The long-term survival of patients with carcinoma of the main hepatic duct junction.

Carcinoma of the main hepatic duct junction tends to invade extensively the bile ducts and hepatic parenchyma, although dissemination is rarely seen. Therefore, extensive resection of the bile ducts combined with hepatic resection is the procedure of choice for treating this disease. From January 1973 to January 1987, 23 of 41 patients underwent resection, giving a resectability rate of 56%. One patient died postoperatively, yielding an operative mortality rate of 4.3%. The 5-year actuarial survival rate is 29.8%. Three patients are now alive and well 6 years and 9 months, 5 years and 10 months, and 5 years and 5 months after the operation. One additional patient who underwent resection in an affiliated hospital has done well for 8 years and 8 months. The results from these four patients treated by curative resection support a strategy featuring curative resection with aggressive surgery.

Cloning and sequence analysis of cDNAs encoding mammalian mitochondrial malate dehydrogenase.

A cDNA clone, named ppmMDH-1 and covering a part of the porcine mitochondrial malate dehydrogenase (mMDH; L-malate:NAD+ oxidoreductase, EC 1.1.1.37) mRNA, was isolated from a porcine liver cDNA library with a mixture of 24 oligodeoxyribonucleotides as a probe. The sequences of the probe were deduced from the known sequence of porcine mMDH amino acid residues 288-293. ppmMDH-1 covered the coding region for porcine mMDH amino acid residues 17-314 and the 3' untranslated region. Subsequently, mouse mMDH cDNA clones were isolated from a mouse liver cDNA library with the ppmMDH-1 cDNA as a probe. One of the clones, named pmmMDH-1 and containing a cDNA insert of about 1350 base pairs, was selected for sequence analysis, and the primary structure of the mouse precursor form of mMDH (pre-mMDH) was deduced from its cDNA sequence. The sequenced coding regions for the porcine and mouse mMDH mRNAs showed about 85% homology. When the deduced amino acid sequence of the mouse pre-mMDH was compared with that of the porcine mMDH, they shared a 95% homology, and the mouse pre-mMDH yielded a leader sequence consisting of 24 amino acid residues and a mature mMDH, consisting of 314 amino acid residues. The leader sequence contained three basic amino acid residues, no acidic residues, and no hydrophobic amino acid stretch. The mouse mMDH leader sequence was compared with those of three other rodent mitochondrial matrix proteins.

[Effects of verbatim information on recognition of S-O-V sentences under incidental learning].

By means of a delayed recognition task, propositions were tested about the retention of verbatim information on Japanese S-O-V sentences based on spreading activation in memory representations. Subjects studied written sentences under incidental instructions, and a recognition test was made after a 10-minute delay. In the test, subjects verified whether the sentences were true or false for (a) original sentences, (b) synonymous distractors (verbs replaced by synonyms), and (c) non-synonymous distractors (verbs replaced by non-synonyms). In Experiment I, subjects verified original sentences more accurately, faster, and more confidently than those having distractors. Results of Experiment II, in which subjects retrieved two related sentences sequentially, showed an inhibition (fan) effect for hits. In addition, there was a facilitation (priming) effect for correct rejections. These findings provide evidence in support of the spreading activation theory of memory.

[Flomoxef treatment of patients with respiratory tract infections].

Flomoxef (FMOX, 6315-S) was administered to 22 patients with respiratory tract infections. The patients consisted of 13 patients with pneumonia, 7 with bronchitis, 1 with bronchiectasis and 1 with pyothorax. The drug was administered by intravenous injection or intravenous drip infusion twice a day with doses of 1 to 2 g and total doses ranged from 17 to 64 g. The following results were obtained. 1. Clinical responses to the therapy were excellent in 1 case, good in 10 cases, fair in 4 cases, poor in 4 cases and not determined in 3 cases. Efficacy ratio was 57.9%. 2. As for adverse reactions, exanthema in 1 patient and stomatitis and numbness of tongue in another patient were observed, but these symptoms improved with cessation of the therapy. Abnormal laboratory test values were observed in 5 cases. From these results it appears that FMOX is a valuable antimicrobial agent against patients with respiratory tract infections.

Isolation and characterization of the human ornithine transcarbamylase gene: structure of the 5'-end region.

We isolated over 20 phage clones carrying the ornithine transcarbamylase (OTC) [EC 2.1.3.3] gene, from two independently constructed human genomic DNA libraries, using as probes either a rat OTC cDNA or several nuclear DNA fragments derived from some of these isolated clones. These clones, classified into 10 different groups, overlapped and spanned a region of more than 85 kilobase pairs of the human genomic DNA. Restriction mapping and Southern blot analyses demonstrated that one of the clones covers the 5'-end region of the OTC gene. We sequenced the 5'-end region of the OTC gene and found that it covered 665 base pairs of the 5'-flanking region, the complete first exon and a part of the first intron (150 base pairs). In the 5'-flanking region, there were two pairs of putative CAAT and TATA boxes and one enhancer core-like sequence, GTGGAAAG. The first exon contained a coding region for most of the OTC presequence, i.e. 26 out of 32 amino acid residues of the presequence, including the initiation methionine.

Familial amyloidotic polyneuropathy diagnosed by cloned human prealbumin cDNA.

