[The sensitized rate of preformed antibodies in the candidate of renal transplant recipient].

It is important to examine the presence of preformed antibodies in hemodialysis patients before renal transplantation, because it causes the graft rejection in organ transplantation. In the 239 hemodialysis patients we analysed 3 preformed antibodies divided TW, BW and BC. Sensitized rate (TW+, BW+ and BC+) was 0% at no blood transfusions (BT), 9.3% at 1-5 BT and 28.9% at 6 more BT. The number of blood transfusions increased the sensitized rate (TW). Sensitized rate (BW+, BC+) was 6.2% at no BT, 9.3% at 1-5BT and 12.0% at 6 more BT, while sensitized rate (BC+) was 33.3% at no BT, 26.7% at 1-5BT and 14.5% at 6 more BT. There was no correlation between the number of blood transfusions and the sensitized rate (BW+, BC+). At one year after the first check, 69.2% of TW positive patients was converted to TW negative and 53.8% of BW positive patients was converted to BW negative each. Further studies of the preformed antibodies are required to assess the clinical course of long surviving patients.

Rat lymphoid cell lines with HTLV-I production. III. Transmission of HTLV-I into rats and analysis of cell surface antigens associated with HTLV-I.

Newborn WKA rats given a single intraperitoneal injection of MMC treated TARS-1, a rat T cell line producing HTLV-I, were shown to accumulate high titred antibodies specific for HTLV-I. Adult WKA rats rejected TARS-1 transplant with transient appearance of anti-HTLV-I antibodies. However, rats maintained under daily administration of Bredinin, an immunosuppressive drug after TARS-1 transplant showed continuous production of antibodies specific for HTLV-I by aging. Type-C virus particles similar to HTLV-I were demonstrated by electronmicroscopy in the short-term cultured spleen cells of these rats. The evidence indicates that HTLV-I can be transmitted into newborn and immunosuppressed adult rats and they may provide a suitable animal model of ATL and related conditions in man, especially for elucidating the virus-host interactions involved in the leukemogenesis of HTLV-I. By using monoclonal antibodies, cell surface antigens associated with HTLV-I were also analysed.

Expression of the HT462 antigen on fresh leukemic T cells and on cells of HTLV-I infected lines.

We have examined expression of antigens defined by HT462 monoclonal antibody (mAb), together with other HTLV-I related antigens using phorbol 12-myristate 13-acetate treated leukemic mature T cells. Thirteen patients with adult T-cell leukemia (ATL), 3 patients in remission states of ATL and 5 patients with non-ATL were examined. All ATL cells expressed the HT462 antigen, however cells from patients in remission did not express the HT462 antigen. A low percentage of cells from 2 out of 5 patients with non-ATL mature leukemic T cells expressed the HT462 antigen, although these cells did not express other HTLV-I related antigens. Cells of HTLV-I infected human cell lines expressed the HT462 antigen. Three HTLV-I infected rat cell lines (TARS-1, TART-1, TARL-2) did not express the HT462 antigen, although cells of these lines expressed other HTLV-I related antigens. Characterization of the HT462 antigen by strip radioimmunoassay based on western blotting technique using cell lysates of HUT102 cells revealed two additional bands (p68, p35) together with previously reported proteins (gp52, p42). Only p68 was seen in western blots using cell lysates of the rat cell lines. These findings further suggest that the HT462 antigen is a cellular component induced in virus transformed human cells and not a virus encoded protein.

Spontaneous regression of an intrarenal arteriovenous malformation.

We report a case of spontaneous regression of a right intrarenal arteriovenous malformation 6 months after initial examination without surgical intervention, arterial embolization or radiotherapy. The patient presented with massive hematuria from the right kidney, which disappeared completely after spontaneous regression.

Chondroid syringoma (mixed tumor of the skin). A clinicopathological study of 13 cases.

