[A case with hepatocellular carcinoma with complete remission due to gamma-IFN therapy].

A 65-year-old male was admitted to our hospital with the aim of closer examination of SOL in cirrhotic liver. By abdominal echography, CT and angiography etc., he was diagnosed as LC with multiple HCC, one in S5 about 2.0 cm, another in S1 about 5.0 cm in diameter, and other four small nodules in right lobe. We treated him with recombinant gamma-IFN at dose of 1.6 X 10(7) units/day for 5 consecutive days biweekly. Although until two months the tumours were gradually enlarged in size, then they became smaller, and at five months later after the beginning of gamma-IFN therapy the tumor stains in angiogram disappeared. Then we performed the surgical treatment. Both macro and microscopical findings of tumor specimens in S5, 2. 2 X 2.0 X 1.9 cm in size, showed complete necrotic tissue. Additionally the yellowish small tumor was found on surface of S8, and it was adenomatous hyperplasia histologically.

Carrier-mediated transport system for cephalexin in human placental brush-border membrane vesicles.

The uptake of cephalosporin antibiotics, cephalexin, was studied with brush-border microvillous plasma membrane vesicles prepared and purified from human full-term placental syncytiotrophoblasts. The uptake of cephalexin by the membrane vesicles was not stimulated in the presence of an Na+ gradient from the outside to the inside of the vesicles, whereas alpha-(methylamino)isobutyrate uptake into the vesicles of the same preparation was stimulated by an Na+ gradient. The equilibrium level of cephalexin uptake decreased with increasing osmolarity of the medium, which indicates that cephalexin is transported into the membrane vesicles. When cephalexin concentrations were varied, the initial rate of uptake obeyed Michaelis-Menten kinetics with Km and Vmax values of 2.29 mM and 2.98 nmol/mg of protein per 60 s, respectively. The uptake of cephalexin was inhibited by structural analogues and sulfhydryl modifying reagents. These results indicate the existence of a carrier-mediated transport system for cephalexin in the human placental brush-border membranes.

The simultaneous use of autologous blood transfusion and hypotensive anesthesia to the radical hysterectomy for uterine carcinomas.

A homologous blood transfusion is often required to replace intraoperative blood loss in a radical hysterectomy. Recently, we have tried the simultaneous use of autotransfusion (predonation and transfusion of autologous blood) and hypotensive anesthesia to eliminate the homologous blood transfusion. Radical hysterectomy was done in 27 cases with uterine cancer from June 1986 to March 1987 at the Kure National Hospital. A homologous blood transfusion was required in 1 of 5 cases with only autotransfusion, none of 5 cases with a simultaneous use of autotransfusion and hypotensive anesthesia, and 6 of 17 cases with neither autotransfusion nor hypotensive anesthesia. The simultaneous use of autotransfusion and hypotensive anesthesia seemed to be effective in reducing the need for homologous blood transfusions.

Acute renal failure secondary to rhabdomyolysis associated with dissecting aortic aneurysm.

Rhabdomyolysis, a massive necrosis of skeletal muscle, is caused by various traumatic and non-traumatic factors. We report on a 30-year-old male in whom dissecting aortic aneurysm diagnosed by computed tomography was responsible for the generation of rhabdomyolytic acute renal failure. The patient was successfully treated by repeated hemodialyses and corrective surgery for aortic regurgitation and dissecting aortic aneurysm (Bentall's operation).

Efficacy of percutaneous transluminal coronary angioplasty in patients with acute myocardial infarction--comparison with intracoronary thrombolysis.

We retrospectively compared the efficacy of percutaneous transluminal coronary angioplasty (PTCA) and intracoronary thrombolysis (ICT) in patients with acute myocardial infarction (AMI). The ICT group consisted of 62 consecutive patients who underwent ICT before the introduction of PTCA for AMI and who were considered to be candidates for PTCA based on review of their cine-films. The PTCA group consisted of 92 consecutive patients who underwent PTCA thereafter. The reperfusion rate was significantly higher in the PTCA group than in the ICT group (92.4% vs 71.4%, p less than 0.01) and the residual stenosis was significantly lower in the former. Furthermore, the incidences of reinfarction and post-infarction angina were significantly lower in the former than in the latter (3.3% vs 12.9%, p less than 0.05 and 6.5% vs 29.0%, p less than 0.001 respectively). Although the degree of improvement in left ventricular function was influenced by the result of reperfusion, it was not affected by the reperfusion method. Therefore, PTCA did not improve left ventricular function more than ICT unless ICT alone failed to achieve reperfusion.

