[Primary adenocarcinoma of renal pelvis and ureter associated with incomplete ureteral duplication: a case report].

We report a rare case of primary adenocarcinoma of the renal pelvis and ureter in an incomplete duplicated ureter. A 69-year-old male with macroscopic hematuria consulted our hospital. Retrograde pyelography revealed left incomplete duplicated ureter and a filling defect in the lower pole of the ureter. Transurethral ureteroscopy was employed to investigate the filling defect, and a papillary tumor was detected extending into the lower segment of the incomplete duplicated ureter. From these findings, the patient was diagnosed with tumor of renal pelvis and ureter, and left total nephroureterectomy was performed. Pathological diagnosis was adenocarcinoma of the renal pelvis and ureter, INF beta, pT2, pR0, pL0, PV0, PN0. No evidence of either tumor recurrence or metastasis was found on the 3-month postoperative follow up. This case is thought to be the first report of primary adenocarcinoma of the renal pelvis in an incomplete duplicated ureter.

[A case of rheumatoid factor-positive pachymeningitis].

We report a case of rheumatoid pachymeningitis involving the posterior fossa. A 71-year-old woman presented with headache, vertigo and dizziness. MRI demonstrated hypertrophic masses in the meninges of the left cerebellar tentorium, cerebellopontine angle and the fourth ventricle. Six months of treatment with betamethasone, prednisolone and azathioprine improved the patient's neurological status. Because an inappropriate immune process and systemic immune disease may cause idiopathic pachymeningitis, the performance of immunological studies for idiopathic cases is mandatory.

Analysis of S-layer proteins of Lactobacillus brevis.

The presence of S-layer proteins in Lactobacillus brevis was examined by SDS-PAGE analysis. Thirty six out of a total of 41 L. brevis strains possessed S-layer proteins of molecular masses ranging from 38 to 55 kDa. Western blot analysis using antisera raised against whole cells of S-layer protein-carrying strains demonstrated the heterogeneity of L. brevis S-layer proteins. No clear relationship was observed between the presence of S-layer proteins or their immunological characteristics and the physiological activity of L. brevis as a beer spoilage organism.

Anemia-inducing substance from plasma of patients with advanced malignant neoplasms.

Patients with advanced malignant neoplasms develop anemia and immunosuppression. During an attempt to identify the causes, we have found that plasma from such patients makes RBCs more fragile in hypotonic buffer, according to results obtained with a coil planet centrifuge. Plasma from these patients suppresses mitogen-stimulated lymphocyte proliferation. In this study, we identified the substance with these effects as a protein. During two-dimensional gel electrophoresis, two isomers with M(r) 50,000 and slightly different isoelectric points near 6.0 were found. Cell fractionation showed that these proteins were in both the cytosol and the nuclear fraction of cells in neoplasms. Another protein with the same antigenicity and a M(r) 100,000 found in the nuclear fraction of cells in neoplasms.

Effects of the autonomic ganglion blocking agent hexamethonium on vasodilator responses mediated by the parasympathetic ganglion on the chorda tympani pathway of the cat.

We investigated the pharmacological properties of a parasympathetic ganglion (a chorda tympani ganglion) that mediates vasodilator responses in the lower lip induced by electrical stimulation of the distal cut end of the chorda tympani or facial nerve root of the cat. These responses were suppressed by prior treatment with the autonomic ganglion blocking agent hexamethonium. We compared the effects of three doses of hexamethonium (1, 3 and 10 mg/kg, i.v.) on the chorda tympani ganglion with their effects on three large ganglia; the otic, submandibular and pterygopalatine ganglia that mediate vasodilator responses. Experiments were conducted on 20 cats weighing 1-3 kg which had been anesthetized with a mixture of urethane (100 mg/kg, i.v.) and chloralose (50 mg/kg, i.v.) then artificially ventilated (pancuronium bromide 0.2 mg/kg per h, i.v.). The chorda tympani ganglion's sensitivity to hexamethonium was similar to that of the otic ganglion but differed from the sensitivities of submandibular and pterygopalatine ganglia. We speculate that transmission through the chorda tympani ganglion is pharmacologically similar to the otic ganglion, although its precise location has yet to be determined.

Angiographic extravasation of contrast medium in acute "spontaneous" subdural hematoma.

Acute spontaneous subdural hematoma is very rare. We have encountered four such cases and verified the arterial origin of the bleeding at operation. None of the patients had a history of head trauma, and each had developed sudden onset of headache and other neurologic deficits, which simulate other cerebrovascular diseases. CT directly revealed subdural hematoma but gave no indication as to the source of the bleeding. Cerebral angiography was performed in all cases, with three of them showing localized extravasation of the contrast material into the subdural space. The extravasation was noted usually in the late arterial phase. This is a useful finding for diagnosing this disease and localizing the bleeding point. It is expected that with more routine use of cerebral angiography in cases of acute spontaneous subdural hematoma, extravasation of the contrast medium will be seen more frequently.

