[Pathomorphogenesis of radio-ulnar synostosis in fractures of forearm bone shafts].

Pathomorphogenesis of radioulnar synostosis which occurred in 4 out of 174 patients treated because of recent and inveterate fractures as well as pseudoarthroses within shafts of forearm bones has been presented. Fractures of both radius and ulna, located at the same level within proximal half, inveterate or neglected, after severe or multiple trauma are at the highest risk of developing radioulnar synostosis.

[Analysis of hand function deficit due to the limitation of forearm rotational movements].

An influence of the forearm rotational movements limitation on everyday and professional life has been presented. The investigation has been carried out with an apparatus simulating various degrees of supination or pronation loss. It almost impossible to compensate for the loss of supination so it affects the function of the hand more then the loss of pronation.

[Mercury pulmonary embolism after voluntary injection].

We describe a case of pulmonary emboli due to mercury in a female drug addict who was HIV positive and had voluntarily injected herself with mercury. The diagnosis was guided by the history and by the characteristics of the chest radiograph and was confirmed by blood and urine levels of mercury and the radiograph taken of the site of the intravenous injection. Three years after the injection the patient did not reveal any signs suggesting disturbance due to the chronic pulmonary exposure to mercury, but there was a reduced carbon monoxide transfer, whose interpretation was difficult due to the past respiratory history and to the drug addiction.

Pulmonary lymphangiomyomatosis. Follow-up and long-term outcome with antiestrogen therapy; a report of eight cases.

Lymphangiomyomatosis is a rare disease which affects young women of childbearing age. Eight women with pulmonary LAM were treated with antiestrogen therapy and were monitored by blood estrogen measurements along with clinical hypoestrogenic symptoms. Treatment ranged from three to nine years. The response to therapy was evaluated by the clinical course, chest x-ray films, pulmonary function tests and overall long-term outcome. Three patients died of respiratory failure after three, five and nine years of treatment. Of the five patients remaining alive, respiratory function deteriorated in four cases, after a transient period of mild improvement lasting three years in two cases. The last patient appeared stable after three years of follow-up. Time course ranged from 4 to 17 years. However, without a control group, we cannot determine whether or not the apparent improvement of the natural time course was due to the hormonal treatment.

Variation of the median nerve in the forearm and the wrist.

Rare type of division of the median nerve in proximal 1/3 of the forearm was described. The finding was made during autopsy of 53 years old white man. A nerve branch that run separately from the main trunk of the median nerve appeared to be the common digital nerve to the 3rd and 4th finger. Between this branch and the main trunk the radial head of the flexor digitorum superficialis muscle was situated, that eventually converged into the tendon of the superficial flexor of the middle finger.

[Wegener's granulomatosis. Therapeutic indications and follow-up course. Apropos of 5 cases].

Five cases of Wegener's granulomatosis treated with cyclophosphamide (CPM) and prednisolone are reported. Four of these patients received intermittent intravenous boluses of cyclophosphamide with the aim of improving the prognosis and renal function and at the same time to attenuate any haematological or vesical toxicity of CPM. The initial response to treatment by boluses of CPM was favourable in all cases but three patients presented with relapses, which were sometimes repeated and boluses of CPM did not enable a remission to be maintained at the time. Recourse to continuous oral therapy in place of bolus therapy proved viable for the maintenance of remission in two cases. The bladder and haematological tolerance to the bolus was satisfactory but an episode of severe neutropenia led to an adaptation of the dose of CPM. The intermittent administration above all the low cumulative dose of CPM obtained with the boluses will explain the better vesical and haematological tolerance observed depending on the capacity to maintain a prolonged remission. The indications for boluses of CPM in the treatment of Wegener's granulomatosis remain uncertain and do not seem to be totally without inconvenience.

[Severe varicella pneumopathy in health adults. Apropos of a case].

The authors report a new case of respiratory distress syndrome which occurred as complication of varicella in an otherwise healthy adult patient. The case is well documented by images and immunological studies which are discussed in the light of recently published data.

[In vitro study of causes of radiation caries].

50 impacted third molars were surgically removed and each tooth was divided in half. One half of each tooth was irradiated with 60 or 120 Gy. Exposure to irradiation did not affect the hardness of enamel or dentin.

Rabbit lung after inhalation of lithium chloride.

Rabbits were exposed to aerosols of lithium chloride in metal concentrations of 0.6 and 1.9 mg/m3 (mass median aerodynamic diameter of 1 micron) for 4-8 weeks, 5 days/week, 6 h/day. The lungs were studied by light and electron microscopy, with particular reference to inflammatory changes, structure of alveolar macrophages and alveolar epithelial cells. Macrophages recovered by lung lavage were studied by light and electron microscopy and their oxidative metabolic activity was measured. The content of phospholipids was analysed in lung tissue. Exposure to lithium produced no significant effects. It thus seems that Li+ is less toxic to the lung than the other metals investigated with the same test system, e.g. Ni2+, Cd2+, Co2+, Cr3+ and Cr6+.

