In vitro study of antral gastrin biosynthesis in response to weaning and corticosterone acetate in rats.

The effects of weaning (abrupt dietary changes from breast milk to solid food) and corticosterone injection on antral gastrin-like immunoreactivity (G-LI) concentrations and antral G-LI biosynthesis were studied in rats. A single dose of corticosterone acetate was injected in one group of 7 days old rats, and a single dose of physiological saline was injected in another. Each group of rats was divided into two subgroups, one fed only rat breast milk until 25 days old and the other weaned at day 21. In non-corticosterone treated unweaned rats, antral G-LI did not increase. In non-corticosterone treated weaned rats, antral G-LI was constant before weaning, then increased 4-fold to the adult level. In corticosterone treated unweaned rats, the antral G-LI on day 11 was twice than on day 7, and thereafter remained constant. In corticosterone treated weaned rats, antral G-LI increased after corticosterone treatment and increased again after weaning to reach the adult level at day 25. Gel filtration of pulse-chase incubated antral samples with L-[methyl-3H]methionine from unweaned rats without corticosterone administration showed Vo, fraction 19 (Fr. 19) and gastrin-34 (G34) peaks, but no gastrin-17 (G17) peak after 60 min of chase incubation, but at 120 min of chase incubation a G17 peak was present; corticosterone-treated and/or weaned (solid food alone) rat samples showed Vo, Fr. 19, G34 and G17 peaks at 30 min of pulse incubation.

Auer rods in mature granulocytes and monocytes.

In a 73-year-old man with acute myelomonocytic leukemia (M4 on FAB classification) Auer rods were demonstrated in mature granulocytes and monocytes in the blood as well as the bone marrow. Cytochemically, a significant number of leukemic blasts were positive for both alpha-naphthyl-acetate esterase and naphthol-AS-D-chloroacetate esterase. Auer rods in mature granulocytes were associated with immature cytoplasm and not infrequently with pseudo-Pelger anomaly. The significance of these observations is discussed.

Nodular sclerosing Hodgkin's disease with massive skin invasion.

Skin invasion is a rare manifestation in cases with Hodgkin's disease and if present mortality rate is very high. In general, almost all patients with advanced skin lesions die of the underlying disease in spite of any chemotherapy, however in case the patient recovered after intensive chemotherapy and irradiation. Moreover, the type of skin manifestation in this case is unique and quite different from the usual. This report describes a well-controlled case of Hodgkin's disease with massive and atypical skin invasion.

Development of antral gastrin-like immunoreactivity and pituitary CCK8/gastrin-like immunoreactivity in rats.

Gel filtration of antral extract of adult rats revealed gastrin-17 and gastrin-34. Gel filtration of anterior pituitary extract showed CCK8 and gastrin-17, whereas posterior pituitary extract showed only a CCK8. Antral gastrin-like immunoreactivity (G-LI) increased after milk feeding and especially after weaning was started. Changes in diet may exert a profound influence on the ontogenic development of antral G-LI, but not pituitary CCK8/G-LI.

Gastrin/cholecystokinin-like immunoreactivity in human blood cells.

In the gastrin and/or cholecystokinin-like immunoreactivity (G/CCK-LI) elution patterns of blood cells in human adults, erythrocyte (RBC) elution pattern has three peaks which are coeluted with gastrin-34 (G34), gastrin-17 (G17) and Vt, and polymorphonuclear leukocyte (PMN) and mononuclear cell (MNC) elution patterns have four peaks which are coeluted with Vo, G34, G17 and Vt. The content of G/CCK-LI in RBC is 1.20 +/- 0.54 f mole/10(8) cells (mean +/- SD). That in PMN and MNC is 1.44 +/- 0.67 p mole/10(8) cells and 1.67 +/- 0.76 p mole/10(8) cells, respectively.

Granulopoiesis in patients with congenital neutropenia.

Granulopoiesis was investigated in five patients with congenital neutropenia (CN) (one Kostmann type, four benign forms). In semisolid agar culture, the marrow cells of all five patients produced normal numbers of CFU-c (colony-forming unit-culture). The size and classification of colonies were normal. In suspension culture in vitro with exogenous colony-stimulating factor (CSF) generated from omental-conditioned medium (OMCM), the myeloid precursors of all patients could proliferate and differentiate into normal polymorphonuclear neutrophils (PMNs). But in the absence of exogenous CSF, myeloid precursors of the patient with Kostmann-type CN did not proliferate or differentiate into PMNs at all. In the four patients with benign neutropenia, however, PMNs were found even without exogenous CSF similar to normal individuals. These results suggest that patients with CN may have normal granulopoietic stem cells with normal proliferative and differentiating capacity in response to exogenous CSF. When a small amount of normal human serum was added to normal marrow cultures stimulated by exogenous CSF, the colony growth increased in a superadditive manner. The enhancing activity of serum from neutropenic patients differed from that of normal serum. Especially, the addition of serum from the patient with Kostmann type CN to normal marrow cultures did not show this enhancement effect. The sera of patients with benign neutropenia had less enhancement effect than did normal control serum. These findings might be interpreted as showing an imbalance between CSF enhancer and inhibitors in the patients' serum.

Effect of vitamin K3 on the first-pass elimination of propranolol in the rat.

The effect of vitamin K3, which inhibits electron transfer from NADPH to cytochrome P-450 via cytochrome c reductase, on the metabolism of propranolol was studied in the rat in vitro and in vivo. For in vitro experiments using rat liver microsomes, the metabolism of propranolol was inhibited by vitamin K3. For in vivo experiments, the area under the blood concentration-time curve and the maximum concentration of propranolol were markedly increased following intraportal infusion of propranolol with vitamin K3, but intraportal administration of vitamin K3 had little effect on propranolol kinetics when propranolol was infused intravenously. Thus, it was concluded that vitamin K3 inhibited the first-pass metabolism of propranolol.

Primary hypothyroidism associated with pituitary enlargement, slipped capital femoral epiphysis and cystic ovaries.

A case of primary hypothyroidism with pituitary enlargement, slipped capital femoral epiphysis and cystic ovaries is reported. The pituitary abnormality and cystic ovaries disappeared dramatically after thyroid hormone therapy. Hip pinning was performed. The recognition of these associations may eliminate unnecessary surgery and lead to the choice of hormone replacement therapy.

Cushing's disease: pituitary microadenoma demonstrated by high resolution computed tomography with thin slices.

A girl 16 7/12 years of age with Cushing's disease secondary to pituitary microadenoma is described. The tumor was very clearly demonstrated by high-resolution contrast-enhanced computed tomography (CT) with thin slices and was removed by transsphenoidal microadenomectomy. In some patients with Cushing's disease levels of ACTH and cortisol are not always high. Therefore, high-resolution contrast-enhanced CT with thin slices should be performed when the diagnosis of Cushing's disease is suspected even when the definite diagnosis cannot be established by endocrinological examinations.

Hyperphenylalaninaemia due to impaired dihydrobiopterin biosynthesis: leukocyte function and effect of tetrahydrobiopterin therapy.

We have described the clinical and biochemical status of two patients with tetrahydrobiopterin (BH4) deficiency due to impaired dihydrobiopterin biosynthesis. BH4 administration appeared to improve the mental and psychological status more than did neurotransmitter replacement therapy alone. This enhancement of activities of daily life was seen with a dose of BH4 as low as 1.25 mg kg-1 day-1. Granulocyte adherence capacity was below normal and recovered after BH4 therapy in both patients. B-cell differentiation capacity was altered either before or after therapy.
