Influence of genetic polymorphism on the metabolism and disposition of encainide in man.

The metabolism of the new, highly effective antiarrhythmic agent, encainide, appears to be polymorphically distributed in a similar fashion to the genetically determined oxidative biotransformation of debrisoquine. Accordingly, the disposition of encainide and known metabolites was investigated after simultaneous acute i.v. (radiolabeled) and single and multiple oral (nonradiolabeled) dosing to two groups of normal subjects characterized as "poor" (PM) and "extensive" (EM) metabolizers of debrisoquine. Pronounced differences in both the plasma concentration/time curves and the 24-hr urinary excretion of encainide and metabolites were observed between the two phenotypes. In the EM group, the oral bioavailability of encainide was only about 25 to 30% because of extensive presystemic (first-pass) metabolism, and no accumulation occurred after multiple oral dosing with 50 mg every 8 hr for 3 days, as the elimination half-life of the drug was about 2.5 hr. The major metabolite formed was O-desmethylencainide which accounted for almost half of the identified urinary metabolites and represented about 10% of the administered dose. This metabolite was present in 5- to 10-fold higher concentrations in the plasma than unchanged drug and accumulated almost 2-fold after multiple oral dosing. 3-Methoxy-O-desmethylencainide also was present at higher concentrations than encainide and accumulated on multiple dosing similarly to O-desmethylencainide. N,O-didesmethylencainide was a minor metabolite only detectable in the urine and N-desmethylencainide was not measurable in either plasma or urine. In contrast, in the PM group, encainide plasma concentrations were 10- to 20-fold higher than in the EMs after both oral and i.v. administration and the elimination half-life was 3- to 4-fold longer.(ABSTRACT TRUNCATED AT 250 WORDS)

Studies on the mechanism of action of 3, 4-dihydro-6-[4-(3,4-dimethoxybenzoyl)-1-piperazinyl]-2(1H)-qu inolinone (OPC-8212), a new positive inotropic drug. Enzyme activities and Ca2+ transport processes of sarcolemma and intracellular organelles.

3,4-Dihydro-6-[4-(3,4- dimethoxybenzoyl )-piperazinyl]-2(1H)- quin olinone ( OPC -8212) is a new positive inotropic agent. In studies designed to elucidate information concerning its mechanism of action, its effect on Ca+ transport and related enzyme activities in sarcolemma, sarcoplasmic reticulum and mitochondria were studied. OPC -8212 was found to have little or no effect on these processes. The mechanism of action therefore is clearly different from other known positive inotropic drugs.

Comparison of catecholamine effects on canine myocardial metabolism and regional blood flow during and after cardiopulmonary bypass.

The acute metabolic and hemodynamic effects of dopamine, dobutamine (both at 10 micrograms . kg-1 . min), and isoproterenol (at 0.05 or 0.1 micrograms . kg-1. min) were determined in dogs following 20 minutes of normothermic global myocardial ischemia. The catecholamines were started 10 minutes before cardiopulmonary bypass (CPB) was discontinued and were continued for 1 hour after bypass. Regional myocardial and systemic blood flow distribution was measured by means of the radioactive microsphere technique. On bypass all catecholamines sharply increased heart rate, myocardial oxygen consumption, and left ventricular blood flow (p less than 0.01). Because the hearts were unloaded, these data suggest that velocity of contraction is an important component of myocardial oxygen consumption. Although these drugs did not lower myocardial adenosine triphosphate (ATP) and creatine phosphate (CP) levels, the significant rise in oxygen consumption suggested that inotropic treatment on bypass may not be beneficial. Furthermore, renal blood flow was diminished in dobutamine-treated dogs (p less than 0.01) and tended to decrease with isoproterenol infusion. No change was seen with dopamine infusion. After bypass, dobutamine treatment increased cardiac output (p less than 0.01) and stroke volume (p = 0.017) with no change in heart rate, myocardial oxygen consumption, high-energy phosphate levels, and total or transmural distribution of left ventricular blood flow. Dopamine infusion did not change cardiac output but did increase oxygen consumption (p less than 0.01). Isoproterenol showed a slight inotropic effect, but frequent ventricular arrhythmias were present during weaning from bypass. In all treatment groups, blood flow in the other systemic beds (cerebral, gastrointestinal, and renal) was similar to that in control dogs. These data suggest that dobutamine is the most efficient of the drugs tested for support of the heart following global myocardial ischemia but, when given during bypass, it appears to decrease renal blood flow.

