Inhibition of immune complex-mediated activation of complement. Effects of agents modulating activation of, and the activated C1 complex.

Several known chemical compounds were shown to selectively inhibit the interaction between immune aggregates and C1q, the activation of C1r-C1s complex by immune aggregate-bound C1q, and the esterolytic activity of the activated C1s, C1s. These reactions are relevant to the functions of the first complement component, C1, and its activation induced by immune complexes. The effects of these inhibitors on tissue injury mediated by immune complex-induced complement activation, such as immune hemolysis, passive cutaneous anaphylaxis, and experimental glomerulonephritis were examined. The results suggest an approximate correlation between the activity shown on the molecular level and that obtained in vivo. One such compound, suramin, was shown to be an effective inhibitor of PCA and the proteinuria manifestation of EGN while not affecting antibody fixation to tissue or histamine-mediated skin reaction. These results suggest that effective suppression of the initial steps of complement activation may be of value of controlling immune complex-mediated tissue injuries in disease.

Regeneration of human liver after hepatic lobectomy studied by repeated liver scanning and repeated needle biopsy.

Regeneration of the residual lobe of the liver after hepatic lobectomy in humans was studied by repeated liver scanning in seven noncirrhotic and three cirrhotic patients. Each patient was studied for several months during the study which lasted from 1-12 years. Regeneration was apparent in noncirrhotic liver remnants following hepatic lobectomy. In the case of a long standing, space occupying lesions such as benign giant cysts, the liver remnant would complete its regeneration process rather early, usually within a few months of hepatic lobectomy. In hepatoma cases, however, regeneration of the residual lobe after hepatic resection usually took five or six months for completion. On the contrary, no definite increase in the size of the liver remnant was seen on repeated liver scanning in cirrhotic patients. Histologic study of the residual lobe was repeated on needle biopsy specimens in two noncirrhotic and four cirrhotic patients. Regenerative hyperplasia of liver cells with large hyperchromatic, or double nuclei never seen in the preresection liver appeared in the liver remnant five, 11, and 27 days after hepatic lobectomy in noncirrhotic patients. In cirrhotics, however, there were no histologic changes between the preresection liver and the postresection remnant studied three, five, 15, 40 days or even two years and 8 months after hepatic lobectomy.

Purification of soluble hepatocellular carcinoma extracts.

When soluble hepatocellular carcinoma (HCC) extract prepared by 3 M KCl solution was passed through a column of Sephadex G-200, four fractions (designated as I, II, III, IV) could be obtained from the elution profile. By leukocyte migration inhibition assay, the active component of soluble HCC extract which represented tumor-associated antigen was shown to be contained in fractions I and II.

Lack of leukocyte migration inhibition by hepatitis B surface antigen in hepatocellular carcinoma patients.

Soluble part of hepatocellular carcinoma (HCC) tissue extracts with or without hepatitis B surface antigen (HBsAg) was tested against leukocytes of 13 histologically confirmed HCC patients. Inhibition of leukocyte migration was observed in 9 out of 13 cases when tested by soluble HCC extract containing HBsAg, while inhibition of lukocyte migration was observed in 8 out of 13 cases when tested by solublp greater than 0.05, by Fisher's exact test). In the meantime, soluble HCC extract with or without HBsAg did not significantly cause inhibition of leukocyte migration in 12 non-HCC patients. Therefore, it is concluded that inhibition of leukocyte migration in HCC patients is caused by the tumor-associated antigen, not caused by HBsAg.

Mononuclear phagocytes from carrageenan-induce granulomas. Isolation, cultivation, and characterization.

Stable cultures of mononuclear phagocytes from carrageenan-induced granulomas in mice have been established after enzymatic dispersion of these lesions. The cells can be maintained for up to 3 wk without division in serum-free media. The mononuclear phagocytes were identified by several criteria. The cells are adherent, phagocytic, contain lysosomal acid hydrolases at high specific activities, secrete lysozyme, and bind soluble aggregates of IgG. The activities of 5'-nucleotidase and leucine aminopeptidase in the cultured granuloma cells showed that they resembled macrophages from thioglycollate-stimulated mice but not unstimulated macrophages in these respects. Supernates from the cultured granuloma cells contain factor(s) which induce the proliferation of thymocytes; the release of such factors by the cells is stimulated by lipopolysaccharide.

