Effects of nitric oxide synthase inhibitor on hemodynamic change and O2 delivery in septic dogs.

To elucidate the role of nitric oxide (NO) in septic shock, we measured hemodynamic and pulmonary gas changes in anesthetized dogs after intravenous administration of bacterial lipopolysaccharide (LPS) with or without NO synthase inhibitor, NG-nitro-L-arginine (L-NNA). Infusion of LPS (250 ng.kg-1.min-1) for 2 h decreased mean arterial pressure over 1-4 h. Although L-NNA (10 mg/kg) blocked LPS-induced hypotension, it decreased cardiac index, oxygen delivery index, arterial pH, and arterial PO2 and increased systemic vascular resistance index in the presence or absence of LPS. Administration of NG-nitro-D-arginine (D-NNA, 10 mg/kg) alone caused fewer hemodynamic effects (increased systemic vascular resistance index and decreased cardiac index) than L-NNA alone. Our study provides evidence that L-NNA prevents endotoxin-induced hypotension but decreases cardiac output and oxygen delivery, effects that may, in part, be due to a nonspecific NO synthase-independent event. Thus clinical use of NO synthase inhibitors for the treatment of septic shock should be cautiously considered.

Effects of chemical subendocardial ablation on activation rate gradient during ventricular fibrillation.

The mechanism by which an endocardial-epicardial activation rate gradient develops after 1 or 2 min of sustained ventricular fibrillation is unknown. We recorded from electrodes on the epicardium and from hook electrodes in the endocardium in three open-chest control dogs during prolonged ventricular fibrillation. The same recordings were also made in seven dogs after right ventricular subendocardial ablation with Lugol solution and in three dogs after substitution of air for the cavitary blood. The effects of these interventions, i.e., Lugol ablation (n = 2) and the exposure to air (n = 2), on the subendocardial Purkinje fiber transmembrane action potential properties were also evaluated in vitro using microelectrode recording techniques. The in vivo studies showed a significant endocardial-epicardial rate gradient in the control dogs and in dogs that had air substituted for the cavitary blood. In comparison, in dogs that underwent chemical subendocardial ablation, the activation cycle lengths for the endocardium and epicardium were not significantly different. The in vitro studies showed that subendocardial Purkinje fiber action potentials could still be recorded for up to 10 min of exposure to air. In comparison, in the tissues subjected to chemical ablation, no transmembrane action potentials could be recorded from either the Purkinje fibers or superficial ventricular muscle cells. We conclude that the development of an endocardial-epicardial activation rate gradient during prolonged ventricular fibrillation depends on the presence of intact subendocardial Purkinje fibers and ventricular myocytes. The retained cavitary blood is not responsible for the development of the rate gradient.

Inhibition of cell growth by macrophage membrane lipid molecule.

Macrophage hybridoma clone 5 suppressed B-cell proliferation induced by lipopolysaccharide (LPS). Since paraformaldehyde-fixed macrophages exerted the effect, macrophage-derived mediators were excluded from the inhibition. The inhibitory property of macrophages was present in the membrane fraction and was recovered in the organic phase after extraction using a chloroform/methanol/water system followed by hexane extraction. Therefore, the inhibitory activity found in macrophage clone 5 was concluded to arise from a lipid component. The inhibitory substance was further purified to a homogeneity by LH20 column fractionation using methanol/chloroform as the mobile phase. The purified lipid did not have any effect on the LPS-mediated induction of MHC class II molecules on the B-cell surface. Moreover, the inhibitory property was shown to affect growth of a wide variety of tumor cell lines of human origin. These results suggest that a lipid molecule found on the cell membrane of cloned macrophage hybridoma may participate in the regulation of cell growth through cell contact.

Infrequent involvement of mutations on neurofibromatosis type 1, H-ras, K-ras and N-ras in urothelial tumors.

