Familial arteriovenous malformations of the brain--two case reports.

Familial arteriovenous malformation (AVM) of the brain is an uncommon entity. A 24-year-old female presented with sudden onset of left hemiparesis due to an AVM with a hematoma in the right frontal lobe, which was surgically removed. Her 23-year-old younger sister presented with motor aphasia and right hemiparesis due to an AVM with a hematoma in the left frontoparietal lobes, which was surgically removed. Thirty-two patients in 15 families have been described in total. The mean age of these patients was 28 years, with 15 males and 17 females. Multiple AVMs occurred in four patients (12.5%), more frequently than in non-familial cases.

Induction of granulocyte-macrophage colony-stimulating factor (GM-CSF) and granulocyte colony-stimulating factor (G-CSF) expression in bone marrow and fractionated marrow cell populations by interleukin 3 (IL-3): IL-3-mediated positive feedback mechanisms of granulopoiesis.

Interleukin 3 (IL-3) induces proliferation and differentiation of mast cell progenitors in vitro, whereas it induces granulocytosis in vivo. In this paper, a positive feedback mechanism of granulopoiesis was studied in order to elucidate the granulocytosis induced by IL-3 in mouse. IL-3 induced expression of granulocyte-macrophage colony-stimulating factor (GM-CSF) and granulocyte colony-stimulating factor (G-CSF) in total bone marrow cells and a marrow adherent cell population. In fractionated marrow cell populations, a different expression pattern of induction of IL-3 stimulation was observed; GM-CSF was expressed in macrophages and fraction 1 (P < 1.061) and 2(1.061 < P 1.074) of bone marrow cells fractionated by equilibrium density centrifugation, G-CSF was expressed in macrophages and fraction 2 and 3 (1.074 < P < 1.097), and interleukin 6 (IL-6) in macrophages and fraction 1 to 3. These results indicate a hierarchical regulation of cytokine production and the existence of a positive feedback mechanism in granulopoiesis. IL-6, induced by IL-3, stimulates stem cells into cycle and induces stem cells to respond to IL-3. The stem cells differentiate to granulocyte-macrophage colony-forming cells by the combined effect of IL-3 and IL-6. IL-3 also induces GM- and G-CSF expression which in turn makes granulocyte-macrophage colony-forming cells differentiate to granulocytes. These factors organize a cytokine network in granulopoiesis.

Enterotoxin-binding glycoproteins in a proteose-peptone fraction of heated bovine milk.

The binding of Escherichia coli heat-labile enterotoxin to caseins, whey proteins, milk fat globule membrane, and proteose-peptone fraction from bovine milk was studied by using the Western blot technique. Two toxin-binding glycoproteins, pp16k and pp20k, with molecular weights of 15,500 and 20,000, respectively, were detected only in a proteose-peptone fraction. These glycoproteins were partially purified by ammonium sulfate precipitation and Toyopearl HW 55 gel filtration chromatography. The binding ability to the toxin was destroyed by periodate treatment or beta-galactosidase treatment, indicating that a carbohydrate moiety, particularly a terminal galactosyl residue, was essential for the binding of the toxin. In contrast, the binding ability was not changed by mild acid treatment, and these glycoproteins did not bind cholera toxin, which can bind to ganglioside GM1, suggesting that the carbohydrate structure of the glycoproteins is different from that of GM1. The N-terminal amino acid sequence and immunoblot analysis indicated that the protein moieties of pp16k and pp20k are identical to alpha-lactalbumin and beta-lactoglobulin, respectively. These toxin-binding glycoproteins were not detected in whey proteins isolated from unheated skim milk, suggesting that they are newly generated during heat treatment of skim milk before the preparation of a proteose-peptone fraction.

Immunohistochemical phenotyping of liver macrophages in normal and diseased human liver.

