Radioimmunoassay screening for etorphine in racing horses.

A commercially available radioimmunoassay kit was used to screen for the presence of etorphine in post-race urines from horses racing in Kentucky. Most horse urines contained small amounts of materials which reacted positively in this immunoassay. These materials are apparently endogenous to the horse and were called apparent etorphine equivalents. The levels of these apparent etorphine equivalents in post-race urines from 70 horses were estimated. Their modal level averaged 0.1 ng/ml, the population distribution was log normal, and individual horses showed levels of up to 0.8 ng/ml.

Computer-based prescription audit as a research, educational and management tool.

An existing dedicated pharmacy computer system (MIDAS system, Alfred Hospital and Health Computing Services, Victoria) used primarily for pharmacy labelling and dispensing, has been rewritten for an in-house minicomputer, with enhancements that allow it to be used in budgetary predictions, drug use monitoring, therapeutic audit and as a research tool. An automatically updated hard copy record of individual patient prescription profiles is generated and represents a valuable addition to the patient medical record. Allied to automatic "sorting" and retrieval techniques, all patients on nominated drugs can be identified and their records examined for particular combination use. Current application to assessment of nifedipine- beta blocker use will be presented, as will an application to cimetidine audit. Prescriptions can be scanned and segregated in terms of numbers of prescriptions in addition to segregation in terms of a particular therapeutic agent. Initial application to audit has detected an unacceptable level of polypharmacy (more than 26% of all prescriptions involve greater than or equal to 5 items and more than 3% involved greater than or equal to 10 items). Statistical information on the volume and unit cost of outpatient prescribing allows a precise estimate of ongoing budgetary commitments, while trends in discharge medication profiles allow a basis for prediction of budgetary trends, particularly with respect to the use of recent additions to the formulary. The system allows for expansion at minimal cost with potentially revolutionary applications, for example, interactive prescriber education if and when "on-line" capabilities are developed. Cross-linking with relevant data-bases will allow a major expansion of applied research into therapeutic drug use and its consequences.

N(2) fixation and h(2) evolution by six species of tropical leguminous trees.

The C(2)H(4)/(15)N(2) and H(2)/(15)N(2) ratios for six species of tropical leguminous trees are reported. C(2)H(4)/(15)N(2) ratios ranged from 2.4 to 4.7; values for the H(2)/(15)N(2) ratios were between 0.6 and 1.4. Relative efficiency values, based on C(2)H(2) reduction, (15)N incorporation, and H(2) evolution during (15)N incorporation varied between 0.68 and 0.84 for the six species. Overall, approximately 30% of the electron flow through nitrogenase was used for H(2) evolution.

Use of a self-help smoking cessation manual as an adjunct to advice from a respiratory specialist.

The purpose of the present study was to determine the effectiveness of a self-help smoking cessation manual for patients with pulmonary disease. All patients were advised to quit smoking by a respiratory specialist. Thirty-five patients were then provided with a manual detailing techniques for quitting; the remaining 40 patients served as a control and were given no additional information or instruction following advice. The cessation rate in the treatment group was 16.7%; and in the control group, 25.6%. This difference was not statistically significant. Approximately half of the patients who received the manual used it, and only 12.5% of the users found it helpful. It was concluded that providing patients with the specific instructions on quitting smoking as presented in the manual did not appear to be useful.

Transport patterns and complications in an isolated Alaska practice.

This study reviews the patterns of transport and complications that occurred in 169 cases of transporting patients over a four-year period for a small, isolated Alaskan community in which the physicians as a matter of policy attempted to avoid heroic emergency surgery. Case descriptions are included describing transport-related complications and mortality and morbidity of patients transferred. A low incidence of mortality and relatively little long-term morbidity was experienced, supporting the feasibility of transferring severely ill patients rather than attempting heroic surgery under marginal circumstances. Although the lack of comparable studies limits the usefulness of these findings in assessing the overall quality of care, appropriate utilization of transport as an extension of the physician's capabilities resulted in a low incidence of complications during the study period.

Treatment of refractory acute nonlymphocytic leukemia with a combination of pyrazofurin and cytarabine.

Six patients with refractory acute nonlymphocytic leukemia were treated with pyrazofurin (15 mg/m2) followed by cytarabine (100 mg/m2) every 12 hours for 6--21 days. All patients cleared their peripheral blood of blast cells but no complete remissions were achieved. Excessive toxicity to skin and mucous membranes was observed. In the doses used, this combination is too toxic for further use. Alternate treatment schedules should be explored.

The preparation of transketolase free from D-ribulose-5-phosphate 3-epimerase.

A procedure for the purification from Candida utilis of transketolase (sedoheptulose-7-phosphate: D-glyceraldehyde-3-phosphate glycolaldehydetransferase, EC 2.2.1.1) free from D-ribulose-5-phosphate 3-epimerase (EC 5.1.3.1) was developed using acetone precipitation, elution from DEAE-cellulose, adsorption of epimerase by thiopropyl-Sepharose, and chromatography on D-ribose 5-phosphate-Sepharose and DEAE--Sephadex. The final product had a specific activity of 43 units/mg, a transketolase/epimerase activity ratio greater than 53 000 to 1, an apparent Km for D-xylulose 5-phosphate and D-ribose 5-phosphate of 77 and 430 microM, respectively, and ran as a single band using electrophoresis on polyacrylamide gel. It was inhibited by D-arabinose 5-phosphate and D-glucose 6-phosphate. During the purification by column chromatography, multiple forms of the enzyme were detected by gel electrophoresis but these gradually disappeared as the enzyme was further purified.

Microtubules: are they involved in the initiation of lymphocyte activation?

Purified human blood lymphocytes were stimulated with concanavalin A or phytohemagglutinin. DNA synthesis was measured with 2-h pulses of [3H]thymidine between 48 h and 73 h after stimulation. Colchicine, at concentrations between 0.1 muM and 10 muM, suppressed consequent DNA synthesis without affecting viability of the cells when added at any time up to 18 h before incorporation of [3H]thymidine was assessed. In concanavalin-A-stimulated lymphocytes, removal of the mitogen by methyl alpha-mannoside only prevented proliferation when added initially, but was without any effect when added after 20 h of stimulation, regardless of when DNA synthesis was measured. Thus, there was a period after 20 h of concanavalin A stimulation, when DNA synthesis was still sensitive to colchicine, but no longer required the presence of the mitogen. Colchicine also suppressed incorporation of [3H]leucine into protein, in resting as well as mitogen-stimulated lymphocytes. Similarly, colchicine decreased amino acid transport, as determined by uptake of alpha-amino-isobutyrate, which appeared to be the rate-limiting step in the incorporation of amino acids into protein in colchicine-treated cells. When the rate of protein synthesis was followed by the relative distribution of ribosomal particles, especially the increase of polysomes in activated lymphocytes, colchicine was without any detectable effect. The early increase in the incorporation of [14C]oleate into phospholipids was identical in the presence or absence of the microtubule-active drug. The data strongly suggest that microtubules are not involved in the initiation of lymphocyte growth or mitogenesis.
