[Aplastic anemia which terminated in hypoplastic leukemia 10 years after diagnosis].

A 57-year-old woman was admitted to our hospital because of further examination of anemia in November, 1978. She was diagnosed as having aplastic anemia, which was not associated with any atypical findings. Treatment with oxymetholone had been effective. However, she developed pancytopenia in January, 1988. A bone marrow aspiration revealed a hypocellular marrow with 75.6% blasts. Following two courses with low dose cytosine arabinoside and vitamin D3, the leukemia improved although she needs red cell transfusions at intervals of one to two months because of persistent pancytopenia. We present a patient with aplastic anemia who developed a hypoplastic leukemia at the tenth year of the disease.

[Diagnostic significance of the number of bone marrow megakaryocytes in cases with decreased platelet production evidenced by platelet kinetic study].

In 18 patients with decreased platelet production proved by reduced platelet turnover, the numbers of megakaryocytes in clot section of marrow aspirate were counted. Platelet counts ranged from 19,000 to 128,000/microliters. The sternal marrow showed decrease of megakaryocytes only in 3 out of 18 cases while megakaryocytes in the iliac marrow were reduced in 6 out of 11 cases. Only one of 11 cases showed decrease of megakaryocyte in both marrows examined. On the contrary, four cases showed normal number of megakaryocytes at both sites. Of 9 cases with normal or increased number of megakaryocytes in sternal and/or iliac marrow, no case showed morphologically abnormal megakaryocytes and only one case had the reduced number of platelet-forming megakaryocytes. Results suggested that the number of megakaryocytes in bone marrow especially in the sternum was not reliable for establishing the diagnosis of decreased platelet production, and that about 50% of patients with thrombocytopenia due to hypoproduction of platelets could be diagnosed pertinently only after the study of platelet kinetics.

[Effects of trospium chloride on the lower urinary tract function].

The effects of trospium chloride on the lower urinary tract function were studied by combined recording of cystometry and sphincter electromyogram in 13 decerebrated dogs. Micturition was induced by bladder filling before and after drug administration. A statistical analysis was carried out on the urodynamic parameters. Trospium chloride at the dose of 0.1 mg/kg showed a small but significant decrease in the threshold pressure during the collecting phase. In the urodynamic parameters of the emptying phase which is considered to be influenced by cholinergic activity, there was a significant decrease in maximum bladder pressure at 0.03, 0.1, and 0.3 mg/kg and a significant increase in residual urine at 0.3 mg/kg. Trospium chloride is probably acting as a antimuscarinic agent in this study. Trospium chloride appears to be useful for the relief of symptoms associated with hyperactive bladder contraction.

[Elevated levels of cytokines in plasma from patients with disseminated intravascular coagulation].

We measured blood concentrations of tumor necrosis factor (TNF), interleukin-1 beta (IL 1-beta), soluble interleukin 2 receptor (s-IL 2r), and interferon alpha (IFN alpha) in 30 patients with disseminated intravascular coagulation (DIC) and compared the results to those of 25 patients without DIC. Plasma levels of TNF, IL 1-beta, and s-IL 2r were higher in patients with DIC than in those without DIC. In one case of acute promyelocytic leukemia, plasma levels of TNF and IL-1 beta increased at the onset of DIC but decreased upon DIC improvement. These findings suggest that activation of the immune system is involved in the development of DIC. However, these concentrations were not markedly increased in patients with leukemia, although blood TNF and s-IL 2 r were markedly elevated in patients with solid cancers. Especially in patients with solid cancers, hyperactivation of the immune system may cause an increase in blood TNF and IL-1 beta and the development of DIC.

[Plasminogen activator in leukemic cell homogenate].

We examined fibrinolytic substances in homogenate of leukemic cells, normal granulocyte or mononuclear cells fraction. Plasminogen activators (PA) were significantly low in normal cells, but they were slightly increased in lymphoblastic leukemia cells and markedly increased in myeloblastic leukemia cells. In almost leukemic cells homogenate, the antigen ratio of tissue type PA (t-PA)/urokinase type PA (u-PA) was about 2.0. In especially AMMoL and AMoL, PA activity had discrepancy between euglobulin lysis time and amidolytic assay using chromogenic substrate. As PA inhibitor (PAI)-II was markedly increased in them. PAI might effect the PA assay. PA activity of leukemic cells homogenate was similar to that without t-PA stimulator and leukemic cells homogenate significantly stimulated t-PA. As both PA activity and antigen were statistically increased in leukemic cells homogenate of patient with disseminated intravascular coagulation (DIC), PA and PA stimulator in leukemic cell might play an important role in hypofibrinogenemia or DIC.