The clinicopathology of 13 cases of chondroid syringoma were examined. The ages of the patients ranged from 26 to 86 years, with an average of 48 years. There were eight males and five females. Ten tumors out of the thirteen appeared on the face. Only one patient out of ten was suspected of recurrence in follow-up information. Histologically, all tumors consisted of epithelial cells, chondroid or myxoid matrix, and other strumal elements. The tumors were histologically classified into two types; twelve tumors had tubular and cystic lumina lined by two layers of epithelial cells, and only one case had small lumina lined by only a single layer. By an immunohistochemical study with a PAP method, positive stainings of keratin were observed in all cases, and S-100 protein in all but one. Ultrastructurally, the tumor cells showed features of an epithelial cell. Some ultrastructural differences were noted between two types of chondroid syringoma. Type I tumor cells had many tonofilaments in cytoplasm, but cytoplasmic filaments in type II were of the intermediate-type.

[Clinical significance of tumor markers in prostatic carcinoma--comparative study of prostatic acid phosphatase, prostate specific antigen and gamma-seminoprotein].

We measured the prostatic acid phosphatase (PAP), gamma-Seminoprotein (gamma-Sm) and prostate specific antigen (PA) in the serum of 862 patients with various urologic diseases including 89 patients with prostatic cancer. We used a PAP radioimmunoassay kit, gamma-Sm enzyme immunoassay kit, Markit-F-PA enzyme immunoassay kit and PA test Wako enzyme immunoassay kit. Serum PA level in advanced prostatic carcinoma (stage C, D) tended to be higher than that in early stage cancer (stage A, B). The Wako kit gave a higher PA than the Markit-F in each stage. The sensitivity rate of Wako PA test was the highest (81%) of all kits. The specificity rate of PAP was the highest (83%), and the accuracy rate of Markit-F PA was the highest (79%). The positive rate in the combined assay of PAP, gamma-Sm and PA in prostatic cancer was higher than that in the single assay of each tumor marker. We regarded PAP, gamma-Sm and PA as clinically different tumor markers, because their serum level did not correlate definitely. No apparent correlation was found between histopathological grade and the level of each tumor marker. The level of PAP, gamma-Sm and PA in the reactivated patients was significantly higher than that of the well-controlled patients. In the reactivated patients, the positive rate of Markit-F PA was the highest (89%) of all the kits.

[Surgical treatment of "inflammatory" aneurysms of the abdominal aorta].

Two cases of "Inflammatory" aneurysm of the abdominal aorta and a review of this type of lesion were presented. The incidence of inflammatory aneurysm of the abdominal aorta in the literature is 2.5 to 15%, but there were no detail reports concerning with this in Japan. The pathogenesis is not clear, but it is evident both macroscopically and microscopically that the inflammatory aneurysms are different from athelosclerotic ones. They are characterized by perivascular peel of inflammatory fibrous tissue. It is possible that this type of aneurysms are merely a variant of Takayasu's disease. Until recently, the diagnosis of this type of aneurysm has not been made before surgery. The symptom of abdominal pain, weight loss, elevated ESR in a patient with abdominal aortic aneurysm are highly suggestive an inflammatory aneurysm. Characteristics of CT scan lead to more frequent preoperative diagnosis of inflammatory aneurysms of the aorta. It reveals a thickened often calcified aortic wall surrounded by a soft tissue mantle. Dynamic scanning shows an enhancing perianeurysnal mass. Graft replacement in these patients is often difficult and associated with increase in morbidity and mortality. At surgery, no attempt should be made to mobilize adjacent viscela in order to avoid injury. Arterial control should be obtained with as little as possible dissection. Some reports refer to successful steroid therapy resolving the inflammatory process and alleviating symptoms. Further research may resolve the treatment of choice for this type of lesion and optimize the timing of surgery.

Immunopathological studies of an autopsy case with Goodpasture's syndrome and systemic necrotizing angiitis.

A rare autopsy case of Goodpasture's syndrome with systemic necrotizing angiitis is reported. The patient, a 56-year-old male, died of respiratory failure with massive pulmonary hemorrhage and renal failure. The autopsy showed widespread hemorrhage in both lungs, diffuse crescentic glomerulonephritis, and systemic necrotizing angiitis in the small arteries. Immunofluorescence studies demonstrated a linear deposition of IgG along the glomerular basement membrane (GBM) as well as the alveolar basement membrane. A granular deposition of C3 was also found along the GBM. Electron microscopy showed that the GBM was irregularly thickened and wrinkled, but electron-dense deposits were indistinct. Anti-GBM antibody activity was detected in the patient's serum and had cross reactivity with normal alveolar basement membrane. The renal eluates contained IgG antibody activity for normal human GBM. These results suggest that glomerulonephritis and pulmonary hemorrhage in the present case were mediated by anti-basement membrane antibodies. We also discussed whether anti-basement membrane antibody is involved in the pathogenesis of systemic necrotizing angiitis.

