[Application of monoclonal antibody for drug delivery system--missile therapy for cancer].

Clinical applications of monoclonal antibody for selective delivery of anticancer drug, toxin and radioisotope were reviewed. The difficulties in preparation and clinical application of chemoimmunoconjugate were pointed out. Also the difficulties of immunotoxin in clinical trial due to its high toxicity had been discussed and it is assumed that development of blocked ricin may overcome many problems related to toxicities. Clinically most advanced immunoconjugate seems to be radioimmunoconjugate. Among three types of immunoconjugates preparation of radioimmunoconjugate is easiest and some promising clinical trials were introduced in this review.

[Detection of apolipoproteins E5 and E7 by a widely used commercial ApoE IFE kit--evidence provided by genotyping].

The purpose of this study is to examine a possibility to detect apolipoprotein (apo) E5 and E7 isoproteins with a commercial kit widely used (Phenotyping Apo E IFE System, Jokoh Co. LTD., Tokyo) and to clarify the frequency of the rare alleles in Central Japan. A total of 1,445 subjects living in Central Japan (1,030 hemodialysis patients and 415 apparently healthy individuals) was phenotyped for apo E isoproteins with the kit using isoelectric focusing with immunoblotting. In 23 subjects, unique apo E4 isoproteins, which migrate more basically than apo E4 isoprotein, were found. These unique isoproteins were proved to be apo E5 or E7 isoproteins by PCR with restriction fragment length polymorphism and with amplification refractory mutation systems, respectively. No significant differences in the frequencies of apo E5 or apo E7 alleles were observed between healthy individuals and hemodialysis patients (0.011 vs. 0.018, NS). The frequencies of apo E5 and E7 alleles were 0.001 and 0.005, respectively, in healthy individuals, and 0.003 and 0.005, respectively, in hemodialysis patients. These data in Central Japan were consistent with those in Western Japan (Matsunaga, et al, Clin Genet, 1995). These results indicate that the commercial kit is applicable to detect the rare alleles, apo E5 and E7 alleles and that the frequencies of the rare alleles in Central Japan are similar to those in Western Japan.

[Experimental study on efficacy of myocardial opacification owing to difference of echocardiographic contrast agents].

UNLABELLED: We studied on opacified duration and concentration of the opacification of the contrast agents. METHOD: The original contrast mediums are Albunex, 5% human albumin and Hexabrix. The used contrast agents were made by sonication of the original medium. Two dimensional echocardiograms were recorded in short axis at papillary muscle level. The evaluation of the opacified duration and the concentration of the opacification were practiced by visual observation and image analyzer. RESULTS AND CONCLUSION: 1) There were no difference among Albunex, sonicated 5% human albumin and sonicated Hexabrix in the myocardial opacification in the visual observation. 2) The opacified duration lasted longest in sonicated 5% human albumin.

[Clinical significance of serum neuron-specific enolase in non-small cell lung cancer].

The present study was a retrospective analysis of serum neuron-specific enolase (NSE) assayed in 302 patients with non-small cell lung cancer (NSCLC) before treatment. Serum NSE ranged from 0.6 to 183.0 ng/ml, with a mean value of 12.2 ng/ml, and exceeded 10 ng/ml in 145 patients (48.0%). The mean levels and rate of NSE positivity in 177 patients whose tumors were categorized as small, medium and large (from the product of the maximum and perpendicular axis of the tumor on chest X-ray), were highest in the large group. There was no significant relationship between serum NSE and the response rate to anticancer chemotherapy. The survival period of the patients with elevated serum NSE (median, 5.7 months) was significantly shorter, than those with normal serum NSE levels (median, 12.1 months). Serum NSE is of prognostic significance in a multivariate analysis (p < 0.01), but not predictive of response to chemotherapy in NSCLC.

[Panhypopituitarism following basilar artery dissection with extensive brainstem and cerebellar infarction: a case report].

We experienced a patient with panhypopituitarism which occurred following extensive brainstem and cerebellar infarction due to dissection of basilar artery (BAD). Panhypopituitarism followed by BAD has not yet been reported in the literature. The patient was a 67-years-old man who was admitted to our hospital because of a sudden onset of consciousness disturbance and vomiting. On the day of admission, he was drowsy and had left IIIrd nerve palsy, skew deviation, occular bobbing, and left hemiparesis. His neurological state was rapidly deteriorated, and fell almost into apneic state, requiring artificial ventilation for a week. Cerebral angiography demonstrated a double lumen sign of the basilar artery confirming the diagnosis of dissection. There were extensive hypodensities in the brainstem and bilateral cerebellar hemispheres on CT. On day 20, he suddenly became shocked following infection. Even after the effective and successful treatment of infection, severe hypotension continued that required administration of chatecholamine agents. Laboratory examinations revealed that he had panhypopituitarism. Supplement therapy with adrenocortical hormones made his circulatory state improved, and could finally be stopped on day 100. In the present case, subclinical hypopituitarism was considered to occur mainly from compression of the pituitary stalk and hypothalamus by the enlarged BAD. Increased intracranial pressure and upward herniation may also have made the pituitary function worse, and the infection finally triggered the adrenal crisis. The BAD occurs more frequently than previously considered, and should be kept in mind as a cause of secondary panhypopituitarism.