[Nasal lymphoma of natural killer cell origin].

We report a case of nasal lymphoma with characteristics of natural killer (NH) cells. A 44-year-old man was admitted to our hospital because of right nasal obstruction. Physical examination revealed a tumor in the right nasal cavity and swelling of the right cervical lymph nodes. Histopathological examination showed diffuse medium sized cell lymphoma. The neoplastic cells expressed CD2, NKH-1, HLA-DR and leukocyte common antigen, but lacked other T-cell, B-cell and myeloid markers. They were in germline configuration for immunoglobulin heavy chain and T-cell receptor genes by southern blot analysis. These findings suggest that they are derived from NK cell.

[Effects of thrombin produced in blood prior to mixing with anticoagulant on the platelet function and blood coagulation tests].

The amount of thrombin produced in the withdrawn blood of healthy donors prior to mixing with anticoagulants was estimated with thrombin-antithrombin III complex (TAT) methods. No samples showed production of thrombin in the blood mixed with anticoagulants within 2 min. After 3 min or more incubation, four donors out of 14 showed elevation of TAT, and a technician could suspect trouble only in a case during taking blood. Although two samples with low thrombin production (5.8, 12 ng/ml, normal: less than 5 ng/ml) showed no changes of APTT or prothrombin time, samples with high thrombin production (74, 98 ng/ml) showed 1.4-2 sec shortening of APTT and 0.8-1 sec shortening of prothrombin time. Platelet counts and platelet volume showed no remarkable changes even when samples with large amount of thrombin were suspended in tubes containing EDTA-2 K. Plasma beta-thromboglobulin concentration was elevated in the blood containing high amount of thrombin and close correlation was observed amount of thrombin in the blood and the amount of plasma beta-thromboglobulin (r = 0.90, P value less than 0.001). Platelet aggregation induced by collagen was not influenced by amount of thrombin produced. ADP-induced aggregation was suppressed in 3 out of 4 cases with thrombin production although no close correlation was observed between maximal aggregation rate and the concentration of thrombin. As improper handling of blood samples including withdrawal of blood leads to misunderstanding of test results. So, blood samples only for these sensitive tests should be taken and mix with anticoagulants within 2 min.

A rapid electrophoretic method for the detection of serum Lp(a) lipoprotein.

A rapid electrophoretic method detecting serum lipoprotein(a) (Lp(a)) was described. When serum lipoproteins were electrophoresed using an agarose gel film containing 0.6% agarose and 3% sucrose, a distinct extra-band was frequently detected between beta- and prebeta-bands. This lipoprotein band was identified as Lp(a) by immunoblotting technique using anti-Lp(a) serum. Using this electrophoretic method, we studied 1,353 subjects with a variety of diseases. The Lp(a)-positive subjects accounted for 8.4% of all. Neither sex nor age influenced the frequency of Lp(a) positivity. Frequencies of definite myocardial infarction in Lp(a)-positive subjects did not differ significantly from those in Lp(a)-negative ones. Those of cerebral infarction in Lp(a)-positive subjects were significantly higher than those in Lp(a)-negative ones. An electrophoretic method described herein is very useful for the rapid detection and screening of Lp(a), and hence for large-scale clinical studies of atherosclerotic risk assessment.

Cyclic AMP-responsive region of the human proopiomelanocortin (POMC) gene.

Transcription of the human proopiomelanocortin (POMC) gene is regulated by cAMP. To identify the region in the human POMC gene responsible for this regulation, we constructed chimeric genes containing different portions of the 5'-flanking region of the human POMC gene fused to the structural sequence encoding the bacterial reporter enzyme chloramphenicol acetyltransferase (CAT). The transcriptional activity of the fusion genes introduced into the rat glial cell line C6 was assayed by measuring CAT activity in the cell lysate. Forskolin, an adenylate cyclase-activating agent, stimulated the expression of POMC-CAT fusion genes. Deletion analysis demonstrated that the region between -417 and -97 bp from the transcriptional origin of the human POMC gene was responsible for regulation by cyclic AMP.

Integrity of the blood-ocular barrier after intravitreal gas injection.

