[IL-1beta and PGE2 production in whole blood and gingival fluid in women with periodontitis and preterm low birth weight].

DESIGN: The aim of the investigation was to evaluate of IL-1beta and PGE2 concentrations in gingival fluid, whole blood as well as IL-1beta, PGE2 production after Escherichia coli lipopolysaccharide stimulation in whole blood in women with preterm low birth weight (PLBW), as compared to the control group. MATERIAL AND METHODS: A case-control study of 88 postpartum women aged 17 to 39 was performed. The case group consisted of 52 women with PLBW and the control group consisted of 36 women giving birth in time. Concentration of inflammatory mediators in gingival fluid, blood serum and IL-1beta, PGE2 production in whole blood after bacterial lipopolysaccharide stimulation were determined by means of immunoenzymatic method. RESULTS AND CONCLUSIONS: The levels of IL-1beta and PGE2 in gingival fluid were significantly higher in all PLBW mothers (also PLBW primiparous) than in the control group. In addition in the primiparous with PLBW group significantly higher PGE2 concentration in blood serum was found compared to the primiparous controls. There were no significant differences between women with PLBW and the controls together with a significantly higher production of IL-1beta and PGE2 in whole blood after LPS stimulation in women with periodontitis and gingivitis compared to subjects with healthy periodontium. Such findings suggest that inflammatory mediator synthesis is mainly result of specific cells exposition to bacterial products. Therefore it seems that more frequent occurrence of the phenotype of hyperactive cells that synthesise these mediators is not responsible for PLBW.

[Periodontitis and preterm low birth weight].

During the past decade an issue of influence of periodontitis on developing of systemic social diseases with polyfactorial etiology (atherosclerosis, myocardial infarction and stroke, diabetes, lung diseases) was taken up. Several researches have also proved that periodontitis could be an independent risk factor for preterm low birth weight (PLBW). The nature of this connection is not clear yet. The aim of this article is to show the current evidence from experimental studies in animals and human case-control studies on the relationship between these pathologies. Potential etiopathogenic mechanisms for periodontitis and preterm low-birth weight mothers have also been demonstrated. It's possible that treatment of periodontitis may reduce the risk of preterm birth.

[Amphetamine and derivatives related death in the aspect of forensic toxicology].

Amphetamine and derivatives are the crucial theme in the problems of forensic toxicology. The paper is based on the study of 20 cases of deaths in which amphetamine and/or derivatives were involved as the cause of death. Toxicological examinations of the autopsy specimens were performed with the use of HPLC/MS method in chemical positive ionization mode (APCI) after liquid--liquid extraction. The toxicological findings obtained in particular cases indicate that the majority of cases under consideration were complex. Among all 20 cases only two, probably suicidal deaths, were a result of amphetamine abuse. Some of them which were violent deaths (murder, gun-shot, hanging, drowning) of people under the influence of drugs. The majority of cases which were probably fatal accidents caused by the interaction of various mixtures of xenobiotics which included also opiates, cocaine, benzodiazepines besides amphetamine and derivatives. The concentrations of xenobiotics detected in these cases fit a relatively large range. One case is worth mentioning. This is a death of a permanent amphetamine user who survived two weeks with hematoma in brain in which amphetamine was detected postmortem. The report also discuss amphetamine and derivatives metabolic problems and contains useful data for medico-legal purposes.

[Chest stab wound comparison in suicidal and homicidal cases].

In view of the appearance of cases of homicides by stabbing in which the killer claims that the victim committed suicide cases of homicides and suicides of this type have been compared. Contrary to the opinions appearing in the medico-legal issues stating that the commonly described features of the suicidal wounds such as tentative wounds, stabbing in the intercostals space, "removal" of clothes do not appear in some of the suicides, a few of these features do appear in each of the examined cases. Having considered the horizontal wound arrangement in the skin, which is a newly described feature, the differentiation between homicides and suicides are more categorical. In addition to that, the low level of alcohol in the blood is an extremely characteristic feature of the suicide by chest stabbing. On the contrary, typical features of suicidal death don't appear in cases of "aggressive self-destruction", self inflicted fatal wounds inflicted by a person with mental disorders.

In passing of judgement for the National Memory Institute, in case of "Cross's Defenders", the concept "state of temporary life hazard", mentioned in Polish criminal code from 1932 year was discussed.

[Unusual case of gunshot by self-made firearm].

A case of suicide with the use of a primitive self-made firearm, constructed with a steel tube was presented. Victim standing on a stool, with hanging loop around neck, shot himself to mouth with fourteen steel balls. At this moment the weapon tore apart wounding hand of the victim.

[Sudden cardiac death of a young male due to fibromuscular dysplasia of the left coronary artery].

Arteriosclerosis and its complications are the most frequent cause of death due to cardiovascular diseases. Other different, considerably rare arterial changes can lead to a fatal result as well. The authors presented a case of a 31-year-old male. Postmortem examination revealed a narrowed lumen and thrombosis of the left coronary artery due to fibromuscular dysplasia and scarification of the myocardium. Hemodynamic disturbances due to left ventricular failure lead to the death of the young adult.

[Lethal injuries caused by sharp tools].

Two interesting cases of fatal impactment on a sharp object were described. Firstly a fall through a glass door, buttocks first and piercing the iliac artery, and a case of falling onto a sharp handle of a pair of pliers carried in the pocket with the subsequent piercing of the heart.
