[Late results of operative removal of esophageal cancer with special reference to blunt dissection].

From 1973 to 1985, 185 patients with squamous cell carcinoma of the esophagus were admitted. For such advanced cases as those with penetration into the adventitia, multidisciplinary treatment with bypass surgery, irradiation, hyperthermia and immunochemotherapy was preferably performed. For those with limited invasion not beyond the adventitia, esophagectomy was performed as a first choice. For carcinoma of the lower thoracic and abdominal esophagus, blunt dissection of the thoracic esophagus was undertaken associated with reconstruction of the alimentary tract using a gastric tube via the posterior mediastinum. Of the 185 patients 90 (48.6%) underwent esophagectomy and 77 (41.6%) bypass surgery. Five-year survival rates after operative removal were 69.2% in stage 0, 40.1% in stages I + II, 38.9% in stage III, 4.5% in stage IV and 30.4% overall.

[Combination chemotherapy with cis-diamminedichloroplatinum (CDDP) and other anticancer agents for patients with advanced esophageal cancer].

Combination chemotherapy with cis-diamminedichloroplatinum (CDDP) and other anticancer agents was performed in patients with advanced esophageal cancer. From July 1982 to September 1984, 16 patients were entered into this study and divided into two regimens. In regimen I, 5 patients were treated with a daily dose of 20 mg CDDP and 10 mg Peplomycin for five consecutive days and 10 mg Mitomycin C on the first day. This course was repeated 2 or 3 times every 4 weeks. As for evaluation, 1 PR (20%) and 4 NC (80%) were observed. In regimen II, nine patients were treated with daily doses of 25 mg CDDP and 2 mg Vindesine for 5 consecutive days, a course which was repeated every 4 weeks. As for evaluation, 1 CR (9%), 1 PR (9%), 4 MR (36%), 2NC (18%), and 3 PD (27%) were observed. Major side effects were renal failure, bone marrow suppression, nausea and vomiting, which were mostly transient. However, more severe bone marrow suppression was observed in regimen I composed with regimen II.

[Cerebrospinal fluid transfer of cefotiam (CTM). Its relationship to CT].

Drip intravenous infusion of cefotiam (CTM) was made on patients who underwent cerebrospinal fluid (CSF) drainage and study was made on CSF transfer of CTM and at the same time on the relationship between CSF transfer of iodine contrast medium and CT scan findings. This study was made on 11 cases of cisternal drainage and 8 cases of ventricular drainage. Cisternal drainage cases were all postoperative cases of ruptured cerebral aneurysm. Cases of ventricular drainage included 4 postoperative cases of ruptured cerebral aneurysm, 1 case of CSF rhinorrhea, 2 cases of brain tumor, and 1 case of ventricular hemorrhage. Drip intravenous infusion of 1.0 g of CTM was made in one hour and at given periods thereafter CSF was collected and measured. CTM transferred to the CSF in cistern at a comparatively high concentration (16.3-0.7 microgram/ml). Hardly any transfer of CTM to the CSF in ventricle was seen in one case of cerebral aneurysm, CSF rhinorrhea, and brain tumor, but transfer was observed in one case of cerebral aneurysm, one case of brain tumor, and case of ventricular hemorrhage. Transfer of iodine contrast medium showed the positive correlation to the transfer of CTM. In cases of brain tumor and ventricular hemorrhage with transfer of CTM with ventricular drainage, enhancement effect of the ventricular wall by the contrast medium could be observed by CT scan. From the foregoing, the following results were obtained. There was good transfer of CTM to the CSF in cistern in postoperative cases of ruptured aneurysm. CTM did not transfer to CSF in the normal ventricle.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of polymorphous adenocarcinoma in the right parotid gland].

The nature of adenocarcinomas of parotid gland origin seems to be still unclear because of the rarity of this lesion. A 65-year-old man with a "polymorphous adenocarcinoma" of the parotid gland is presented. This tumor is characterized by a marked diversity in the histologic manifestations, presenting dominantly solid, tubular, pseudoadenoid cystic sarcomatoid patterns. The tumor cells showed distinctive signs of high grade malignancy such as the occurrence of mitotic figures and involvements of lymphatic vessels and/or blood vessels. "Polymorphous adenocarcinoma" is a histologically distinctive neoplasm unlike other adenocarcinomas of parotid gland origin.

A cytochemical study on the salivary gland pleomorphic adenoma (mixed tumor) and the fetal and adult salivary gland.

We studied the pleomorphic adenoma of the salivary gland ultrastructurally and cytochemically [Mg2+-activated adenosine triphosphatase (Mg2+-ATPase)], compared it with normal human fetal and adult salivary glands, and evaluated the histogenesis of this tumor. In the adult salivary gland, reaction products shows Mg2+-ATPase activity were localized in the plasma membranes of myoepithelial cells adjacent to the acinar cells or intercalated duct cells. However, in the salivary gland of the 16-week fetus, they were seen along all adjoining plasma membranes of the cells of terminal buds and duct-like structures. The present case of pleomorphic adenoma comprised two histological components: solid and myxomatous areas. Reaction products were seen along adjoining plasma membranes of both light and dark cells in solid areas.

Enhanced antitumor effects of bleomycin and moderate hyperthermia by additional use of ethanol.

The combined effects of moderate local hyperthermia, bleomycin and ethanol were investigated with Lewis lung carcinoma tumors in female C57BL/6 mice. Different combinations of treatments were performed on days 4 and 7 after tumor implantation. Combined treatment of 41 degrees C hyperthermia and bleomycin resulted in mild reductions of tumor growth. Hyperthermia plus bleomycin led to marked reduction in tumor growth under the condition of a 43 degrees C temperature. The antitumor effects of 41 degrees C hyperthermia combined with bleomycin were enhanced by the additional use of ethanol, and these effects were more remarkable than those of 43 degrees C hyperthermia and bleomycin. A possible application of these findings to clinical therapy for advanced esophageal cancer was discussed.