[Clinical study of cefuzonam in the field of obstetrics and gynecology].

Cefuzonam (CZON, L-105) was used clinically for the treatment of obstetrical and gynecological infections at a dosage of 1 g once or twice daily by intravenous drip infusion. The results obtained are summarized as follows. 1. Clinical effects of CZON were analyzed in 10 patients, including 5 patients with intrapelvic infections, 3 with intrauterine infections, and 1 each with adnexitis and an external genital infection. Excellent responses were observed in 1 patient (11.1%), good responses in 7 (77.8%), poor responses in 1 (11.1%), and one remaining case was unevaluable. The efficacy ratio was 88.9%. 2. Upon the treatment, eradications of causative bacteria were observed in all 4 cases tested. Staphylococcus sp. (2 strains), Staphylococcus epidermidis (1 strain), Klebsiella oxytoca (1 strain) and Pseudomonas putida (1 strain) were all eradicated by the CZON treatment. 3. The safety of the drug was analyzed in the 10 patients and rash occurred in 1 patient as a side effect. 4. One patient showed eosinophilia after the CZON treatment. It appeared that CZON would be useful for the treatment of obstetrical and gynecological infections.

[Augmentation of murine organ-associated natural immune responses by cepharanthin].

Oral administration of cepharanthin (bisbenzylisoquinoline alkaloid) to mice was effective in the organ-associated immune responses. Cepharanthin, as a biological response modifier (BRM), augmented natural killer (NK) activity of leukocytes isolated from the spleen, lymph node, lung, and liver. The degree of NK augmentation was almost the same in these organs without lymph node, and reached to the maximum levels 3 days after cepharanthin administration. In addition, adherent leukocytes (greater than 95% macrophages) isolated from the lung or liver following cepharanthin administration also exhibited an augmented macrophage-mediated cytotoxicity. These results suggest that such organ-associated immune responses may play an important role in the antitumor and/or antimetastatic effects mediated by cepharanthin.

[An experimental study on recovery of lacerated muscles after repair].

We often encounter laceration of skeletal muscle in daily clinical practice, but few studies have been reported regarding structural and functional recovery of lacerated muscle after repair. This study used the lateral head of the gastrocnemius muscles of rats to evaluate the sequential healing processes after laceration. Electrophysiological study using a self-designed 8-channel arrayed multielectrode revealed recovery of the electrical conductivity of the muscle itself in all cases 8 weeks after operation. Histological specimens showed gradual regeneration of muscle fibers across the suture site, and in 8 weeks the suture site was filled with muscle fibers. The tension of the muscle also recovered by degrees, reaching 60% of that of the contralateral muscle 6 weeks after operation, but it remained at that level even after 6 months. This study showed the possibility of structural and functional recovery in a reunited muscle.

[Clinical significance of serum DU-PAN-2 surveyed by solid-phase sandwich EIA for DU-PAN-2].

The clinical usefulness of DU-PAN-2 as a tumor marker was investigated. DU-PAN-2 antigen was detected by enzyme immunoassay based on sandwiching the antigen between monoclonal antibodies to DU-PAN-2. The EIA kit was supplied from Kyowa Medex Co. The mean DU-PAN-2 concentration in sera from 53 healthy donors was 51.8 U/ml and the mean plus 2 S.D. value was 156.2 U/ml. A level of 160 U/ml was used as the cut-off value. Serum DU-PAN-2 was positive in 7.5% of healthy donors, 16.9% of 83 patients with benign disease and 37.7% of 159 cancer patients. Of the cancer patients, those with pancreatic cancer, bile duct cancer and primary liver cell cancer had a high positive frequency of more than 70%. On the other hand, of patients with benign diseases, a high positive incidence of 40% or 50% was observed in patients with chronic hepatitis or liver cirrhosis. From this result, DU-PAN-2 appeared to be useful as a tumor for pancreatic cancer and bile duct cancer. Determination of the molecular weight of DU-PAN-2 in serum from a pancreatic cancer patient by Sepharose 4B gel filtration demonstrated the existence as a high-molecular-weight protein between about M.W. 500-10,000 K Da.

