Strategy for defining the clinical content of family medicine.

The clinical content of family medicine is yet to be defined and arranged into a structured system of knowledge. To provide this systematic approach, it is suggested that a set of problem "profiles" could be identified as basic to family medicine, and binary decision and problem-solving chains identified for each. In addition, each problem profile might be expected to provide a consistent "masquerading" format for specific unusual problems. Certain "red flags" would be expected to be relevant to specific uncommon problems of medicine. A method of identifying these "masquerades" and "red flags" is proposed.

Respiratory function and symptoms: an environmental-epidemiological study of rubber workers exposed to a phenolformaldehyde type resin.

Rubber workers exposed to a hexamethylenetetramine-resorcinol (HR) adhesive system reported an excess of acute symptoms and significant reductions in expiratory flow rates at low lung volumes. Smokers exhibited greater reductions in flow rates than nonsmokers. The greatest reductions in ventilatory capacity were associated with HR exposure. There was a moderate association of reduced flow rates with respirable particulate in the HR exposed workers only.

The modified bovine heterograft in vascular access for chronic hemodialysis.

The results of 100 modified bovine heterografts constructed in 93 patients for subcutaneous arteriovenous fistulas for chronic hemodialysis have been reviewed. Fifty-seven patients had the bovine heterograft inserted as the primary method of vascular access. Twenty-eight patients had complications consisting of graft thrombosis and stenosis, graft infection, and hemorrhage that required additional surgical procedures. Three patients with diabetes mellitus developed ischemia of the hand. Fourteen patients have died but none of the deaths could be attributable to the use of the bovine heterograft. In our experience the modified bovine heterograft has enjoyed a higher patient acceptance as compared to other methods of vascular access for hemodialysis. The chief advantage of the bovine graft has been the ability to use the graft in any patient as a means of immediate dialysis.

Family resemblance in abilities: the Wechsler scales.

This note reports data on parent-child and spouse resemblance on the Wechsler intelligence scales, the WISC and WAIS. Test data on parents and one male 10-year-old child from 55 Canadian families are used to estimate father-son, mother-son, midparent-son, and father-mother correlations on 11 comparable subscales and on the verbal, performance, and total IQ aggregates. Heritabilities for these same scales are estimated as the regression of offspring on midparent values.