The neurofibromatosis type 1 (NF1) gene is considered a tumor-suppressor gene whose product acts upstream of ras. The ras gene is an oncogene very commonly detected in human cancers and consists of three families, H-ras, K-ras and N-ras. These genes are converted to active oncogenes by point mutations in codon 12, 13, or 61. Examination was made of the mutations of these genes in 39 urothelial malignant tumors (31 bladder cancer, 6 renal pelvic tumor, and 2 ureter tumors) using polymerase chain reaction single-stranded conformation polymorphism and direct sequencing methods. Three of 39 (7.7%) cases showed mobility shifts in the ras family gene but no point mutations in NF1 and N-ras genes could be detected. Mutations were found in 1 case in H-ras at codon 13 (GGT-GTT/GGT) and K-ras at codon 12 in 2 cases (GGT-GCT/GGT, GGT-GTT/GGT). All 3 cases had progressed far beyond grade 2 and stage pT2. It follows from the above that NF1 and ras gene mutations are infrequent in the pathogenesis of urothelial tumors.

Immunostimulatory and antitumor activities of monoglycosylceramides having various sugar moieties.

Ten kinds of monoglycosylceramides (MonoCers), having the same ceramide portion and different sugar moieties, were synthesized and their immunostimulatory and antitumor activities were examined. The manner of combination between sugar and ceramide has been demonstrated to affect the manifestation of immunostimulatory and resultant antitumor activities of MonoCers, and in the case of D-MonoCers having the D-sugar, alpha-D-MonoCers (sugar combined to ceramide in an alpha-configuration) show stronger activities than beta-D-MonoCers. Furthermore, the form of sugar, not the furanose-form but the pyranose-form, and the 2"- and 4"-hydroxyl groups of the pyranose-form of sugar, seemed to play an important role in the manifestation of the activities of alpha-D-MonoCers.

Transdermal delivery of theophylline using an ethanol/panasate 800-ethylcellulose gel preparation.

A novel ethylcellulose gel preparation using an ethanol/panasate 800 (tricaprylin) (40/60) binary lipophilic vehicle was developed and applied to the transdermal delivery of theophylline. The in vitro skin permeability of theophylline across excised hairless mouse skin was improved by the use of this ethanol/panasate 800 (40/60) binary vehicle compared with either ethanol or panasate 800 as a single vehicle, and the addition of lauric acid as a permeation enhancer to the binary vehicle shortened the lag time and increased the permeated amount of theophylline up to 6 h (initial permeation amount). The in vitro permeability of theophylline from the ethanol/panasate 800 (40/60)-ethylcellulose gels decreased with an increase in the content of ethylcellulose in the gels. In the in vivo evaluation using abdominal rat skin, the ethanol/panasate 800 (40/60)-7% (w/w) ethylcellulose gel produced a good feature as a sustained-release preparation, with a relatively high bioavailability (BA) of theophylline, and dose dependency was observed.

Optimization of preparative conditions for polylactide (PLA) microspheres containing ovalbumin.

Polylactide (PLA) microspheres containing ovalbumin (OVA) as a model protein were prepared by a water-in-oil-in-water (w/o/w) emulsion solvent evaporation method. The optimization of preparative parameters for the PLA microspheres containing OVA were performed, and the in vitro characteristics of the obtained microspheres were examined. Firstly, a smaller internal aqueous phase volume was found to be advantageous in obtaining high loading efficiency. Secondly, the addition of 2-10% (w/v) NaCl into the external aqueous phase (0.5% (w/v) polyvinyl alcohol solution) also improved OVA loading efficiency. Prepared products showed a sharp release of OVA at the initial phase, but the following phase was characterized by a slow release rate of OVA that continued at least 28 d. The release rate of OVA from microspheres made of PLA with a molecular weight of 15400 was faster than that from microspheres made of PLA with a molecular weight of 58300. However, the LA/GA (lactide/glycolide) ratio was not likely to have much effect on the release profile of OVA. Finally, the effect of PLA microsphere particle size on the release profiles of OVA was examined. The extent of burst release at the initial phase increased as the mean diameter of prepared PLA microspheres decreased. For example, the PLA microspheres with a small mean diameter (5.0 microns) showed a 40% burst release, but almost 30% of OVA remained in the PLA microspheres (confirmed by HPLC method) after the 28 d release test, suggesting the possibility of using this carrier as a long-acting protein delivery system.

Hypoxic injury of immature cardiac myocytes under various hypothermic conditions using an in vitro cell-culture model.