The authors studied the effect of various intravitreal gases on the integrity of the blood-ocular barrier using a fluorophotometric technique. The gas (0.3 ml) was injected into the vitreous cavity of a pigment rabbit after paracentesis. Air and two long-acting gases, sulfur hexafluoride and perfluoropropane, were studied. The eyes that received only paracentesis served as controls. Fluorescein concentrations in the anterior chamber were determined by slit-lamp fluorophotometer 30 minutes following intravenous fluorescein administration. Fluorophotometry was repeated on days 1, 3, 7, and 14 after the gas injections. The eyes that received sulfur hexafluoride and perfluoropropane showed increased aqueous fluorescein concentrations in comparison with the control eyes and the air-injected eyes, although conventional slit-lamp biomicroscopic examination detected no anterior chamber inflammatory signs in these eyes. The aqueous fluorescein leakage decreased to the normal level after the intravitreal gas bubble was absorbed. These results suggest that intravitreal gas bubbles induce subclinical breakdown of the blood-aqueous barrier, which becomes re-established soon after the gas is absorbed.

[Neoplastic angioendotheliosis of B-lymphocyte origin: an autopsy report].

A 56-year-old male presented with fever of unknown origin and subacute dementia which progressed to death with seizure, coma and acute deterioration of general conditions. He had splenomegaly but not skin eruption or lymph node swelling. Autopsy findings showed that mononuclear tumor cells were widespread within the lumens of small blood vessels, indicating the features of neoplastic angioendotheliosis. The involved organs were shown to be brain, lung, adrenal grand, testis, bone marrow, heart and thyroid gland. To determine the origin of tumor cells, an immunohistochemical study was carried out using a panel of monoclonal antibodies. The results indicated that the tumor cells were of B-lymphocyte origin. These findings support the possibility that neoplastic angioendotheliosis is a lymphoma with proliferation in small blood vessels throughout the body.

Intensive chemotherapy followed by autologous bone marrow transplantation for a patient having wide-spread embryonal carcinoma in the central nervous system.

A patient with an intracranial embryonal carcinoma, which was resistant to cisplatin-based combined chemotherapy and radiotherapy, developed spinal seeding. Alpha fetoprotein (AFP), human chorionic gonadotropin (HCG) and the beta chain of HCG (NCG beta) in serum or cerebrospinal fluid immediately returned to elevated levels despite sequential chemotherapy and resection. Accordingly, we treated this patient with spinal irradiation and intensive combination chemotherapy (cisplatin etoposide, vinblastine, cyclophosphamide and actinomycin D) followed by autologous bone marrow transplantation. With this treatment, the clinical manifestations showed a tendency to improve and, furthermore, the serum levels of AFP and HCG returned to normal. These findings suggest autologous bone marrow transplantation to be a potential approach to the treatment of patients with embryonal carcinomas.

Hereditary complete deficiency of lactate dehydrogenase H-subunit.

We report the second known case of a patient, a 45-year-old Japanese woman, with hereditary complete deficiency of lactate dehydrogenase (LDH; EC 1.1.1.27) H-subunit. Total LDH activity in her serum was abnormally low (35 U/L, normal reference interval 195-360). LDH activity in her erythrocytes was also low, but all the other glycolytic enzyme activities in her erythrocytes were within normal limits. Electrophoresis of her serum, erythrocytes, lymphocytes, thrombocytes, and saliva showed only one band, the LDH M4 isoenzyme. LDH activities in her saliva and lymphocytes exceeded the reference interval. Her erythrocytes contained fructose 1,6-diphosphate 26 mumol/L (normal range 4-13), dihydroxyacetone phosphate 75 mumol/L (normal range 8-22), glyceraldehyde 3-phosphate 57 mumol/L (normal range 4-14), and pyruvate 45 mumol/L (normal range 31-63). The family study of three generations showed that this deficiency was inherited in an autosomal recessive mode.

2-Mercaptoethylamine, a competitive inhibitor of spermidine synthase in mammalian cells.

Spermidine synthase from rat ventral prostate was inhibited by 2-mercaptoethylamine (MEA). Inhibition of spermidine synthase by MEA was competitive with respect to one of the substrates putrescine, but not competitive with respect to the other substrate decarboxylated S-adenosylmethionine. MEA markedly depressed spermidine and spermine contents in human erythroid leukemia K562 cells, suggesting that these changes resulted from the inhibitory effect of MEA on spermidine synthase in situ.
