An epidemiological study on cancer in certified arsenic poisoning patients in Toroku.

A mortality study has been made to assess the carcinogenic effect of arsenic exposure by analyzing a group of certified chronic arsenic poisoning patients at a small mining village in Japan. When compared with expected number of deaths, based on sex-, age-specific mortality of Japanese in 1975, '80 and '85, all cancer, cancer of respiratory system and cancer of bladder, kidney and other and unspecified urinary organs showed a significant excess over the expected number. Divided by employment history, death due to all cancers, to respiratory cancer and to urinary tract cancer showed a significant excess among former workers of the mine. Classified into four cohorts by both employment history and smoking habit, death from respiratory cancer showed a significant excess only in the cohort respiratory cancer showed a significant excess only in the cohort of former workers with smoking history, suggesting an interaction between exposure to arsenic and smoking. Eight cases among ten respiratory cancers and two urinary tract cancer cases were accompanied by cancer of other organs, including Bowen's disease.

Accumulation and metabolism of 2,4,6-trichlorophenyl-4'-aminophenyl ether by carp.

1. Accumulation of 2,4,6-trichlorophenyl-4'-aminophenyl ether (CNP-amino) in carp was investigated for 14 days. CNP-amino in carp could be divided into free CNP-amino [CNP-amino(I)] and bound CNP-amino [CNP-amino(II)]. 2. The bioconcentration factors (BCF) of CNP-amino(I + II) in carp were 90 +/- 38 (mean +/- SD, n = 3) for muscle, 402 for liver, 501 for kidney and 5368 for gallbladder after 14 days exposure. 3. 2,4,6-Trichlorophenyl-4'-acetamide phenyl ether (CNP-acetamide) was detected as metabolites of CNP-amino in muscle and viscera of carp.

Bioconcentration and excretion of diazinon, IBP, malathion and fenitrothion by carp.

1. Bioconcentration and excretion of diazinon, IBP, malathion and fenitrothion were studied for carp (Cyprinus carpio L.). 2. The concentrations of these pesticides in muscle and viscera of the carp reached plateaus in 12-48 hr exposure. 3. The average values of bioconcentration factors (BCF) for diazinon were 20.9 in muscle, 60.0 in liver, 111.1 in kidney and 32.2 in gallbladder over the 168 hr exposure period. Similarly, those values were 4.3-26.7 for IBP, 2.7-17.3 for malathion, and 36.0-157.1 for fenitrothion. 4. The excretion rate constants of malathion (hr-1) were 0.13 for muscle, 0.12 for liver, 0.08 for kidney and 0.06 for gallbladder. Those of diazinon, IBP and fenitrothion (g.ng-1.hr-1) were 0.002-0.024 for muscle, 0.001-0.020 for liver, 0.0004-0.004 for kidney and 0.002-0.023 for gallbladder, respectively.

[Temperature-dependent pseudoleukocytopenia and in vitro studies on the underlying mechanism].

Spuriously low electronic white-cell count was obtained on EDTA-anticoagulated blood from a 54-year-old male suffering from liver cell carcinoma. In an attempt to understand better the phenomenon, in vitro studies were performed. The blood smear anticoagulated with EDTA revealed remarkable agglutinations of leukocytes, mainly neutrophils but not lymphocytes. The EDTA-treated blood showed maximal leukocytes-agglutinations at room temperature but no agglutination at 37 degrees C. The agglutinated leukocytes, moreover, were dispersed by incubating the blood at 37 degrees C. Although agglutination occurred in some degree in the blood anticoagulated with Na-heparin or Na3-citrate, it was much less pronounced compared to the EDTA-treated blood. Remarkable leukocytes-agglutinations were induced when EDTA-anticoagulated blood from healthy donor was mixed with serum from the patient at room temperature. These results suggest that the phenomenon of in vitro leukocyte agglutination and consequent pseudoleukocytopenia is due to leukocyte agglutinin in serum from the patient. The mechanisms were discussed comparing with pseudothrombocytopenia caused by platelet agglutinin.

[Immunohistochemical analysis of T-lymphocyte subsets on the sarcoid lymph nodes using the computerized analytic system for the light microscopic figures: the relation to the histological maturation stages of the lymph nodes].

