Biomechanical analysis of rehabilitation in the standing position.

Biomechanical analysis of the two-dimensional models composed from roentgenographic pictures and electromyographic analysis of the shear force exerted on the tibia during standing on both legs were conducted in 21 young adult males. The simultaneous contraction of the quadriceps and hamstrings was observed in all electromyograms. Amplitude observed on electromyograms of the hamstrings increased as the trunk flexion angle increased. The calculated average values of shear force were negative at every knee flexion angle (negative value means posteriorly directed force). As the trunk flexion angle increased, posterior drawer force increased at knee flexion angles of 30 degrees and 60 degrees. The simultaneous contraction of the quadriceps and the hamstrings was considered to be the main factor that influenced these results. Standing on both legs with knee and trunk flexion was considered to be applicable in the early stages after anterior cruciate ligament reconstruction.

Compensatory muscle activity in the posterior cruciate ligament-deficient knee during isokinetic knee motion.

We performed electromyographic analyses in 12 patients with posterior cruciate ligament-deficient knees to compare electrical activity in the quadriceps, hamstring, and gastrocnemius muscles between the uninjured and involved limbs. Each patient performed concentric isokinetic knee motion at 30 and 60 deg/sec in both limbs separately. Torques in knee extension and flexion and surface electromyograms from the quadriceps, medial hamstring, and medial gastrocnemius muscles were simultaneously recorded. The uninjured limb served as a control for each patient. Before generation of flexion torque, the gastrocnemius muscle was electrically activated significantly earlier in posterior cruciate ligament-deficient knees than in uninjured knees at each velocity. However, we could not find any significant differences between the posterior cruciate ligament-deficient and uninjured knees in torque curves and electromyelograms of the quadriceps and hamstring muscles. This study suggests that early contraction of the gastrocnemius muscle may be a part of a compensatory mechanism in posterior cruciate ligament-deficient knees.

The effect of cyclic displacement on the biomechanical characteristics of anterior cruciate ligament reconstructions.

This study was conducted to clarify the influence of cyclic displacement on the structural properties of four types of femur-graft-tibia complexes used to reconstruct the anterior cruciate ligament. Forty hindlimbs from pigs were used. In two groups, bone-patellar tendon-bone grafts were secured with interference screws (group A) or the suture-post technique (group B). In two groups, multistrand flexor tendons were fixed using the tape-staple technique (group C) or the sutures-tied-over-a-button technique (group D). In each group, five femur-graft-tibia complexes underwent tensile failure tests without cyclic displacement. The other five complexes underwent 5000 cycles of cyclic elongation for 2 mm, and then underwent the tensile failure tests. The initial stiffness significantly decreased after cyclic displacement in each group, although there were no significant differences in the linear stiffness and the ultimate failure load between the tests with and without cyclic displacement. These findings suggest that 5000 cycles of repetitive elongation of the femur-graft-tibia complex by 2 mm does not jeopardize the graft fixed with the procedures used in this study, despite a slight but significant increase of an anterior-posterior laxity of the knee.

Association of mixed connective tissue disease, Sjögren's syndrome and autoimmune hepatitis: report of a case.

A 51-year-old female was admitted to the hospital due to liver dysfunction. She had suffered from arthralgia, Raynaud's phenomenon, dry mouth and dry eyes. Laboratory data showed positive LE cells and anti-RNP. The diagnosis of mixed connective tissue disease (MCTD) was made according to the criteria of the international symposium on MCTD and Antinuclear Antibodies (August 1986, Tokyo). Liver biopsy showed chronic active hepatitis. The results of sialography, lip biopsy and rose-bengal test were compatible with Sjögren's syndrome (SjS). This is the report of a rare case with MCTD associated with SjS and autoimmune hepatitis.

Trans-catheter hepatic arterial injection of lipiodol soluble anti-cancer agent SMANCS and ADR suspension in lipiodol combined with arterial embolization and local hyperthermia for treatment of hepatocellular carcinoma.

The clinical effect and safety of Lp-TAE alone and combined with radiofrequency (RF) capacitive hyperthermia (HT) were evaluated in 20 patients with hepatocellular carcinoma (HCC) associated with cirrhosis of the liver. After the oily carcinostatic agents were administered by Lp-TAE, HT, at a temperature of greater than 42.5 degrees C, was induced for 40 min, twice a week by an RF of 8 MHz for a total of 10 to 38 times. The response rate was 40% in the 10 cases that were treated with Lp-TAE combined with HT and 20% in the 10 cases that were treated with Lp-TAE. The patients who were treated with Lp-TAE combined with HT had a tendency to have better survival rates than those of the Lp-TAE group (p less than 0.099). The main side-effects of Lp-TAE combined with HT were low-grade fever, localized pain, myelo-suppression and liver dysfunction, but these were transient and eventually disappeared.

Vimentin expression in benign and malignant lesions in the human mammary gland.

