Deferoxamine given at reperfusion improves function of the cold-stored rat heart.

In this study deferoxamine (DF), a strong iron chelator, was administered either before storage or during reperfusion, in an attempt to inhibit the iron-dependent hydroxyl radical production and improve the functional recovery of the cold-stored/reperfused cardiac explant. Excised rat hearts were flushed with Krebs-Henseleit buffer (KHB), arrested with a cardioplegic solution, CP11-EB, with or without DF, and immersion stored in CP11-EB at 0 degree C for 16 hr. To assess function, the stored hearts were reperfused in the working mode with KHB for 30 min. Experimental groups included: (i) DF treatment during prestorage flush [CP11-EB + 0.01 mM (n = 5), 0.05 mM (n = 13), 0.1 mM (n = 5), 0.2 mM (n = 5), or 0.75 mM DF (n = 5)]; (ii) DF treatment during reperfusion [KHB + 0.3 mM (n = 5), 0.6 mM (n = 7), 0.75 mM (n = 11), 1.0 mM (n = 6), 1.5 mM (n = 4), or 2.5 mM DF (n = 7)]; and (iii) untreated group (n = 8) received no DF during flush or reperfusion. Function of unstored hearts (n = 7) including aortic flow (AF, 54.6 +/ 2.6 ml/min); cardiac output (CO, 76.5 +/- 3.3 ml/min), systolic pressure (SP, 135.7 +/- 1 mm Hg), diastolic pressure (DP, 70.7 +/- 3.8 mm Hg), and work (96.7 +/- 6.4 g-meter/min) served as controls. Functional recovery of the untreated group was AF, 59%; CO, 58%; SP, 71%; DP, 73%; work, 41% of control values. DF treatment at any dose during the initial flush did not improve functional recovery.(ABSTRACT TRUNCATED AT 250 WORDS)

Variable selectivity of the Hitachi chemistry analyzer chloride ion-selective electrode toward interfering ions.

Chloride measurements by ion-selective electrodes are vulnerable to interference by anions such as iodide, thiocyanate, nitrate, and bromide. We have found that the degree of interference of these anions on the Hitachi chemistry analyzer chloride electrode varies from electrode to electrode and this variation can even occur within the same lot of membrane. This variation is not dependent upon the length of time the cartridge has been in the analyzer because no correlation existed between the usage time and the electrode response to interfering ions. Neither is this variation due to the deterioration of the electrode because all electrodes tested had calibration slopes within the manufacturer's specification. Our study, however, showed that even after repeated exposure to a plasma sample containing 2 mM thiocyanate, the chloride electrode was still able to accurately measure the chloride in plasma without thiocyanate, thus confirming that a carryover effect does not exist from a previous thiocyanate-containing sample.

Changes in interstitial adenosine during hypoxia: relationship to oxygen supply:demand imbalance, and effects of adenosine deaminase.

OBJECTIVE: The aim was to determine the changes in coronary blood flow and intramyocardial interstitial fluid (ISF) adenosine and adenosine metabolites during systemic hypoxia, and to evaluate (1) whether the increase in ISF adenosine during hypoxia is augmented if the hypoxic hyperaemia is prevented, and (2) the effects of adenosine deaminase on ISF adenosine and coronary blood flow during sustained hypoxia. METHODS: Anaesthetised dogs were instrumented with a flow probe around the left anterior descending coronary artery to measure coronary blood flow and with a microdialysis probe in the myocardium perfused by this artery to sample intramyocardial ISF. Dialysate purine metabolite levels were used as indices of ISF levels. Systemic hypoxia was induced by a reduction in the FIO2. RESULTS: Acute systemic hypoxia (PaO2 approximately 3.9 kPa) resulted in a 60% increase in dialysate adenosine, along with increases in dialysate levels of the adenosine metabolites inosine (74%), hypoxanthine (33%), and xanthine (32%). If the hypoxic hyperaemia was prevented, dialysate adenosine increased by 180% during hypoxia, and the increases in adenosine metabolites were augmented as well. During sustained hypoxia, intracoronary administration of adenosine deaminase decreased dialysate adenosine below prehypoxia levels, but did not alter the hypoxic hyperaemia. CONCLUSIONS: While ISF adenosine is increased during acute systemic hypoxia and is increased in relation to the oxygen supply:demand imbalance, consistent with a role of adenosine in hypoxic hyperaemia in the heart, adenosine is not necessary for the maintenance of a sustained increase in coronary blood flow during hypoxia.

