Sparse distribution of hepatocyte growth factor-producing cells inside hepatocellular foci in rats treated with hepatocarcinogens.

The distribution of hepatocyte growth factor (HGF)-synthesizing cells in rat liver during development of glutathione S-transferase P form (GST-P)-positive nodules after diethylnitrosamine initiation followed by promotion with 2-acetylaminofluorene plus partial hepatectomy (PH) was investigated using in situ hybridization. HGF-producing cells were non-parenchymal in nature, and were suspected to be mainly of Kupffer type. They were mostly located outside GST-P-positive lesions, in the surrounding parenchyma. In the oval cell proliferation phase 1 week after PH, they increased and they were mainly localized around the portal triads. It is concluded that HGF is directly involved in an endogenous paracrine growth pathway controlling proliferation in oval cells and in normal, but not GST-P-positive, hepatocytes.

Epidemiologic survey of lower urinary tract symptoms in Asia and Australia using the international prostate symptom score.

BACKGROUND: The prevalence of lower urinary tract symptoms was determined by survey as an initial step in estimating the significance of benign prostatic hyperplasia (BPH) in Asia and Australia. METHODS: The symptom index (0 to 35) and quality-of-life (QOL) index (0 to 6) of the international prostate symptom score were measured in 7588 men in 9 Asian countries and 146 men in Australia. RESULTS: The percentages of Asian men considered to be symptomatic (symptom index > or = 8) were 18%, 29%, 40%, and 56% in the age groups of 40 to 49, 50 to 59, 60 to 69, and 70 to 79 years, respectively. For Australian men, these figures were 36%, 33%, and 37% in the 50 to 59, 60 to 69, and 70 to 79 year age groups, respectively. CONCLUSIONS: Our estimates indicate that the prevalences of symptomatic men in Asia and Australia are similar to or greater than those in Europe and America, and suggest BPH is similarly common in these areas.

Functional outcome and late complications in patients with continent urinary reservoirs.

BACKGROUND: We reviewed the functional outcome and late complication of continent urinary reservoirs (CUR) constructed with a cecocolonic segment, including the Indiana pouch, in 37 patients treated in our clinic. METHODS: The CUR procedure was performed on 37 patients, creating partially detubularized (PD) reservoirs in 9 patients, totally detubularized (TD) reservoirs in 16 patients and reservoirs with an ileal patch (IP) in 12 patients. Continence was achieved by the nipple valve in 10 patients and by ileal plication in 27 patients. The mean follow-up period was 46 months (range, 15 to 87 months). The function of the reservoir was evaluated by measurement of the intrareservoir pressure. RESULTS: Patients with the TD reservoir had less frequent appearance of involuntary, phasic elevation of the intrareservoir pressure (30.8%) than those with the PD reservoir (62.5%). In contrast, this phasic elevation was found in only 1 patient with an IP reservoir. The IP reservoir had the largest capacity accompanied by the lowest maximum intrareservoir pressure. Total incontinence was observed in 2 patients with ileal plication due to disruption of the plicated sutures on the terminal ileum. Frequent difficulty in catheterizing the reservoir was found in 2 patients, and reservoir-ureter reflux was found in 3 renal units. The serum chloride level was significantly elevated after surgery, however, in most patients the levels remained within normal limits. CONCLUSION: Our experience of the outcome and late complications of reservoirs indicates that the cecocolonic reservoir with an ileal patch and stapled ileal plication, i.e., the Indiana pouch, is a better choice for continent urinary diversion for patients who need a cystectomy.

Morphological and pharmacological characterization of guinea-pig prostatic smooth muscle cells in vitro.