The distribution pattern of vimentin in mammary gland lesions, including benign lesions (36 cases) and adenocarcinomas (40 cases), was studied with the use of a monoclonal antibody. In myoepithelial cells of mastopathy, the positive vimentin staining was seen in 6 cases (33.3%). Gynecomastia showed strong vimentin expression in myoepithelial cells. Fibroadenomas (14 cases) expressed positive vimentin staining in 8 cases (57.1%) with various staining intensities in myoepithelial cells. Lactating mammary gland tissue was positive for vimentin staining in narrow myoepithelium. In adenocarcinoma, positive reactions in tumor cells were found in 17 cases (41.2%) of solid tubular, 12 cases (33.3%) of papillo-tubular, and 11 cases (54.5%) of scirrhous carcinomas. The staining intensity for vimentin was weak in modified myoepithelial cells and neoplastic cells. Malignant cells in mammary carcinomas with vimentin reactivity usually expressed keratin staining; thus a co-expression of vimentin and keratin was found in mammary carcinomas.

Heterogeneity of keratin expression and actin distribution in benign and malignant mammary diseases.

Immunoreactivity of monoclonal anti-cytokeratin KL1, PKK1, K8.12 and anti-actin antibodies in 101 cases of diseased human breast lesions showed irregular keratin distribution in luminal cells of terminal ductal-lobular unit and basal layer cells of the interlobular and main duct. Actin staining was confined to myoepithelial cells. Benign lesions showed great heterogeneity in luminal cells of the terminal ductal-lobular units. Breast carcinoma showed a reduced staining for keratins, heterogeneity of keratin expression was found in solid tubular carcinoma, and actin was usually absent: however, papillo-ductal or comedo type had actin positive myoepithelial cells around carcinoma foci.

Abnormality of the post-proline-cleaving enzyme activity in mice with systemic lupus erythematosus-like syndrome.

Previous studies indicated the importance of hydrolytic enzymes in the pathogenesis of autoimmune diseases. In the present study, we examined the activities of such enzymes in various organs of the hybrids of the New Zealand Black and New Zealand White mouse (NZB/W mouse) as a laboratory model of human systemic lupus erythematosus. Of the 18 enzymatic activities tested, the activities of the post-proline-cleaving enzyme showed a particular behavior in the spleen of NZB/W mouse. The enzymatic activity progressively increased with age in contrast to the reverse tendency in the control. This phenomenon was not found in any organs other than the spleen. The activity of this enzyme showed a high level of correlation to that of proline-iminopeptidase in most organs tested from control animals. However, these correlations were almost completely absent in the spleen of NZB/W mice. This may suggest an important pathogenetic role for the post-proline-cleaving enzyme in immunological disturbances in this model animal.

Peritoneal implantation of endodermal sinus tumor of the pineal region via a ventriculoperitoneal shunt. Cytodiagnosis with immunocytochemical demonstration of alpha-fetoprotein.

A 17-year-old male developed an endodermal sinus tumor in the pineal region that metastasized to the peritoneal cavity via a ventriculoperitoneal shunt and caused ascites. Neoplasm was diagnosed by cytologic examination of the peritoneal fluid, and the precise histologic nature of the neoplasm was confirmed by immunocytochemistry, which demonstrated alpha-fetoprotein in the neoplastic cells.

Iliofemoral venous thrombectomy (follow-up studies of 88 patients).

A review of our 93 consecutive thrombectomies performed on 88 patients for acute and subacute iliofemoral venous thrombosis led us to the following conclusion. 1. Venous thrombectomy is an effective and safe operation and should be continued to be utilized in the management of iliofemoral venous thrombosis. 2. Thrombectomy should be recommended for in many types of patients, and the indication should include not only those whose symptoms occurred less than 2 weeks prior to the operation, but also those whose symptoms had been present for more than 2 weeks before surgery, in as much as the patients are suffering from severe pain, swelling and incapacity to walk. 3. The thrombectomy is useful for relief of complaints and plays an important role in the prevention of postphlebitic syndrome.

Combined chemoendocrine treatment with tegafur and tamoxifen for advanced renal cell carcinoma.

Ten patients with advanced renal cell carcinoma were given combined chemoendocrine treatment with tegafur and tamoxifen. Patients were orally administered 800 mg/body tegafur and 20 mg/body tamoxifen daily. One patient was assessed as CR, three as PR, two as NC and four as PD. One out of two patients with estrogen receptor (ER)-positive tumors and three out of four patients with ER-negative tumors responded favorably to this treatment. Side effects observed during treatment included nausea and vomiting (Grade I in three patients and Grade II in five), leukopenia (Grade II in four patients and Grade III in two), Grade II liver dysfunction in two patients, and slight pigmentation in two patients. This treatment can be considered efficacious for renal cell carcinoma.

Pharmacokinetic correlation between experimental and clinical effects on human non-small cell lung cancers of cis-diammineglycolatoplatinum (254-S) and cis-diamminedichloroplatinum.

