Serum cardiac enzymes in stroke.

Serum cardiac enzyme levels (CK, LDH, SGOT) were estimated and the ECG recorded for 4 days following admission of 288 patients (Group I) to a stroke intensive care unit. Sixty-four of these patients, subsequently found not to have strokes, served as controls. Mean serum levels of all 3 cardiac enzymes were elevated in 8% of the 224 patients with stroke. The mean serum enzyme levels in patients with transient ischemic attacks (TIA) did not differ from controls. In a second group of 230 patients with stroke (Group II) serum CK levels were measured and the isoenzyems were fractionated to determine the tissue source of the enzymes. One hundred and one patients had raised total CK values and 25 of these (11%) had raised CK-MB (heart) iso-enzyme, the remainder having CK-MM (skeletal muscle) fraction. No serum CK-BB (brain) iso-enzyme was detected in any patient. Patients with positive serum levels of CK-MB has more evidence of acute myocardial ischemia on ECG (p less than 0.05), and more cardiac arrhythmias (p less than 0.001) than those with normal CK levels. Scattered areas of myocytolysis were found in the myocardium at autopsy in one patient. The acute rise in serum cardiac enzymes which we have recorded in the initial stages of stroke suggest that acute myocardial involvement is a commoner complication than is generally recognized. Also, since the CK-MB rises were modest and progressive, it is more likely that this acute myocardial dysfunction is a consequence, rather than a cause, of the acute cerebrovascular lesion.

Physician knowledge and practice patterns relating to diabetic nephropathy.

OBJECTIVE: To assess the knowledge and practice patterns of primary care physicians relating to the detection and treatment of diabetic nephropathy and to compare these findings with current standards of care. DESIGN: National survey. PARTICIPANTS: 1,000 randomly selected physicians. SETTING: Primary care. INTERVENTION: Mailed questionnaire consisting of a six-page "Diabetes Survey." RESULTS: Among the 1,000 primary care physicians, 950 were considered eligible for the survey. Sufficiently completed surveys were returned by 216 physicians, yielding an adjusted response rate of 22%. In general, patients with type 1 diabetes were monitored more intensively than those with type 2 diabetes. Primary care physicians were more likely to monitor patients for overt proteinuria (86%) than microalbuminuria (58%). Of the physicians who claimed to monitor patients for microalbuminuria, 39% chose inappropriate methods for detection, which lowers the percentage of physicians who correctly monitored for microalbuminuria to 37%. More than 95% of the respondents were aware of the benefits of angiotensin-converting enzyme inhibitors (ACEIs) in delaying the progression of diabetic nephropathy. Patients with proteinuria (86%) were more likely to be treated with an ACEI than were patients with microalbuminuria (79%). If a patient presented with proteinuria but without hypertension, the use of ACEIs fell to 75%. Rates of referral to a nephrologist were low at early stages of the disease (3% to 11%) and remained relatively low (28%) at later stages, even when serum creatinine was greater than 3 mg/dL. CONCLUSION: Further efforts directed toward education of primary care physicians about diabetic nephropathy have the potential to improve the care of patients with this disorder. These findings demonstrate a unique opportunity for pharmacists to become educators to physicians in their community.

The impact of the decreased prevalence of smoking in Canada.

Using data on the prevalence of smoking among different age and gender groups, we estimated the impact of the prevalence of smoking on the present and future mortality of Canadians. In 1989, the life expectancy of male smokers at the age of 45 was 7 years less than non-smokers, among female smokers life expectancy was 4 years less than non-smokers. The total number of smoking-attributable deaths from all causes was estimated for every decade from 1969 to 2019. A decrease in smoking prevalence from 1989 levels to 10% overall in 1999 would result in 15,000 and 6,000 fewer smoking-attributable deaths among women and men, respectively. Continuing the existing pattern of smoking among men and women will result in an increase in the total number of smoking-attributable deaths among women and an increase in the smoking-attributable deaths from lung cancer in both sexes.

Feeding practices and dental caries in an urban Canadian population of Vietnamese preschool children.

The aim of this project was to determine the severity of nursing caries, and to examine contributing behavioral factors, in a group of Vietnamese families in British Columbia, Canada. The data collected became the basis for a community-based oral health promotion program. Information on feeding, dental health practices, and dental caries were collected for 60 mother/child pairs. For children > or = 18 mos, prevalence of nursing caries was 64 percent. Sixty-five percent of all children had a naptime bottle, and 85 percent > or = 18 mos had a "comfort" bottle that was carried around, and drunk from during the day. Milk was the most common beverage. A "comfort" bottle was significantly related to the presence of nursing caries, P = 0.02; a naptime bottle had a less significant association, P = 0.07. Dental knowledge questions revealed that all mothers knew that a child who had a "comfort" bottle could get tooth decay, but 63 percent thought that cavities were not a problem in baby teeth.
