Long-term survivors in resected and nonresected small cell lung cancer.

Long-term survival (greater than or equal to 3 years) was evaluated in 164 patients with small cell lung cancer (SCLC). Thirty-seven patients underwent surgical resection, and 127 did not. All but one resected patient received combination chemotherapy. Of the 20 (12%) long-term survivors, 13 (35%) were resected, and 7 (6%) were not. Eleven of these resected patients had pathologically confirmed stage I disease. All of the 7 nonresected patients achieved complete remission by treatment, 6 of these having presented with limited disease. In addition, all patients received thoracic irradiation combined with chemotherapy. Two of the 20 patients who survived beyond 3 years developed a second malignancy 11.3 and 12 years, respectively, after initial treatment for SCLC. In conclusion, surgical resection for stage I, and probably stage II SCLC followed by chemotherapy may be an appropriate therapeutic approach. For advanced limited disease, thoracic irradiation, in addition to chemotherapy, seems to improve long-term survival.

Effects of l-menthol and dl-camphor on the penetration and hydrolysis of methyl salicylate in hairless mouse skin.

Plasters containing methyl salicylate with and without l-menthol and dl-camphor were prepared and topically applied on hairless mouse skin to investigate the effects of l-menthol and dl-camphor on the skin penetration and hydrolysis of methyl salicylate in the skin. The in vitro hydrolysis of methyl salicylate was also investigated using the skin homogenate. It was found that l-menthol with dl-camphor enhanced the skin penetration of methyl salicylate, and they inhibited both the in vivo and in vitro hydrolysis of methyl salicylate to salicylic acid.

Esterase-like activity of human serum albumin. VII. Reaction with p-nitrophenyl 4-guanidino-benzoate.

The reaction of p-nitrophenyl 4-guanidinobenzoate (NPGB) with human serum albumin (HSA) was examined kinetically at various pH's and 25 degrees C. The Michaelis constant (Ks in M) and the catalytic rate constant (k2 in s-1) were determined. The ratio of k2 to k0 (hydrolysis rate constant of NPGB in s-1) at pH 7.4 was 75.6, indicating the esterase-like activity of HSA. The effects of the reversible binding of site-specific drugs and the chemical modification by site-specific reagents on the HSA activity indicated that HSA has multiple reactive sites towards NPGB. Results of the reaction in the presence of excess NPGB over HSA also suggested the existence of multiple active sites. The pH-profile for k2 showed inflection points at about pH 6.0 and pH 10.0, suggesting the involvement of groups with pKa's of 6.0 and 10.0 in HSA.

Influence of pregnancy on the serum prolactin level following prolactinoma surgery.

We studied the influence of pregnancy on the serum prolactin levels after surgery in 35 patients with prolactinomas. Eighteen out of 22 patients with postoperative normalization of the prolactin level were able to achieve repeated spontaneous pregnancies and endocrine cure by surgery alone, but the other 4 had recurrent hyperprolactinemia. Thirteen patients with persistent hyperprolactinemia after surgery achieved one pregnancy with or without bromocriptine treatment, but did not achieve a second one owing to an increase in the prolactin level after delivery. None of the patients with postoperative hyperprolactinemia showed spontaneous normalization of the prolactin level or restoration of menstruation after delivery. In these 13 patients, the mean increase from the postoperative to the post-delivery prolactin level was less for the patients with bromocriptine-induced pregnancy than for those with spontaneous pregnancy. It is concluded that pregnancy did not adversely affect the serum prolactin level in the patients with a normal postoperative level.

Acceleration of H+ extrusion via Na(+)-H+ exchange in guinea-pig ventricular papillary muscle under intracellular acidic condition.

We investigated mechanisms by which intracellular pH was regulated under intracellular acidic condition in resting guinea-pig ventricular papillary muscles in vitro. Intracellular sodium ion activity (aiNa), intracellular and surface pH (pHi and pHs) were measured with Na(+)- and H(+)-selective microelectrodes and resting tension was measured. By exposure to 0 mM K solution aiNa and resting tension increased progressively while pHi decreased but reached the steady level of pH 6.95. pHs which was lower than external bulk pH (pHo) decreased progressively by exposure to 0 mM K solution. In 4 mM K solution, amiloride (1 mM), an inhibitor of Na(+)-H+ exchange, induced a reversible decrease in both aiNa and pHi, and an increase in pHs. Changes in pHi and pHs induced by application of amiloride in 0 mM K solution were larger than those in 4 mM K solution. The rate of decrease in pHi induced by amiloride became larger at longer exposure to 0 mM K solution. Lowering pHo from 7.4 to 6.4 induced a larger decrease in pHi in 0 mM K solution than that in 4 mM K solution. Lowering pHo from 7.4 to 5.4 reversed the difference between pHs and pHo. These results suggest that in guinea-pig papillary muscle, Na(+)-H+ exchange is active to regulate intracellular H+ under resting condition and under intracellular acidic condition, H+ extrusion via the Na(+)-H+ exchange would be accelerated not only by the net thermodynamic driving force for Na+ and H+ but also by other factors.