Effectiveness of antiscatter grids in digital radiography. A phantom study.

INTRODUCTION: With ever-increasing interest in bedside radiography using digital imaging techniques, one question often asked is whether and which antiscatter grid should be used to reduce the effects of scattered radiation. In this article, the authors quantitatively analyze and compare images obtained with a flexible circular hole (FCH) grid, a conventional 6:1 focused grid, or without a grid. METHODS: Scatter-to-primary ratios (SPRs), contrast signals, and contrast-to-noise ratios (CNRs) were measured for fixed patient exposure and were compared at various locations in the images of an anthropomorphic chest phantom. RESULTS: Although both grids resulted in greater contrast signals, a significant improvement of the CNR was achieved only in the upper and middle mediastinum regions when the conventional grid was used. With the FCH grid, the CNR is degraded at most locations in the chest image. DISCUSSION: Our results indicate that at fixed exposure level, the use of either grid yields only marginal improvement, at best, in bedside digital chest radiography.

Adenosine deaminase inhibitor in cardioplegia enhanced function preservation of the hypothermically stored rat heart.

Adenosine (ADO) has been shown to be protective to the ischemic-reperfused myocardium. This study tested the hypothesis that inhibition of myocardial adenosine deaminase during cold storage will elevate tissue ADO content, improve the cardiac function, and preserve ATP. The isolated rat hearts (6-9 hearts/group) were flushed with a cardioplegic solution containing 0-75 microM erythro-9-(2-hydroxy-3-nonyl)adenine (EHNA) and immersion-stored at 0 degree C for 9 hr. Function was assessed after 30 min working reperfusion. Function of the unstored hearts (n = 11, mean +/- SE) including heart rate (293 +/- 13 bpm), aortic flow (AF; 52.5 +/- 1.1 ml/min), coronary flow (CF; 23.5 +/- 1.3 ml/min), cardiac output (CO; 76.0 +/- 2.1 ml/min), systolic pressure (SP; 136 +/- 2 mmHg), diastolic pressure (DP; 63 +/- 1 mm Hg), work (90.5 +/- 3.4 g-m/min), and coronary vascular resistance (CVR; 2.77 +/- 0.14 mmHg-min/ml) served as controls. Heart rate in all stored hearts returned to normal after reperfusion. Recovery of other function in no-EHNA group was: AF, 52 +/- 7; CF, 55 +/- 5; CO, 53 +/- 6; SP, 79 +/- 4; DP, 93 +/- 3; work, 47 +/- 7; and CVR, 171 +/- 15% of control. EHNA improved functional recovery in a dose-dependent fashion. At the optimal concentration of 25 microM, the recovery was: AF, 83 +/- 6; CF, 68 +/- 4; CO, 78 +/- 5; SP, 90 +/- 3; DP, 105 +/- 5; work, 77 +/- 8; and CVR 151 +/- 9% of control. ADO A1 receptor antagonists, 8-phenyltheophylline (1 microM) and 1,3-dipropyl-8-cyclopentylxanthine (0.1 microM) blocked the effects of 25 microM EHNA; the recovery of CO was reduced to 65 +/- 3 and 50 +/- 2% of the control, respectively. Tissue ADO content in 25 microM EHNA hearts at the end of storage was 95 +/- 19 nmol/g dry wt, which was significantly elevated from 15 +/- 3 nmol/g dry wt in no-EHNA hearts. EHNA also caused a 45-fold increase in the release of ADO over no-EHNA group during the first 10 min of reperfusion. But EHNA treatment did not cause any change in either end-storage or end-reperfusion myocardial ATP levels. Thus EHNA in cardioplegic solution inhibited cardiac ADO catabolism during long-term hypothermic storage and improved function preservation partially via an ADO A1 receptor-mediated mechanism without invoking ATP conservation.

High frequency of HLA-DQB1*0301 allele in patients with pure ocular cicatricial pemphigoid.

