Transcatheter arterial embolization using iodized oil (lipiodol) mixed with an anticancer drug for the treatment of hepatocellular carcinoma.

The therapeutic results of Lp-TAE (transcatheter arterial embolization with Gelfoam particles preceded by the infusion of a mixture of lipiodol and an anticancer drug via the proper hepatic artery) were evaluated in hepatocellular carcinomas (523 non-resected and 24 resected cases). Excellent therapeutic effects were confirmed not only for the main tumor but also for the daughter nodules by a histological examination of the liver tissues resected after Lp-TAE. The cumulative 1-year, 2-year and 3-year survival rates in the 523 non-resected cases were 60.4%, 42.9% and 28.0% respectively. These survival rates were all higher than those achieved by Gelfoam TAE. The above results suggest the usefulness of Lp-TAE in the treatment of hepatocellular carcinoma.

Plasma levels of atrial natriuretic peptide in patients with liver cirrhosis and its relation to ascites and renal function.

Plasma immunoreactive alpha-human atrial natriuretic polypeptide (Ir-alpha-hANP) was measured by radioimmunoassay in 21 cirrhotics and 10 normal subjects. Average of Ir-alpha-hANP level in cirrhotics was significantly higher than in normal subjects (125.8 +/- 79.6 versus 28.7 +/- 12.2 pg/ml, P less than 0.001). In cirrhotics without ascites, Ir-alpha-hANP levels were positively correlated with creatinine clearance (Ccr) and urinary sodium excretion, suggesting that alpha-hANP was closely related to renal circulation and sodium homeostasis. On the contrary, in cirrhotics with ascites Ir-alpha-hANP levels were negatively correlated with Ccr. Urinary sodium excretion in cirrhotics with ascites and Ccr more than 50 ml/min was positively correlated with Ir-alpha-hANP levels. However, cirrhotics with ascites and Ccr less than 50 ml/min excreted little sodium in spite of high Ir-alpha-hANP levels. On the basis of the Ir-alpha-hANP before and after treatment of ascites, cirrhotics with ascites were subdivided into 2 groups. In group I Ir-alpha-hANP decreased from high values and in group II it was further elevated from slightly high values by treatment. The difference in renal function and plasma volume may account for the difference in Ir-alpha-hANP changes in the 2 groups.

IL-2 antibody production in lupus mice.

We show here that antibody responses to IL-2 were detected in NZBWF1 and BXSB mice but not in MRL/lpr mice in vivo and in vitro. Lipopolysaccharide (LPS), in the presence or in the absence of T cells, also induced anti-IL-2 antibody production in NZBWF1 and BXSB mouse spleen cells but not in MRL/lpr spleen cells in vitro. Anti-IL-2 antibody production increased progressively with age. Supernatants (SN) obtained from NZBWF1 B cell cultures were able to elicit anti-IL-2 antibody production in LPS-stimulated spleen cells of normal BALB/c mice, whereas SN from NZBWF1 T cells culture had no detectable effect.

Contragestive effects of the antigestagenic agent, RU486, in rats and rabbits.

The contragestive effects of the antiprogesterone RU486 in pregnant rats and rabbits were investigated. Administrations of increasing doses of RU486 in intact or castrated pregnant rats induced abortion in a dose dependent manner. RU486 had less effect when administered on the 16th day of gestation. Antiestrogens (tamoxifen, clomiphene citrate), an inhibition of 3 beta-hydroxysteroid dehydrogenase (epostane) showed a synergistic effect when used in combination with RU486. A decrease in serum progesterone levels was observed after RU486 administration. A larger dose of RU486 was necessary to induce abortion in pregnant rabbits compared to rats. The maximum blood concentration was greater and the area under the plasma level curve was larger in rats than in rabbits following administration of RU486.

An experimental study on the pathway of iron transfer from macrophages to erythrocytes in rat liver.