[Drug sensitivity and expression of beta-lactamase in enterococci isolated at Okayama University Hospital].

The following results were obtained from a study of Enterococci isolated at the Okayama University Hospital from 1990 to 1994. 1) The isolation frequency of Enterococci was about 6% for each year. 2) The percentage of Enterococcus faecalis (E. faecalis) among Enterococci decreased each year, but the percentage of Enterococcus faecium (E. faecium), and Enterococcus avium (E. avium) increased each year. 3) Results of drug sensitivity tests revealed that the incidence of ofloxacin (OFLX) resistant E. faecalis and E. avium increased each year. Also, the resistance of E. faecalis to ampicillin (ABPC), the primary medication choice for treating E. faecalis, increased each year. 4) A few vancomycin (VCM) non-sensitive Enterococci were isolated. These bacteria showed low-sensitivity toward ABPC, imipenem (IPM), gentamicin (GM), and OFLX, and the extent of multiple drug resistance increased each year. 5) One hundred strains of Enterococci were selected to examine the expression of beta-lactamase using the broth method and the iodine-starch method. However, beta-lactamase producing strains were not detected.

Influence of different doses of methyl ethyl ketone on 2,5-hexanedione concentrations in the sciatic nerve, serum, and urine of rats.

Rats were injected subcutaneously with 2,5-hexanedione (2,5-HD 2.6 m mol/kg) alone (HD group) or with 2,5-HD and methyl ethyl ketone (MEK) (2.6 m mol/kg of each agent, HD&MEK group) or with 2,5-HD 2.6 m mol/kg and 5 times that dose (13.0 m mol/kg) of MEK (HD&5MEK group). The concentration of 2,5-HD in serum and in the sciatic nerve was determined 0.5, 1, 2, 4, 8, and 16 h after administration. Urinary 2,5-HD concentration was determined from the beginning of administration up to 16 h afterward. 1) The concentration of 2,5-HD in the serum, the sciatic nerve, and the urine was increased significantly (p < 0.05) in the co-administered groups; the higher the MEK doses were, the greater was the increase. 2) The clearance of 2,5-HD from both the serum and the sciatic nerve was delayed in the co-administered groups. The highest concentration in serum and the sciatic nerve appeared at 1 and 2 h respectively. After administration, the biological halflife (t1/2) of 2,5-HD from 1 to 8 h in serum was 6.5, 5.8 and 12.0 h for the HD, HD&MEK, and HD&5 MEK groups respectively. From 8 to 16 h, the t1/2 in serum was 1.2, 3.2 and 16.6 h for the HD, HD&MEK, and HD&5MEK groups, respectively. In nerve tissue, the prolongation of clearance in the co-administered groups was greater than that in serum, the t1/2 from 2 to 8 h being 5.2, 9.6 and 19.9 h for the HD, HD&MEK, and HD&5MEK groups, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Endovascular treatment of an extracranial internal carotid artery aneurysm at the skull base with mechanically detachable coils--case report.

A 45-year-old male experienced a transient ischemic attack manifesting as left blurred vision and right hemiparesis. Angiography demonstrated an extracranial internal carotid artery aneurysm at the entrance of the carotid canal just above the C-1 vertebral arch. Intraluminal occlusion of the aneurysm was achieved with mechanically detachable coils under local anesthesia, and the carotid flow was preserved. There was no complication during or following the procedure. Treatment of aneurysms using detachable coils is an important alternative to surgical treatment when surgical access to the lesion is difficult.

[Fournier's gangrene in a patient with perirectal abscess: a case report].

A case of an 81-year-old man with Fournier's gangrene was reported. The patient visited our hospital complaining of scrotal swelling and redness. Perirectal abscess was found and ultrasound study revealed thickness of scrotal skin and normal testes. The patient was immediately treated with antibiotics, incision and debridement of the scrotal skin. The lesion healed 8 weeks later.

[Antitumor effects of liposome-entrapped carboplatin (Lipo-CBDCA) after intraperitoneal administration in rats bearing carcinomatous peritonitis].