Two factors, is classification and its immunogenetics, may have contributed to an incomplete understanding of the pathogenic mechanism in the disease group commonly termed 'cicatricial pemphigoid'. We have previously demonstrated that pure ocular cicatricial pemphigoid (OCP) is a unique clinical and immunopathological entity. In an effort to better define the immunogenetic characteristics for patients with pure OCP, we performed HLA typing for the class II MHC gene HLA-DQB1*0301 allele, by amplifying genomic DNA extracted from patients with polymerase chain reaction, followed by hybridization with a sequence-specific oligonucleotide probe on dot-blotted and Southern-blotted amplified DNA samples. Ninety percent (9/10) of patients with pure OCP had the DQB1*0301 allele, in contrast to 52% (17/33) of normal individuals who had the DQB1*0301 allele. The high frequency of the HLA-DQB1*0301 allele in patients with pure OCP may point to a distinct immunogenetic pattern and possibly a distinct pathomechanism for the disease.

Ventilatory responses to hypoxia in the toad Bufo paracnemis before and after a decrease in haemoglobin oxygen-carrying capacity.

The ventilatory and cardiovascular effects of a decreased O2-carrying capacity of the blood were evaluated in the toad Bufo paracnemis. Pulmonary ventilation was monitored using a pneumotachographic technique. Chronic arterial catheters served to record both cardiac frequency and blood pressure and enabled the withdrawal of blood samples for analysis of pH and partial pressure of O2 (PO2). Haemoglobin concentrations were determined by the cyanmethaemoglobin method. The ventilatory response to hypoxia was not affected by the reduction in blood O2-carrying capacity, which suggests that PO2 rather than O2 content is the regulated variable. The reduction in capacity is accompanied by an increased normoxic heart rate and by a reversal of the hypoxic tachycardia normally observed.

THE RELATIVE CONTRIBUTIONS OF RED AND WHITE BLOOD CELLS TO WHOLE-BLOOD ENERGY TURNOVER IN TROUT

Previous studies addressing energy turnover in fish blood have ignored the possible influence of white cells. The present investigation quantified the contribution of white and red cells to whole-blood energy turnover in trout (Oncorhynchus mykiss) before and after adrenergic stimulation. All experiments were carried out on cells kept in their native plasma. White cells were found to have an almost twenty times higher rate of oxygen consumption than red cells. Furthermore, white cells were responsible for essentially all whole-blood lactate production. Our data therefore show that white cells account for almost half of the energy turnover in trout blood. Adrenergic stimulation elicited a significant increase in total as well as ouabain-sensitive (a Na+/K+-ATPase inhibitor) red cell oxygen consumption. However, the ouabain-sensitive red cell oxygen consumption amounted to approximately 23 % of the total red cell oxygen consumption, regardless of adrenergic stimulation. Therefore, energy-consuming processes other than Na+/K+-ATPase activity are probably involved in the increased red cell oxygen uptake after adrenergic stimulation.

Solid pelvic masses caused by endometriosis: MR imaging features.

OBJECTIVE: Solid fibrotic nodules of endometriosis can simulate peritoneal metastases on imaging studies. To assist with this distinction, we analyzed the MR appearance of solid masses of pelvic endometriosis. MATERIALS AND METHODS: A search of pathologic and surgical records of patients who had pelvic MR imaging with a phased-array multicoil disclosed 13 patients who had endometriosis proved at laparotomy. MR images were reviewed retrospectively by two unblinded radiologists for the signal intensity, size, and enhancement pattern of solid peritoneal masses. Eight solid masses in six patients were detected on MR images: four lesions were in the cul-de-sac, two in the bladder wall, and two in the rectal wall. Four of the masses were excised at surgery and one was sampled by surgical biopsy; microscopy showed abundant fibrosis with small clusters of endometriotic glandular tissue. Three masses were inspected at surgery and found to represent dense fibrosis caused by endometriosis. RESULTS: Solid masses of endometriosis ranged in size from 1 to 5 cm. Seven of the eight masses had similar features on MR images: intermediate signal intensity on T1-weighted spin-echo images with punctate foci of high signal intensity, low signal intensity on T2-weighted images, and enhancement after administration of contrast material. CONCLUSION: MR imaging shows enhancing solid masses in some patients with endometriosis. These masses have MR features that might be useful in the differentiation between solid foci of endometriosis and peritoneal metastases.

Electrochemical therapy (ECT) for thyroid adenoma during acupuncture anaesthesia: analysis of 46 patients.