The purpose of this study was to evaluate the functional and biochemical effects in immature cardiac myocytes under hypoxic and hypothermic conditions. Cardiac myocytes were isolated from neonatal rat ventricles and cultured for 4 days, after which 12.5 x 10(5) myocytes/flask were incubated under 3% hypoxic conditions at 4 degrees C, 10 degrees C, 15 degrees C, 20 degrees C, 25 degrees C, and 37 degrees C for 6, 12, and 24 h. After each hypoxic incubation, creatine kinase (CK) and lactate dehydrogenase (LDH) were measured in the incubation medium. The myocytes were then cultured for an additional 24 h at 37 degrees C to evaluate the recovery of the myocyte beating rate. In the 4 degrees C and 37 degrees C groups, the myocyte beating rate recovery markedly decreased with increasing incubation times from 78.1% and 97.2% at 6 h to 0.0% and 38.4% of the control, which was the beating rate prior to hypoxic incubation, at 24 h, respectively. However, in the 10 degrees C, 15 degrees C, and 25 degrees C groups, this value decreased significantly only at 24 h. In the 20 degrees C group, beating rate completely recovered in 24 h. A marked increase was found in the release of CK and LDH in the 4 degrees C group from 28.5 mIU/flask and 232.9 mIU/flask at 6 h to 83.7 mIU/flask and 640.7 mIU/flask at 24 h, respectively. However, in the 25 degrees C and 37 degrees C groups, this release was significantly increased only at 24 h.(ABSTRACT TRUNCATED AT 250 WORDS)

Detection of serum antibody against bovine coccidiosis by using Eimeria tenella antigen.

Antigens of Eimeria tenella sporulated oocysts were applied to detect serum antibody of calf infected with Eimeria. Polypeptides of E. tenella oocyst were strongly proved with the infected bovine serum in immunoblotting. The infected serum also showed higher absorbances than that of normal serum in enzyme linked immunosorbent assay (ELISA) using E. tenella oocyst antigen. Suitable conditions for the ELISA were obtained by using soluble antigen of E. tenella oocysts at 2.5 micrograms/ml and bovine serum at 200 fold dilution.

Low incidence of hemorrhagic infarction following coronary reperfusion with nasaruplase in a canine model of acute myocardial infarction. Comparison with recombinant t-PA.

An examination was made of the coronary thrombolytic effects of nasaruplase in a canine model of acute myocardial infarction. The model was produced by selective injection of an artificial thrombus into the coronary artery stenosed by laser ablation. Intravenous nasaruplase (8 U/kg/min) showed an equivalent thrombolytic effect to a recombinant tissue-type plasminogen activator (rt-PA, 10,000 IU/kg/min) as assessed by reperfusion rate (78.6 versus 79.2%) and reperfusion time (37.4 +/- 5.2 versus 37.0 +/- 2.5 min). Nasaruplase decreased the plasma alpha 2-plasmin inhibitor (alpha 2-PI) level by 28% immediately after reperfusion, but hardly altered fibrinogen or plasmin-alpha 2-plasmin inhibitor complex (PIC) levels. By contrast, rt-PA significantly decreased plasma alpha 2-PI and fibrinogen levels, by 84% and 92% respectively, and, increased PIC level more than 70-fold. Hemorrhagic infarction occurred in 2 of 14 animals in the nasaruplase group and in 9 of 19 animals in the rt-PA group. In these animals, significant correlations were found between the ratio of the hemorrhagic infarction area to total infarct area and the plasma alpha 2-PI (r = -0.740, p < 0.05) or fibrinogen (r = -0.798, p < 0.05) concentrations, as well as between the recovery rate of left ventricular regional wall motion and the plasma alpha 2-PI (r = 0.924, p < 0.01) or fibrinogen (r = 0.864, p < 0.01) concentrations. It is concluded that nasaruplase is a potent thrombolytic agent which preserves left ventricular function with a lesser rate of hemorrhagic infarction than rt-PA. Further, nasaruplase administration results in recovery of left ventricular regional wall motion and systolic function, such as Vmax.

Acute myeloblastic leukemia associated with an intermediate state between the healthy carrier state and adult T-cell leukemia.