In order to reveal the pathway of iron release from macrophages, a 59Fe-labelled ferric hydroxide-potassium polyvinyl sulfate complex (Fe-PVS) was injected intravenously into anemic rats and the level of radioactivity in the liver, spleen, bone marrow, blood plasma and red blood cells (RBC) was estimated at various time intervals after the injection. Histochemical observation of ferric iron and ferritin in the liver was also made on anemic rats treated using unlabelled Fe-PVS. Fe-PVS injection promoted the recovery of anemia causing a rapid increase in the RBC number, with activated erythropoiesis occurring in the spleen and bone marrow. Soon after the injection, most of the radio iron was found in the liver with a small amount in the circulating erythrocytes, bone marrow and spleen. The iron level in the liver decreased gradually with a rapid increase in the iron level of the erythrocytes which reached a very high level 6 days after the 59Fe-PVS injection. Histochemical observations showed a heavy deposition of ferritin in the Kupffer cells 3 days after Fe-PVS injection. This deposition was minimized after 6 days with an increase in the level of ferritin in the parenchymal cells in the central area of acini. The level of radioferritin estimated biochemically in the nonparenchymal cell fractions of the liver revealed that the level dropped by about one third approximately 3.5 days after the Fe-PVS injection, showing the stimulated ferritin release at this stage. Results indicate that Kupffer cells in the liver play an important role in ferritin synthesis from the phagocytized iron compounds and that the iron is supplied for erythroid cell proliferation.

Urinary prostaglandins and renal function in obstructive jaundice.

Changes in urinary prostaglandin E2 (PGE2), 6-keto PGF1 alpha, and thromboxane (TXB2) excretion in 12 patients with obstructive jaundice were observed in relation to renal function and the renin-angiotensin (R-A) system. In obstructive jaundice before percutaneous biliary drainage the creatinine clearance (CCr) was significantly lower (p less than 0.001) and the PGE2 and plasma angiotensin II (AII) concentrations were significantly higher (p less than 0.005 and p less than 0.005, respectively) than those in normal subjects. Both 6-keto PGF1 alpha and TXB2 were widely distributed. When CCr returned to normal after drainage, PGE2 and plasma AII also returned to normal, but when CCr decreased after drainage, PGE2 and plasma AII increased. Before drainage, PGE2 correlated negatively with CCr (r = -0.72, p less than 0.01) and positively with plasma AII(r = 0.69, p less than 0.02). 6-Keto PGF1 alpha correlated positively with serum total bilirubin (r = 0.66, p less than 0.02). The percentage change in PGE2 after drainage correlated negatively with that in CCr (r = -0.95, p less than 0.005). The percentage chang in plasma AII correlated positively with that in urine PGE2 (r = 0.94, p less than 0.005) and negatively with that in CCr (r = -0.85, p less than 0.02). These results suggest that PGE2 is closely related to the R-A system and might assist in the maintenance of renal circulation in obstructive jaundice.

Hb Hope, beta 136(H14)Gly----Asp, in a diabetic Japanese female and its functional characterization.

A beta-variant hemoglobin, first misjudged as a marked elevation of Hb A1, was found in a 68-year-old Japanese female with diabetes mellitus. This hemoglobin was isolated by Bio-Rex 70 chromatography combined with chromatofocusing, and was found to be Hb Hope, beta 136(H14)Gly----Asp, by classical and high performance liquid chromatographic peptide mapping techniques. Intrinsic oxygen affinity of this hemoglobin was approximately one-third as compared with that of Hb A0. This property was still observed in the constituent beta subunits isolated. Effects of such allosteric effectors as H+ (at a fixed concentration of Cl-), anion (Cl-), 2,3-diphosphoglycerate and carbon dioxide were more or less depressed. Among others, a marked reduction in the carbamate effect should be noted in a structural interpretation of the functional modifications. Subunit cooperativity, on the contrary, was not different from that in Hb A0 (n = 2.8-2.9). Explanation of these altered functions were attempted on the basis of the altered structure. The reduced stability of Hb Hope is also described.

[A case of persistent müllerian duct syndrome associated with seminoma].

A case of persistent Müllerian duct syndrome associated with seminoma is presented. A 28-year-old man was hospitalized with the complaint of a palpable mass in the left lower abdomen and lumbago. Physical examination revealed a normal right scrotal testicle and a small right inguinal hernia that did not contain a mass, but no testicle was found in the left scrotum. Laparotomy was performed for suspected tumor of an intraabdominal testis. At abdominal exploration the left testis was found to be cross-ectopic to just behind the pubic symphysis and to be replaced completely with a large tumor, which was supplied by the left spermatic vessels and had a vas deferens emptied to the right seminal vesicle. In addition another cord-like structure was traced into the right inguinal canal. The tumor was dissected with these structures. On histological examination the tumor was pure seminoma and the cord-like structure proved to be an immature uterus, Fallopian tubes and seminal vesicle.