Properties and characterization of a highly purified sarcoplasmic reticulum Ca2+-ATPase from dog cardiac and rabbit skeletal muscle.

Sarcoplasmic reticulum (SR) Ca2+-ATPase was purified from dog cardiac and rabbit skeletal muscle using Triton X-100 at optimal ratios of 0.5 for cardiac and 0.5 to 1.0 for skeletal SR. The yields of Ca2+-ATPase were 4 to 5 and 1 to 2.2 mg/100 mg of cardiac and skeletal SR protein, respectively. The enzyme activities were 547 +/- 67 mumol ADP/mg/h for cardiac and 1192 +/- 172 mumol ADP/mg/h for skeletal Ca2+-ATPase. Removal of excess Triton X-100 increased the enzyme activities to 719 +/- 70 and 1473 +/- 206 mumol ADP/mg/h, respectively. The residual content of Triton X-100 for cardiac and skeletal Ca2+-ATPase was 20 and 5 mol/mol of enzyme, respectively. Maximum levels of phosphoenzyme were 4.4 +/- 0.2 and 5.6 +/- 0.6 nmol/mg in each case. A single protein band of 100 kDa was obtained for each purified Ca2+-ATPase by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The preparations were stable at -80 degrees C for 5 months in the presence of 1 mM Ca2+. The phospholipid content of the purified enzyme was 2-fold greater than that of native cardiac and skeletal SR microsomes. Repeated washing of the purified enzyme preparation did not alter the phospholipid content or the specific activities.

Intercellular junctions in the human fetal membranes. A freeze-fracture study.

Freeze-fracture replicas of the human reflected and placental amnion and chorion laeve at term were studied in order to give a systematic survey of the nature and extension of the intercellular junctions in the fetal membranes. No differences could be detected between the reflected and placental amniotic epithelium. In both the replicas never displayed plasma membrane differentiations typical of occluding junctions, while communicating junctions were occasionally and desmosomes frequently seen. In the chorionic trophoblast maculae occludentes, communicating junctions and desmosomes were regularly encountered. It is assumed that the maculae occludentes are remnants of occluding junctions which early in gestation possibly seal off the chorionic cavity; it appears improbable that they contribute significantly to the permeability properties of the chorionic trophoblast, since it is known from previous ultrastructural studies that large open intercellular channels cross the chorionic trophoblast. Thus the absence of occluding junctions, which could act as effective permeability barriers, in both epithelial components of the fetal membranes suggests that the factors able to influence the amniotic fluid turnover or the paraplacental protein exchange are the geometrical relationships and physico-chemical properties of the intercellular channels in the amniotic epithelium and chorionic trophoblast. In addition, communicating junctions were present between fibroblasts in the chorion laeve but not in the amnion, possibly indicating differences in the functional state of these cells and/or their extracellular microenvironments.

Fine structure of human chorionic membrane. Ultrastructural and histochemical examinations.

Normal human term chorionic membrane was examined by electron microscopy and histochemistry. In comparison with previous studies of the human chorion, our findings showed the following new morphological aspects: (1) The cellular junctions between adjacent trophoblastic cells were made up not only of desmosomes but also of gap junctions and occasional tight junctions. (2) The epithelial basement membrane was stratified and discontinuous. (3) Besides the fibroblasts and macrophages (Hofbauer cells), the connective tissue of the human chorion also contained myofibroblasts. Histochemically, the surface coat of the plasma membrane of the chorionic epithelial cells, the microfibrils, and the proteoglycans associated with the collagen fibrils showed a positive reaction to ruthenium red. This was not so for the basement membrane of the chorionic epithelium. Since the tight junctions are seldom present in the chorionic membrane, they are apparently not significant for the paracellular protein transfer. Similar to the amnion epithelium, it is assumed that the intercellular spaces and the discontinous basement membrane of the chorionic epithelium represent a paracellular route for non-placental protein transfer. As the myofibroblasts in human amnion, we suggest that the myofibroblasts in the chorionic membrane may contribute to the protection of the fetal membranes from overdistension.

Prevention of ATP catabolism during myocardial ischemia: a preliminary report.

