Medullary thyroid carcinoma and pheochromocytoma accompanied with nodular hyperplasia in multiple endocrine neoplasia type 2.

Three patients with familial multiple endocrine neoplasia type 2 underwent total adrenalectomy and subsequently near-total or total thyroidectomy. The overt pheochromocytomas were present bilaterally in two patients and unilaterally in another patient. In addition, multiple nodules measuring a few mm in diameter were demonstrated in all adrenal medullas examined. All patients exhibited bilateral medullary thyroid carcinomas which varied in size from a few minute nodules accompanied by the microscopic C-cell hyperplasia to distinct large tumors. The thyroid C-cell hyperplasia appeared to be confined to the upper and middle thirds of the lateral lobe, and micronodules of the C-cell hyperplasia enlarged to coalesce, forming one large carcinoma mass. On the other hand, nodular hyperplasia of the chromaffin cells was distributed diffusely throughout the adrenal medulla and each nodule appeared to develop individually into a large pheochromocytoma. These findings suggested that, at least in the family members at a high risk for multiple endocrine neoplasia type 2, the development of both medullary thyroid carcinoma and pheochromocytoma was always preceded by a multicentric nodular hyperplasia. On the basis of the pathogenesis of these tumors, the most rational surgical approach was presented.

A retrospective study on the prognosis of aged patients with epilepsy.

A retrospective follow-up study on the prognosis of 90 patients with epilepsy above 40 years of age has resulted in: 1. In the aged patients, the duration of epilepsy (mean: 23 years) was longer than the period of treatment they underwent (mean: 14 years). The delayed commencement and/or interrupted medication along with the patients' incompliance of taking drugs accounted for the main reasons for at least 70% of the patients. 2. The partial epilepsy (80%) especially with complex symptomatology (55%) were predominating in the aged group. And also, that the tonic-clonic seizures were liable to convert to the complex partial seizures in their thirties and forties, was found in this study. 3. Although almost all the patients (97%) had more or less clinical attacks even at the time of survey, 75% or more decrease in seizure frequency have been exerted for one and a half year, regardless of the seizure types in the majority of the patients (81%). This seems to offer an explanation that an appropriate sociomedical strategy is necessitated for patients with long-standing epilepsies.

[Electroencephalography of Moyamoya disease].

Attempts were made to evaluate the EEG findings in 16 children and 12 adults with Moyamoya disease. (1) The children revealed specific findings such as hemispheric posterior slow (HP slow), centrotemporal slow (CT slow) and re-build-up after the end of hyperventilation. (2) HP slow was mainly observed in EEG examined within one year after the initial onset. In children in whom EEG was performed more than three years after the onset, chronic suppressive findings were found on EEG. (3) Buildup after the end of hyperventilation was revealed in almost all the children, which we called "Re-build-up". (4) No specific findings were verified on EEG in adult Moyamoya, beside slight abnormalities.

Inhibition of leucocyte migration by potassium chloride extracts of human tumor cells in ascites and from tissues.

Human gastric cancer-associated antigens were solubilized from fresh ascites and surgical specimens by 3M KCl extraction. The antigenicity of these extracts was measured by leucocyte migration inhibition assay with an improved modification devised in this laboratory. Antigens extracted mutually reacted with autologous and allogeneic leucocytes taken from cancer patients. The out of 11 (27%) patients and 7 out of 28 (25%) patients with various cancers showed positive migration index values against 13 ascites tumor cell antigens in the autologous and allogeneic reaction systems, respectively. Reactions were positive in 4 out of 10 patients with gastric cancer against autologous antigens, and in 10 out of 26 (38%) of the patients against allogeneic tumor antigens. On the other hand, one patient with gastric cancer reacted well with various sources of antigens extracted from allogeneic cells of gastric cancer, while the other patient with gastric cancer did not react with any of five gastric cancer antigens tested so far. These findings suggested that the cell-mediated immune response of patients with gastric cancer against autologous and allogeneic tumor antigens extracted was detectable in the test system. It is considered at present that the eM KCl antigen may be relate to human leucocyte antigen, and some common antigenic reactants capable of reacting with several gastric cancer may be involved in this reaction.

Morphologic demonstration of tubular obstruction in acute renal failure.

We used the qualitative Hanssen technique in albino rats to seek morphologic demonstration of tubular obstruction in two types of acute renal failure: one induced by folic acid and another by methemoglobin. Immediately after the intravenous injection of folic acid, 250 mg/kg body weight, the animals became almost anuric. Two to three hours after the injection, sodium ferrocyanide remained within the proximal convoluted tubules. After the intravenous injection of methemoglobin, 0.5 to 1.0 g/kg body weight, the animals became oliguric but not anuric. Sodium ferrocyanide injected with methemoglobin was seen mainly in distal tubules and collecting ducts 2 to 3 hours after the injection. The degree of tubular dilatation was more marked in the former model than in the latter, in agreement with the degree of oliguria. These morphologic findings were taken to indicate that the above two types of acute renal failure were caused by tubular obstruction rather than by intrarenal vasoconstriction and subsequent cessation of glomerular filtration. Treatment of intractable ascites by continuous reinfusion of the sterilized, cell-free and concentrated ascitic fluid.

The new method for continuous reinfusion of sterilized, cell-free and concentrated ascitic fluid is described, and utilized in 72 patients with intractable ascites in both malignancy as well as liver cirrhosis and is described with satisfactory results. The management by repeated ascites reinfusion of patients with benign massive ascites has been possible. This method is capable of being applied to patients with malignant ascites. Symptomatic relief and prolonged survival time is anticipated. The method described in this study is simple and free of adverse effects.

Use of profound hypothermia induced by surface cooling in open-heart surgery.

Profound hypothermia with body-surface cooling for use in open-heart surgery is considered a difficult anesthetic technique because of the problems of controlling ether anesthesia. This paper describes our hypothermia technique in detail, with emphasis on its particular usefulness in open-heart procedures in neonates and infants. The results are based on our experience with 281 consecutive patients. Guidelines for safe and effective performance of this technique were established on the basis of continuous monitoring of the electrocardiogram, electroencephalogram, and arterial blood pressure. We conclude from our experience that open-heart operation under profound hypothermia is a safe, effective, and extremely promising method.

Local prognosis after combined external and interstitial radiation therapy for carcinoma of the tongue.

Of 36 patients with early carcinoma of the tongue treated with interstitial irradiation, 25 were controlled, 10 recurred and one developed local necrosis. Of 57 advanced cases treated with telecobalt therapy followed by interstitial irradiation, 15 were controlled, 20 recurred and 22 developed local necrosis. The frequency of necrosis following the combined irradiation was unacceptably high. Interstitial therapy alone may be a more adequate method for the primary tumour.