[A case of ischias as a complication of internal iliac artery embolization].

A case of severe right radicular ischias following embolization is reported. Gelfoam powder and pledgets were used for right internal iliac artery embolization in the case of intractable bladder cancer hemorrhage. It is supposed that previous radiotherapy and fine embolic material may be the contributing factors to this complication.

Genetic differences between two substrains of NZB mice.

We have compared the NZB/B1NJ (/NJ) and NZB/B1Pt (/Pt) mouse substrains with respect to a number of immunological and biochemical polymorphisms. In agreement with earlier findings by other workers, we detected histocompatibility antigen difference(s) between the two substrains. Serological analysis indicated that /NJ and /Pt expressed the same allotypes of H-2, Thy-1 and Lyt-1 antigens, while they differed for the Lyt-2 antigen. Amongst 15 biochemical polymorphisms, which we assayed, /NJ and /Pt differed for four, namely Pep-3, Mup-1, Gpd-1 and Hbb. In addition, the two NZB substrains differed for the Hc marker (haemolytic complement).

TCGF(IL 2)-receptor inducing factor(s). II. Possible role of ATL-derived factor (ADF) on constitutive IL 2 receptor expression of HTLV-I(+) T cell lines.

Interleukin 2 (IL 2) receptor (IL 2-R) is constitutively expressed on T cell lines established from the patients with adult T cell leukemia (ATL), which is a human T cell leukemia lymphoma virus (HTLV-1)(+) T4(+)-leukemia endemic in Japan, the United States, and other countries. Many of these cell lines continuously produce an acidic lymphokine, ATL-derived factor (ADF), which preferentially induces the synthesis and expression of IL 2-R on a sensitive HTLV-1(-) non-T cell line (YT). The induced IL 2-R was characterized by the binding of 125I-IL 2 and flow cytometry by using fluoresceinated anti-human IL 2-R monoclonal antibodies (anti-Tac). Scatchard analysis with 125I-IL 2 showed ADF induced high-affinity receptor sites on YT cells. To test the possibility that ADF produced by HTLV-1(+) T cells is involved in the abnormal expression of IL 2-R, we studied the effect of ADF on an HTLV-1(+) IL 2-dependent T cell line (ED) in which the beta-chain gene of the T cell antigen receptor (T beta) was rearranged. Unlike IL 2-independent HTLV-1(+) cell lines that constitutively expressed Il 2-R, the IL 2-R expression on ED cells declined in the absence of crude IL 2 or recombinant IL 2. When either ADF or recombinant IL 2 was added to the culture of ED cells, there was a dose-dependent enhancement of IL 2-R expression in 24 hr. ADF and IL 2 showed a synergism in the IL 2-R induction, and both factors were needed to induce the maximal receptor expression in these T cells. The lack of IL 2 production by ADF-treated YT, as well as ED cell line suggested IL 2 may not be involved in the IL 2-R induction by ADF. Northern blot hybridization with human IL 2-R cDNA probe showed the increase of IL 2-R mRNA in YT cells after ADF-treatment. ADF also enhanced IL 2-R expression of a rat T cell line transformed by HTLV-1(TARS-1), as demonstrated with anti-rat IL 2 receptor monoclonal antibodies (ART-18). An ADF-like IL 2-R-inducing factor was also detected in the conditioned medium of two HTLV-1(+) rat T cell lines (TARL-2 and TART-1), which constitutively expressed a higher number of Il 2-R than TARS-1 cells.(ABSTRACT TRUNCATED AT 400 WORDS)

Autoimmune disease and malignant lymphoma associated with host-versus-graft disease in mice.