BACKGROUND: Prostatic smooth muscle is thought to play a major role in the pathogenesis of bladder outlet obstruction in patients with benign prostatic hypertrophy. However, the physiology of prostatic smooth muscle cells remains largely unknown, in part due to the lack of a suitable model system. We therefore sought to establish an in vitro culture of guinea pig prostatic smooth muscle cells. METHODS: Immature guinea pig prostate was treated by enzymatic digestion and the cells obtained were used to initiate the primary culture. After 3 to 4 passages, cultured smooth muscle cells were examined morphologically by immunocytochemistry and electron microscopy. The contractile properties of cultured smooth muscle cells were also examined. RESULTS: The cultured prostatic cells demonstrated hill and valley morphology, which is a hallmark of smooth muscle cells in vitro, and stained positively for desmin. In addition, electron microscopic examination of ultrastructural morphology revealed myofilaments. Confluent cultures of prostatic smooth muscle cells showed a clear, dose-dependent contractile response to phenylephrine. Furthermore, contraction of the prostatic smooth muscle cells by 10(-6) mol/L phenylephrine was completely inhibited by pretreatment with 10(-6) mol/L terazosin. CONCLUSIONS: An in vitro culture of prostatic smooth muscle cells was established. This culture is likely to provide a powerful tool for elucidating the physiology and pathophysiology of prostatic smooth muscle.

Transurethral balloon laser thermotherapy for urinary retention in patients with benign prostatic hyperplasia who are at high surgical risk.

BACKGROUND: Twelve patients with benign prostatic hyperplasia and urinary retention, who were considered to be poor candidates for prostatectomy, were treated by transurethral balloon laser thermotherapy (TUBAL-T). The mean patient age was 78.9 years (range, 66 to 93 years) and the mean duration of bladder catheterization was 11 weeks (range, 2 to 48 weeks). METHODS: Irradiation into the prostatic tissue was done through 360 degrees with a neodymium: yttrium aluminum garnet (Nd:YAG) laser balloon placed in the prostatic urethra, with pain relief provided by using local topical anesthesia. The total laser dose was from 45,000 to 123,376 J, with an average of 73,089 J. The irradiation time was from 40 to 54 minutes, with an average of 45.2 minutes. RESULTS: Spontaneous voiding became possible in all patients at a mean of 2.8 days (range, 1 to 7 days) after irradiation. The mean catheter-free period was 20.5 months (range, 6 to 34 months), with the longest being 34 months. The international prostatic symptom scores, quality-of-life scores, and peak uroflow rates showed substantial improvement after laser thermotherapy. To date, long-term resumption of spontaneous voiding was successfully achieved in 9 of 12 cases (75%). CONCLUSION: TUBAL-T is safe and effective alternative for treatment of urinary retention in patients with benign prostatic hyperplasia who are considered to be at high surgical risk.

Hemangioma of the prostatic urethra: hematospermia and massive postejaculation hematuria with clot retention.

The case of a 53-year-old man with hematospermia and massive postejaculation hematuria that caused urinary retention is described. This is the sixth case in the English and Japanese language literature. Cystourethroscopic examination revealed that a solitary raised tumor was present just distal to the vermontanum, and that bleeding was from its apex. Histologic examination of an excisional biopsy sample showed features compatible with hemangioma.

Experimental bystander encephalitis induced by immunization with HTLV-I-producing T cells in mice.

ICR mice were immunized with HTLV-I carrier T lymphocytes (MT-2 cell line) and then inoculated intracerebrally with these cells. After non-specific traumatic hemorrhage, perivascular cell infiltration was noted diffusely throughout the brain on day 2 and lasted for over 2 weeks. HTLV-I antigens were detected in both sides of the cerebral hemisphere by Western blotting analysis. Tissue damage consisting of demyelination, axonal degeneration, and astrogliosis was observed most heavily on days 10 and 14. Non-immunized mice inoculated with the cells showed only transient hemorrhage. In cases using the HTLV-I free T-cell line (MOLT-4) inflammatory cell infiltration and tissue damage was much less conspicuous and disappeared after day 10. These experiments support the hypothesis that HAM/TSP is a bystander disease initiated and progresses via HTLV-I-infected T-cell invasion into the spinal cord of a patient who has been infected and sensitized to the virus.

[PCR method for detection of K1 antigen and serotypes of Escherichia coli isolated from extraintestinal infection].

A portion of the neuS gene concerning Escherichia coli K1, was amplified by the PCR to detect the K1 antigen among extraintestinal infection with E. coli. The correlation between K1 antigen and O:H serotype was investigated. The predominant serotypes were O7:H-, O16:H6, O18ac:H7 from cerebrospinal fluid with meningitis, O16:H6 from blood with septicemia, O1 : H7, O7:H-, O16:H6, O18ac:H7, O75 : H- from urine with cystitis. The same O:H serotypes tended to be found frequently irrespective of source. K1 antigen was found O1:H7, O7:H-, O16:H6, O18ac:H7, and almost all the strains possessed to H6 or H7.