Detection and semiquantitative determination of antibodies against Plasmodium falciparum in human blood serum: NBT-reduction by polymorphonuclear leukocytes.

Nitroblue tetrazolium (NBT) reduction by polymorphonuclear leukocytes (PMN) has been employed for the detection of specific opsonizing antibodies against Plasmodium falciparum in sera from individuals exposed to malaria parasites. Specific antibody-antigen complex is known to trigger the metabolic activation of normal PMN, as measured by NBT-test. In the sera from 16 out of 17 patients tested the NBT-reduction of normal PMN was, in the presence of P. falciparum antigen, significantly higher than that obtained with pooled normal serum from individuals without malaria background. This enhancement was more pronounced in the presence of complement. NBT-reduction was elevated to a lower extent when human anti-P. falciparum sera were substituted with anti-P. vivax or P. ovale sera. Furthermore, no enhancement was noted when red blood cells lysate was used as antigen. The results indicated the presence of specific opsonizing antibodies against P. falciparum in the patient sera. Oxygen-derived free radicals formed by PMN during the stimulation are suggested as the neutrophil mediated protection against malaria.

Selenium effects on human neutrophilic granulocyte function in vitro.

The effects of an inorganic selenium salt on phagocytic functions of human neutrophilic granulocytes from donors with a low activity of glutathione peroxidase have been investigated. Granulocytes were exposed for 60 min in vitro to sodium selenite in two physiological concentrations (100 and 200 ng Se/ml) and one unphysiologically high concentration (2000 ng/ml). The spontaneous and chemotactic migration, the nitroblue tetrazolium reduction, the phagocytosis of fluorescein-labeled yeast particles and the intracellular killing of staphylococci were then studied in such granulocytes and compared to control cells, which had not been exposed to selenium. The migration and nitroblue tetrazolium reduction abilities of granulocytes were not affected by selenium exposure. The phagocytic and bactericidal activities were significantly increased in granulocytes exposed to selenium in physiological concentrations. However, at 2000 ng Se/ml these activities were found to be equal to or lower than control levels. Thus selenium supplementation might enhance phagocytic and bactericidal functions of human granulocytes, thereby improving the host defense against bacterial infections.

[Thoracic actinomycoses. General illustrated review of 4 surgical cases].

Actinomycosis of the lung is rare and difficult to diagnose. Its good prognosis after medical treatment contrasts with the high frequency of surgical excision, due to the fact that pre-operative diagnosis is exceptional. Following a review of the recent bacteriological, radiological and therapeutic data available in the literature, 4 personal cases are presented. In all 4 cases actinomycosis was diagnosed post-operatively on the presence of yellow grains in the histological specimens. In 2 cases surgical excision proved impossible, but an almost complete radiological recovery could be obtained after prolonged treatment with co-trimoxazole. This shows how important it is to be aware of the disease, to improve its diagnostic approach and to remember its sensitivity to antibacterial agents, even in well organized forms.

Serum opsonic activity after pneumococcal vaccination in splenectomized individuals as measured by a nitroblue tetrazolium (NBT) reduction micromethod.

19 children who were splenectomized after traumatic rupture and 15 healthy siblings were vaccinated with a 14-valent pneumococcal vaccine (Pneumovax). The serum opsonic activity against types 3, 6A, 7F, 14, 19F and 23F was evaluated before the vaccination and after 2 and 12 months by a new micromethod based on nitroblue tetrazolium reduction of granulocytes. Before the vaccination, opsonic activity was found more often in the splenectomized than in the controls, 38 and 29%, respectively. However, patients and controls responded similarly to the vaccine. The serum opsonic activity changed from negative to positive after vaccination in 31% of the splenectomized, as well as the controls. The results indicate that the protection against pneumococcal infection after vaccination may not be adequate against all pneumococcal types in all patients.

NBT-reduction for detection of specific, opsonizing antibodies against haemophilus parainfluenzae.

Specific, opsonizing antibodies against Haemophilus parainfluenzae were demonstrated in serum by measuring their enhancing effect on the nitroblue tetrazolium (NBT)-reduction of normal human granulocytes, phagocytizing the homologous bacteria. Patients with septicemia and/or endocarditis and carriers of H. parainfluenzae were studied. In late phase sera of the first group such antibodies were detected in 6 of 9 cases. Opsonizing antibodies against H. parainfluenzae were not detected in any of the carrier sera. A dose-response effect was observed when a positive late phase serum was tested in serial 2-fold dilutions. The enhancement of the NBT-reduction was eliminated by absorption of serum with homologous antigen. The detection of opsonizing antibodies, essential for the immune defense against H. parainfluenzae might be helpful for evaluation of the pathogenicity of this organism, which so far is considered as a rare causative agent.
