A comparative study of three fast algorithms to estimate cerebral blood flow and distribution volume using N-isopropyl-p-[123I]iodoamphetamine and two SPECT scans.

To estimate regional cerebral blood flow (rCBF) and distribution volume (Vd) using N-isopropyl-p-[123]iodoamphetamine (IMP) and two SPECT data (early and delayed data) measured at two discrete time points, three fast algorithms are implemented, which are the table look-up method (TLU), the linear least-squares fitting method (LLSF), and the linear search method (LS). A two-compartment model was used as the IMP kinetic model. These algorithms were applied to the early and delayed data measured by the two scan protocols, t1 = 12 min, t2 = 54 min (scan A) and t1 = 12 min, t2 = 85 min (scan B). The estimated rCBF and Vd values using the present three algorithms were in good agreement with those using the nonlinear least-squares fitting technique and dynamic SPECT data. The quantitative value and signal to noise (S/N) ratio of rCBF images by TLU was not changed whether scan A or scan B was used. However, the S/N of the Vd image by TLU using scan B improved over that using scan A. The same tendency was observed in the results of LLSF and LS. The CPU times for the parametric imaging of CBF and Vd images for one slice (64 x 64 matrix) were 0.04, 0.09, and 0.14 s using TLU, LLSF, and LS, respectively. In conclusion, the present three algorithms gave reasonably accurate estimates of rCBF and Vd using two SPECT data, and are well suited for parametric imaging because of their good statistical properties and high computational efficiency.

Polymorphic drug metabolism: studies with recombinant Chinese hamster cells and analyses in human populations.

Most promutagens and procarcinogens exert their genotoxicity after undergoing metabolic activation. Metabolism of chemicals is an important factor in limiting the extent of the action of a chemical. In this study, we established cell lines which carried cDNAs coding for human CYP1A2 and N-acetyltransferase (NAT); the latter functions as O-acetyltransferase for N-hydroxyarylamines formed by CYP1A2. A cell line which expresses CYP1A2 together with P450 reductase activated aflatoxin B1, but not heterocyclic amines. A cell line which carries CYP1A2 and polymorphic NAT (NAT2) in addition to P450 reductase efficiently activated IQ and some other heterocyclic amines. However, a cell line which carries CYP1A2 and monomorphic NAT (NAT1) had only low activity toward the same heterocyclic amines. In order to determine the presence of and frequency of genetic polymorphisms of CYP1A2 and NAT2 in humans, we performed in caffeine phenotyping test on 205 Japanese volunteers. Analyses of metabolic ratios of urinary metabolites showed a bimodal distribution, indicating that about 86% and 91% of Japanese were extensive metabolizers (EM) of CYP1A2 and NAT2, respectively. The genotype NAT2 determined by the PCR-RFLP method agreed completely with the phenotype. To determine the mechanism of the differences in CYP1A2 activity, genomic DNA from peripheral lymphocytes of poor metabolizers (PM) and EM was subjected to DNA sequencing. No differences in nucleotide sequence were observed between PMs and EMs in the exons, exon-intron junctions and 5'-flanking region of the CYP1A2 gene.(ABSTRACT TRUNCATED AT 250 WORDS)

Inhibition of UDP-glucuronosyltransferase by aglycons of natural glucuronides in kampo medicines using SN-38 as a substrate.

7-Ethyl-10-[4-(piperidino)-1-piperidino]carbonyloxycamptothecin (CPT-11), a potent anticancer agent for lung and gynecological cancers, is metabolized in vivo to the active compound, 7-ethyl-10-hydroxycamptothecin (SN-38), which is subsequently conjugated to SN-38-glucuronide by UDP-glucuronosyltransferase (UDP-GT). Three purified aglycons of natural glucuronides, baicalein, luteolin and glycyrrhetic acid, inhibited UDP-GT activity towards SN-38 as a substrate. The inhibitory potencies of these aglycons toward UDP-GT were similar to that of 1-naphthol. Based on these results, together with our previous finding that the corresponding glucuronides used in the present study strongly inhibited beta-glucuronidase in gut flora, we propose that materials in Kampo (Japanese herbal) medicines containing these aglycons of natural glucuronides could be used in vivo to decrease the enterohepatic circulation of SN-38 and other drugs.