BALB/c mice neonatally injected with (BALB/c X C57BL/6)F1 hybrid spleen cells develop host-versus-graft disease (HVGD) with immunopathological features characteristic of either systemic lupus erythematosus (SLE), or immunoblastic lymphadenopathy (IBL) in man. HVGD mice manifest polyclonal hypergammaglobulinemia with various autoantibodies, generalized lymphadenopathy, hepatosplenomegaly, immune complex glomerulonephritis and evolve in the end to malignant lymphoma. The necessary prerequisite for HVGD induction between donor and host can be summarized as follows: Histoincompatibilities in the H-2 region between donor and host are needed; predominant F1 donor cells needed for HVGD induction are, if not sole, steroid resistant, nylon wool nonadherent and Thy-1 positive T cells; the role of donor T cells is not only to present H-2 complex to the host, but also to interact with and proliferate in the host; strain differences are found for the susceptibility of HVGD induction in the host. It has been found that HVGD evolves to a malignant lymphoma of host T cell origin. The HVGD mouse model may, therefore, contribute to the understanding of the cell to cell interactions at work in the pathogenesis of IBL, as well as SLE, in man.

[A case of spontaneous rupture of renal cell carcinoma].

A case of spontaneous rupture of renal cell carcinoma is reported. A 53-year old man was admitted with the chief complaint of sudden gross hematuria and right flank pain on December 28, 1979. On the following day, the clinical impression was right ruptured kidney, and therefore right nephrectomy was done. Pathological diagnosis was renal cell carcinoma. He received the post-operative irradiation of a total of 5,000 rads. He was seen five years later, at which time there was no evidence of local recurrence or distant metastasis of cancer. Thirty three cases of spontaneous rupture of renal cell carcinoma were collected from Japanese and English literature. Most common chief complaint is abdominal or flank pain. Excretory urography, ultrasonography, CT scan and angiography are useful, but it is difficult to diagnose preoperatively when the tumor is small. Therefore, it is important to suspect occult cancer when a reasonable cause of rupture is undetermined. In these indeterminate cases primary nephrectomy should be considered strongly.

Rat lymphoid cell lines with human T cell leukemia virus production. I. Biological and serological characterization.

Cocultivation of spleen cells, lymph node cells, and thymocytes of female Wistar-King-Aptekman rats with short-term cultured male adult T cell leukemia (ATL) cells in the presence of 5-bromo-2'-deoxyuridine (BrdUrd) resulted in the establishment of rat lymphoid cell lines, TARS-1, TARL-2, and TART-1. Cytogenetic analysis of the three cell lines showed a female rat karyotype with 42 chromosomes. The surface phenotypes of TARS-1 and TART-1 were those of rat T cells. TARL-2 was only positive for rat Ia and leukocyte common antigens. The cell lines continuously produced a type C retrovirus, human T cell leukemia virus (HTLV) and expressed ATL-associated antigens. TARS-1 and TART-1, but not TARL-2 were transplantable into newborn syngeneic rats and nude mice. These results strongly indicate that HTLV not only immortalizes, but also transforms rat T cells in vitro. Adult rats immunized with either TARS-1 or TARL-2 produced antibodies specific for HTLV. The biochemical analysis of the antigens that reacted with rat sera revealed that they are the two HTLV-specific polypeptides, p24 and p28.

Lupus nephritis. Clinicopathology and immunopathology of 80 biopsy cases.

The clinicopathology and immunopathology of 80 cases of lupus nephritis in a total of 1,187 biopsies were presented. Histologically, they consisted of 11 cases of mesangial lupus nephritis (13.8%), 51 cases of diffuse proliferative lupus nephritis (63.7%), and 18 cases of membranous lupus nephritis (22.5%). No cases of focal proliferative type, minimal changes or end-stage kidneys were included. Ten cases were male and they had no qualitative and quantitative differences with female ones in any respects. The microtubular structures were found in glomerular endothelial cells with a 70% frequency and in the interstitial endothelial cells with a 46% frequency. In the latter case, microtubular structures tended to be localized in the perinuclear space as well as in the endoplasmic reticulum. Spherical microparticles were found in 46% of all cases and the incidences were significantly higher in the membranous type. Organized deposits with straight parallel microfilaments were found only in a single case. Electron-dense deposits with fingerprint pattern were not seen. Immunohistochemically, various immunoglobulins, complement components, and fibrinogen were found deposited more frequently in diffuse proliferative lupus nephritis. IgG was the predominant immunoglobulin deposited in capillary loops in membranous types, and IgA in the mesangium in mesangial types. Extraglomerular deposition of immunoglobulins and complements were found in 26.3% of all cases.
