Histological evaluation of radiochemotherapy for prostatic cancer: early results of a pilot study.

Antiandrogen therapy has been widely used for prostatic cancer, yet many authors have reported on the limited efficacy of hormonal therapy for this disease. The present report is based on an histological evaluation of chemotherapy and radiotherapy in combination for prostatic cancer. To determine the usefulness of this combined therapy, 15 prostatic cancer patients were treated with a combination of Bleomycin and Adriamycin (BA therapy), or with only Adriamycin. Radiotherapy was administered to all cases. Twelve of the 15 patients were evaluable histologically with effects ranging from Grade I to Grade IV in Shimosato's classification. We conclude that further investigation, including a randomized study of hormonal therapy, is necessary.

Inverted papilloma with malignant change of renal pelvis.

A case is presented of a rare type of renal pelvic tumor. Morphologically the structure of the lesion resembled an inverted papilloma of the urinary bladder, but the tumor cells composing the lesion underwent malignant transformation.

Radiation-induced bladder carcinoma.

Two cases are presented of radiation-induced bladder carcinoma which followed prior irradiation for cervical carcinoma of the uterus. One was a sixty-eight-year-old woman with bladder carcinoma fourteen years after irradiation (total dose of 4,500 rad) for cervical carcinoma of the uterus. The other was a sixty-four-year-old woman with bladder carcinoma twenty-five years after irradiation with 150-K volt apparatus for cervical carcinoma of the uterus. From the late radiation change of the skin, it was estimated that the total dose of prior radiation might be 4,000 rad or more. Both had high-grade, high-stage transitional cell bladder carcinoma, and the former was with marked mucus-forming adenomatous metaplasia.

Carcinosarcoma of urinary bladder.

The case of an eighty-two-year-old man with carcinosarcoma of the urinary bladder is presented. Morphologically, coexistence of two elements, transitional cell carcinoma and proliferation of spindle-shaped cells, is demonstrated, between which the transition is observed. The literature is reviewed, and the histogenesis of the sarcomatous element is discussed.

Eosinophilic ureteritis? Regional ureteritis with marked infiltration of eosinophils.

The case of a thirty-seven-year-old man with eosinophilic granuloma of the ureter is discussed. Preoperative x-ray studies disclosed distal ureteric regional stenosis and moderate hydronephrosis with small stone shadows. A segmental ureterectomy and vesicoureteral neostomy were performed. Pathologic examination of the removed specimen revealed markedly infiltrated eosinophils of the ureteric wall. The literature is reviewed and the causation of eosinophilic granuloma discussed.

Inverted papilloma of bladder. Two cases with questionable malignancy and squamous metaplasia.

Two new cases of inverted papilloma of the urinary bladder are presented: one is a benign tumor with marked squamous metaplasia, and the other shows questionable malignancy in a single polypoid mass. This malignant transformation cautions against overconfidence in the benign nature of inverted papilloma of the urinary bladder. The authors believe that careful pathologic examination is necessary for inverted papilloma of the bladder.

Intravesical instillation of adriamycin combined with low-dose irradiation for superficial bladder cancer.

Ten patients with superficial low-grade bladder cancer were treated with intra-vesical instillation of doxorubicin hydrochloride (Adriamycin) with low-dose irradiation (total dose of 800 rads), and the therapeutic effect was evaluated clinically and histopathologically. In clinical evaluation based on cystoscopic findings, all 10 patients showed marked response to the combination therapy. In 3 patients the main tumors disappeared, in 6 the tumors were reduced by 50 per cent or more with or without disappearance of daughter tumors, and in 1 the tumor was reduced by 50 per cent or less with disappearance of daughter tumor. Histopathologically the tumor cells could not be confirmed in 3 patients; the tumor cells were almost nonviable with some viable cells in 2; the tumor cells were almost viable with some nonviable cells in 4; and 1 showed no effect.

Effect of diabetic sera on the conversion of eicosapentaenoic acid (EPA) to prostaglandin I3 by cultured bovine aortic endothelial cells.

The effects of sera from diabetic patients and healthy donors on the synthesis of prostaglandin I2 (PGI2) and PGI3 in vitro were studied in confluent bovine aortic endothelial cells cultured with EPA. The products 6-keto-PGF1 alpha and delta 17-6-keto-PGF1 alpha were measured by GC/SIM as markers of PGI2 and PGI3 formation in growth medium after 60 min of incubation. PGI2 and PGI3 synthesis with 10% diabetic sera were less than with sera from healthy donors (p < 0.05). However, the total prostacyclin production (PGI2 and PGI3) in the cell cultures incubated with 10 microM EPA and 10% diabetic sera approximated that of the cultures incubated with the sera of healthy donors without EPA. These results suggest that the diabetic sera inhibits PGI2 and PGI3 synthesis in the cultured endothelial cells, and that EPA intake may reduce the complications of diabetes mellitus, such as microangiopathy and vaso-occlusive diseases, and enhances the production of PGI3 which seems to exert a strong anti-aggregatory effect.

A study of the electrical bio-impedance of tumors.

A new system of impedance measurement over a frequency range of 0 to 200 kHz was developed by a three-electrode method. In this study, the electrical impedances of various tumors were measured in vivo in 54 patients with breast disease (31 breast cancers, 13 fibroadenomas, and 10 fibrocystic diseases) and 57 patients with pulmonary disease (44 lung cancers, 5 metastatic pulmonary tumors, 4 pulmonary tuberculoses, and 4 organized pneumonias). On the basis of those impedance measurements and the equivalent circuits in vivo, we calculated the extracellular resistance (Re), intracellular fluid resistance (Ri), and cell membrane capacitance (Cm) in tissues, all of which were compared among the various diseases. It was found that Re and Ri were significantly higher in breast cancers than in benign tumors and normal breast tissues and that Cm was significantly lower in breast cancers than in other tissues. On the other hand, Re and Ri were significantly higher, and Cm was significantly lower, in normal lung tissues than in pulmonary masses. Re and Ri were significantly higher, and Cm was significantly lower, in malignant tumors than in organized pneumonias. The results showed that these parameters (Re, Ri, and Cm) exhibit significant differences among various tissues and tumors, suggesting possible applications in tumor diagnosis.
