The treatment of Perthes' disease.

The results of surgical and conservative treatments of a total of 235 patients with unilateral Legg-Calvé-Perthes disease are reported. Results of the two modes of treatment with those of an untreated patient group are presented. The long periods of follow-up have shown that surgical treatment yielded better results than the weight-relief of the hip by a splint. Following femoral osteotomy, the course of the disease became more rapid. Based on their experiences, authors recommend the surgical treatment of Legg-Calvé-Perthes disease.

[Elongation of the leg with the Kawamura-method (author's transl)].

Results of leg lengthening with the Kawamura method are reported here. This procedure was in 8 cases, where a 2,8--4,0 cm (i.e. 10%) lengthening was achieved. There were 2 transient peroneal palsies and in one case bony union was achieved with autologous bone transplantation for delayed healing. In correctly selected cases even 3--8 cm shortening can be safely managed with this method.

Nerve cells of the rabbit, cat, monkey and human caudate nucleus: a Golgi-study.

The Golgi architecture of the nucleus caudatus in rabbit, cat, monkey and human material was analysed. The brains were prepared by Golgi--Kopsch impregnation. The fairly divergent types of neurons practically belong to two main groups: projective or efferent neurons and local- or interneurons. The efferent neurons -- as it has been confirmed by experimental studies -- establish far contacts with various regions of the brain. According to the Golgi picture, their axon emits several collaterals, which produce local connections. The interneurons have only locally arborizing axon. The are mainly responsible for intrinsic connections of the nucleus. In both main groups of caudate neurons, several types and subtypes were observed. The number of the types and subtypes of the neurons show an increasing tendency in the phylogenetic series.

[Development in the treatment of congenital dislocation of hip (author's transl)].

Authors review the development in the treatment of congenital dislocation and dysplasy of hip and demonstrate the results with various methods. Results with treatment according to Lorenz (1040 cases) were unsatisfactory because of frequent occurance (66.3 percent) of osteochondritis. The treatment with Frejka cushion (360 cases) did not improve basically the results. Pavlik's strapping, especially when started under 1 month of age gave excellent results (3156 cases). In operative treated the results of 332 Colonna operations were unsatisfactory because of consecutive deformation of the hip and restriction of movements. Open reduction with varisation derotation osteotomy (474 cases) provided good acetabular development especially in cases where articular cartilage and upper limbus could be saved. Salter's operation is successful when proper indications are considered, but should be performed only after 1--2 years of observation of the acetabular development in non improving cases. The Chiari procedure can improve the weight bearing surfaces in cases of steep acetabular roof flat acetabulum and consecutive subluxation. In 557 cases of varisation derotation osteotomy subcapital coxa valga occured frequently. Basic principle of treatment remains prevention, early recognition and treatment after birth.

The mechanism of genetic predisposition in congenital dislocation of the hip.

The important role of polygenic acetabular configuration and monogenic joint laxity has again been proved in the aetiology of congenital dislocation of the hip. According to the findings reported these two genetic predispositions seem to be unrelated. The time of diagnosis in accetabular dysplasia type and joint laxity type did not differ, thus the neonatal and late-diagnosed cases do not seem to be two clear-cut entities.

Two family studies on congenital dislocation of the hip after early orthopaedic screening Hungary.

Two family studies involving 1767 and 379 index patients in Budapest and Bekes county, respectively, were undertaken to examine the effect of early orthopaedic screening on the recurrence risk of congenital dislocation of the hip. About 14%, 2.1-2.3%,1.2-1.4%, and 4.7-6% of sibs, parents, uncles and aunts, and cousins, respectively, had congenital dislocation of the hip in these two surveys. The recurrence risks were eight-fold and four-fold higher in brothers and sisters, four times higher in parents, 2.5-fold higher in uncles and aunts, and 2.0-2.5 times higher in cousins, respectively, than in the general population. This family pattern seems to fit best with a model of polygenic-multifactorial inheritance. In earlier studies higher recurrence risks were found. These may be explained by the change of diagnosis due to early orthopaedic screening which may increase the possibility of over diagnosis and the treatment of mild cases which previously recovered spontaneously.