This report describes an intermediate state between the human T-cell lymphotropic virus type I (HTLV-I) healthy carrier and adult T-cell Leukemia (ATL) who developed acute myeloblastic leukemia (AML, FAB subtype M2). The polyclonal integration of HTLV-I proviral DNA was demonstrated in the peripheral blood lymphoid cells, whereas AML cells had no HTLV-I proviral DNA. The patient achieved remission after combination chemotherapy but cells with lobulated nuclei persist at a low level and the polyclonal integration of HTLV-I proviral DNA is still demonstrated. We suggest that the patients with the integration of HTLV-I proviral DNA might develop secondary neoplasms more frequently than healthy carriers and this case stresses the need to exercise caution with these patients. The relationship between HTLV-I and AML is briefly discussed.

[A study of the efficacy of thrombolytic therapy for acute myocardial infarction in elderly patients].

We compared the short and long term outcome between an elderly group (E; aged 70 year more, 17 cases) and a younger group (Y; less than 70 years, 95 cases). Reperfusion rates were similar in both groups (E; 70.6% vs Y; 67.3%). Reocclusion rates at predischarge CAG were similar in both groups (E; 70.6% vs Y; 67.3%). Reocclusion rates at predischarge CAG were similar in both groups (E; 7.2% vs Y; 6.8%). Hospital cardiac deaths in E were higher than Y (E; 5.9% vs Y; 4.2%). Intestinal bleeding in E was more frequent than in Y (E; 5.9% vs Y; 3.2%). We concluded that thrombolytic therapy for acute myocardial infarction in elderly patients was useful, however bleeding complication in elderly patients were higher than in younger patients.

[Clinicopathological study of nonpalpable and nonvisible (stage T1c) prostate cancer].

Clinical and pathologic characteristics of stage T1c disease (nonpalpable and nonvisible cancer) was studied retrospectively in men who underwent radical prostatectomy in order to better understand this disease entity. Findings in stage T1c disease (16 patients) were directly compared with those in stage T2b disease (11 patients). No significant difference was observed between these groups with regard to age, preoperative serum prostatic acid phosphatase level, prostatic weight, numbers of tumor foci, total tumor volume, volume of index cancer and tumor grade (p > 0.05). Preoperative prostate specific antigen concentration was significantly lower in stage T1c group (p < 0.05). Substantially larger number of patients with stage T1c disease had pathologically organ confined disease when compared with T2b group (86.7% versus 45.5%, p < 0.05). Seventy-five percent (12/16) of stage T1c and 90.9% (10/11) of stage T2b disease were considered clinically significant. Most of stage T1c disease is organ confined and clinically significant. Clinical and pathologic features of these tumors are similar to those in T2b disease. Twenty-five percent of stage T1c disease, however, are small and thus may potentially be overtreated. Enhanced detection of prostate cancer achieved with modern technology can lead to undesirable treatment of clinically insignificant tumors. Preoperative diagnostic modalities which can reliably distinguish groups of tumor with different biological potential are needed to overcome this contradiction.

[Early detection of prostate cancer-results of a prostate specific antigen-based detection program at a "multiphasic screening"].

Determination was made of serum PSA in the early detection of prostate cancer in a population of 1,227 men aged 55 years or older who visited a "human dock". "Human dock" is a unique health examination facility open to public in Japan. Recommendation for biopsy was made based solely on this parameter when its value exceeded 2.0 ng/ml in IMx immunoenzymetric assay (Dinabot Co.). Of these, 162 (13.2%) had elevated values. The proportion of males with serum PSA greater than 4.0 ng/ml was only 3.6%. Of the 109 males who underwent ultrasound guided biopsies, 17 cancers were detected, the cancer detection rate thus being 1.4%. Most of these cases (82.4%) were clinically localized cancers. Eight patients with cancer had serum PSA levels below 4.0 ng/ml including 6 less than 3.0 ng/ml. Radical prostatectomy was conducted on 14 patients. All had histologic features of clinically significant cancer and 64.3% were pathologically confined. Oriental elderly males appear to have lower serum PSA than western counterparts. The optimal cutoff of serum PSA for early detection should be examined further in oriental male populations. Differences in the incidence of prostate cancer between ethnic groups may have been overestimated in previous studies owing in part to unawareness by the physician and general public. Though the application of PSA for early detection will likely disclose greater numbers of prostate cancers in Japan, whether early detection reduces mortality by minimizing risk of death from cancer remains a point to be clarified.