Lymph nodes obtained from 19 patients with sarcoidosis were classified into four groups according to their histological maturation stages, which were the early stage, the former mature stage, the late mature stage and the healing stage. Immunohistochemical analysis of CD4 positive cells and CD8 positive cells were performed using monoclonal antibodies to these lymph nodes. Then the number of the positive cells per square millimeters of a granuloma was estimated using a computerized analytic system for light microscopic figures. The number of CD4-positive cells per square millimeter of a granuloma was almost constant from the early stage to the mature stage, but inclined to decrease in the healing stage; on the other hand, the number of CD8-positive cells per square millimeter of granuloma decreased extremely in the mature stage, compared with the early stage and the healing stage. Moreover the CD4/CD8-positive cell ratio in granulomas, calculated from these data, was elevated in the mature stage, compared with the early and the healing stages. From the above results, the CD4/CD8-positive cell ratio in a granuloma was demonstrated to be a valuable index for evaluating the histological activity of the granuloma of sarcoid lymph nodes. Furthermore, the decrease of CD8-positive cells in mature stage granuloma was considered to be related to the maturation of the epithelioid cell granulomas in sarcoidosis.

[A case of Hurthle cell carcinoma in the superior mediastinum].

This is a report for an unusual case of oxyphilic cell adenocarcinoma originating from anterior mediastinum in a 36-year-old male who complained of his neck lymphoadenopathy. 201Tl whole body scanning showed increased uptake of the left supraclavicular and upper mediastinal regions. There was no history of prior operation nor irradiation to the thyroid or neck region. Chest CT scanning also demonstrated the tumor in the superior mediastinum, but the mass has no increased 67Ga uptake. No tumor in the thyroid lobes was apparent on thyroid scintiscanning, ultrasonography and neck CT. The mediastinal tumor resection, the right and left thyroid lobectomy and the neck lymphonodectomy were completed. Serial sectioning of the resected thyroid lobes failed to show any tumorous tissue. The light microscopic features of a Hurthle-cell tumor arising in the mediastinal ectopic thyroid and diagnosed by neck lymph node biopsy were presented. The tumor was clinically malignant, having metastasized to the bone and the neck lymph nodes. Non surgical treatment including radioactive iodine (131I) therapy and combination chemotherapy were disappointing. The patient deteriorated progressively and died 16 months after thyroidectomy. We describe here an unusual case of a papillary growing and thyroglobulin producing tumor in the superior mediastinal region without evidence of a primary thyroid gland tumor.

[A case of giant osteoma in the occipital bone].

A 51-year-old female patient, admitted with a chief complaint of dizziness, had bulging of the occipital area, which had started insidiously. On admission, a bony hard swelling with a smooth surface was noted in the left occipital region, extending over the midline of the skull. Plain X ray films revealed a uniform shadow of an osteoid mass inside the occipital bone. A tomogram indicated that the mass originated in the diploë or outer table of the skull, although there was no evidence of destruction of the inner table. CT scan revealed mixed density mass with high density of bone and an irregularly shaped low density area. In the T1-weighted images of MRI, the entire mass showed a low intensity, while in the T2-weighted image the mass showed 3 areas with different intensities. Bone scintigraphy was performed with 99mTc-MDP and revealed an image of accumulation in the mass. Carotid and vertebral angiograms were negative for evidence of feeding arteries to the mass, for tumor stain, and for occlusion of either sigmoid sinus or transverse sinus. The mass was diagnosed as a giant osteoma, and was totally removed. The excised tumor measured 10 x 9 x 5 cm, and inspection at operation revealed it to be composed of a fragile osteoid portion and a comparatively soft connective tissue portion. The histological diagnosis of the mass was that it was a spongy osteoma including a comparatively large amount of connective tissue. The postoperative recovery of the patient was satisfactory and her dizziness disappeared.(ABSTRACT TRUNCATED AT 250 WORDS)

[Acquired von Willebrand syndrome with autoimmune hemolytic anemia].

