In vitro bioassay for human erythropoietin based on proliferative stimulation of an erythroid cell line and analysis of carbohydrate-dependent microheterogeneity.

The human erythroleukemia cell line TF-1 was employed for the determination of proliferative stimulation induced by recombinant human erythropoietin (rhEpo). Potencies of various intact and sugar-trimmed rhEpo preparations were estimated using the International Standard for Human r-DNA-derived Epo (87/684) as a reference for activity. The cellular response was measured in a multi-channel photometer using a colorimetric microassay, based on the metabolism of the tetrazolium dye 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide to formazan, by viable cells. The linear part of the log dose-response relationship encompassed 2.5-90 pM and activity of rhEpo preparations was measured at doses between 3 and 60 pM. The assay was designed as a parallel line test, using three or four concentrations for potency determinations, which fulfills pharmacopoeial requirements for assay validity. Inter-assay relative standard deviation varied between 4.1% and 12.6% and most assays revealed potencies with limits of error within 87-113%. In order to acquire an additional means for an efficient probing of physiologically relevant features of rhEpo, a luminiscence-dependent Western detection system, based on a combined isoelectric focusing/sodium dodecyl sulphate-polyacrylamide gel electrophoresis separation, was established. As opposed to conventional electrophoresis the two dimensional approach enabled the disclosure of minor truncations in the rhEpo-attached glycan moieties using picomolar quantities of the hormone. Moreover, the separated isoforms of rhEpo were quantified by computer-assisted densitometry and compared with the 87/684 standard. Accordingly, results obtained by the cellular response were balanced against the general pattern observed and the relative amounts of separated rhEpo isomers as determined by the quantitative Western analysis. The method described should be suitable for potency assessments of pharmaceutical formulations of rhEpo.

[Psychomotor physical therapy--patients' starting point and their evaluation of the therapeutic results].

Psychomotor physiotherapy is a technique which aims, through relief of muscular tension and inhibited breathing, to give the patient a better understanding of the connection between mental and somatic reactions, establish improved coping patterns. In a three-year, prospective registration of 152 patients referred to psychomotor physiotherapy the expectations of the patients were evaluated, along with the presented symptoms. After 6-12 months of treatment 75% experienced a relief of symptoms, and 50% a change in symptom profile. At follow-up of 109 patients after 2 1/2 years we found a significant improvement in symptoms as well as in parameters for everyday coping, and a marked reduction in use of drugs. There was a correlation between an academic education and response to treatment.

Intraluminal duodenal diverticulum. Report of a case diagnosed by endoscopy.

Intraluminal diverticulum of the duodenum is a very rare congenital anomaly arising in the second part of the duodenum in close relation to the papilla of Vater. 39 cases have been reported in our available literature. The clinical symptoms and signs are nonspecific, and characteristically the preoperative diagnosis has in all cases been based on the roentgenographic appearance of the lesion. In this paper we present a case diagnosed by duodenoscopy, the radiological studies including hypotonic duodenography revealing no pathology.

Primary purulent meningococcal pericarditis with tamponade.

A case of primary purulent meningococcal pericarditis, presenting with hemodynamic derangement due to tamponade, is reported. Treatment was successfully fulfilled with antibiotics and surgical drainage, using continuous percutaneous subxiphoidal catheter drainage with the Seldinger technique. Less than ten cases of this potentially lethal entity have been reported in the literature.

Popliteal aneurysms.

Four typical cases of popliteal aneurysms are reported and the recent literature is briefly reviewed. The aneurysm should be operated upon once the diagnosis is made, whether symptomatic or not. The treatment of choice is proximal and distal exclusion of the aneurysm and a vein bypass, using the medial approach.
