[A case of polyneuropathy associated with osteosclerotic myeloma].

A 50-year-old woman presented with sensorimotor polyneuropathy in the lower limbs. Biopsy of the muscle revealed neurogenic changes, and electrophysiological study and pathology of the biopsied nerve indicated myelinoaxonal degeneration. Her symptoms responded to neither plasmapheresis nor prednisolone. Pelvic radiography disclosed an osteosclerotic lesion that was subsequently diagnosed as IgA-lambda plasmacytoma by biopsy. Radiation therapy for this solitary osteosclerotic myeloma improved the patient's polyneuropathy. In Japan, reports of cases of polyneuropathy associated with osteosclerotic myeloma without multiorgan disorders as in our case have not previously been described; thus, such cases may be misdiagnosed. Therefore, in patients with polyneuropathy of unknown etiology, a systemic bone survey and repeated searches for M-protein are important. Treatment of bone lesions in patients with solitary osteosclerotic myeloma may improve the associated polyneuropathy.

[A case of ill-defined vitiliginous lesions in the posterior polar regions of both eyes].

In a 34-year-old man, who was examined for binocular visual disturbance as chief complaint at the department of ophthalmology, we found ill-defined vitiliginous lesions of a size about 1/5 approximately 1/7 the diameter of the papilla in the fundal posterior polar regions of both eyes. Fluorescein angiography (FA) revealed hyperfluorescence of the vitiligo. Blood tests showed increased herpes simplex and herpes zoster varicellosus antibody titers and positive antinuclear antibodies (spekled type). Following steroid treatment, the vitiligo remitted, but reappeared later. Indocyanine green infrared fluorescein angiography (IA) revealed hyperfluorescence of the vitiligo. Fundus findings suggested multiple evanescent white dot syndrome (MEWDS), but were judged to be negative because 1. the vitiliginous lesions were morphologically different; 2. the onset site of the vitiligo was more posterior and polar-sided, 3. onset of the vitiligo was binocular, 4. the vitiligo recurred and 5. late hypofluorescence was not observed in IA.

[Management of elderly patients with incidentally discovered unruptured aneurysms].

Long-term natural history of unruptured cerebral aneurysms is not found frequently. Hence, the indications for surgery on unruptured asymptomatic cerebral aneurysms are still unclear. The benefit of treatment ultimately depends on the relative risk of subsequent aneurysm rupture in untreated patients versus the risk involved in surgery. Clinical features of fourteen elderly patients aged over 70 years with incidentally discovered unruptured aneurysms were analyzed. Two were male and 12 were female, with ages ranging from 70 to 82 years (mean: 74.5 years). Aneurysms were located in the anterior circulation in 13 patients and in the posterior circulation in 1 patient. One patient had multiple aneurysms, that is, bilateral middle cerebral aneurysms. The size of all these aneurysms was less than 10 mm. The indication for surgery was determined case by case. General information about the natural history of incidentally discovered aneurysms was given to the patients and their relatives. Informed consent was based on the fact that subarachnoid hemorrhage was associated with a poor prognosis, while excellent operative results were common in patients with unruptured aneurysms. Five patients agreed to surgical treatment. Four of them, their ages being 70, 70, 72 and 72 years old, with aneurysms located on the middle cerebral arteries, underwent neck clipping of their aneurysms with no operative morbidity or mortality. However, the remaining one patient was not recommended for surgery in spite of her consent to having it, because of her high age (82 years) and the location of her aneurysm (intracavernous internal carotid artery). Consequently, 10 patients who didn't receive surgery were followed-up at periods ranging from 3 months to 7 years. Two patients developed rupture of the aneurysms, either proven (one patient) or presumed (one patient). The former patient made an uneventful recovery after surgery, but the latter died. None of the remaining eight patients have experienced rupture of the aneurysms. It is our clinical impression, however, that they harbor an unruptured aneurysm with at least mild trepidation. With the rapid aging of the population, withholding aneurysm surgery merely because a patient is elderly may not necessarily be the most appropriate decision. Our conclusions are as follows: (1) Elderly patients in their early seventies are apt to agree to having surgical treatment for their unruptured aneurysms. (2) The cases reported herein show that asymptomatic middle cerebral artery aneurysms were able to be clipped very safely. (3) Most patients have experienced a decrease in quality of life from knowing they are living with an unruptured aneurysm.

