In addition to promoting vital communication among medical professionals, documentation helps fulfill regulatory, legal, and payer reimbursement requirements, thereby ensuring that nursing contributions to patient well-being are recognized and that access to care is preserved.

The revised erythropoietin claims monitoring policy that will govern reimbursement by the Centers for Medicare & Medicaid Services for Epoettin alfa and darbepoetin alfa administered to patients on dialysis takes effect on April 1, 2006. The new policy provides several significant improvements that should enhance clinicians' ability to manage anemia rationally. However, several aspects of the new policy are ambiguous and will require further clarification and ongoing evaluation to determine the effect on practice patterns and clinical outcomes.

The Centers for Medicare and Medicaid Services issued a revised erythropoietin claims monitoring policy that governs reimbursement for Epoetin alfa and darbepoetin alfa in patients on dialysis effective October 1, 2006. The new policy reinforces the upper Hb target level of 12.0 g/dL. However, the new provisions also allow increased latitude for making timely dose adjustments in response to the variability in Hb levels that occurs due to changes in patient status.