Acquired von Willebrand syndrome is reported in a 31-year-old woman with autoimmune hemolytic anemia (AIHA). The patient, who had no family history of bleeding, presented with a hemorrhagic diathesis of recent origin. Routine coagulation studies showed a normal platelet count, prolonged bleeding time and abnormal glass bead retention. Plasma levels of factor VIII-von Willebrand factor (F VIII-vWF) were low and ristocetin-induced platelet aggregation (RIPA) was decreased. In vitro, the patient's plasma exhibited inhibitory activity against vWF: ristocetin cofactor activity (vWF: Rco) but had no effect on RIPA. The multimeric pattern of the patient's vWF resembled that of patients with inherited type II von Willebrand disease, in that the largest multimers were missing. Clinical improvement resulted after treatment of AIHA with prednisolone (PSL), and F VIII-vWF returned to normal levels with normalization of vWF multimers. However, following tapering the dose of PSL, plasma levels of F VIII-vWF again decreased.

[A case of primary leiomyosarcoma of the lung].

We reported a case of 66-year-old female with leiomyosarcoma of the right lower lobe of the lung with the complaint of abnormal shadow of the chest roentgenogram. On December 2, 1987, partial resection was performed and the tumor was totally removed. Histological examination revealed primary leiomyosarcoma of the lung. By electron microscopic observation, pinocytotic vesicle was not observed, but myofilaments were observed in the cytoplasm.

Immunobiology of aging and cancer.

Although the incidence of neoplasms is increased in the elderly, some tumors appear to grow more slowly in old as compared to young patients. We have used the B16 melanoma to explore the relationship between age, T-lymphocyte function, and the rate of tumor growth. Increasing age is associated with a decreased rate of tumor growth and impaired T-cell function in C57BL/6 mice. Furthermore, when T-cell immunity in young mice is compromised by thymectomy, B16 tumor growth is decreased. Mixed cell transfer studies demonstrated that T cells from young but not old donors stimulate the growth of B16 melanoma cells in young lethally irradiated recipients. Recently, T cells from young but not old mice have been reported to produce angiogenic factors. As tumors from young mice have a richer vascular supply than in old mice, it appears that one mechanism for the age-associated decrease in B16 melanoma growth is the decreased capacity of T cells from old mice to generate angiogenic factors. However, parabiosis of young and old mice showed that local factors in old mice also limit the stimulatory influences of T cells from young mice. Novel therapeutic approaches to limit the growth of tumors might result from a greater understanding of the production of and response to angiogenic factors produced by T cells.

[The marrow granulocyte reserve in lung cancer patients].

The marrow granulocyte reserve (MGR) of patients with lung cancer have been estimated by measuring the maximum neutrophil increment after administration of hydrocortisone. The MGR was found to have decreased in four of 19 untreated patients, but the reason for this decrement was not clear. Further, there were no differences in the MGR between the cell types or cancer stages. The MGR in patients 70 years old or older tended to be decreased. After chemotherapy or radiation therapy, the MGR depressed significantly. However, on administration of OK 432, the MGR significantly increased in two patients who had received radiation therapy and whose MGR had been deficient initially.

[A sporadic case of protein C deficiency].

A congenital deficiency of protein C (PC) is reported in a 42-year-old male, suffering from his first spontaneous episode of deep venous thrombosis in the left lower limb. The only defect found in laboratory assays for hemostasis and hepatic function was half normal level of PC, measured by both immunological and functional assays. To confirm congenital PC deficiency, the functional activity levels of PC were compared with those of other vitamin K-dependent factors during stabilized anticoagulant therapy under stable conditions. Although the patient's father had a history of a cerebral vascular accident, his PC level was found to be within normal levels. The patient's mother, free from thromboembolic events, also had a normal PC level. So the patient seemed to be a sporadic case. However, the patient's 14-year-old son, who has been asymptomatic to this time, has the same PC deficiency state.

[A case of multiple inflammatory tracheo-bronchial polyps associated with chronic sinusitis].

A 57 year-old woman was aware of breath odor and visited to our hospital. There were no abnormal findings in her physical examination and chest X-ray. Multiple polyps in the trachea and both main bronchi were discovered by fiberoptic bronchoscopy. They were 0.1-0.5 cm in diameter. In a histological examination, squamous metaplastic epithelial cells and numerous inflammatory cells infiltrating subepithelially were seen. No malignant cells were found. The patient had suffered from chronic sinusitis for 15 years, and it was suggested that the development of tracheo-bronchial polyps was associated with chronic sinusitis.