[A case of choroidal metastasis from bronchial atypical carcinoid tumor (well-differentiated neuroendocrine cell carcinoma)].

We describe a case of choroidal metastasis from bronchial atypical carcinoid tumor (well-differentiated neuroendocrine cell carcinoma). In this report, we compared the images of indocyanine green infrared fluorescence angiography (IA) to the histological appearance of the metastatic tumor. A 48-year-old male patient presented with metastatic choroidal tumors in both eyes. The IA findings revealed high intensity of fluorescence in the center of the tumor, but low intensity at the edge. Afterwards, the patient underwent enucleation of his left eye because of secondary glaucoma. Microscopic examination of the resected eyeball disclosed two histological patterns: one was that of typical carcinoid tumor; the other atypical carcinoid tumor. In addition, the tumor blood vessels had proliferated more densely in the center of the tumor than at the edge. Only two case reports in the English literature, as for as we know, have previously described the precise histological examination of both choroidal metastasis and primary bronchial carcinoid tumor. This case suggests that the extraordinary IA images are caused by the histologic character of the metastatic tumor: 1. rich vascularity, 2. variety of distribution of the tumor vessels.

Effects of plasmapheresis in a rabbit model of cancer cachexia.

We developed a model of cancer cachexia by transplanting VX2 tumors into rabbits and investigated the effects of plasmapheresis in this model. AIS activity was identified in supernatants obtained from VX2 tumor by phenyl Sepharose chromatography. Fractions eluted with 0.5 M NaCl showed the ability to damage the red blood cell membrane and suppress immunity. Rabbits exhibited marked weight loss, anemia and immunodeficiency 40 days after transplantation of VX2 tumors. Plasmapheresis inhibited plasma-induced damage of the red blood cell membrane and suppression of immunity. Twice-weekly plasmapheresis resulted in gradual normalization of cellular immunity. Weight loss and survival were also improved. No side effects were observed.

[Application of ultrafast 3D-CT in cervical spine injury].

The authors investigated the usefulness of the ultrafast three-dimensional computed tomography (UF-3D-CT) in cervical spine injury. At Osaka City General Hospital, between April 1995 and March 1998, the authors examined 38 patients with cervical spine injury using UF-3D-CT. The ultrafast CT reduces scanning time. There were no complications associated with the examination. We examined 12 patients with vertebral body fracture, 11 patients with lamina fracture, 8 cases with subluxation including locking facet, 2 patients with atlanto-axial dislocation and 8 patients without bony abnormality. Results suggested that UF-3D-CT was very useful for helping us to understand the bony deformity and the relationship between the bone and the important vessels. UF-3D-CT is very useful for preoperative evaluation and pathophysiological evaluation in cervical spin injury.

Purification and partial characterization of an antigen specific to Lactobacillus brevis strains with beer spoilage activity.

Certain Lactobacillus brevis strains are resistant to hop-derived compounds such as isohumulone and are able to grow in beer. In this study, we raised an antiserum against our beer spoilage laboratory strain L. brevis 578 which reacted with 23 of 24 beer spoilers and two of 13 non-spoilers in precipitation reactions using 0.5 M NaOH cell extracts. This specific antigen to the beer spoilage L. brevis strains (SABSL) was demonstrated to be located beneath the S-layer proteins by agglutination reactions using S-layer protein-stripped cells obtained by treatment with 0.1 M NaOH. SABSL was purified using an affinity column coupled with an antibody against SABSL. The purified antigen was hydrolyzed with 2 M HCl and the hydrolyzate was analyzed by thin-layer chromatography and enzymatic analysis. The results showed that SABSL contains glycerol, phosphate, glycerophosphate, D-galactose and D-glucose. D-Galactose and D-glucose accounted for 4.7% and 0.1% of the composition, respectively. Melibiose, but not mannose, inhibited the precipitation reaction. Intense precipitation reactions were obtained with fractions which did not bind to the ConA-column. These results indicate that the immunodominant component of the SABSL is galactose and the SABSL determinant is most probably a galactosylated glycerol teichoic acid. The antiserum raised against the beer spoilage strain L. brevis 578 could distinguish between Pediococcus beer spoilers and non-spoilers in precipitation reactions.