Participation of food antigens in increased polyclonal antibody production induced by alcohol.

In order to obtain a better understanding of hyperglobulinemia in chronic alcoholism, we investigated whether food antigens participated in the effects of long-term oral alcohol (AL) administration on serum immunoglobulin levels and polyclonal antibody production in spleen, Peyer's patch and bone marrow of C57BL/6 mice in specific pathogen free (SPF) circumstances. In animals fed antigen food (AF), three weeks of oral AL administration elicited an increase in polyclonal IgA antibody production in Peyer's patch. Moreover, seven weeks of oral AL administration elicited increases in both polyclonal IgA antibody production and serum IgA level. However, in animals fed antigen free food (AFF), oral AL administration failed to elicite increases in serum IgA level and polyclonal antibody production in spleen, Peyer's patch and bone marrow. Furthermore, in animals fed AF or AFF, oral AL administration failed to elicite increases in polyclonal IgM and IgG antibody production in any organ and serum IgM and IgG level. It is suggested that food antigens participated greatly in the elevation of serum IgA level in chronic alcoholism and that Peyer's patch is the major site of polyclonal IgA antibody production.

[The clinical study of 23 cases in patients with bladder cancer over 85 years old].

We treated 23 patients with bladder cancer over 85 years old. The male to female ratio was 3.6. Six cases (26%) were of low grade (G1) cancer and 16 (70%) were of high grade (G2, G3), and the other one was of unknown grade. Superficial cancer (less than or equal to pT1) was seen in 11 cases (48%), and invasive cancer (greater than or equal to pT2) was 12 cases (52%). The 3-year survival rate was 38% for all, that for superficial cancer 55%, and that for invasive cancer 0%. Fifteen cases (65%) were treated by TUR and the prognosis of invasive cases in this group was poor.

The role of renal macrophages in the aglomerular kidney of the sea-horse (teleost) in the removal of exogenous macromolecules from circulating blood.

Mechanisms involved in the removal of exogenous macromolecules from circulating blood were investigated in the aglomerular nephrons of the kidney of the sea-horse, Hippocumpus kuda Bleeker, using either native anionic or cationized probes. After intraperitoneal injection of native anionic horse-spleen ferritin (HS-AF) or cationized one (HS-CF) into the sea-horse, kidneys and gills were examined morphologically at intervals of 1 h to 14 days. Histochemical and immunohistochemical studies and transmission electron microscopy of the kidneys revealed that most of the injected HS-AF or HS-CF were taken up by macrophages in the sinusoids of hemopoietic tissue through pinocytosis, being gradually accumulated into their phagolysosomes. By the 14th day, the injected ferritin particles were degraded and the ferric agglomerates were concentrated within the phagolysosomes. Then the macrophages heavily laden with densely packed ferric catabolites migrated into the hemopoietic area forming macrophage agglomerates or macrophage centers. Some HS-AF and HS-CF particles infiltrated into the tubular basement membrane, where they were taken up by the tubular epithelial cells through pinocytosis, translocated into the phagolysosomes, fragmented into small ferric catabolites and then excreted into the urinary space. In contrast, examination of the gills revealed neither HS-AF, HS-CF particles nor their histochemically detectable ferric catabolites in either the interstitial space including basal lamina or the alveolar epithelia. It seems that no appreciable egress of exogenous substances occurs from the gill into the environmental water.

[Neocarzinostatin therapy of advanced renal cell carcinoma].

Fifteen patients with advanced renal cell carcinoma underwent chemotherapy of neocarzinostatin (NCS). First, 6-8 mg of NCS was infused into renal artery at angiography; second, 6-8 mg was infused by the same route just before ligation of renal artery at nephrectomy, if possible; and third, 2 mg was given intravenously at 2-week intervals, about 10 times. One patient who suffered from metastatic renal cancer on rs. tibia was treated by femoral arterial injection of 2 mg NCS 5 times. By treatment of NCS for advanced renal cell carcinoma 2 out of fifteen patients achieved complete regression, one patient showed partial regression and four a minor response. Effective rate (CR + PR) of NCS for metastatic renal cancer was 20%, and the response rate (CR + PR + MR) was 47%. We consider that NCS is presently the most effective drug for renal cell carcinoma.