The enhancement of ATP regeneration following global myocardial ischemia in dogs by both ATP catabolic enzyme blockade and precursor infusion was investigated. The breakdown of AMP to adenosine is catalyzed by 5'-nucleotidase and this enzyme was inhibited during the ischemic period with either concanavalin A (Con A, 3 mg/kg) or alpha, beta-methyleneadenosine 5'-diphosphate (AMP-CP, 250 microM). To provide additional ATP precursors, adenine (30 mg/kg) and ribose (25 mg/kg) (A/R) were also infused into the coronary vasculature during ischemia and recovery on cardiopulmonary bypass. Left ventricular myocardial ATP levels in control animals decreased to 52% of preischemic values during aortic cross clamping, but ATP levels in dogs treated with AMP-CP + A/R fell to only 67% of preischemic values (P less than 0.05). During reperfusion, ATP levels in Con A + A/R (3.43 +/- 0.26 mumol/g wet wt) and AMP-CP + A/R (3.77 +/- 0.42) treated animals were higher than values found in control dogs (2.73 +/- 0.16, P less than 0.05). Infusions of A/R alone without enzyme inhibition did not increase ATP regeneration. The adenine nucleotide energy charge ratio was also increased by enzyme blockade with either inhibitor when combined with precursor infusion. On bypass, left ventricular myocardial blood flow (measured by the microsphere technique) was increased by 140% (P less than 0.01) over control values in all groups receiving A/R; therefore, enhanced ATP levels were not merely the result of increased flow. Renal blood flow was not adversely affected by this combination of drugs as has been previously found with adenosine infusion and inhibition of adenosine catabolism.

[Fine structure of the septum in twin pregnancy].

The structure of the septum in bigeminal pregnancy was examined via the electron microscope after normal pregnancy. The cellular junctions were desmosomes and "gap junctions" in the amnionic epithelium, whereas in the chorionic epithelium "gap junctions" and "tight junctions" were found occasionally besides desmosomes. The basement membrane was multilayered and discontinuous both in the amnion and in the chorion. Since, physiologically, the desmosomes and the gap junctions cannot be regarded as a diffusion barrier against proteins, and since the rare tight junctions are without any significance for the passage of the proteins through the septum, and, furthermore, since a discontinuous basement membrane cannot exercise any action in the nature of a barrier, these structural peculiarities of the septum seem to indicate that the intercellular spaces and the discontinuous basement membrane of the amnion and of the chorion in the septum may represent a paracellular path for the passage of the proteins between the two amniotic cavities.

Ischaemia-induced changes in canine cardiac sarcoplasmic reticulum.

Effects of myocardial ischaemia on sarcoplasmic reticulum (SR) of dog hearts were investigated. Regional ischaemia was produced by occlusion of the left circumflex artery, and a microsomal fraction enriched in vesicles of SR was isolated from subendocardium (Endo) and subepicardium (Epi) of control and ischaemic areas of the heart. No significant changes occurred in ischaemic Epi. A loss of in vitro activities (ie calcium transport and ATPase) was found for SR from ischaemic Endo which paralleled the changes in the histology of the tissue. At 5 min of coronary occlusion, Ca2+ binding and Ca2+-ATPase activities of SR from ischaemic Endo were normal. A decrease in the activities of SR was first evident at 15 min after the occlusion, decreased further at about 30 min and remained at that level at 60 min of ischaemia. The maximal rate of Ca2+ uptake did not parallel the Ca2+-binding and Ca2+-ATPase activities. The degree of cAMP-dependent phosphorylation by endogenous and exogenous protein kinase was not different between SR from control and ischaemic areas. A participatory role of SR in the ischaemic impairment of left ventricular systolic and diastolic performance is discussed.

Selective inhibition of Na+-induced Ca2+ release from heart mitochondria by diltiazem and certain other Ca2+ antagonist drugs.