Impairment of antigen-specific antibody production in transgenic mice expressing a dominant-negative form of gp130.

gp130 is a common signal-transducing receptor component for the interleukin 6 family of cytokines functioning in, for example, immune, hematopoietic, and nervous systems. In this study, to investigate the physiological functions of gp130 and to determine the pathological consequences of impaired gp130 signals, we have generated transgenic mice expressing a cytoplasmically truncated form of mouse gp130. Expression of this form of gp130 in lymphocytes significantly suppressed interleukin 6-induced tyrosine phosphorylation of endogenous gp130 and a downstream signaling molecule, signal transducer and activator of transcription 3, indicating that this form has a dominant negative function. In spite of the impaired gp130 signals, the development of lymphocytes in the transgenic mice appeared normal in terms of surface marker phenotypes. These mice, however, exhibited severe defects in antigen-specific antibody production of most immunoglobulin isotypes other than IgM after immunization with 2,4-dinitrophenol-conjugated ovalbumin. These results demonstrate in vivo that gp130 is essential for antigen-specific antibody production.

Cortical laminar necrosis in brain infarcts: chronological changes on MRI.

We studied the MRI characteristics of cortical laminar necrosis in ischaemic stroke. We reviewed 13 patients with cortical laminar high signal on T1-weighted images to analyse the chronological changes in signal intensity and contrast enhancement. High-density cortical lesions began to appear on T1-weighted images about 2 weeks after the ictus. At 1-2 months they were prominent. They began to fade from 3 months but could be seen up to 11 months. These cortical lesions showed isointensity or high intensity on T2-weighted images and did not show low intensity at any stage. Contrast enhancement of the laminar lesions was prominent at 1-2 months and became less apparent from 3 months, but could be seen up to 8 months.

Vertebral artery occlusion after subarachnoid hemorrhage from a dissecting aneurysm of the vertebral artery: case report.

BACKGROUND: Generally speaking, occlusion of the vertebral artery is a finding of a dissecting aneurysm associated with completed stroke. We present a case of a vertebral dissecting aneurysm that produced subarachnoid hemorrhage (SAH). Angiography on the day of hemorrhage, however, demonstrated complete occlusion of the vertebral artery. CASE PRESENTATION: A 44-year-old hypertensive woman suffered a sudden onset of headache and vomiting followed by loss of consciousness due to SAH. Angiography on the day of hemorrhage revealed a complete occlusion of the right vertebral artery just distal to the dissecting aneurysm. This is the first report of such a case. The patient was still considered to be at significant risk of rerupture. Craniotomy and clip occlusion of the right vertebral artery and the origin of the right posterior inferior cerebellar artery were carried out to trap the thin-walled sac. At discharge, the patient had recovered completely except for some left limb ataxia, which subsequently disappeared. CONCLUSIONS: Two options are available for the treatment of such a case: surgical or medical treatment. We employed surgery. Which is the preferred approach, however, is a difficult judgment to make at this juncture.

Spontaneous spinal subarachnoid hematoma of unknown pathogenesis: case reports.

OBJECTIVE AND IMPORTANCE: The occurrence of spontaneous spinal subarachnoid hematoma of unknown pathogenesis is extremely rare. In the cases reported to date, the hematoma, located dorsal to the spinal cord (dorsal type), has caused paraplegia and has required emergency surgical intervention. CLINICAL PRESENTATION: We examined two patients who sustained spontaneous spinal subarachnoid hematoma. Both experienced sudden back pain, but there were no symptoms of spinal cord compression. Magnetic resonance imaging revealed spinal subarachnoid hematoma located ventral to the spinal cord (ventral type). INTERVENTION: Both patients were treated conservatively, and follow-up examinations have revealed that they have remained neurologically normal for 7 years and 6 months, respectively. CONCLUSION: We postulate that there are two types of spontaneous spinal subarachnoid hematoma of unknown pathogenesis (ventral and dorsal types), each of which presents a distinct clinical picture and prognosis. Ventral type hematoma may be one of the causes of acute back pain, and because of its benign prognosis, surgical treatment may not be necessary.