We attempted to correlate the in vitro and in vivo antitumor activities of cis-diammineglycolatoplatinum (254-S), a novel platinum complex, and cis-diamminedichloro-platinum (CDDP) against the established culture cell lines and xenografts of human non-small cell lung cancer (NSCLC) with their clinical effects, based on the previous finding that the cytotoxicity of CDDP depends on the area under the curve (AUC). The concentration of 254-S and CDDP inhibiting the in vitro growth of 4 cultured NSCLC lines by 50% (IC50) was 0.82-7.8 and 0.53-4.2 micrograms/ml, respectively, showing a similar level. Of the 4 cell lines, only the most sensitive line, RERF-LC-AI, showed an IC50 close to a specific concentration (0.50 for 254-S and 0.32 micrograms/ml for CDDP) that reproduces in vitro the clinical AUCfree (24.8 and 5.34 micrograms-hr/ml) of the respective drugs. We treated 6 lines of human NSCLC xenografts implanted in nude mice with 254-S and CDDP at a particular dose (13.2 and 3.7 mg/kg) that is equivalent to the clinical doses with respect to the plasma AUCfree. 254-S and CDDP exhibited significant antitumor effects on 2 and 1 of the 6 lines, respectively. These in vitro and in vivo findings were considered to be relatively well correlated with the reported clinical response rates of 15-19% for 254-S and 14-15% for CDDP.

[Clinical features and long-term outcome of non-Q wave myocardial infarction in the elderly: comparison with Q wave myocardial infarction].

The clinical features and long-term outcome of non-Q wave myocardial infarction (NQMI) in the elderly were assessed in 24 patients with NQMI and compared with those in 48 patients with Q wave myocardial infarction (QMI). NQMI patients had a significantly lower maximal peak of serum creatine phosphokinase activity and lower incidence of pump failure during the acute phase. In-hospital mortality did not differ significantly between the NQMI and QMI patients. Evaluation of acute-phase coronary angiographic features within 6 hours of onset found a significantly higher incidence of infarct-related vessels in the NQMI patients, but the frequency of multivessel disease and the level of collateral flow did not differ between the two groups. Fifteen of the 24 NQMI patients and 34 of the 48 QMI patients underwent emergency coronary revascularization procedures of percutaneous transluminal coronary angioplasty (PTCA) or intracoronary thrombolysis. Successful recanalization was more frequent and the time to recanalization was shorter in the NQMI patients. The requirement for coronary revascularization (PTCA or coronary artery bypass graft) in the chronic phase for residual stenosis did not differ significantly between the two groups. Left ventricular ejection fractions were significantly better in the NQMI patients. The short- and long-term outcomes in elderly patients with NQMI and QMI were good and did not differ between the two groups, probably reflecting the active performance of revascularization in the acute and chronic phases.

A combined chemo-endocrine treatment with tegafur, adriamycin, methotrexate and tamoxifen for advanced renal cell carcinoma.

Eight patients with advanced renal cell carcinoma were given a new chemo-endocrine treatment with tegafur, adriamycin, methotrexate and tamoxifen. Estrogen receptor was detected in five cases from renal or metastatic tumors. The patients were medicated with 800-1, 200mg of tegafur and 20 mg of tamoxifen daily po and 20mg of adriamycin and 10 mg of methotrexate iv intermittently at two week intervals. Two patients were regarded as CR, two as PR, one as NC and three as PD. Two out of three with estrogen receptor and one out of two without estrogen receptor responded favorably to this treatment. Side effects observed during the treatment were Grade II nausea/vomiting in six, Grade II leukopenia in three, Grade I thrombocytopenia in two, and Grade I hepatoxicity in three patients. The patients were found to be enjoying a good quality of life during the treatment because of lowered toxicity. This treatment can be regarded as a good treatment modality for advanced renal cell carcinoma.

Antitumor and endocrine effects of an aromatase inhibitor (CGS 16949A) on DMBA-induced rat mammary tumor.

The antitumor and endocrine effects of a new nonsteroidal aromatase inhibitor were studied using DMBA-induced rat mammary carcinomas. CGS 16949A (CGS) was administered orally once daily for 3 weeks. A marked antitumor effect was noted at doses of more than 1 mg/kg, and weight gain was dose dependent. Histologic examination showed atrophic changes of the tumors and decreased expression of estrogen receptor levels. To investigate the mechanism of the antitumor effect of CGS, we studied the changes in endocrine function in rats given this agent. In the groups receiving more than 1 mg/kg of CGS, the serum estradiol and prolactin levels were lower and the serum luteinizing hormone level was higher than in the control group. In rats treated with CGS, ovary weight was increased, while uterus and pituitary weights were decreased. These changes were dose dependent. In contrast, the serum corticosterone level and adrenal weight remained unchanged. We conclude that CGS inhibits the growth of hormone-dependent tumors by changing the hormonal environment.

Single vertebral compression and involvement of the posterior elements in tuberculous spondylitis: observation on MR imaging.

To examine the atypical presentation of tuberculous spondylitis we evaluated seven cases seen in our hospitals during a five-year period (1989-1994) with procedures including MR imaging and CT scan. Three of the seven cases showed collapse of the single vertebral bodies with relatively spared disk spaces. Posterior elements were also involved on MR imaging in three cases, but the involvement of adjacent disks and vertebral bodies militated against metastases.