Amino acids and peptides. XL. Synthesis of Ac-Tyr-Val-Ala-Asp-MCA using newly developed acetylating reagent.

2-Acetoxy-3-benzyl-5-methyl-6-isobutylpyrazine was prepared by cyclization of H-Phe-Leu-CH2Cl, followed by acetylation with acetic anhydride. This pyrazine derivative can react with amino groups of amino acids or peptides to produce acetyl amino acids or acetyl peptides without acetylation of hydroxy group of Tyr, Ser and Thr. Using this acetylating reagent, Ac-Tyr-Val-Ala-Asp-MCA, which is a specific substrate of the interleukin-I (IL-I) processing enzyme, was prepared.

S-oxidation of (+)-cis-3,5-dimethyl-2-(3-pyridyl)-thiazolidin-4-one hydrochloride by rat hepatic flavin-containing monooxygenase 1 expressed in yeast.

1. Rat hepatic flavin-containing monooxygenase 1 (FMO1) expressed in yeast catalyzed the S-oxidation of (+)-cis-3,5-dimethyl-2-(3-pyridyl)thiazolidin-4-one hydrochloride (SM-12502) in vitro. 2. S-oxidation was inhibited by 1-(1-naphthyl)-2-thiourea and thiobenzamide, known inhibitors of FMO, but was not enhanced by n-octylamine, a known enhancer of FMO. 3. The rate of S-oxide formation from SM-12502 was about four-fold lower than that from (+/-)-trans-3,5-dimethyl-2-(3-pyridyl)thiazolidin-4-one hydrochloride (SM-9979) and enantioselectivity and diastereoselectivity of the S-oxidation reaction were observed. 4. The ability of the recombinant yeast to produce the S-oxide from SM-12502 was maintained for long periods and exemplifies the recombinant yeast as a bioreactor to produce a large amount of the S-oxide.

Efficient complementary DNA directed expression of human fetal liver cytochrome P450 (CYP3A7) in insect cells using baculovirus.

CYP3A7 is a form of cytochrome P450, which is expressed specifically in human fetal livers. NPVHF1, a recombinant baculovirus containing the entire coding region of CYP3A7, was constructed and infected to Spodoptera frugiperda (Sf9) cells. Upon infection with NPVHF1, the Sf9 cells expressed the CYP3A7 to the maximum content of 0.2 nmol per mg of whole cell lysates 72 hours after infection. A 5.5-fold expression level (1.1 nmol per mg of whole cell lysates) was attainable when cultured in the presence of externally added hemin. A catalytic activity of the CYP3A7 expressed in the Sf9 cells was confirmed by the umu gene expression mutation assay, in which aflatoxin B1 was activated to a mutagen by the expressed CYP3A7 in the presence of NADPH-cytochrome P450 reductase and cytochrome b5. From these results, it is concluded that the baculovirus expression system enables the high-level expression of CYP3A7 and will be a very useful tool for the characterization of CYP3A7.

[A study on accuracy of rCBF measurements using the conventional microsphere method with N-isopropyl-p-[123I]iodoamphetamine and SPECT].