[Aortic and mitral valve aneurysms complicated with infective endocarditis: a case report].

A rare case of aortic and mitral valve aneurysms complicated with infective endocarditis was accurately diagnosed by transesophageal echocardiography. A 57-year-old man with severe aortic regurgitation due to infective endocarditis was admitted to our hospital. Transthoracic echocardiography showed an aortic valve aneurysm on the right coronary cusp and perforations on the other cusps. Transesophageal echocardiography demonstrated a small aneurysm on the anterior leaflet of the mitral valve which was not clearly visualized by transthoracic echocardiography. Color Doppler echocardiography revealed severe aortic regurgitation and mild mitral regurgitation without perforation of the mitral valve aneurysm. Aortic valve replacement and mitral valvuloplasty of the anterior mitral leaflet were performed. The right coronary cusp of the aortic valve showed marked thinning with infiltration of inflammatory cells. The postoperative clinical course was uneventful.

Fluorine-18-fluorodeoxyglucose and carbon-11-methionine evaluation of lymphadenopathy in sarcoidosis.

UNLABELLED: Uptake of 18F-fluorodeoxyglucose (FDG) and 11C-methionine (Met) in mediastinum and hilar lymph nodes was studied using PET in 31 patients with sarcoidosis. The aim of our study was to examine whether these different tracers play a differential role in clinical assessment of pulmonary involvement. METHODS: Fluorine-18-fluorodeoxyglucose and 11C-Met PET were administered on different days. The differential absorption ratio of these tracers was calculated for the region of interest with the highest level of activity. Clinical reassessment of sarcoidosis was made at least 1 yr after the first PET examination. In seven patients whose lymph nodes still remained visible by other imagings at the time of reevaluation, the same PET study was performed again. RESULTS: Both FDG and Met were accumulated in the lymph nodes in all but one patient. The FDG and Met uptake ratios in all patients were not correlated, but they could be divided into the FDG-dominant group (FDG/Met uptake ratio > or = 2) and the Met-dominant group (FDG/Met uptake ratio < 2). Within each group, the FDG and Met uptake values were correlated. The rate of improvement assessed by clinical status and chest radiographs was considerably higher in the FDG- (78%) than in the Met-dominant group (33%). In the seven patients of the repeated PET examination, their FDG/Met uptake ratios were generally unchanged after 1 yr. CONCLUSION: The results suggest that the FDG/Met uptake ratio using PET may reflect the differential granulomatous status in sarcoidosis and be a useful tool for pretreatment evaluation.

[Right-to-left shunt and atrial septal aneurysm in stroke patients: a contrast transesophageal echocardiographic study].

BACKGROUND: Transesophageal echocardiography (TEE) has been used to detect cardiac abnormalities including right-to-left shunt (RLS) and atrial septal aneurysm (ASA). The aim of this study was to elucidate frequency of RLS and ASA detected by contrast-TEE, and to evaluate the role of these abnormalities in stroke patients. METHODS AND RESULTS: We investigated prevalence of RLS and ASA in 504 patients suspected of stroke by using TEE. (mean age 62 +/- 12 y.o., stroke 491, non-stroke 13). The RLS was detected in 64 patients (12.7%), in whom 49 patients (9.7%) had the patent foramen ovale (PFO). The ASA was demonstrated in 10 patients (2.0%), and accompanied well with the RLS (six of the 10, 60%). The prevalent ratios of the RLS (33.8%), the PFO (26.8%), and the ASA (11.3%) in 71 patients with embolic stroke (brain embolism or TIA) of unknown cause were significantly higher than those in the other 433 patients (9.2%, 6.9%, 0.5%, respectively, p < 0.0001). Multiple logistic regression analysis showed that both the PFO and the ASA were independent risk factors (odds ratio: 3.8: p = 0.0002 and 16.6: p = 0.0008, respectively) for embolic stroke of unknown cause. CONCLUSION: It seems that the RLS, PFO and the ASA play a roll in developing embolic stroke of unknown cause.