EGF receptor ligands are a large fraction of in vitro branching morphogens secreted by embryonic kidney.

Much attention has recently focused upon hepatocyte growth factor (HGF) as a potential regulator of epithelial branching morphogenesis. However, since neither the HGF nor c-met "knockout" mice show abnormal kidney branching morphogenesis, we sought to analyze the relative importance of HGF in in vitro branching morphogenesis compared with other factors secreted by the embryonic kidney. Exploiting an assay that employs kidney epithelial cells (murine inner medullary collecting duct, mIMCD3) seeded in collagen cocultured with the embryonic kidney, we found that a tyrosine kinase inhibitor that is highly specific for the epidermal growth factor (EGF) receptor (EGFR), tyrphostin AG1478, inhibited mIMCD3 cell process formation (an early step in branching tubulogenesis) by 40%, whereas high concentrations of neutralizing anti-HGF antibodies had a lesser effect (20% inhibition), suggesting that EGFR ligands account for a larger fraction of branching morphogens secreted by the embryonic kidney than HGF. In addition, when an embryonic epithelial cell line derived from c-met (-/-) mice was cocultured with the embryonic kidney, these c-met (-/-) cells underwent process formation. EGFR ligands but not HGF were able to induce branching tubulogenesis in these cells. All EGFR ligands tested, including EGF, transforming growth factor-alpha, heparin-binding EGF, betacellulin, and amphiregulin, induced mIMCD3 cell tubulogenesis. EGFR ligands caused upregulation of urokinase, urokinase receptor, and matrix metalloprotease-1, and tubulogenesis could be inhibited by the metalloprotease inhibitor 1,10-phenanthroline. Our results support the notion that multiple parallel and potentially redundant growth factor-dependent pathways regulate branching tubulogenesis.

Effects of oxytocin, prostaglandin F2 alpha and prostaglandin E2 on intracellular free calcium concentrations of longitudinal muscle cells isolated from term pregnant rat myometrium.

The effects of oxytocin, PGF2 alpha and PGE2 on [Ca2+]i of isolated longitudinal muscle cells of term pregnant rats were investigated using Fura-2. Oxytocin, PGF2 alpha and PGE2 induced an initial rapid increase followed by a secondary gradual increase in [Ca2+]i in the presence of 1.5 mM Ca2+. The initial maximum increases in [Ca2+]i were obtained at 6 s for oxytocin, 20 s for PGF2 alpha and 30 s for PGE2 after addition of the stimulants. The EC50 values obtained from the curves were 2.0 nM for oxytocin, 250 nM for PGF2 alpha and 2,200 nM for PGE2. On the other hand, the increases in [Ca2+]i induced by the stimulants were nearly abolished by removal of the extracellular Ca2+. The stimulants induced biphasic increases in [Ca2+]i which were highly extracellular Ca(2+)-dependent processes, and the order of potencies for the stimulants was oxytocin > > PGF2 alpha > PGE2 in terms of affinity as well as magnitude. These results might indicate differences in the weight of the physiological role of the stimulants for regulating uterine contractility.

Pre-operatively determined prognostic factors in metastatic renal cell carcinoma.

OBJECTIVES: The aim of this study was to clarify the pre-operatively determined factors affecting the prognosis in patients with metastatic renal cell carcinoma (RCC). METHODS: We analyzed 69 patients with metastatic RCC who received nephrectomy. Multivariate analysis by Cox's proportional hazards model and survival studies were applied. RESULTS: Tumor thrombus in a renal vein or vena cava was the most significant poor-prognostic factor, followed by multiple-organ metastases and three or more abnormal findings of factors related to acute-phase proteins, in this order. Seven patients having these three risk factors died of cancer within 7 months after nephrectomy. On the other hand, half of the patients without risk factors survived for more than 2 years. CONCLUSIONS: Risk status should be used for selection of metastatic-RCC patients for treatment of the primary site.

A benign form of isolated angiitis of the central nervous system in puerperium: an identical disorder to postpartum cerebral angiopathy?