The effect of Ca2+ antagonist drugs on Ca2+ uptake and Na+-induced Ca2+ release by isolated rabbit heart mitochondria has been studied using arsenazo III and dansylaziridine-labeled troponin C as Ca2+ indicators. Mitochondria accumulated 20 nmol of Ca2+/mg of protein by a respiration-dependent process. A23187, carbonyl cyanide p-trifluoromethoxyphenylhydrazone, or Na+ induced a release of Ca2+ from Ca2+-loaded and ruthenium red (RR)-treated mitochondria. Ca2+ antagonist drugs had no effect on the Ca2+ uptake process or on Ca2+ release induced either by A23187 or by carbonyl cyanide p-trifluoromethoxyphenylhydrazone but they were effective in inhibiting the Na+-induced Ca2+ release process. The drug concentrations that resulted in 50% inhibition of Na+-induced Ca2+ release were 7, 12, 13, 66, and 150 microM for diltiazem, prenylamine, fendiline, nifedipine, and verapamil, respectively. In the absence of RR, the net amount of Ca2+ released by Na+ was less than that which occurred in the presence of RR due to the simultaneous operation of both the Ca2+ uptake and the Ca2+ release processes. Under these conditions, the inhibition of Ca2+ uptake by RR brought about a complete release of Ca2+ from the mitochondria, and the inhibition of the Na+-induced Ca2+ release by diltiazem resulted in an apparent uptake of Ca2+ by the mitochondria. These results indicate that diltiazem and certain other Ca2+ antagonist drugs may be selective inhibitors of the Na+/Ca2+ antiporter in heart mitochondria much like ruthenium red is a selective inhibitor of the Ca2+ uniporter.

Squamous cell carcinoma arising in branchiogenic cyst: branchial cleft carcinoma.

Squamous cell carcinoma that developed in branchial cleft cyst in a 53-year-old male is described. The tumor was focally invasive. The tumor fulfilled criteria advocated by Martin for substantiating carcinoma as primary in the branchial cleft cyst. The patient has survived for 4.5 years after radical neck dissection and radiation without manifesting primary or recurrent tumor locally or elsewhere. One of the major criteria in diagnosing squamous cell carcinoma as primary tumor in the branchiogenic cyst is to prove that it has arisen in normal epithelial lining [2, 3]. In our case there is transition of the tumor from normal squamous epithelial lining of the cyst to malignancy.

Effects of dopamine and dobutamine on the myocardial and systemic circulation during and following cardiopulmonary bypass in dogs.

The effect of dopamine (Dp) and dobutamine (Db) on myocardial and systemic blood flow (BF) distribution was compared in dogs being weaned from cardiopulmonary bypass (CPB) after 20 min of normothermic global myocardial ischemia. Drug infusions (10 mcg/kg/min) were begun and BF was measured (radiolabeled microspheres) prior to weaning and 60 min off CPB. On CPB: Pump flows, by design, were similar (100 ml/kg/min) in Dp, Db, and saline control (NS) dogs. Dp and Db significantly (p less than 0.05) increased myocardial BF. Dp did not alter renal, visceral, and skeletal muscle BF and only increased BF to the cervical spinal cord and medulla. Db, however, significantly reduced renal (-49%), splenci (-58%), pancreatic (-27%), and colonic (-47%) BF but increased perfusion in essentially all of the central nervous system. Off CPB: Cardiac output during Dp and Db infusions was significantly greater than NS dogs (107 +/- 7 vs 152 +/- 12 vs 82 +/- 10 ml/kg/min resp.); the greater increase for Db resulting from a larger stroke volume. Dp and Db significantly increased myocardial BF. Dp increased splenic (+77%), gastric (+139%), and gallbladder (+125%) BF but had no effect on renal, hepatic, skeletal muscle, and intestinal BF. Db infusion maintained renal BF similar to NS and elevated BF to most visceral organs. The results of this study show that the myocardium responded to inotropic stimulation despite the previous ischemic insult but the BF changes varied among regional vascular beds with Dp and Db infusions during and following CPB. Of the 2 drugs, Db showed the greater inotropic response off CPB, a similar increase in myocardial perfusion and greater visceral organ bloodflow.

Chromosomal replication asynchrony of a human breast carcinoma cell line. I. studied by continuous BrdU incorporation and G-banding analysis.

Continuous BrdU incorporation and the Giemsa staining technique were used to study the cell cycle kinetics of a human breast tumor cell line. It was found that the interchromosomal replication pattern of the neoplastic cell was significantly different from that of normal cells in two respects. First, the pattern is highly asynchronous; within a single cell there are chromosomes at different replication cycles; that is, some chromosomes complete their DNA replication before others begin. Second, the replication schedule for the chromosomes, as identified by superimposing the BrdU-Giemsa technique on the trypsin G-banding technique, is relatively consistent within the cell line but differs from that of normal cells. Some chromosomes that replicate late in normal human lymphocytes and fibroblasts replicate early in this cell line. In contrast to the unusual interchromosomal replication pattern, gross analysis of the intrachromosomal replication schedule shows no apparent difference from that reported for normal cells. The asynchrony phenomenon reported here may be associated with the etiology of aneuploidy in neoplasia.
