 0.001). Platelet survival, studied by malonaldehyde formation in three patients during the period of decreasing parasitaemia, revealed a shortened life span to 2--3 days in comparison to 7--10 days in normal controls. In all patients platelet counts rose to threefold the initial values within 5 days after clearance of parasites. The results demonstrate that, first, thrombocytopenia is a common feature in human malaria, second, thrombocytopenia induced by malaria is due to shortened life span in the peripheral blood and, third, some interaction is present between platelets and malaria plasmodia or parasitized red cells.

Combination of techniques for concentration and identification of microfilariae from peripheral blood.

Human blood containing a known number of Dirofilaria immitis or Brugia malayi microfilariae was incubated with Giemsa-stain or methylene blue solution and then subjected to density gradient centrifugation in Percoll-solution at a density of 1.090 g/ml. This resulted in complete separation of microfilariae from cellular components. Subsequently the microfilaria-containing layer was altered through a polycarbonate membrane. The stained parasites are then easily recognized and identified on the completely transparent membrane. The average recovery was 80%. This technique proved useful as a preparatory method and as a diagnostic tool for the detection of microfilariae in patients with low microfilaraemia.

Plasmodium falciparum malaria and human red cells. II. Red cell genetic traits and resistance against malaria.

In 702 Nigerian children under 6 years of age the incidence and the severity of malarial infection was studied with respect to haemoglobin types and red cell glucose-6-phosphate dehydrogenase variants. The results suggest that Hb AS as well as the female genotype GdA-/GdB offer selective advantage against the disease. Parasite densities in carriers of these genotypes were significantly lower than in other subjects. Whereas protection by Hb AS was found mainly in children between 2 and 4 years of age, the advantage afforded by GdA-/GdB was similar in all age groups. Possible mechanisms are discussed.

Plasmodium falciparum malaria and human red cells. I. A genetic and clinical study in children.

In a holoendemic malaria region of Nigeria children of both sexes aged from 9 months to 6 years with an acute febrile illness were examined for malaria. In 461 children with malaria, predominantly P. falciparum, and in 241 children without malaria, haemoglobin levels, haemoglobin types, red cell glucose-6-phosphate dehydrogenase variants and parasite densities were recorded. The G6PD status was determined by a combination of spectrophotometric enzyme quantitation, electrophoresis and the cytochemical methaemoglobin elution technique. Malaria morbidity and parasitaemia decreased with increasing age. Frequencies of haemoglobin types and G6PD variants were not significantly different in the malaria and the non-malaria series. Haemoglobin values were significantly lower in children with malaria to about the same extent in HbAA and HbAS subjects, but no close correlation existed between haemoglobin level and parasite density. Details of the G6PD classification and the effect of malaria on enzyme activity are discussed.

[Psychic development of children under conditions of longterm isolation (author's transl)].

Three children with severe combined immunodeficiency have been isolated for 19-22 months. Two of the children have been placed into the isolation system directly after delivery; on child entered the isolation system at the age of 5 months. The influence of this situation on the psychic development of the children was studied by the means of direct observation as well as by psychometric test methods. An additional child which has been isolated between the age of 6 and 32 months was retested at the age of 4 and 6 years. The reported observation which include those of the parents and the team allow the conclusion, that a normal development is possible under these extreme conditions, when an optimal clinical and psychological support is maintained.

Rhesus incompatibility and aplastic anemia as the consequence of split chimerism after bone-marrow transplantation for severe combined immunodeficiency.

A patient with severe combined immunodeficiency received three transplants of bone marrow from the HLA-B- and -D-identical mother. The first transplantation led to a severe graft-versus-host reaction followed by immunological reconstitution. A split chimerism was found with engraftment only of the maternal lymphocytes. Five months after the transplantation an autoimmune hemolytic anemia was observed which was due to rhesus incompatibility as well as polyspecific antibodies. At the same time agranulocytosis developed and 9 mth after the first transplantation the child suffered from aplastic anemia. Two further attempts failed to engraft the maternal hematopoiesis. The child died during the treatment with cyclophosphamide as conditioning for a third transplantation.