Between February 1989 and April 1993, 46 patients with thyroid adenoma were treated by electrochemical treatment (ECT) during acupuncture anaesthesia. After three months to four years follow up period, the cure rate was 97.8%. ECT is a new technique which provides simple, effective and safe treatment of thyroid adenoma. Treatment of benign tumours with ECT represents a new application.

[Pingyangmycin-induced chromosome damage in lymphocytes of laryngeal cancer patients].

Lymphocytes from 28 untreated laryngeal cancer patients and 23 healthy controls were cultured in vitro and exposured to pingyangmycin (bleomycin A5), a clastogen. The lymphocytes were arrested in metaphase and analyzed. The total chromosome aberration rate, mean chromatid break rate per cell and cell aberration rate were 1.98% +/- 0.05%, 0.57% +/- 0.35%, and 42.8% +/- 12% respectively for laryngeal cancer patients. However, for healthy controls these values were 0.94% +/- 0.04%, 0.28% +/- 0.12%, and 27% +/- 12% respectively. Statistical analysis showed there are significant differences between the two groups. The data indicate that under our experimental conditions chromatid break rate 0.40 can be considered to be a borderline value, 0.80 hypersensitive value. For any individual, if the chromatid break rate is 0.40 or more, one should be ranked as having cancer risk. If 0.80 or more, then, highly cancer risk.

[Study of vocal function following partial laryngectomy with computer analysis system].

Vocal function of 28 patients following partial laryngectomy was studied with HGF-1 personal computer system. The system consists of spectrogram analysis of correlogram and sonogram. The result showed that different phonetic positions have different vocal functions. Vocal function of the patients who had new vocal cord and patients who received supraglottic hemilaryngectomy is nearly normal. Whereas vocal function of patients with supraglottic construction and speaking shunt is very poor. This system appears to be an objective and quantitative tool for vocal function study.

Evaluation of the antitumor activity of human IL-4 by in vitro and in vivo assays.

The characteristics of rhuIL-4 induced cytotoxicity was detected in vitro by using 51Cr release assay and the anti-tumor activity of rhuIL-4 induced killer cell was evaluated in vivo by using a human tumor model in nude mice. huIL-4 can induce LAK activity from peripheral blood lymphocytes (PBMC) stimulated with phytohemagglutinin (PHA). Compared with the LAK activity induced by rhuIL-2, the cytotoxicity of the killer cells induced by rhuIL-4 to K562 and Raji cells was lower, but that to TBL-E, a human lymphoid leukemia cell line established in our laboratory, and PHA-activated blast cells (PHA-blasts) was of similar magnitude. In the cytotoxicity assay using PHA-blasts, the addition of PHA increased the IL-4-induced killer cell cytotoxicity by 131%, but had no effect on IL-2-induced killer cell cytotoxicity. This implies that IL-4 mainly induces CTL-like activity, while IL-2 mainly induces NK-like activity. An experimental human tumor model in nude mice was established by injection of TBL-E human leukemia cells. The anti-tumor activity of rhuIL-4 was evaluated by injection of human LAK cells induced from PHA-blasts by rhuIL-2+rhuIL-4 and human cytokines into tumor-bearing nude mice. The results showed that human LAK cells effectively inhibit the tumorigenicity of TBL-E cells in nude mice with an inhibition rate of 61%. The antitumor effect of rhuIL-2 was better than that of rIL-4, and the antitumor effect of rhuIL-2+rhuIL-4 was similar to that of rhuIL-2, though the former delayed the occurrence of tumors. Our data imply the potential application of human IL-4 in clinic, and provide an animal model to evaluate the anti-tumor activity of human cytokine(s) with species specificity.

[The clinical characters and treatment of lung cancer in youth].

From January 1970 to December 1990, 67 patients with lung cancer aged younger than 40 years, were treated in the PUMC Hospital. There were 43.3% with SCLC and 37.3% with adenocarcinoma. Twenty-eight patients underwent resection of the lung. Thirty-five cases received chemotherapy or radiotherapy only. The clinical and pathological characteristics of lung cancer in youth, and the influence of surgical and non=surgical treatments to the patients with lung cancer are discussed. The authors consider that the bronchoscopic and sputum smear examinations are very helpful for diagnosing lung cancer in the early stage. Postoperative adjuvant chemo- and radio-therapy is suggested because of the high incidence of SCLC and adenocarcinoma in the young patients with lung cancer.