To evaluate the accuracy of conventional microsphere method for the measurement of regional cerebral blood flow (rCBF) based on the microsphere model with N-isopropyl-p-[123I]iodoamphetamine (123I-IMP) and SPECT, we performed simulation analysis and clinical studies. Although the microsphere method requires early SPECT scan at a few minutes after injection of the tracer for the accurate measurement of rCBF, the conventional microsphere method, which is generally used, requires more delayed SPECT scan with long scan-duration. In the conventional microsphere method, the delayed SPECT image is corrected to the image at a few minutes after injection of the tracer by using the monitored entire brain activity. By the simulation analysis based on the 2-compartment model (influx; K1 and outflux; k2) using the input function and the entire brain activity obtained from eight subjects respectively, it was found that the conventional microsphere method overestimated the rCBF in the practical range of rCBF and Vd (= K1/k2) given in the 2-compartment model. When the values of rCBF and Vd in the 2-compartment model were given at 0.5 (ml/g/min) and 30 (ml/g) respectively, the rate of overestimation of rCBF by the conventional microsphere method was determined to be 17.3 +/- 0.7% (mean +/- S.D.). Also in clinical studies for eight subjects, the conventional microsphere method overestimated the rCBF compared with those evaluated by non-linear least squares fitting (NLLSF) analysis based on the 2-compartment model. Those results agreed well with the simulation analysis, suggesting the validity of the simulation. The rCBF values clinically estimated by the conventional method were, however, significantly correlated with those calculated by NLLSF analysis, and there were not so much difference between the two quantitative rCBF images obtained by the conventional microsphere method and the microsphere method. Therefore, we conclude that the conventional microsphere method is clinically useful in spite of the overestimation of rCBF.

Chiral inversion of drug: role of intestinal bacteria in the stereoselective sulphoxide reduction of flosequinan.

Chiral inversion at a sulphoxide position of flosequinan enantiomers [(+/-)-7-fluoro-1-methyl-3-methylsulphinyl-4-quinolone] occurred in conventional rats but not in either germ-free rats or rats treated with antibiotics after an oral administration of each enantiomer. Thus, it was postulated that the chiral inversion occurred by mechanisms mediated by intestinal bacteria. The intestinal content isolated from conventional rats reduced R(+)- and S(-)-flosequinan to produce the sulphide, while intestinal content from rats treated with antibiotics did not reduce the drug. Several strains of facultative anaerobes possessed a high flosequinan reducing activity. Escherichia coli, Klebsiella oxytoca and Klebsiella pneumoniae reduced R(+)-flosequinan to the sulphide stereoselectively. On the other hand, Enterobacter aerogenes and Micrococcus agilis exclusively reduced S(-)-flosequinan. The sulphide, which could be produced by intestinal bacteria from R(+)- and S(-)-flosequinan, was readily absorbed upon an oral administration to rats, and was oxidized fairly rapidly to R(+)- and S(-)-flosequinan and further to the sulphone form. Based on these data, it has been confirmed that chiral inversion at the sulphoxide position of flosequinan enantiomers occur via stereoselective reduction of sulphoxide by intestinal bacteria to form the sulphide, followed by oxidation of the sulphide in the body to produce R(+)- and S(-)-flosequinan.

Stereoselective S-oxidation of flosequinan sulfide by rat hepatic flavin-containing monooxygenase 1A1 expressed in yeast.

Rat hepatic flavin-containing monooxygenase (FMO) 1A1 expressed in yeast catalysed the S-oxidation of flosequinan sulfide (7-fluoro-1-methyl-3-methylthio-4-quinolone) to R(+)-flosequinan (sulfoxide form, R(+)-7-fluoro-1-methyl-3-methylsulfinyl-4-quinolone) but not to S(-)-flosequinan, and did not catalyse the oxidation of R(+)- and S(-)-flosequinan to flosequinan sulfone. The Km and Vmax for the stereoselective S-oxidation were 33 microM and 6.2 nmol per min per mg of microsomal protein, respectively. The S-oxidation was inhibited by 1-(1-naphthyl)-2-thiourea and thiobenzamide. n-Octylamine activated the S-oxidation with little change in stereoselectivity. The ability of the recombinant yeast to produce R(+)-flosequinan from flosequinan sulfide could be maintained for at least 2 days and exemplifies the value of a recombinant yeast expressing FMO as a stereoselective bioreactor.