We examined the distribution of Lipo-CBDCA after intraperitoneal administration and antitumor effects in rats. The serum levels of platinum in Lipo-CBDCA were lower than in free-CBDCA intraperitoneal or intravenous administration at 15 and 30 min. after administration. After 3 hours, Lipo-CBDCA showed higher levels of serum platinum than free-CBDCA. These data showed the slow release of Lipo-CBDCA. The antitumor effects of Lipo-CBDCA were studied in rats with peritoneal dissemination due to AH 130 tumors. Intraperitoneal treatment with Lipo-CBDCA prolonged the life span significantly compared with Lipo-CBDCA. No side effects of chemotherapy were found in the liver, kidney, spleen or small intestine. A gastric cancer patient suffering from carcinomatous peritonitis with remarkable ascites was treated with Lipo-CBDCA intraperitoneally. After several injections of Lipo-CBDCA, the ascites disappeared completely and the CEA level of ascites decreased dramatically. These results indicate that intraperitoneal chemotherapy with Lipo-CBDCA may be more effective than free-CBDCA to manage carcinomatous peritonitis, and may be therapeutically useful without toxic side effects.

[Assessment of serum estrogen levels by RIA with HPLC during hormone replacement therapy].

We studied the serum estradiol and estrone levels in 146 peri and postmenopausal women, and in 38 women who had complained of various climacteric disturbance symptoms during daily hormone replacement therapy (HRT) with conjugated equine estrogen (CEE) 0.625 mg and medroxyprogesterone acetate (MPA) 2.5 mg. Serum estradiol and estrone were measured before treatment, and at 6 months, and after one year of the HRT therapy by HPLC-radioimmunoassay. In 146 peri and postmenopausal women, the serum level of estradiol was from 3 to 6pg/ml. The serum level of estradiol in 38 women after HRT significantly increased (p < 0.01) from 3.34 to 23.6 pg/ml at 6 months, and 21.5 pg/ml at 12 months. The serum level of estrone significantly increased (p < 0.01) from 26.6pg/ml to 156.7pg/ml at 6 months, and 137.2pg/ml at 12 month. These results are very useful for deciding on the doses of hormones and the expected serum estradiol level in HRT for Japanese women.

[Primary squamous cell carcinoma of the ureter: a case report].

A case of squamous cell carcinoma of ureter is presented. A 64-year-old male suffering from right lower abdominal pain and gross hematuria visited our hospital. Right hydronephrosis was found by ultrasound examination. Intravenous pyelography revealed a right non-functioning kidney. Abdominal computed tomographic scanning showed right hydroureteronephrosis and a soft-tissue density mass in the right lower ureter. Retrograde pyelography demonstrated a filling defect in the right lower ureter. Squamous cell carcinoma was suspected by cytological examination. On the basis of the above findings, right nephroureterectomy with partial cystectomy was performed. Pathohistological diagnosis was squamous cell carcinoma of the ureter, G3, INF gamma, pT3, pR0, pL1, pV1, pN1. No evidence of either tumor recurrence or metastasis was found for 6 months after the operation. Sixty-one cases of primary ureteral squamous cell carcinoma, including our case, were collected from the Japanese literature and characteristic clinical features of the tumor are discussed.

[Body surface ST mapping in daily life using multi-lead ambulatory electrocardiograph].

To extend the usefulness of multi-lead ambulatory electrocardiography, we attempted to construct body surface isopotential maps of ST segment using our newly designed multi-lead ambulatory electrocardiograph. Material consisted of 40 patients (pts) with angiographically proven coronary arterial stenosis. By a commercially-available 4-channel recorder and a specially designed adapter, 30-lead ECGs were sequentially recorded together with body position signals for 24 hours. Compensation for ST distortion and interpolation of ST level on time scale techniques were performed before construction of ST maps. The extension of ST depression area in pts with single LAD disease were larger than in pts with single LCX or RCA disease. As time elapsed, after occurrence of ischemia the extension and location of ST depression area were changed. These results were thought to be reasonable. In conclusion, the ambulatory recording of body surface ST maps may become useful for study of myocardial ischemia in daily life.

[Multi-lead ambulatory electrocardiograph for recording body surface ST maps in daily life].

We attempted to construct ambulatory body surface isopotential maps of ST segment using our newly designed multi-lead ambulatory electrocardiograph. The multi-lead ambulatory electrocardiograph. The multi-lead ambulatory ECG system consisted of a commercially-available 4-channel recorder and a specially designed adapter, by which 30-lead ECGs were recorded together with body position signals for 24 hours. The electrode positions were selected from the 87 lead points for body surface mapping according to Yamada et al. Compensation for ST distortion and interpolation of ST level on time scale techniques were performed before the construction of ST maps. The extension and location of ST depression area thus obtained after compensation and interpolation were thought to be reasonable. In conclusion, the ambulatory recording of body surface ST maps is possible using our new device and may become useful for study of myocardial ischemia in daily life.