A diagnosis of familial amyloidotic polyneuropathy (FAP) can be made by use of restriction endonuclease NsiI, a cloned human prealbumin cDNA and Southern blot procedures. Digests of DNAs from 10 disease-free individuals showed two bands (6.6 kb and 3.2 kb) complementary to a human prealbumin cDNA, whereas digests from 11 individuals with FAP exhibited two additional bands (5.1 kb and 1.5 kb). We interpret these changes in pattern to be the result of a restriction site for NsiI located in the altered codon and associated with the mutant prealbumin gene. All these individuals with FAP were heterozygous for the prealbumin gene, carrying one normal and one mutant gene.

[The influence of the rapid rise of intraluminal pressure of the upper urinary tract on ureteral action potentials].

The present investigation was undertaken in order to examine the ureteral action potentials following transient rapid rise of the intraureteral pressure. In 21 adult mongrel dogs, physiological saline and 5% formalin were injected into the left and right obstructed ureters, respectively to produce rapid rise of intraureteral pressure to 60 cmH2O for 7 minutes, and then ureter was released from obstruction. In these experiments, recordings of ureteral electromyogram and intraureteral pressure were made. Histological changes of pelvi-ureteral system were also observed microscopically. Results 1. After injection of physiological saline, discharge intervals were significantly shortened, but normal discharge intervals were restored when the ureters were released from obstruction. Under conditions of reobstruction, the discharge intervals were shortened and intraureteral pressure inducing the shortest discharge interval was 50 cmH2O. 2. After injection of formalin, discharge intervals were significantly shortened, and the action potentials disappeared in 1 minute 28 seconds to 4 minutes 51 seconds after the injection in 18 out of the 21 ureters. No discharge was observed at 7 minutes after the injection in 16 out of the 18 ureters. In 14 out of the 16 ureters, however, the action potentials were restored when the ureters were released from obstruction or after the obstruction was readministered. Under conditions of reobstruction, discharge intervals were shortened, and intraureteral pressure inducing the shortest discharge interval was 20 cmH2O. 3. Histologically, the loss of epithelium, as well as edema and vasodilatation in lamina propria, were observed only in the mucosa of the middle portion of the ureters receiving formalin. The results of the present study show that a transient rapid rise of intraluminal pressure of the upper urinary tract caused by the injection of physiological saline or formalin, dose not have a significant effect on the ureteral action potentials, and that disappearance of the action potentials is refered to pharmacological action of formalin.

[Effects of levels-of-processing on retention of target and nontarget words in a selective attention task].

Effects of levels-of-processing on retention of visually presented target and nontarget letter words were studied in relation to the amount of processing resources expended on the attended task. Attention was directed to targets by the shadowing. Targets and nontargets were distinguished by sensorial or semantic attributes. Processing resources expended were measured by reaction time at an auditory signal during the shadowing. Experiment I, with 20 subjects, showed that recall rate of targets and nontargets was higher and the expended resources were greater, in semantic than in sensorial processing. However, in Experiment II, with 48 subjects, the task difficulty was balanced with respect to shadowing errors and the expended resources were found no more different with different levels-of-processing, while d' measures of targets and nontargets were greater in semantic than in sensorial condition. It was concluded that the effects of levels-of-processing depended not on the resources expended, but on discriminative attributes.

Mitochondrial DNA-like sequences in the human nuclear genome. Characterization and implications in the evolution of mitochondrial DNA.

Thirty-three phage clones carrying DNAs homologous to human mitochondrial DNA (mtDNA) were isolated from two independently constructed human gene libraries, and the region and extent of homology of mtDNA-like sequences carried by these clones were examined in hybridization experiments. Each phage clone contained DNA sequences homologous to various parts of the mtDNA and the extent of homology differed from clone to clone. From the efficiency of the library screening, it was estimated that human nuclear DNA contains at least several hundred copies of mtDNA-like fragments. Four clones carrying nuclear DNA sequences homologous to the mitochondrial Unidentified Reading Frame (URF) 4 and URF5 regions were chosen for further studies, and their structures were analyzed by DNA sequencing. Comparison of these mtDNA-like sequences with that of mtDNAs of several mammalian species revealed conservation of a part of the structures present in direct ancestral mtDNAs. The mtDNA fragments seem to have been continuously integrated into mammalian nuclear DNA during evolution.

Structure of the human prealbumin gene.

Using cloned human prealbumin cDNA as a probe, Southern blot hybridization of human genomic DNA revealed that the prealbumin gene consists of an unique, single-copy DNA. The nucleotide sequences of the entire human prealbumin gene, including both 581 base pairs of the 5'- and 95 base pairs of the 3'-flanking sequences, were determined. The gene spans about 7.0 kilobase pairs and consists of four exons and three introns. As in most eukaryotic genes, the consensus TATA and CAAT sequences are found 30 and 101 nucleotides, respectively, upstream from the putative cap site, and a polyadenylation signal sequence AA-TAAA is found in the 3'-untranslated region. Unexpectedly, two independent open reading frames provided with respective regulatory sequences were found within the gene: one in the first intron and the other in the third intron.

Long-term survival of patients with hepatocellular carcinoma combined with liver cirrhosis. Report of two patients.

Two patients with hepatocellular carcinoma combined with liver cirrhosis lived more than 5 years after hepatic resection. One patient with liver cirrhosis combined with schistosomiasis japonica survived 5 years and 2.5 months after left lobectomy and died of liver failure. Two recurrent tumors, each 1 cm in diameter, were present in the remaining lobe but were not responsible for death. Another patient is well 9 years and 2 months after extended right lobectomy. The tumor was bulky, and the resected specimen was 2800 g in weight. These facts show that patients with hepatocellular carcinoma combined with liver cirrhosis can survive for a long period even if a large tumor is involved.