[Clinical experience with the artificial urinary sphincter AMS 800 for male incontinence].

BACKGROUND: The aim of this study is to evaluate the therapeutic efficacy of artificial urinary sphincter AMS 800 implants for male patients with urinary incontinence. METHODS: Eleven male patients with urinary incontinence were treated by implantation of the artificial urinary sphincter AMS 800 between 1988 and 1992. Patient age at the surgery ranged from 14 to 79 years, with a mean age of 58 years. At presentation, 9 patients (82%) had true incontinence and 2 had overflow incontinence, and medical treatments and/or surgical procedures (Teflon injections in 2 and Sling procedure in 1) had been attempted previously elsewhere in all patients. The etiologies of incontinence were post-prostatectomy (transurethral resection in 4 and radical retropubic prostatectomy in 4) in 8 patients (73%) and myelomeningocele, spinal cord injury, pelvic trauma in one each. There were 5 patients with abnormal cystometrogram, and 2 of them were performing intermittent self catheterization. Vesicoureteral reflux was determined in 2 patients preoperatively, which were surgically corrected one year before AMS800 implant in a patient and simultaneously in another patient. The cuff was placed around the bulbous urethra (9), pendulous urethra (1) or bladder neck (1). The device was activated 6 weeks post implantation, and the assessment of therapeutic effects was started 2 weeks after activation. RESULTS: Follow up ranged from 3 weeks to 75 months, with a mean of 56 months, if 3 cases done explantation due to periprosthetic infections were excluded. After AMS 800 implantation 5 patients (45%) were completely continent, 4(36%) required the use of not more than 1 pad per day, while a patient was not satisfied with the results and another patient was not definitive because of early explantation before device activation. Hence complete or near complete continence was achieved in 9 patients (81%). A patient is performing intermittent self catheterization in conjunction with the AMS 800 without any complications up until now. There were 3 periprosthetic infections (27%) associated with 2 cuff erosions, that consequently required explantation. Besides periprosthetic infection, neither complications nor mechanical device failures was experienced. CONCLUSION: In conclusion, the AMS 800 artificial urinary sphincter was safe with reasonable mechanical reliability and offered acceptable and satisfactory urinary control to the selected patients with incontinence.

Structure-antitumor activity relationship of semi-synthetic Spicamycin derivatives.

New derivatives of spicamycin modified at the fatty acid moieties of the molecule were synthesized and their structure-activity relationships were examined. The antitumor activity was greatly influenced by modification of the fatty acid moieties to tetradecadienoyl or dodecadienoyl analogues exhibiting better antitumor activity against COL-1 human colon cancer xenograft than SPM VIII.

Synthesis and antitumor activities of glycine-exchanged analogs of spicamycin.

A series of SPM VIII analogs were synthesized to investigate the effect of the amino acid moiety on the antitumor activity. The L-threonine analog and the glycylglycine analog of SPM VIII showed much higher cytotoxicity to P388 murine leukemia cells (IC50 5.8 nM and 0.11 nM, respectively) than SPM VIII (IC50 25nM). However, replacement of the glycine moiety with other amino acids greatly reduced the antitumor activity against COL-1 human colon cancer xenograft model. This study indicated that the glycine moiety of SPM VIII is crucial for the antitumor effect.

Induction of IgE antibody production in mice with different DPT-vaccine preparations.

Antibody production in mice after immunization with diphtheria-pertussis-tetanus (DPT) vaccine was investigated. Six lots of the vaccine produced in the same year by six manufacturers in Japan were chosen. Production of IgE antibody specific to either diphtheria or tetanus toxoid varied among vaccine preparations used, although there was no apparent difference in IgG antibody production after immunization. In addition, the level of IgE antibody specific to diphtheria toxoid was correlated with that of IgG1 antibody and inversely with that of IgG2 antibody. These results suggest that each vaccine preparation may induce a distinct pattern of antibody production and, therefore, the type of immune response induced by vaccination may vary among vaccine preparations.
