[Indications for and the postoperative results of mitral valve prosthesis].

The influence of 20 hemodynamic and clinical indices on the postoperative results was studied in 528 patients with mitral valve prosthetics. One year after the operation 78.7% of the patients are with improved functional capacity, 18.8% are without change and 2.5% are in worse condition. The lack of improvement correlates moderately with the postoperative complications (r = 0.4701) and to a larger degree with the number of patients in the II functional class before the operations (r = 0.7342). The deterioration of the functional capacity is in relation to the complications by operations on the tricuspid valve (r = 0.8773), but there is also considerable correlation with the low ejection fraction (r = 0.6425), previous commisurotomy (r = 0.5871) and uncorrected tricuspid valvular disease (r = 0.6314). The operative lethality rate is 6.25%.

[Analysis of the autopsy data on 48 cases of infectious endocarditis in patients over 14].

Forty eight cases with infections endocarditis (IE) are analyzed--27 males and 21 females (75% at the age of 31-60). Only in 12.5% IE has developed on intact valvular apparatus. In 83.4% the preceding valvular pathology is rheumatic heart defect (RHD), and congenital cardiac malformations and valvular dysplasia--8.3% each. The mitral-aortic valvular defects predominate in RHD, followed by aortic, tricuspid and mitral ones. In 83.3% X IE is with valvular localization, in 4.2%--parietal and in 12.5%--on valvular prostheses. Vegetations are highly specific for valvular IE (100%) as well as ulcerations (90%) and perforations (45%). Ruptures of extra-valvular structures are established in 8.3%. The inflammatory alterations in myocardium are in 62.5%. The pericardial changes are found in 14.6%, mainly in the cases with valvular prosthesis. Septic changes are established in all cases in the spleen and regeneration of bone marrow, and vascular changes--in 85.4%. The cases with embolism and infarctions predominate in the kidneys (79.1%), followed by the cases with Löhlein's focal nephritis (18.6%). The inflammatory changes in liver are rare--6.9%. In 37.5% changes in brain were established, embolism according to incidence being first, followed by cerebral hemorrhages, cerebral abscesses and purulent leptomeningitis Staphylococcus aureus proved to be most frequent causative infectious agent established et necropsy.

[Case of infectious endocarditis in hypertrophic cardiomyopathy].

A patient with hypertrophic cardiomyopathy and infectious endocarditis has been described, confirmed clinically and bacteriologically. L-forms of alpha-streptococcus were isolated from the hemocultures. Aortic valve was affected in the course of the septic process. That morbid state was very favourable influenced by the treatment with high dose penicillin, combined with streptomycin. The control hemocultures 40 days after the discontinuation of the treatment proved to be negative.

[Pregnancy, labor and puerperal problems of women with acquired heart defects].

A total of 592 women in child-birth, at an average age between 15 and 42, mean 26.8 +/- 8.4 years, have been studied. The women with mitral valve defects predominate--83.9% (mitral stenosis--39.8%, mitral insufficiency--18.5% and combined mitral stenosis and insufficiency--41.7%), followed by the women with combined) aorto-mitral defects--9.8% and those with isolated aortic defects--6.3% (out of them with aortic stenosis 35.1%), aortic insufficiency--43.2% and combined aortic defect 21.7%). Pulmonary hypertension proved to have 83 women (14%), all with mitral and mitral-aortic defects. Cardiac insufficiency was established in 130 patients (22%-53.8%, stage I, 38.5%, stage II A and 7.7%--stage II B). Cardiac complications during pregnancy were observed in 29.6% of the patients, during delivery--in 5.4 and in puerperium--in 7.4%. Three women have died--one during delivery and two--during the early puerperal period. The percentage of the women that have given birth by via naturalis is 81.6% and the women that needed assistance--18.4%. A total of 587 viable children have been born (7 twins), 9--non-viable and 3--stillborn. A proper evaluation, in advance, of the cardiac affection and adequate behaviour during pregnancy and labour are necessary for the successful outcome of the pregnancy and delivery in females with valvular heart defects.

[Roentgeno-clinical comparisons in atrial septal defects in adults].

A total of 117 adult patients with atrioseptal defect were studied by routine X-ray method and cardiac catheterization. The X-ray indices of pulmonary blood circulation and heart were juxtaposed that reflect the hypervolemia and hypertension with oxyhemometric volumetric percentage of the shunt, the sytolic pressure in pulmonary artery and the extent of the defect established at operation. A correlation was established between the X-ray determined stage of the pulmonary changes (from the stage of isolated hypervolemia to anatomic-fixed hypertension) and the pressure. A proportionalily was established in the degree of the shunt with the dimension of the heart with X-ray big hearts. With the small hearts, the shunt is unpredictable. There was a considerable difference depending on he size of right auricle, on the extent of the defect and the shunt mainly in the big auricles. With small auricles, the defect and shunt could be of different extent. The risk not to perform catheterization because of a small shunt in auricles with a length of 55 mm is acceptable. At the same time a hemodynamically insignificant shunt could be expected with right pulmonary artery to 17 mm. The right ventricular chanjes could be due mainly to volumetric loading.

[Longitudinal 3-year follow-up of the status of patients with chronic non-coronarogenic myocardial disease among the adult population of the Razgrad District].

Via a comprehensive study of the patient groups from the hospital, polyclinic and follow-up care, within the frames of Razgrad district, 252 cases with positive data for chronic noncoronary myocardial diseases were accurately specified. The basic task that was set, was to follow up the dynamics in the progress of the myocardial disease. One hundred and eight patients (54,76 +/- 6,15%) manifested no change in their state in the course of 3 years, 25,40 +/- 5,37% had and improvement in their state as a result from the therapy carried out. In 17,46 +/- 68% the patients had a deterioration in their state and 2,38 +/- 0,96%--died. The analysis of the data reveal that the quickest and most unfavourable evolution had the patients with myocardial pathology.

[Model of formation of chronic volume loading in the right heart].

The authors describe an experimental model for examining the changes occuring in the haemodynamics of the small circuit as a result of created artificial defect of the interatrial septum. An original operative technique is used, by which the complete clamping of the right lung artery and vera is avoided and lung complications are prevented. The performed control haemodynamic studies up to one vear after the operation support the efficiency of the proposed model.

[Terminal bundle-branch block (TBB) in acute and chronic overloading of the right heart].

The authors performed a model loading of the right heart ventricle in 40 dogs in 17 dogs--acute tension loading by creating a partial ligature (banding) of the pulmonary artery; the dogs were examined in respect to the changes hemodynamics and ECG for a period of three hours. In the second group of 10 dogs a chronic tension loading of the right heart was created by a partial ligation of the pulmonary artery (banding); the dogs were followed--up in respect to hemodynamic and vectocardiographic changes for a period of one year. In the other group of 10 dogs a chronic loading of the right heart: was formed by operative route and interatrial defect was obtained; the dogs were followed-up in respect to hemodynamics and vectrocardiographic changes for a period of one year. The authors described the changes in the intraventricular conduction of vectro cardiograms, made by the three plane system of Akulincev, and these vectrocardiograms were compared with the type and degree of loading as well as with the morphologic changes occurred in the heart. The authors indicated the nonspecificity of TBB as a sign of volume loading of the right heart ventricle.