Undifferentiated carcinoma of the parotid gland. Case report with electron microscopic findings.

Two cases of undifferentiated carcinoma of the parotid gland were studied by light and electron microscopy. Light microscopy showed nests of ovoid cells with scanty cytoplasm and pyknotic nuclei in two cases. One case was the small-cell type, and another one was the large-cell type histopathologically. Electron microscopy showed two distinct cell types in each tumor: Case 1 (small-cell type). - An epithelial-like cell, and an irregular-shaped cell containing bundles of filaments suggesting myoepithelial differentiation. Case 2 (large-cell type). - An epithelial-like cell, and a large cell containing secretory-like granules. These findings support a salivary duct epithelial origin for these tumors.

Chronic sialadenitis of the submandibular gland (so-called Küttner tumor).

Two cases of chronic sialadenitis of the submandibular gland (so-called Küttner tumor) are reported. Clinically, they are asymptomatic except for firm swellings which are similar to neoplasms. Light microscopic observations revealed that the lesion is characterized by various degrees of atrophy and/or destruction of acini, infiltration of inflammatory cells, periductal fibrosis and lymph follicle formation. With the electron microscope, a marked increase of collagen fibers are observed throughout the gland parenchyma, and the secretory granules of acinal cells in the remaining acini remarkably decreased in number. We suggest that an autoimmune mechanism may play a role in case 1 and that an ascending infection through the duct system may be a main factor in case 2.

Neuromuscular junctions of the posterior cricoarytenoid muscle in the cat.

The motor end plate of the cat posterior cricoarytenoid muscle is examined by using electron microscope and histochemical method. Furthermore, we studied the localization of acetylcholine receptors by fluorescent staining of snake neurotoxins (alpha-bungarotoxin and erabutoxin b) and autoradiography of 125I-labeled alpha-bungarotoxin. The fine structure of the motor end plate of this muscle is approximately identical to that of other singly innervated skeletal muscles. The subneural apparatus is activated by cholinesterase (ChE). ChE reaction appears markedly in primary and secondary synaptic clefts. No remarkable difference is found between TMR-labeled neurotoxin and FITC-conjugated neurotoxin. Fluorescent staining is associated with subsynaptic muscle membranes and not with motor nerve terminals. The autoradiogram revealed heavy accumulations of grains at the junctional folds.

[Local hyperthermo-chemotherapy plus ethanol for advanced esophageal cancer].

Eight esophageal squamous cell cancer patients were treated with local hyperthermochemotherapy plus ethanol in addition to bypass surgery and supraclavicular esophagostomy. Local hyperthermochemotherapy plus ethanol treatment was as follows: The esophagus was perfused for one hour using esophagostomy with warmed saline (42-44 degrees C) containing 6-10% ethanol, the overflowing saline was aspirated. Simultaneously, 15 mg Bleomycin or 10 mg Peplomycin was administered intravenously and 5 mg Bleomycin or Peplomycin was administered directly to the esophageal site. This procedure was repeated twice a week for a total of 10 to 24 times. Six of the 8 patients (75%) thus treated showed a shortening of the duration of the shadow defect in X-radiograms. In one case, the lesion was removed and the patient is doing well 9 months after the operation.

[Fundamental and clinical studies on T-1982 (cefbuperzone), a new cephamycin antibiotic, in the field of pediatrics].

Studies on T-1982 (cefbuperazone), a new cephamycin antibiotic, were carried out in the field of pediatrics, and the following results were obtained. 1. Peak MIC of T-1982 against S. pyogenes (group A) lately isolated was 0.39 micrograms/ml, and the drug was active even against highly resistant strains of macrolides, lincomycin, tetracycline and chloramphenicol. 2. Peak MICs of T-1982 were 0.78 microgram/ml against B. pertussis, 0.2 microgram/ml against E. coli and less than or equal to 0.05 microgram/ml against K. oxytoca, and the drug was also active against ampicillin-resistant bacteria. 3. Serum levels and urinary excretions of T-1982 were investigated in 6 cases. When given at a dose of 20-28 mg/kg by 1 hour intravenous drip infusion, serum concentrations of T-1982 attained the peak level of 63.5-75.9 micrograms/ml at the end of administration and sustained the level of 0.9-2.6 micrograms/ml at 6 hours, the serum half-life (T 1/2) ranging 70-82 minutes. Approximately 20-72% of the dose were excreted in the active form into urine within 6 hours. 4. Twenty-seven cases of acute pediatric infections were treated with T-1982 mainly by intravenous drip infusion, and satisfactory clinical results were obtained in all the cases of angina lacunaris, bronchitis, bronchopneumonia, pertussis, sepsis caused by Serratia and acute urinary tract infections caused by ampicillin-resistant E. coli. The efficacy rate was 96.3%. In this study the drug was administered chiefly at a daily dose of 50-70 mg/kg 2-3 times a day for 2-12 days. 5. Gram-positive cocci (S. aureus, S. pneumoniae, S. pyogenes) and Gram-negative rods (H. influenzae, H. parainfluenzae P. vulgaris, B. pertussis, S. marcescens, E. coli) were eradicated by the treatment with T-1982. 6. No noticeable side effects were observed, except for temporary increase of eosinophil in 2 cases and slight elevation of GOT in 1 case.