The phenotypical heterogeneity of human liver macrophages was analyzed with monoclonal antibodies that recognize antigens specific for the monocyte-macrophage lineage. Most liver macrophages in normal and diseased liver were positive for CD68, whereas fewer matured macrophages were detected by 25-F9. Comparative staining of mirror sections revealed some to be doubly positive and others to be singly CD68 positive. Quantitative analysis confirmed the difference, suggesting heterogeneity of maturation in liver macrophages. Most liver macrophages in the normal liver were negative for CD14, a receptor for lipopolysaccharide and lipopolysaccharide-binding protein complexes. Liver macrophages in liver diseases were activated to express CD14 at varying degrees and were involved in the clearance of lipopolysaccharide-lipopolysaccharide-binding protein complexes. Fc gamma RI, a receptor for monomeric IgG that is involved in antibody-mediated cell cytotoxicity, was negative in the normal liver, but was expressed in liver macrophages at inflammatory sites (e.g., in piecemeal and focal necrosis) in diseased livers. Fc gamma RII was expressed in most liver macrophages, as well as in sinusoidal endothelial cells; Fc gamma RIII was expressed in a smaller number of liver macrophages. Expression of Fc gamma RII and Fc gamma RIII was increased in chronic active hepatitis. These results suggest that liver macrophages are heterogeneous in maturation and function and that they are activated in liver diseases as shown by the novel expression of CD14 and Fc gamma RI. The restricted expression of Fc gamma RI indicates that Fc gamma RI-positive macrophages, in cooperation with cytotoxic T lymphocytes, may play an important role in liver cell injury through antibody-mediated cell cytotoxicity.

[Long-term prognosis of the interferon-treated patients with chronic hepatitis C: compared with the natural course].

To evaluate the prognosis of the interferon (IFN)-treated patients with chronic hepatitis C (CH (C)), the clinical course and outcome of patients with CH (C) who were administered IFN more than 5 years before was compared with the natural course of untreated patients with CH (C) who were followed-up for 5 years. Good responders to IFN therapy showed normalized serum ALT level thereafter except one patient who were positive for serum HCV-RNA and had relapse of hepatitis. Histological improvements were observed in good responders. In groups of poor responders and non-responders, the fluctuation of serum ALT and histological changes after IFN therapy seemed to be similar to the natural course. Several problems on the prognosis are discussed.

Cloning and sequence analysis of cDNA encoding rabbit vitamin D-binding protein (Gc globulin).

The cDNA sequence of rabbit vitamin D-binding protein (DBP) was determined by sequencing clones isolated by immuno-screening and PCR amplification from a rabbit liver cDNA library. The 1641-nucleotide sequence included the 5' untranslated region and extended to the poly(A) tail. It encoded a protein of 460 amino acid residues in addition to a signal sequence of 16 residues, and had 84%, 79% and 78% sequence identities with human, rat and mouse DBP cDNA, respectively. In comparison with DBP amino acid sequences of other species, there are at least four heterogeneous regions. In contrast, a putative actin binding domain was conserved in the evolution process.

Glibenclamide reduces the coronary vasoactivity of adenosine receptor agonists.

Experiments in guinea pig heart Langendorff preparations assessed the effect of KATP channel blockade on the coronary vasoactivity of adenosine and 17 analogs chosen to represent a variety of purine and ribose modifications. Although glibenclamide is a functional antagonist that acts at the level of an effector rather than at a receptor, it caused parallel rightward shifts of agonist dose-response curves. The size of the shift of EC50 differed according to the kind of analog: the ranking was, generally, N6-phenethyladenosines > 2-aryl-aminoadenosines = 2-(1-alkyn-1-yl)adenosines > N6-cycloalkyladenosines = adenosine-5' -uronamides. The coronary vasoactivity ranking of agonists in the presence of supramaximal concentrations of glibenclamide was 2-(1-alkyn-1-yl)adenosines = 2-aralkoxyadenosines > 2-aralkylaminoadenosines > 2-arylaminoadenosines > N6-substituted adenosines. Glibenclamide did not affect the vasoactivity of adenosine itself, perhaps because avid uptake by endothelial cells prevented penetration of the agonist to receptors deeper in the vascular wall. The results exclude a model consisting of one kind of receptor acting exclusively through a KATP channel, argue against one kind of receptor coupled to a KATP channel as well as to an additional effector but is consistent with two kinds of vasodilatory adenosine receptors, one of which activates a KATP channel. The identity of the adenosine receptor coupled to the KATP channel is uncertain; the other receptor has the pharmacological profile of an A2a-adenosine receptor.

[Clipping of aneurysm arising from the posterior communicating artery itself by contralateral craniotomy: a case report].