Urethral recurrence of an urachal carcinoma: a case report.

A 68-year-old man presented with microscopic hematuria. Cystoscopy revealed a papillary and pedunculated tumor in the bladder dome which, on punch biopsy, proved to be adenocarcinoma. Partial cystectomy and urachal remnant resection were performed. Histopathologically, the tumor was diagnosed as an urachal tumor. Four months after surgery, multiple posterior urethral tumors were found; punch biopsy revealed adenocarcinoma, which was quite similar to the previous tumor. Based on the histopathological examination of the transurethrally resected tissue, the urethral tumor was diagnosed as a recurrence of the urachal carcinoma. No evidence of either tumor recurrence or metastasis was found within the 15 months following the second surgery.

Endoscopic injection of Teflon for correction of primary vesicoureteral reflux in children.

BACKGROUND: Treatment of vesicoureteral reflux by endoscopic injection of Teflon paste has recently demonstrated a good success rate. This report describes our experience in treating vesicoureteral reflux in children with particular reference to follow-up data. METHODS: Between December 1993 and November 1994, endoscopic injection of Teflon paste into the submucosa was performed on 18 children (29 ureters) to correct vesicoureteral reflux. RESULTS: After treatment, reflux was eliminated in 75.9%, decreased in 13.8%, and unchanged in 10.3% of the ureters. The success rate was lower for high grade reflux cases and ureteral orifices with abnormal shapes. CONCLUSION: Long-term follow-up for up to 2 years demonstrated that although late recurrence may occur in a small proportion of cases, the procedure is safe, simple and effective for correcting vesicoureteral reflux.

Effects of transforming growth factor-beta on the production of parathyroid hormone-related peptide in a human ovarian cancer cell line in vitro.

OBJECTIVE: We examined the effects of transforming growth factor-beta 1 (TGF-beta 1) on intracellular parathyroid hormone-related peptide (PTHrP) production in a human ovarian cancer cell line (HOC-21). METHODS: Various concentrations of TGF-beta 1 were added to a culture medium, and the numbers of cells were counted. HOC-21 cells were cultured with or without TGF-beta 1, and intracellular PTHrP was measured by a radioimmunoassay kit that recognized the carboxy-terminal protein while counting the number of cells. Furthermore, a 125I-TGF-beta 1 binding assay was carried out. Values were analyzed statistically using an analysis of variance followed by an unpaired t-test. RESULTS: Five ng/ml of TGF-beta 1 inhibited the cell growth on Day 1 after plating. The TGF-beta 1 significantly (p < 0.05) stimulated intracellular PTHrP production in a time-dependent manner, also by Day 1. A 125I-TGF-beta 1 binding study revealed that HOC-21 cells expressed a high affinity for the TGF-beta 1 receptor. CONCLUSION: The suppression of human ovarian cancer cell proliferation by TGF-beta 1 might be involved in the production of PTHrP.

Imaging of Lactobacillus brevis single cells and microcolonies without a microscope by an ultrasensitive chemiluminescent enzyme immunoassay with a photon-counting television camera.