Role of the liver in alcohol-induced alteration of high-density lipoprotein metabolism.

The effect of alcohol intake on the serum levels of high-density lipoprotein cholesterol (HDL-ch), apoprotein A-I (apo A-I), and apoprotein A-II (apo A-II) was investigated in 15 habitual alcohol drinkers without liver injury (group A), 17 with mild hepatic damage (group B), and five with advanced liver disease (group C). The serum level of HDL-ch was higher in group A than in 10 nondrinkers (P less than 0.01) but lower in group B (P less than 0.05) and markedly lower in group C (P less than 0.01). The level of apo A-I was also higher in group A (P less than 0.05), although the level of apo A-II was not. To elucidate the effect of alcohol on apo A-I production by hepatocytes, the synthesis of apo A-I and albumin by cultured rat liver cells in the presence of ethanol was also investigated. Ethanol enhanced the production of apo A-I but not that of albumin. The results suggest that serum HDL is elevated in habitual alcohol drinkers without liver injury and that the elevation of HDL in blood is mainly dependent on the increase of apo A-I synthesis in the liver.

Enhancing effects of oral adjuvants on anti-HBs responses induced by hepatitis B vaccine.

Hepatitis B (HB) vaccine is very promising for the prevention of HB infection. There exist, however, some non-responders to current vaccination trials. In this study, taurine, parotin and lithium were selected as adjuvants which can be administered orally. The mechanisms of these three materials as adjuvants and their effects on HB vaccine were investigated in mice. For instance, taurine induced polyclonal antibody production and exhibited adjuvant activity. Although taurine did not have any activity on the proliferation of thymocytes nor stimulate IL-2 production, taurine did induce IL-1 production by macrophages. It was considered that taurine-induced IL-1 would play an essential role in the proliferation and differentiation of B cells. Parotin also induced polyclonal antibody production and exhibited adjuvant activity. These effects of parotin were not affected even if macrophages or T cells were depleted, and parotin itself had an IL-1-like activity. Therefore, it was considered that parotin acted directly on B cells by its IL-1-like activity and mitogenic activity, resulting in the proliferation and differentiation of B cells. Lithium induced neither polyclonal antibody production, nor IL-1 or IL-2 production. However, when given with an antigen, lithium activated the humoral immune system, resulting in the augmentation of antibody production. Oral administration of taurine, parotin and lithium were capable of restoring antibody responses to HB surface antigen (HBsAg) in HBsAg-nonresponder mice. Furthermore, taurine, parotin and lithium enhanced the adjuvant effects of aluminium contained in the present HB vaccine. These observations indicate that use of these oral adjuvants may open new perspectives in the field of human HB vaccination.

The circulating alpha 1-antitrypsin-elastase complex attacks the elastic lamina of blood vessels. An immunohistochemical study.

The aim of the study was to determine the destination of the alpha 1-antitrypsin-elastase complex, which is found in circulating blood after the peroral administration of elastase. The complex was made in vitro by mixing hog pancreatic elastase with human alpha 1-antitrypsin and then injected intravenously into rats and mice. Tissues taken at various times after injection were subjected to histochemical staining using an antibody against elastase. Light microscope observations revealed dense deposition of reaction products in the elastic lamina of the arterioles; moderate or slight deposits were seen in the tissues surrounding arteries, in the tubular epithelial cells of the proximal convoluted tubules in the kidney, and in the pancreatic ducts. Immunoelectron microscopy revealed heavy deposition of the reaction product in the elastic lamina of the small arteries and arterioles; some dissolution of the elastic fibers was also evident. Pinocytic uptake of the alpha 1-antitrypsin-elastase complex was observed on the abluminal surface of endothelial cells and in smooth-muscle cells bordering the elastic lamina of arterioles. The endothelial cells of the arteries and arterioles retained their normal morphological appearance, although local desquamation was observed in some animals. The results indicate that, when the alpha 1-antitrypsin-elastase complex is present in the circulating blood, it is incorporated into the elastic lamina through the endothelial layer. This results in liquefaction of the lamina, desquamation of endothelial cells and leakage of the complex into the perivascular tissues via the vascular walls. However, some of the complex seems to be excreted very quickly from the kidneys.