Production and localization of enzymes on soft gel cultivation.

Production and localization of glutaminase and leucine aminopeptidase (LAP) in soft gel cultivation were compared with those in koji and liquid cultivations. The enzymes were detected only in the whole-mycelial-mat fraction by soft gel cultivation, but in both intracellular and extracellular fractions by the other two methods. The enzyme species of glutaminase and LAP in soft gel cultivation were analyzed by ion exchange and gel filtration column chromatographies. Three species of glutaminase and four (or five) species of LAP were formed in the whole-mycelial-mat fraction. The intracellular and extracellular fractions of the koji and liquid cultivations contained different species of enzymes.

[Surgical indication for T1N2 disease: comparison of survival among resected T1N2, T2 N2 and non-resected T1 (M0) disease].

Based on the outcome of 28 patients with pathologically confirmed T1N2 disease who underwent surgical resection, we evaluated surgical indication for this stage. Twenty three patients underwent complete resection and 5 patients incomplete resection. The histologic type of tumor was adenocarcinoma in 20 patients and squamous cell carcinoma in 8 patients. The 5-year survival rate was 22% for all resected patients and 28% for completely resected patients. These survival rates were better than those (16% and 23%, respectively) in patients with T2N2 disease. However, there was no significant difference of survival among incompletely resected T1 N2, and non-resected T1 (M0) patients. Although 9 of 28 T1N2 patients survival more than 3 years, all but one patient underwent complete resection of tumor and had clinical N0 or N1 before surgery. In conclusion, surgery should be indicated only for patients with completely resectable tumor, even in T1N2 disease.

Prenatal diagnosis of Duchenne muscular dystrophy by restriction fragment length polymorphism analysis with pERT 87 intragenomic deoxyribonucleic acid probes.

Prenatal diagnosis of DMD was performed with three intragenic genomic probes and chorionic villus sampling. A total of 8 unrelated families with at least one DMD were analysed. DNA was extracted from peripheral white blood cells for carrier testings (50 individuals). For prenatal detection, it was extracted from chorionic villi obtained by chorionic villus sampling at 9 menstrual weeks. DNA was digested with an appropriate restriction enzyme followed by overnight electrophoresis in 1% agarose gels. DNA was transferred from the gel to nylon membrane according to the protocol of an alkaline transfer method. The pERT 87 probes were labeled by nick translation. The membranes were hybridized overnight after prehybridization. After washing, the membranes were exposed to X-ray films to make autoradiograms for restriction fragment length polymorphism analysis. Fetal sex was determined by a rapid screening test with a Y chromosome-specific repeat sequence. Out of 8 fetuses, 4 were males and 4 were females. All of 4 male fetuses were determined to be unaffected. Out of 4 female fetuses, 3 were diagnosed as non-carriers, and the carrier status of the remaining one was not able to be determined because her mother was not informative for all testings.

[A case of pituitary adenoma with simultaneous secretion of TSH and GH detected by double immunostaining method].

A rare case of simultaneous hypersecretion of thyroid stimulating hormone (TSH) and growth hormone (GH) in a pituitary adenoma is reported. A 59-year-old male complaining of general fatigue, dyspnea on exertion and finger tremor was admitted. Examination on admission, he revealed with hyperthyroidism and hypersecretion of TSH and thyroid hormones. Administration of TRH did not further increase serum TSH level, and administration of T3 also had no effect on TSH secretion. CT scan showed a pituitary macroadenoma 13mm in diameter. MRI demonstrated a homogenously hypointense mass with Gd-DTPA enhancement in the left side of the sella turcica. The entire chromophobic adenoma was removed by trans-sphenoidal surgery. Immunostaining of the specimen showed that the cytoplasm of the adenoma cells was positive for both TSH and GH. Double immunostaining using avidin-biotin-peroxidase complex (ABC) method and immunogold silver staining (IGSS) method, showed that the adenoma cells had been secreting both GH and TSH at the same time. After the adenomectomy, the hyperthyroidism disappeared, and all altered indicators of pituitary function returned to normal.

[Successful repair of subvalvular annular aneurysm of the left ventricle].

A 62-year-old woman was admitted to our hospital due to congestive heart failure. On the chest X-ray film, cardiomegaly and prominent pulmonary congestion were revealed. She became well with bed rest and medication of digitalis and diuretics. Echocardiogram established the diagnosis of severe mitral regurgitation. In addition, cineangiography of the left ventricule showed cystic left ventricular aneurysm which originated from below the posterior mitral annulus. Operation was performed on July 16, 1990. Under cardiopulmonary bypass, we reflected the apex of the LV upwards, but failed to identify the aneurysm. Then, left atrium was opened through Dubost incision. As regards mitral valve structures, there were no abnormal findings except moderate enlargement of it's orifice. After resection of the mitral valve, orifice of the aneurysm (0.5 X 2.0 cm) was recognized, which was closed with the buttressed sutures. Afterwards, MVR was performed with 25 mm Medtronic Hall valve. The patient had a smooth postoperative course without complication.