[Disappearance of embolic signals on transcranial Doppler sonography following antiplatelet therapy in a patient with transient ischemic attacks].

Transcranial Doppler sonography (TCD) has been increasingly used to detect embolic signals. A 49-year-old woman had repeated episodes of weakness and numbness of the right limbs. She was diagnosed as having transient ischemic attacks (TIAs). A cranial Gadolinium-enhanced MRI revealed multiple small lesions in the cortical-subcortical areas of the left middle cerebral artery territory. A TCD examination on admission detected 4 embolic signals on the left MCA trunk in a 30-minute examination period. TIAs completely ceased after the initiation of an antiplatelet therapy with aspirin. Embolic signals were not detected in later TCD studies. The embolic signals on the TCD strongly indicated that TIAs are caused by an embolic mechanism. TCD is potentially useful for assessing the efficacy of antiplatelet and anticoagulant therapies in patients with TIAs.

[Outcomes of 50 leukemia patients who received bone marrow transplants from unrelated donors].

Fifty leukemia patients were given bone marrow transplants (BMTs) from unrelated donors at Meitetsu Hospital. We studied the outcomes of their transplants from two perspectives: leukemia disease stage and acute graft versus host disease (GVHD). The probability of disease-free survival for standard-risk, high-risk, and super-high risk patients was 65%, 29%, and 8%, respectively. The main causes of death were septicemia, cardiac and renal failure, and relapse of leukemia in the high- and super-high risk patients, and grade III-IV acute GVHD in the standard-risk patients. The incidence of grade II-IV and grade III-IV acute GVHD was 32% and 17%, respectively. All 7 patients in whom grade III-IV severe acute GVHD developed died. We conclude that better control of acute GVHD and treatment of early stage complications are clearly important to improving the outcome of BMTs from unrelated donors, especially for high and super-high risk patients.

[Postpartum depression and social support].

OBJECTIVES: To ascertain the state of a mother's depression three months after childbirth and to what sort of or to whose social support it is related. METHOD: In October 1993, a questionnaire survey was conducted on the attributes, state of depression by Zung Self-rating Depression Scale, and social support of 300 mothers who received health examinations of their 3 to 4 months' old infants at five health centers in Tokyo. Relationship between depression and social support of 256 mothers (rate of valid answer 85.3%) was examined by one-way analysis of variance, Pearson's product moment correlation coefficient and multiple regression analysis. RESULTS: The depression score averaged 37.3 points, with 73 persons (28.5%) scoring 40 to 47 points (light) and 27 persons (10.5%) scoring 48 or more points (medium level or higher). The following were the variables which individually showed a significant relationship to the depression score after controlling for age, education, number of children, type of family, and whether or not the mother was working: The emotional support score from the husband and his parents, such as the frequency in which the husband "listened to the mother's worries and anxieties" "was attentive or considerate to the mother" and "helped in feeding the child", the frequency in which the husband's parents "could be consulted on worries the mother had about childbirth, child care and child development" and "nursed and played with the child". It was found that the better the state of such support, the less the state of depression. On the other hand, support from the mother's parents, neighbors, and friends had no bearing on depression. CONCLUSION: The level of depression of the surveyed group was the same as that of the general female public. Postpartum depression was related to emotional support from the husband and emotional and practical support from the husband's parents. Therefore, from the aspect of preventing depression, we believe it is important that, firstly, the mother and family should understand the importance of support and improve the support by the family, and, secondly, the mother herself should improve her ability to cope.

[A case of glioblastoma manifesting 49 years after lobotomy].

We report a case of glioblastoma manifesting 49 years after a lobotomy. He was diagnosed as having schizophrenia at age 20 and was operated on with a standard lobotomy when he was 27 years old. He had led a useful life after 40 years old without medication. Because of hallucination and delusion, he was hospitalized at the end of 1996. CT showed a well enhanced tumor adjacent to the cavity made by the lobotomy in the left frontal lobe. This is the second case report of glioblastoma just beside the cavity formed by lobotomy. The relationship between glioblastoma and old lobotomy is discussed, especially in regard to morphology and CT findings.

Corneal marginal ulcer in relapsing polychondritis: treatment with keratoepithelioplasty.

This article describes a case of corneal marginal ulcer caused by relapsing polychondritis (RPC) that was treated with keratoepithelioplasty. The patient underwent keratoepithelioplasty with lamellar keratoplasty in the left eye. No relapse occurred in the nasal side in which corneal lenticules were grafted, whereas vascular invasion developed in the temporal side in which no lenticules were grafted. Keratoepithelioplasty was an effective surgical procedure to prevent a recurrence of corneal marginal ulcer in a patient with RPC.