[Basic fetoprotein (BFP)].

Monoclonal antibodies to basic fetoprotein (BFP) were produced by hybridoma technology and the immunoreactivity of monoclonal anti-BFP to cancer tissues was investigated by an immunohistological technique. The investigation indicated that one type of monoclonal anti-BFP had a higher specific immunoreactivity to cancer. With this monoclonal anti-BFP, an enzyme immunoassay (MAb-EIA) was developed based on a competitive binding technique using biotin-labeled BFP and peroxidase-conjugated avidin. By the MAb-EIA and an enzyme immunoassay developed with conventional polyclonal anti-BFP (PAb-EIA), BFP was determined in sera from 95 normal donors, 238 patients with benign disease and 580 patients with cancer. Cut-off values of 90 ng/ml in MAb-EIA and 80 ng/ml in PAb-EIA were used. On comparing MAb-EIA with PAb-EIA, we found similar positive incidence for normal donors (MAb-EIA 0% PAb-EIA 2.1%) and patients with benign disease (MAb-EIA 26.0%, PAb-EIA 24.4%), but higher positive results in MAb-EIA (46.7%) for patients with cancer compared with those in PAb-EIA (33.8%).

[Fundamental and clinical studies of imipenem/cilastatin sodium in the field of obstetrics and gynecology].

Fundamental and clinical studies of a new carbapenem antibiotic, imipenem/cilastatin sodium (MK-0787/MK-0791), were carried out to evaluate the drug in treating infections of the female genital organs. The following results were obtained: In 4 patients undergoing hysterectomy, the penetration of MK-0787 and MK-0791 into female genital organ tissues was studied following a 30-minute intravenous drip infusion of MK-0787/MK-0791 500 mg/500 mg. Plasma levels of MK-0787 and MK-0791 in cubital venous blood following the drip infusion peaked at 0 minute with values of 51.2 micrograms/ml and 61.9 micrograms/ml, respectively, then decreased to 1.9 micrograms/ml and 0.7 microgram/ml, respectively, at 201 minutes. MK-0787 penetrated readily to female genital organ tissues and levels of the drug exceeded 0.5 approximately 1.9 micrograms/g in various organ tissues at 201 minutes following an intravenous drip infusion of 500 mg/500 mg of MK-0787/MK-0791. Clinically, MK-0787/MK-0791 was used for the treatment of obstetrical and gynecological infections at a dosage of 500 mg/500 mg twice daily by intravenous drip infusion. Clinical effects of MK-0787/MK-0791 were analyzed in 22 patients, including 9 patients with intrauterine infections, 7 with intrapelvic infections, 5 with adnexitis, and 1 with an external genital infection. Excellent responses were seen in 5 patients (22.7%), good responses in 15 (68.2%), and poor responses in 2 (9.1%). The efficacy ratio was 90.9%. After the treatment, 15 out of 18 isolates were eradicated for an 83.3% eradication rate. All strains of S. epidermidis (2 strains), Eubacterium lentum (1 strain), Peptococcus sp. (1 strain), beta-Streptococcus (1 strain), Gram-positive rods (2 strains), Enterobacter cloacae (1 strain), Bacteroides bivius (1 strain), Pseudomonas sp. (1 strain), Pseudomonas cepacia (1 strain) and Gram-negative rods (1 strain) were eradicated by the MK-0787/MK-0791 treatment. The safety of the drug was analyzed in 22 patients, effects occurred in 3 (13.6%). Among those 3 patients, diarrhea occurred in 1 patient, rash in another and nausea and vomiting in yet another. One patient had an increased BUN after the MK-0787/MK-0791 treatment. In the other 21 patients, no abnormalities in creatinine, GOT, GPT, or T. Bil. values were observed. It may be concluded that MK-0787/MK-0791 is useful for the treatment of obstetrical and gynecological infections.

Primary hypothyroidism associated with pituitary enlargement, slipped capital femoral epiphysis and cystic ovaries.

A case of primary hypothyroidism with pituitary enlargement, slipped capital femoral epiphysis and cystic ovaries is reported. The pituitary abnormality and cystic ovaries disappeared dramatically after thyroid hormone therapy. Hip pinning was performed. The recognition of these associations may eliminate unnecessary surgery and lead to the choice of hormone replacement therapy.