Asbestos and nonasbestos fiber content in lung tissue of Japanese patients with malignant mesothelioma.

BACKGROUND: Pulmonary fiber content of both asbestos and nonasbestos types were evaluated in Japanese patients with malignant pleural mesotheliomas. METHODS: Pulmonary fiber content was analyzed in 16 patients and 16 case-matched control subjects by transmission electron microscopy with energy-dispersive X-ray analysis using a low-temperature ashing procedure. RESULTS: The geometric mean content of total asbestos was significantly higher in the patients (22.0 x 10(6) fibers/g dry lung) than in the control subjects (2.24 x 10(6) fibers/g dry lung) (P < 0.01). When the asbestos content was analyzed by fiber type, the geometric means were also consistently and significantly higher among the patients compared with the control subjects (P < 0.01). Results were as follows: (1) amosite: patients 3.94 times 10(6) versus control subjects 0.23 x 10(6); (2) crocidolite: patients 3.56 times 10(6) versus control subjects 0.35 times 10(6); (3) total amphiboles: patients 16.0 times 10(6) versus control subjects 0.77 times 10(6); and (4) chrysotile: patients 3.76 times 10(6) versus control subjects 1.01 times 10(6). However, when individual total asbestos content was considered, 7 of the 16 patients (44%) had levels lower than the highest value noted among the control subjects. Pulmonary fiber content of patients and control subjects also revealed the presence of nonasbestos fibers. The geometric mean of nonasbestos fibers was significantly higher in the patients (87.3 x 10(6)) than in control subjects (33.8 x 10(6)) (P < 0.01). The major type of nonasbestos fibers in both groups was aluminum silicates. The mean of ratios of nonasbestos fiber contents to total asbestos contents in the patients and control subjects was 7.0 and 17.3, respectively. CONCLUSIONS: The results were mainly in agreement with the findings of earlier investigations, but fiber content of both chrysotile and nonasbestos fiber as well as those of amphibole asbestos were significantly higher in the patients than in the control subjects.

Molecular cloning and sequence analysis of hamster CYP2E1.

Two cDNA clones, pHSj31 and pHSj3, coding for CYP2E1 were isolated from a hamster liver cDNA library. The sequence analyses revealed that they encoded the same polypeptide of 493 amino acid residues (M(r) = 56,616) and differed from the length of their 3'-untranslated region. The deduced amino acid sequence of hamster CYP2E1 showed approx. 90% identities with those of the rats and mice, and approx. 80% identities with those of the rabbits, monkeys and humans. The NH2-terminal 35 deduced amino acid sequences of hamster CYP2E1 were completely identical with purified protein, ha P-450j. The Northern blot analysis showed that CYP2E1 was expressed in livers and to lesser extents in kidneys and lungs.

The pins and rubbers traction system for treatment of comminuted intraarticular fractures and fracture-dislocations in the hand.

The authors have developed a new skeletal traction system for comminuted intraarticular fractures and fracture-dislocations in the hand. The system consists of two or three Kirschner wires and rubber bands, and is easy to assemble. It is more compact and comfortable than the banjo splint, and equally effective, and it allows early motion of the affected digits. A description of the technique is followed by the clinical results of seven cases of severe articular injuries in the hand. At the time of follow-up, the average range of the affected PIP joint motion was about 80 degrees. The final active motion of the injured DIP joint ranged from 0 to 40 degrees in flexion and that of the affected thumb (trapezial fracture) was not limited. The average follow-up period was 13.1 months.

Kawasaki disease with a concomitant primary Epstein-Barr virus infection.