[Angiographic diagnosis and management of bleeding from the digestive organs].

Gastrointestinal bleeding is recently seen less often by the angiographer. This is mainly due to advances in endoscopy, and nuclear medicine. When patients with gastrointestinal bleeding are referred, endoscopic diagnosis and therapy should be performed at first. However, when it is impossible to diagnose or to control the bleeding, angiography must be considered as soon as possible. Intra-abdominal bleeding should be diagnosed by angiography at first. In both cases, embolization is generally safe and effective depending on the advance of occlusive agents.

[Detection of coronary artery disease by adenosine triphosphate stress echocardiography: comparison with adenosine triphosphate stress thallium myocardial scintigraphy and coronary angiography].

The clinical feasibility and usefulness of adenosine triphosphate-2Na (ATP) stress echocardiography for the detection of coronary artery disease (CAD) were assessed. Two-dimensional echocardiography and thallium-201 single photon emission computed tomography (SPECT) during ATP infusion were performed simultaneously in 58 consecutive patients (41 men and 17 women; mean age 66 +/- 12 years) with suspected CAD. ATP was infused intravenously at 0.16 mg/kg/min for 5 min and thallium was injected at 4 min. All patients underwent coronary angiography within 2 weeks of ATP echocardiography and ATP SPECT. An ischemic response during ATP infusion was detected by echocardiography as the development or worsening of a wall motion abnormality compared with the baseline and by SPECT as a perfusion defect that filled totally or partially during redistribution. Significant coronary artery stenosis was defined as > or = 75% diameter stenosis in a major epicardial vessel. The severity of the stenosis was classified as follows: Group A, lesions with significant coronary artery stenosis (> or = 75%, < 90%); Group B, lesions with severe coronary artery stenosis (> or = 90%) without collateral circulation; Group C, lesions with severe coronary artery stenosis (> or = 90%) with collateral circulation. Significant CAD was present in 43 of 58 patients. The overall sensitivity, specificity and accuracy of ATP echocardiography for detecting significant CAD were 70%, 100% and 78%, respectively, and those of ATP SPECT were 98%, 87% and 95%, respectively. In patients without previous myocardial infarction, the sensitivity of ATP echocardiography was 67%. The sensitivity of ATP echocardiography and ATP SPECT for detecting myocardial ischemia were 59% and 95% in patients with 1-vessel disease, 75% and 100% in those with 2-vessel disease, and 88% and 100% in those with 3-vessel disease, respectively. The induction of wall motion abnormality by ATP echocardiography was highly concordant with ATP SPECT imaging in patients with multivessel disease. Although the sensitivity of ATP echocardiography improved in patients with multivessel disease more than in those with single-vessel disease, detection of all diseased vessels was achieved in only 10% of patients with multivessel disease. The sensitivity of ATP echocardiography and ATP SPECT for detecting myocardial ischemia in individual vessels were: right coronary artery, 58% and 74%; left anterior descending artery, 59% and 97%; left circumflex artery, 27% and 68%. ATP-induced transient perfusion defects were associated with transient wall motion abnormality in only 57% of segments. The sensitivity of ATP echocardiography and ATP SPECT for detecting myocardial ischemia in patients with severe coronary stenosis were: Group A, 32% and 66%; Group B, 60% and 93%; Group C, 80% and 95%. The sensitivity of ATP echocardiography was significantly higher in the lesions with collateral circulation than in those without collateral circulation. ATP stress echocardiography is useful for detecting myocardial ischemia in patients with multivessel disease and in patients with severe coronary artery stenosis (> or = 90%). In particular, transient wall motion abnormality tends to be detected in the segments perfused by collateral circulation.

[The prevention and treatment for restenosis in coronary angioplasty].

Since the invention of Gruentzig AR in 1977, percutaneous transluminal coronary angioplasty (PTCA) has become a widely accepted therapeutic modality for the treatment of patients with ischemic heart disease (IHD). Restenosis which occurs within six months is a major problem of PTCA. Several clinical trials of pharmacological regimen have failed to show significant improvements in restenosis. Palmaz-Schatz stent could improve in restenosis. STRESS and BENESTENT DCA also could improve in restenosis when aggressive debulking was succeeded (BOAT and OARS). The recent IVUS studies have demonstrated that vessel remodeling (vessel shrinking) was the major factor of restenosis. Stent contributes to the prevention of the vessel shrinking, which leads to the improvement in restenosis. However, the rapid increase in stent implantation rates with widening indications cancels the beneficial effect of stent.
