[Forceps deliveries in epidural or general anesthesia (author's transl)].

It was the aim the present study to evaluate maternal morbidity and fetal outcome of 832 pelvic outlet forceps deliveries during the years 1974 through 1977 with special reference to the method of anesthesia (Epidural anesthesia (EA) with Bupivacain 0,5% or general anesthesia (GA) with Methohexital-Na and Succinylcholin). Maternal vaginal and/or cervical injuries were seen significantly (p less than 0,001) more often in the forceps group than in spontaneous, vacuum, or breech deliveries. Vaginal injuries occurred less often (p less than 0,01) in the EA- than in the GA-group. Forceps deliveries were not associated with increased maternal morbidity in fetuses weighing 3750gr or more. The cardiotocography scores (Hammacher et al. 1974) of the 30 minutes preceeding delivery (final scores) were better with EA than with GA (p less than 0,01). Arterial cord pH and 1 minute Apgar values were similar in both groups. The same was true for fetuses with a birthweight of 3750gr or more. The time for cervical dilatation from 7 to 10 cm and duration of the second stage of labor did not influence maternal morbidity or fetal outcome, regardless of the method of anesthesia.

[Interrater reliability of amp symptoms (author's transl)].

Two psychiatrists examined 48 patients (25 depressed and 23 schizophrenic). Each documented the symptoms on AMP sheets 3 (psychopathologic symptoms) and 4 (somatic signs, first column only). The study deals with 139 AMP symptoms. Seventy could be judged concerning symptom exists/does not exist. Of these 70 symptoms, 45 showed a good or moderate interrater reliability. Specific symptoms had a better reliability than nonspecific. Symptoms described by the patients had a better reliability than those judged by the doctor alone. The results indicate that expanded use of the AMP system in its present form is problematic. Work on a new version of the AMP system has already begun.

[Parturition under peridural anesthesia (author's transl)].

Basing on a study of 205 "matched pairs", it is shown to what extent peridural anesthesia (PA) influences the course of parturition, the type of termination of birth, and the intrapartal and post-partal condition of the child. The method of PA application is accurately described in the section "Material and Methods". The first stage of labor, or period of dilatation (PD) is not in any way effected by PA. However, PA will almost double the expulsion period (EP). Hence, elective termination of birth via technically simple, complication-free outlet forceps is usually propagated and employed significantly more often. Emergency sections or delivery by forceps due to mandatory indication, are not more frequent under PA than otherwise. The intrapartal condition of the child, measured with the cardiotocogram employing in the score according to Hammacher et al. (6) before and after application of PA, as well as during the last 30 minutes of birth (final score), is influenced only irrelevantly. The Apgar scores one minute and five minutes post partum, as well as the arterial umbilical cord pH values, do not differ significantly between the peridural and the control groups. The authors agree with the widespread opinion that, at the present moment, PA is the best obstetric analgesic method with simultaneous maximum possible safety for the fetus.

[Value of the abdominal electrocardiogram for cardiotocography (author's transl)].

The value of abdominal Electrocardiogram (abd. ECG) for cardiotocography has been studied prospectively in 183 ante- and 93 subpartum patients. The indications for using abd. ECG have been worked out and the influence of various factors on the quality of registration were tested. It was found that neither location of the placenta nor obesity nor the state of the membranes influenced the quality of abd. ECG-records. Furthermore abd. ECG was used successfully in cases with advanced labor.

[AMP system: longitudinal studies of psychopathologic and somatic symptoms. An empiric investigation of the problem of side effects (author's transl)].

The psychopathologic and somatic state of 681 psychiatric patients were investigated in longitudinal studies within the scope of different psychopharmacologic trials. Most psychologic symptoms occur less frequently, whereas somatic symptoms are found to increase, which suggests side effects from psychotropic drugs. The analyses show that the AMP-system can be reduced by 49 (possibly 45) symptoms, as initial findings and developments show that these symptoms are rare, and do not allow of differentiation between diagnostic groups.

[Taxonomy and assessment of psychological investigation methods in psychotropic drug trials (author's transl)].

Psychological tests are bound with specific goals. Four goal aspects are differentiated: status vs. processdiagnostics, normoriented vs. criterionoriented diagnostic, testing vs. inventarization, measurement of true scores vs. decision oriented diagnostic. Every diagnostic procedure is characterized by a specific personality theory and theory of measurement (classical vs. probabilistic test model). The diagnostic procedures traditionally used for evaluating drugs prefer status diagnostic, normoriented diagnostic, testing and measurement of true scores. Similar one-sidedness in personality theory and theory of measurement restrict validity and usefulness of psychological tests. In clinical practice we find a theoretically and empirically unjustified restriction in the selection of measurement devices on ratings and questionnaires. If we suppose multidimensionality of drug induced changes, we must apply a multimethod approach in outcome studies and use beside ratings and questionnaires behavior observations, objective tests, psychophysiological and neurophysiological measures. We propose a descriptive taxonomy of methods for planning multimethod outcome and process studies. From this taxonomy the methods of measurement for effects and side effects of drugs may be derived. The necessity of the multimethod approach is confirmed by empirical research. With this concept, the following neglected research questions become more obvious: concordance and discordance, synchrony and desynchrony of methods of measurement. We conclude with recommendations for clinical practice and research of outcome and process effects in drug therapy.

[Methodologic studies of the Hamilton rating scale for depression (author's transl)].

Within the scope of psychopharmacologic therapies, the Hamilton Rating Scale for Depression was applied 3 times on each of 197 psychiatric patients. An extended version (24 symptoms) of the Hamilton Scale was employed, which included the original symptoms. Various formal analyses (frequency of symptoms, influence of sex, item analyses, estimation of reliability, factor analysis) allow the following conclusions: With the original version, the single factor solution is the most consistent; several factors are not acceptable. With the extended version (used in the U.S.A.) the single factor solution can be equally recommended, but the two factor solution could also be taken into consideration, whereby a distinction is made between retarded depression and somatic depression aspects. The total score of the Hamilton Scale is satisfactorily consistent and hardly dependent on sex.

[Development of drug, alcohol and tobacco consumption. Comparison between 2 studies from 1971 and 1974 in the canton Zurich].

Consumption of drugs, alcohol and tobacco has been compared in two populations of 19-year-old men, of 1971 (2785) and 1974 (1617), drawn from three different geographical regions of the Canton of Zurich. The hypothesis that the percentage of drug users has risen over the last three years was not confirmed: as previously, 25% have had experience of drugs. As expected, however, there was more regular and heavy drug users in 1974 than in 1971. Also, there is a more marked trend towards extension of drug consumption to rural areas than in 1971. The percentage of cigarette smokers has remained stable, but per capita cigarette consumption has increased. Contrary to expectation, alcohol consumption showed the sharpest rise in all three regions. The study suggests that drug consumption is not to be considered in isolation but in conjunction with the use of alcohol and tobacco.

[Studies with the AMP system: comparison of symptoms from Berlin and Zürich].

Symptoms of 1830 psychiatric patients from Berlin and of 783 patients from Zurich were assessed using the AMP-system. The symptoms were analysed according to their frequency and their potency to differentiate diagnostic groups. The following 4 categories of symptoms proved to be useful: frequently occurring and specific or unspecific, rarely occurring and specific or unspecific. Factor analyses of the frequently occurring unspecific symptoms yielded three general dimensions, i.e., "depression", "disturbance of memory and thinking" and "hostility".
