[Foreign bodies in the upper airways of children - problems relating to diagnosis and treatment].

The authors share their experience and systematic observations with regard to the correlation between "chronic pneumonia" and foreign bodies in the respiratory tract, based on a survey of 480 bronchoscopic examinations for a period of 10 years. It was established that misdiagnosed foreign bodies in the lower respiratory tract account for 12.5% of all these cases. The percentage of undiagnosed bilateral foreign bodies is unusually high, as well as that of the left-sided foreign bodies, although to a smaller degree. The study also shows that very often in pediatric practice foreign bodies are wrongly diagnosed as bronchopneumonia and bronchial asthma.

[The conservative surgical treatment of kidney tumors].

Seven cases are reported of patients with renal tumors in whom organ-preserving operations were performed. Two patients had only one kidney, the contralateral had been removed in the past. They had only partial resection of one third of the kidney performed. In five patients, in whom parenchymal renal tumor was accidentally detected, with normal contralateral kidney, enucleation of the tumor was performed. On histologic examination one had hypernephroma, three--adenoma with atypism, one--leiomyoangiolipoma. The size of all enucleated tumors varied from 25 to 55 mm with distinct fibrous pseudocapsule, without proliferation in it and without vascular invasion of the neoplastic process. It is assumed that patients with tumor of the only kidney when possible should be subjected to organ-preserving operation. In the event of a small encapsulated tumor and normal contralateral kidney, organ-preserving operation may be performed when there is low-grade malignancy.

[Endorectal ultrasonic study of the prostate].

Endorectal ultrasound examination, which has been developed mainly by Japanese and Danish scientists is being applied at I. Clinical Hospital "Dr. Kiril Khavezov" in Sofia since 1984. In this report the method is briefly described and results presented of 91 examinations performed in 76 patients with miscellaneous prostate diseases: 55 patients with adenoma, 17 with cancer, 3 with prostatitis and 1 with abscess. Characteristic of adenoma were the changes in the size and form of the gland; two types of changes in the form are described. Various structural changes, associated with asymmetry and impaired integrity of the prostate capsule were seen in cancer. In cases of prostatitis the gland was enlarged without structural changes. Only the the abscess was visualized as a liquid zone with irregular shape. The value of this method is that it furnishes information on the morphologic pattern of the whole gland, as well as on its correlations with the adjacent structures. Endorectal ultrasound examination of the prostate can not be a substitute for the cytologic and histologic methods when a neoplastic process is suspected.

[Peroral use of a lidocaine preparation].

The authors studied the therapeutic effect of lidocainee peroral forms in case of rhythm disorders, accompanying with various cardiac diseases. The results obtained revealed that in 76 per cent of the cases with ventricular extrasystoles, the treatment result was very good, with complete disappearing of extrasystoles. Good effect was found in 18 per cent, with the recording of some rare, single extrasystoles towards the end of the treatment, and the treatment had no effect on 6 per cent of the patients. The drug proved to be effective in the cases with supraventricular extrasystoles. Lasting result was attained in 89.36 per cent of the patients treated. Due to the favourable therapeutic effect and good tolerance the authors recommended lidocain peroral form for wide application in the treatment of rhythm disorders.

[Clinical dynamic ECG observations of the physical load tolerance of myocardial infarct patients].

Dynamic ECG observations are reported after a full day interval motor programme, applied with four motor regimes on patients with myocardial infarction under clinical conditions. The motor programme was used for the treatment of 241 patients, distributed into 2 groups. Observation on the effect of peak physical loading at the end of each motor regime was also reported. The authors concluded that the tolerance to physical loading was adequate, being confirmed by the absence of significant ECG dynamics even with the peak loadings, performed at the end of all motor regimes.

[Pathomorphological studies of the testes in various forms of hypogonadism in young men].

The authors studied 32 biopsies of testes of patients, aged 18 to 26 with various forms of male hypogonadism: Klinefelter's syndrome--16, primary hypotrophy pogonadism with other known and unknown etiology--6, cryptorchism--1, secondary hypogonadism--4, later puberty--3. The materials were studied by the standard histological techniques. The authors report about definite structural changes with the various forms of hypogonadism, manifested in the elements of seminal tubules, Sertoli's cells, basal membrane of the ducts and Leydig's cells, situated in the interstitial spaces.

[Clinical trial of the Bulgarian digitalis preparation, Digilanide C used intravenously].

The Bulgarian digitalis preparation Digilanid C for venous application has a satisc factory effect on the majority of the patients (67 per cent) with light and moderate chronic cardia insufficiency and espically in case of tachycardia in auricular fibrillation. The comparison of the results between the group of 46 patients, treated with Digilanid C and the control group of 20 patients treated with Isolanid (Richter) do not reveal essential differences in the major part of the results obtained.
