[Effect of alfacalcidol on bone and calcium metabolism in elderly women].

Oral administration of active vitamin D3 can reduce the loss of bone mass and the incidence of fractures in patients with osteoporosis in Japan. We conducted a prospective study to confirm the effects of 1 alpha(OH)D3 (Alfacalcidol, Alfarol Chugai Tokyo) on bone and calcium metabolism in elderly women with osteoporosis. Enrolled in the present study were 16 elderly osteoporosis women aged 72.6 +/- 4.5 years to whom 1 microgram of 1 alpha(OH)D3 was administered daily. Fasting blood and urine were obtained at baseline, 1 week, 4 weeks, 12 weeks and 24 weeks after the treatment. Monitored parameters were vitamin D metabolites, intact-PTH, bone alkaline phosphatase (BAP), osteocalcin (OC), deoxypyridinoline (DPD) and pyridiuium crosslinked type I collagen telopeptides (CTx). Serum 1 alpha, 25(OH)2D and PTH levels were significantly increased (p < 0.01) and decreased (p < 0.05), respectively at 1 week after commencing administration. There was a significant decrease of DPD (p < 0.05) at 12 weeks after commencing administration compared to the baseline levels. Serum levels of BAP and OC were found elevated at 1 week, and decreased at 12 weeks. In conclusion, the present study clinically confirmed that 1 alpha(OH)D3 stimulates bone formation in vitro. Long-term administration of 1 alpha(OH)D3 indirectly suppressed bone resorption through the suppression of parathyroid function in the elderly.

Effect of diabetes on uremic dyslipidemia.

Elevated intermediate-density lipoprotein (IDL), a remnant lipoprotein, is an independent risk factor for atherosclerosis in patients with end-stage renal disease (ESRD). Since the presence of diabetes mellitus further increases the risk of cardiovascular mortality in ESRD, we examined the effect of diabetes on IDL among ESRD patients. The subjects were 330 healthy control subjects and 287 patients with end-stage renal disease including 80 patients with type 2 diabetes. As compared with the healthy subjects, the nondiabetic ESRD patients had increased plasma triglyceride and IDL cholesterol. Diabetic patients with ESRD showed a further increase in plasma triglyceride and IDL cholesterol compared with the nondiabetic group. However, the difference in IDL levels between the ESRD groups was no longer significant when subjects were stratified by plasma triglyceride. Plasma triglyceride was correlated with IDL cholesterol. Increased hemoglobin A(1c) was significantly associated with IDL cholesterol in a multiple regression model including age, gender, and the presence of ESRD. Such an association was no longer significant in another model including plasma triglyceride as an additional covariate. Further analysis indicated the positive effects of diabetes and hyperglycemia on plasma triglyceride. These results indicate that increased IDL in ESRD is further deteriorated in the presence of diabetes, and that the adverse effect is accounted for at least partly by hypertriglyceridemia associated with chronic hyperglycemia.

[Evaluation by metabolic markers of bone].

Metabolic markers of bone are useful for the evaluation of future bone mineral loss and risk of fractures. Recent studies suggest the markers are one of indicators of suppression of turnover by anti-resorptive treatment. Although only urinary markers are reimbursed and numbers of measurement are limited by health insurance, Japanese standard values, and minimum significant changes were already published. Effectiveness of the treatment by bisphosphonate and HRT can be evaluated by the measurement of the markers individually. The compliance of the treatment can be improved by the information of the changes of markers by treatment. Utilization of serum markers may change the selection of treatment modalities.

[Bone mineral measurement in evaluation of osteoporosis treatment].

BMD measurement is the most common practical method to evaluate the efficacy of the treatment. As the signal/noise ratio of lumber BMD is better than the other part of BMD measurement, the best site for the evaluation of therapy is lumber spine. If evaluation at lumber spine is difficult, hip BMD, followed by peripheral BMD may be selected. Special care must be taken if peripheral BMD is measured, because of low signal/noise ratio. The interval between BMD measurements is usually 6 months, and it should be longer than this period, if peripheral BMD is utilized.