A case of isolated angiitis of the central nervous system (IACNS) in puerperium with a clinical presentation of bilateral frontal lobe hemorrhage is described. Her symptoms and arteriographic abnormalities were resolved by institution of corticosteroid. After cessation of steroid treatment she showed no evidence of relapse. This case indicates that there appears to be a benign form of IACNS, although the etiology of postpartum IACNS is not known. The postpartum cerebral angiopathy described in the literature may be an identical disorder, since there are no clinical points of specific distinction between them.

[The background factors influencing loss of sexual intercourse after transurethral resection of the prostate (TUR-P)--A study using the Sapporo Medical University-Questionnaire].

BACKGROUND: Sexual dysfunction after transurethral resection of the prostate (TUR-P) depends on multiple factors including preoperative erectile function itself. Using the Sapporo Medical University-questionnaire, we analyzed background factors contributing to loss of sexual intercourse after the operation. PATIENTS AND METHODS: We sent the self-administered questionnaires to 1,000 patients who had received TUR-P. Answers to questions were ranked as scores. Multiple regression analysis was used to identify factors significant for loss intercourse after TUR-P. RESULTS: The response rate was 63.5%. Patient with incomplete answers and those having diseases affecting voiding or sexual function and antiandrogen medication were excluded from the study. Leaving 536 patients for the final analysis. Scores for sexual desire and erectile function in the patients who received TUR-P were lower than those of age-matched healthy males. When we divided the patients according to frequency of sexual intercourse before and after TUR-P, there was distinct differences in age, scores for sensory disturbance, depression, the cooperative value of the sexual partner, sexual desire, erectile function, nocturia, residual sensation and urgency between those maintaining sexual intercourse and those having lost it. Multiple regression analysis using these factors indicated that the scores of erectile function was the most significant determinant for maintaining sexual intercourse after TUR-P in patients aged from 50 to 69 years, followed by the cooperative value of the sexual partner, residual sensation and sensory disturbance. In those 70 old and older, the score for erectile function was the most significant determinant, followed by the cooperative value of sexual partner. CONCLUSION: Our results suggest that, irrespective of age, the cooperation of the patient's sexual partner as well as erectile function significantly influence the maintenance of sexual intercourse after TUR-P. In addition, voiding condition and psychological status after TUR-P may affect the maintenance of sexual intercourse in patients in their fifties and sixties.

[Long-term clinical course of Peyronie's disease treated conservatively].

BACKGROUND: To evaluate the natural history of Peyronie's disease and to determine the suitable time for surgical treatment, we investigated changes of clinical findings of the disease over time. PATIENTS AND METHODS: We evaluated changes of plaque size, penile curvature and erectile dysfunction in 10 patients with Peyronie's disease. The mean follow-up period was 36.8 months. RESULTS: At the first examination, the mean plaque size of patients with erectile dysfunction was larger than that of those without erectile dysfunction, suggesting a positive association between size and erectile function. There was no significant relationship between plaque size and penile pain or penile curvature. The mean size of plaques decreased significantly compared with that at the first examination (p < 0.05). However, penile curvature, pain and erectile dysfunction persisted during the follow-up period in all but one patient, who showed improvement of penile curvature. CONCLUSION: These results suggest that we may recommend a shorter observation period until surgery that we have done for some patients who want to be surgically treated.

[A clinical study on combination therapy of antimicrobial agents for complicated urinary tract infection--with special reference to combination with clarithromycin].

PURPOSE: To confirm the clinical efficacy of the combined therapy to complicated urinary tract infection (UTI), we conducted a comparative clinical study of the combined therapy with ciprofloxacin (CPFX) and clarithromycin (CAM) acting an biofilm elimination or CPFX alone in patients with complicated UTI. PATIENTS AND METHODS: The study was carried out in patients with complicated UTI having WBCs with 5/hpf or more in urinary sediment and bacteriuria at least 10(4) CFU/ml. The combined therapy was CPFX and CAM, each 600 mg/day, for 14 days, and the single therapy group CPFX, 600 mg/day, for 14 days. On Day 7 and 14, the eradication rate and efficacy rate (according to the criteria of the Japanese UTI committee) were determined. In the patients with indwelling catheter, the surface of the catheter tip was observed under a scanning electron microscope (SEM) on Day 14. RESULTS: In both cases with and without catheters, clinical efficacy was higher in the combined therapy group than in the single therapy group. In particular, the efficacy rates at 14 Day were significantly higher in the former group. Furthermore, we investigated the therapeutic effect in the below MIC breakpoint of CPFX in complicated UTI. The combined therapy group showed a higher clinical efficacy in both cases with and without indwelling catheter than the single therapy group, although there was not statistically significant. Biofilm on the surface of the catheter tip was eliminated in 75% of the combined therapy group. However, none of the biofilm was eliminated in the single therapy group. CONCLUSION: From the above results, we surmise that the combined use of CPFX and CAM will show some degree of efficacy in eliminating both the causative organism and its biofilm in the complicated UTI.