Ethnicity and patterns of help-seeking.

This study reconstructs and analyzes pathways of help-seeking traveled by severely disabled mental patients in the community. Ethnicity is a prime factor in differentiating patterns of help-seeking in that Chinese patients are kept for prolonged periods of time within their families in the beginning of pathways, while Anglo-Saxons and Middle Europeans are referred by their families or themselves to multiple social and mental health agencies. Native Indians are referred by persons other than family members or themselves between social and legal agencies in the community. Eventually all of these pathways lead to psychiatric inpatient intervention. There are major impediments to treatment in these patterns, most notably isolation and deterioration of patients within confines of families or lack of coordination of agencies in the community. Recommendations aimed at the improvement of treatment in the community are presented in light of these findings.

Service delivery issues in Asian-North American communities.

The authors describe some of the sociocultural factors (e.g., moralistic, religious, psychological, and familial characteristics) that influence the help-seeking behavior of Chinese psychiatric patients in North America. They propose a replicable approach for investigating sociocultural patterns relevant to service delivery to Chinese in other communities.

Hepatic tumors, surgical treatment and its results.

During the period from March 1954 to June 1976, 489 cases of hepatic tumors were experienced at the Department of Surgery, National Taiwan University Hospital. The pathological classification are: Hemangioma 5; Cystic hamartoma 2; Hemangioendothelioma 2; Non-parasitic localized cyst 5; Polycystic liver 9; Tuberculoma 1; Actinomycosis 1; Hyperplasia 1; Adenoma 1; Primary liver sarcoma 2; Primary liver cancer 442; and Metastatic liver cancer 18. Of the 489 hepatic tumors, two cases were treated with excision, 148 cases with hepatic lobectomy and nine cases of polycystic liver were treated with fenestration. Benign hepatic tumors are all well up to eight years after the operation, 14 cases of primary liver cancer survived five to 17 years after hepatic lobectomy.

Quantitation of immune complexes by competitive inhibition of binding of Clq to insoluble IgG aggregates.

An assay system for the binding of Clq to insoluble IgG aggregates was found useful for the quantitation of immune complexes in biological fluids. The assay, both easily and rapidly performed, is based on the competition of Clq binding substances with IgG aggregates. Serum from rheumatoid arthritis patients showed an increase in Clq binding substances over normal serum and this increase could be abolished by pretreatment of the serum with D-penicillamine.

The doctor's wife: mental illness and marital pattern.

It is a clinical impression that physicians' wives present in disproportionately large numbers as psychiatric patients; that in the vast majority severe marital problems are present; and that the marital relationships show a similar pattern. The purpose of this paper is to ascertain whether the latter two impressions have any basis in fact. Accordingly, a random sample of twenty physicians' wives who had been in-patients in the University of British Columbia Health Sciences Centre Hospital during the period March 1, 1969 to May 31, 1973, and whose husbands had been interviewed, was selected. The records were reviewed to obtain personal data, pertinent psychiatric history, diagnosis, the personality of husband and wife and information on the marital relationship. These conclusions emerged: 1. Ninety per cent of the patients had a primary diagnosis of depressive neurosis. 2. Ninety-five per cent of the patients had a secondary diagnosis of personality disorder, hysterical personality and passive-aggressive personality in order of frequency. 3. In 90 per cent of the patients there was a history of suicidal preoccupation or attempt. 4. In 55 per cent of the patients there was a history of significant drug and/or alcohol abuse. 5. The patients were more frequently ward management problems. 6. A common marital pattern was noted: a dependent, histrionic wife and an emotionally detached husband. 7. The cases were characterized by their complexity, severity, long duration and difficulties involving the patient and spouse in an appropriate treatment plan. The authors discuss the implications of this study, particularly its significance in provoking examination of what measures might be utilized for early detection and intervention with those physicians and physician-marriages at risk.