A case of aneurysm arising from the posterior communicating artery itself clipped by contralateral frontotemporal craniotomy (pterional approach) is presented. A 65-year-old female developed sudden severe headache and chest pain in January of 1993. Neurological examination on admission revealed consciousness disturbance such as stupor and nuchal stiffness. CT-scan showed marked subarachnoid hemorrhage. She also suffered from acute myocardial ischemia and cardiac failure. Cerebral angiograms after recovery from cardiac dysfunction demonstrated three saccular aneurysms arising from the dilated right posterior communicating artery itself, the junction of the left internal carotid artery and the posterior communicating artery, and the bifurcation of the left middle cerebral artery. The left IC-PC junction aneurysm was thought to be ruptured because of its size and contour, so left frontotemporal craniotomy was undertaken. By the left pterional approach, successful clipping of all three aneurysms involving the one arising from the contralateral posterior communicating artery was achieved. The aneurysm at the posterior communicating artery itself was found to arise from the non-branching site and to project inferiorly, thus the successful clipping through the prechiasmal cistern could be performed without compromising any small perforating arteries.

[A case of aseptic meningoencephalitis in a patient with secondary Sjögren syndrome with systemic lupus erythematosus].

A 26-year-old woman, who had been treated with prednisolone 30 mg/day for secondary Sjögren syndrome (SjS) with systemic lupus erythematosus (SLE), was complicated with aseptic meningoencephalitis (AME). Cerebrospinal fluid (CSF) cell counts were 26/3, total protein 126 mg/dl, glucose 41 mg/dl. The CSF, blood, nasopharyngeal swab were cultured for bacteria, mycobacteria, viruses and fungi, but no evidence for an infectious etiology was obtained in this patient. The pair sera study for viruses titer were performed, but no significant titer elevation was observed. Head magnetic resonance imaging showed remarkable atrophy of bilateral occipital lobe and dilatation of posterior horn of bilateral ventricles. The high 2-5AS enzyme activity in the CSF suggested indirectly that the cause of this meningoencephalitis was virus infection. In this case microbiological studies were negative, so central nervous system SLE and/or central nervous system SjS were not completely neglected.

[A case of pineal teratoma with interesting radiological findings].

A rare case of mature pineal teratoma with interesting radiological findings in a 16-month-old infant is reported. The patient was referred to our clinic because of generalized convulsions. A CT scan showed marked hydrocephalus and a low density mass lesion without contrast enhancement in the pineal region. A CT cisternography demonstrated the lesion as a filling defect area on the image produced immediately after the emergent V-P shunt, and a filling area 24 hours later. The lesion was of signal intensity on T-1 weighted MR image and of high signal intensity on T-2 weighted MR image, equal to CSF intensity. These radiological findings were compatible with an arachnoid cyst in the quadrigeminal cistern, so we performed the excision of both anterior and posterior cyst walls using the occipital tentorial approach. However, the histology of the cyst wall was not compatible with that of an arachnoid cyst, showing neuroepithelial-like cell lining with positive staining for cytokeratine. The postoperative follow-up on MRI was continued for 31 months. An MRI performed 9 days after the first operation showed, compared with the gray matter, iso-signal intensity area and high signal intensity area on T-1 weighted image. At first, we thought this was because of bleeding of the pineal gland brought on by the operative maneuver. However, the mixed intensity lesion shown on T-1 weighted images gradually expanded and distorted, and finally showed the typical MR images for a teratoma. The operative findings using the occipital transtentorial approach were typical for a teratoma.(ABSTRACT TRUNCATED AT 250 WORDS)

[Myocardial protection during multiple coronary artery bypass grafting--a comparison of antegrade and combined antegrade-retrograde cardioplegia].

We compared the effectiveness of antegrade cardioplegia (A group, 100 cases) and combined antegrade-retrograde cardioplegia (A + R group, 66 cases) on myocardial protection in patients undergoing coronary artery bypass grafting for more than 3 vessels. The two groups were similar in patients' age, the extent of coronary artery diseases and preoperative risk factors, cardiac output and ejection fractions. The mean number of grafts was 3.3 in A group and 3.6 in A + R group (p < 0.05). The mean number of ITA grafts used was 1.2 in A group and 1.4 in A + R group (p < 0.05). Aortic cross-clamp time was 114 minutes in A group and 138 minutes in A + group (p < 0.01). The serum CK-MB concentration on postoperative day 1 was 38 IU/L in A group and 25 IU/L in A + R group (p < 0.05). The left ventricular stroke work index 3 hours after reperfusion was 33 gm min/m2 in A group and 43 gm min/m2 in A + R group (p < 0.05). The maximum dose of catecholamine required was similar in two groups. Intra-aortic balloon pump was used in 11 patients (11%) in A group and in 2 patients (3.6%) in A + R group (p < 0.05). We conclude that the combined method of intermittent antegrade and continuous retrograde perfusion of cardioplegic solution provides better myocardial protection than intermittent antegrade cardioplegia alone for multivessel coronary artery bypass surgery.