A two year old boy exhibited not only clinical manifestations which suggested a recurrence of Kawasaki disease (KD) but also evidence of a primary infection by Epstein-Barr virus (EBV) including tonsillitis, splenomegaly and atypical lymphocytosis in the peripheral blood. An inverted CD4/CD8 ratio in lymphocyte subsets suggested the presence of infectious mononucleosis (IM). Epstein-Barr virus titers (viral capsid antigen-immunoglobulin G 1:20; Epstein-Barr virus-associated nuclear antigen < 1:10) showed an acute EBV infection and the presence of EBV genome in the blood was determined by the polymerase chain reaction technique. In Japan, the peak incidence of KD and IM is in children under 4 years of age. From the investigation of EBV titers, it has been reported that some patients with KD develop an associated, unusual primary EBV infection. Kawasaki disease concurrent with a primary EBV infection as in this case, suggests the possibility of an etiologic agent related to the KD rather than to the EBV infection itself.

Occurrence of autoantibody to protein disulfide isomerase in rats with xenobiotic-induced hepatitis.

The occurrence of an autoantibody to protein disulfide isomerase (PDI) in rats after administration of various hepatotoxic drugs was investigated by immunoblotting and radioimmunoassay. An anti-PDI autoantibody was detected with high frequency in rats treated with D-galactosamine, acetaminophen with diethylmaleate, and carbon tetrachloride with diethylmaleate. The antibody-positive rate was relatively low in the groups of rats given carbon tetrachloride, acetaminophen or DL-ethionine alone. The anti-PDI antibody was not detected in rats treated with diethylmaleate alone. Although the mechanism of the production of the anti-PDI autoantibody is unclear, the occurrence of anti-PDI antibody correlated with high serum GPT activities. It is suggested that the autoantibody plays an important role in the development and persistence of drug-induced hepatitis.

Occurrence of autoantibody to protein disulfide isomerase in patients with hepatic disorder.

The occurrence of autoantibody to protein disulfide isomerase (PDI) was examined in sera from patients with alcoholic liver disease, liver cirrhosis, systemic lupus erythmatosus (SLE) and cancer. Judging by results of Western blotting using a purified preparation of PDI, the anti-PDI antibody was hardly seen in most sera from healthy controls but was raised in patients with alcoholic liver disease, liver cirrhosis, SLE and liver cancer. When the antibody titer to PDI was measured by radioactivity in a radioimmunoassay, positive values (cut off value; mean + 2SD = 256 cpm) were seen in 46% (11/24) of the patients with moderate alcoholic liver disease (mean radioactivity, 339 cpm) and in 54% (7/13) of those with severe alcoholic liver disease (mean radioactivity, 664 cpm). Only 8% (4/48) of the healthy controls were positive (mean radioactivity, 50 cpm). Four of six patients with hepatoma were positive, while none of the seven patients with non-hepatic cancer were positive. The positive proportions in the patients with liver cirrhosis and SLE were 64% (9/14) and 40% (4/10), respectively. These results suggest that the occurrence of anti-PDI antibody would play an immunological role in the progression of hepatic disorders.

Lymphoid interstitial pneumonia as a pulmonary lesion of idiopathic plasmacytic lymphadenopathy with hyperimmunoglobulinemia.

A case of idiopathic plasmacytic lymphadenopathy with hyperimmunoglobulinemia is reported. A 71-year-old man was admitted to the hospital because of the abnormal shadow on chest roentgenogram. Chest X-ray taken on admission showed remarkable diffuse infiltration and pleural thickening. Laboratory examinations revealed an elevated total protein level of 10.1 g/dl, and a gammaglobulin level of 7.0 g/dl including 6,790 mg/dl IgG. Mediastinal lymphadenopathy was observed on a chest CT. The patient underwent open lung biopsy. Heavy infiltration of lymphocytes and plasma cells were seen in the moderately fibrotic pulmonary interstitium. LIP was diagnosed. Lymph node biopsy was also performed. Follicular hyperplasia with prominent germinal centers and plasma cell proliferation in the interfollicular area were seen. Treatment with prednisolone resulted in an improvement in the chest X-ray findings, as well as a diminished polyclonal hypergammaglobulin level.