An ultrasensitive chemiluminescent enzyme immunoassay (CLEIA) was developed for the rapid detection and quantification of Lactobacillus brevis contaminants in beer and pitching yeast (Saccharomyces cerevisiae slurry collected for reinoculation). L. brevis cells trapped on a 47-mm nucleopore membrane (0.4-micron pore size) were reacted with a peroxidase-labelled Lactobacillus group E antibody and then subjected to an enhanced CLEIA analysis with 4-iodophenol as the enhancer. The combination of a nucleopore membrane with low background characteristics that enables the antigen-antibody reaction to proceed through the pores of the membrane and a labelled antibody prepared by the maleimide hinge method with minimal nonspecific binding characteristics was essential to minimize background in the detection of single cells. An ultrahigh sensitive charge-coupled device (CCD) camera equipped with a fiber optics image intensifier permitted the imaging of single cells. A clear correlation existed between the number of luminescent spots observed and the plate count [y (CLEIA) = 0.990x (plate count) + 15.9, where n = 7, r = 0.993, and P < 0.001]. Microscopic observation confirmed that the luminescent spots were produced by single cells. This assay could be used to detect approximately 20 L. brevis cells in 633 ml of beer within 4 h. Our ultrasensitive CLEIA could also be used to detect microcolonies approximately 20 microns in diameter which had formed on a membrane after 15 to 18 h of incubation. This method, which we called the microcolony immunoluminescence (MIL) method, increased the signal-to-noise ratio dramatically. The MIL method could be used to detect a 10(0) level of L. brevis contamination in 633 ml of beer and a 1/10(8) level of L. brevis contamination in pitching yeast within 1 day (15 to 18 h to form microcolonies and 2 h for CLEIA).

A specific oligonucleotide primer for the rapid detection of Lactobacillus lindneri by polymerase chain reaction.

A polymerase chain reaction (PCR) method was developed for the rapid detection of the beer-spoilage heterofermentative lactic acid bacterium Lactobacillus lindneri. Three strains, the Chinese brewery isolate DA1, the Japanese commercial beer isolate BG2, and the Japanese brewery isolate SE3, which were serologically classified as belonging to L. lindneri, were used in this study. After sequencing the 16S rDNA of the isolates DA1 and BG2 and the typical beer-spoilage heterofermentative Lactobacillus brevis L63, these sequences were compared with published data. A L. lindneri specific PCR primer, DA-40, was then constructed based on the V1 variable region of 16S rDNA. The specificity of PCR using the L. lindneri specific primer DA-40 and the universal primer 907r was examined using five L. lidneri strains: the three isolates described above and two strains from culture collection, DSM 20690 and DSM 20692. A variety of beer-spoilage lactic acid bacteria, including 71 Lactobacillus strains and 13 Pediococcus strains, were also included in this examination. No PCR product was obtained from any DNA with the exception of the five L. lindneri strains, indicating that the L. lindneri specific primer DA-40 was highly specific. The detection limit for L. lindneri in beer was 63 CFU/100 mL of beer.

Measurement of N- and C-terminal-region fragments of parathyroid hormone-related peptide in milk from lactating women and investigation of the relationship of their concentrations to calcium in milk.

Parathyroid hormone-related peptide (PTHrP) is found in very high concentrations in the milk of various mammals. However, little is known about its physiological role in this fluid. To obtain detailed profiles of PTHrP in milk, we measured the concentrations of PTHrP in human milk by two different region-specific assays, PTHrP(1-87) (N-PTHrP) and PTHrP(109-141) (C-PTHrP). We also examined the correlations between PTHrP and Ca concentrations in milk as well as the correlations between PTHrP and secreted milk volume. The levels of N-PTHrP and C-PTHrP were relatively low after delivery and gradually increased to 13.87 +/- 2.40 nmol/l (mean +/- S.E.M.) and 56.39 +/- 11.31 nmol/l respectively on the 10th day postpartum. N-PTHrP concentration remained steady until the 6th month postpartum when weaning starts, at which point it decreased slightly. C-PTHrP levels changed in a similar way to N-PTHrP levels but were 2- to 5-fold higher. Milk Ca concentration, and content, correlated with C-PTHrP concentration, and content (r = 0.422 and r = 0.769 respectively; P < 0.0001) but not with N-PTHrP. N-PTHrP concentration in the milk samples on the 4th day postpartum correlated with the volume of milk secreted during the 24 h before the samples were taken (r = 0.524, P < 0.01), but C-PTHrP concentration did not. These results suggest that PTHrP in human milk may play some role in the maintenance of lactation through the N-terminal region and in promoting Ca transfer into milk via the C-terminal region.