[Long-term surgical results of microprolactinomas].

Sixty three patients with microprolactinoma were studied by long-term follow-up after surgical cure. Postoperative normalization of prolactin (PRL) was seen in 38 (60%) of the total 63 cases, and hyperprolactinemia recurred in 5 (13%) during the follow-up period. Spontaneous restoration of menstruation occurred in 45 (71%), and, in 41 (65%), regular menstrual cycle as continuing at the time of the last follow-up. 34 (72%) of 47 patients who wanted children became pregnant, and 31 of these had a subsequent successful delivery. Clinical cure by surgical treatment alone, followed by spontaneous restoration of menstruation and continuation of regular cycle during the follow-up period after pregnancy and delivery was seen in 41 (65%) of the total cases. From these results, it was concluded that transsphenoidal surgery should be the treatment of first choice for microprolactinoma when complete cure is required.

Spontaneous rupture of hemangioma of the liver: treatment with transcatheter hepatic arterial embolization.

A case of spontaneous rupture of a cavernous hemangioma of the liver with a successful hepatic resection after transcatheter hepatic arterial embolization is reported. Twenty-eight cases of spontaneous rupture of hepatic hemangioma have been reported in the medical literature. Each of the cases (19 adults and nine children) were reviewed, including the present one reported here. Surgical treatments were carried out on 20 patients, of whom only five survived. Ruptured hemangiomas ranged in size from 3.0 to 25.0 cm, and many were located on the inferior surface of the liver. Surgical resection should be considered for a large hepatic hemangioma located on the inferior surface of the liver if the tumor has a high probability of rupture. Once the hemangioma has ruptured, emergent hepatic resection is recommended for low-risk patients, whereas high-risk patients should receive transcatheter hepatic arterial embolization prior to elective hepatic resection.

[Pituitary apoplexy with localized hematoma in the suprasellar region].

The authors report a case of a patient with a pituitary tumor, in which pituitary apoplexy occurred only in the suprasellar part of the tumor. A 26-year-old woman suffered from abrupt worsening of vision and headache. A CT scan showed a dumb-bell-shaped tumor extending to the suprasellar region, in which an irregularly shaped low density area suggesting a liquidized hematoma was seen. The rapid worsening of her symptoms was highly indicative of pituitary apoplexy. The first operation was performed through the transsphenoidal route. There was no evidence of intratumoral hematoma in the intrasellar tumor, which could be removed successfully. However, the suprasellar mass could not be reached because of the hardness of the diaphragma sellae and the presence of a normal pituitary gland. MRI and CT cisternography after this first operation showed a narrow opening of the diaphragma sellae. The second operation using right frontotemporal craniotomy disclosed a suprasellar mass, which consisted of an old hematoma and a necrotic tumor. The tumor was sub-totally removed. The patient's visual acuity improved after the second operation. Although transsphenoidal surgery is the treatment of choice in patients with pituitary apoplexy, the selection of the surgical route should be made only after careful neuroradiological evaluation with regard to the extrasellar extension of the tumor.

Endoscopic screening of early esophageal cancer with the Lugol dye method in patients with head and neck cancers.

The poor prognosis for esophageal cancer could be improved if lesions were detected at an early stage. To detect early esophageal cancer, endoscopic screening of the esophagus with the Lugol dye method was performed in patients with head and neck cancers who were asymptomatic but regarded as being at high risk for synchronous or metachronous esophageal cancer. Of 178 patients screened, 9 had esophageal cancer (5.1%). Eight of these patients (89%) were at early stages with no lymph node metastasis. Most of the lesions (9 of 13 lesions) were not detectable by barium studies or ordinary endoscopic study. The epidemiologic statistical analysis of the patients confirmed that they had a significantly high observed and expected number (O/E) ratio (39.7; P less than 0.001). These results demonstrate the value of endoscopic screening of the esophagus with the Lugol dye method in patients with head and neck cancers and imply that endoscopic screening with the Lugol dye method may be useful for detecting early esophageal cancer in individuals at risk for other causes.

Ca2(+)-calmodulin-dependent protein kinase II phosphorylates various types of non-epithelial intermediate filament proteins.

We have investigated the actions of Ca2(+)-calmodulin (CaM)-dependent protein kinase II on various types of non-epithelial intermediate filament proteins, vimentin, desmin, glial fibrillary acidic protein (GFAP) and neurofilament triplet proteins. Most of these filament proteins could serve as substrates. The effects of phosphorylation on the filamentous structure of vimentin were investigated in sedimentation experiments and by using electron microscopy. The amount of unassembled vimentin increased linearly with increased phosphorylation. However, the extent of the effect of phosphorylation on the potential to polymerize was also affected by the MgCl2 concentration, under conditions for reassembly. The actions of Ca2(+)-CaM-dependent protein kinase II on non-epithelial intermediate filaments under physiological conditions are given attention.