[Relationship between grade and stage of renal pelvic and/or ureteral tumor and associated bladder tumor].

OBJECTIVE: The purpose of this study was to analyze bladder tumor (BT) associated with renal pelvic and/or ureteral tumor (RUT), with emphasis on the relationship between the grade and stage of these tumors. METHODS: We reviewed 77 cases of RUT operated upon at our hospital during the last 14 years. We herein define stage T1b tumor as invasive since this stage of tumor is considered to have a different biologic behavior and potential compared to stage Ta tumor. RESULTS: Of the 77 patients, 29 had associated BTs: 8 with a concomitant BT, 3 with BT antedating RUT, and 18 with a subsequent BT. In addition, one patient with preceding BT and two with concomitant cases developed BT after the RUT operation, permitting 21 cases to be categorized in the subsequent BT group. Thirty one out of 32 cases of associated BTs had the same or a lower grade of RUT while one-patient who initially had G2 T1b RUT developed G3 T2 BT 5 years after total nephroureterectomy. In the preceding cases, two patients with Ta BTs, had Ta RUTs, and one patient with T1b BT antedated T2 RUT. In the concomitant BTs, all four cases with stage Ta RUT exhibited Ta BT, and another four with invasive RUT presented invasive BT. In the subsequent cases, 18 out of 21 patients developed Ta BT. CONCLUSION: In the preceding and concomitant cases, associated BT & RUT tend to have the same nature, that is, to be either invasive or non-invasive. In contrast to these cases, 18 out of 21 patients in subsequent cases developed Ta BT. However, since three out of 10 patients with invasive RUT subsequently developed invasive BTs1 careful follow-up for invasive RUT should be mandatory to detect recurrent invasive BTs in their early stage.

[Maximum increase of penile circumference as a parameter for evaluation of penile erectile capacity].

PURPOSE: Male erectile capacity has been often evaluated with the maximum changes of penile circumference during nocturnal penile tumescence (NPT) by erectometer. However, it has not been fully established that the changes actually reflect penile rigidity. In this paper, we evaluated how the maximum change (the increase in circumference or its rate) of the circumference is related to penile rigidity. PATIENTS AND METHODS: The study included 116 men with or without complaints of sexual dysfunction. The maximum increase (penile circumference on erection-that when flaccid) and increase rate ¿(penile circumference on erection-that when flaccid)/penile circumference when flaccid¿ of penile circumference and penile rigidity were studied during artificial erection induced by intracavernous injection of vasoactivedrugs. RESULTS: When the maximum increase of penile circumference on erection was 21 mm or more, more than 80% of men achieved adequate rigidity of the penile for vaginal penetration. However, if the increase was 10 mm or less, all men had poor rigidity. In men having an increase of 11 mm or more and less than 21 mm, more than 80% did not achieve adequate rigidity if their penile circumference in the flaccid state was less than 95 mm. In contrast, 80% or more of the men showed penile rigidity sufficient for vaginal penetration when the flaccid circumference was 95 mm or more. Studies on the maximum increase rate revealed results similar to those in the study on the maximum increase. These two parameters had a similar diagnostic accuracy for evaluating penile rigidity. CONCLUSIONS: A higher prediction rate for penile rigidity was achieved by adding the parameter of penile circumference in the flaccid state to the maximum increase or increase rate of penile circumference. Thus, evaluation of erectile capacity by maximum changes (the increase in circumference or its rate) of penile circumference with an erectometer can be used for screening of men with sexual dysfunction.
