Blood pressure, plasma norepinephrine (NE) and epinephrine (E) levels in responses to oral glucose load (OGTT) and to head-up tilting (Tilt) were measured in 14 hypertensive (HT) and 17 normotensive subjects (NT). In response to OGTT, NE significantly increased within 30 minutes (+25 +/- 8%; p less than 0.05) in NT, followed by a decline to the basal level, while it remained unchanged in HT (+5 +/- 8%). E decreased similarly during OGTT in both groups. Mean blood pressure, however, fell only in HT (-3.3 +/- 1.1%; p less than 0.05) but did not change in NT. Blood pressure, NE and E responses to Tilt did not differ between NT and HT. These results suggest that hypertensive subjects have some defects in sympathetic activation in response to glucose loading.

The influence of a family history of hypertension on body mass index, serum lipids, uric acid, blood sugar and haematocrit was investigated in 579 young normotensives aged 19-35 years with or without a parental history of hypertension. Body mass index, total cholesterol, triglycerides and uric acid were significantly higher in offspring with a parental history of hypertension than in those without. There were no differences in blood sugar and haematocrit between the two groups. Multivariate analysis revealed that a parental history of hypertension influenced levels of body mass index, triglycerides and uric acid, independently of the level of blood pressure.
We evaluated whether or not increased sodium (Na) concentrations of cerebrospinal fluid (CSF) and stimulated activities of brain renin-angiotensin system (RAS) contribute to an enhanced hypertension by salt overload in spontaneously hypertensive rats (SHR). Long-term salt loading (1% NaCl solution as drinking fluid) accelerated the development of hypertension in SHR, but did not alter the blood pressure (BP) in normotensive Wistar-Kyoto rats (WKY). CSF Na concentration was elevated in uninephrectomized (Nx) group as compared to that in control SHR, while in WKY CSF Na was not influenced by the treatment. A fall in BP by intravenous AVP antagonist or hexamethonium was greater in salt-loaded SHR than in controls. This hypotensive response to the combined blockade of AVP and SNS correlated with CSF Na in SHR but not in WKY. Plasma concentration of AVP and epinephrine tended to increase in relation to the degree of salt loading in SHR but not in WKY. Pressor responses to intracerebroventricular (ICV) angiotensin II (AII) and NaCl were greater in SHR than in WKY, although these responses were not influenced by chronic salt load in either SHR or WKY. The enhanced hypertensive action of ICV NaCl in SHR was abolished by pretreatment with ICV AII antagonist. Chronic saline drinking enhanced the depressor effect of ICV captopril in SHR but not in WKY. These observations suggest that salt overload in SHR may cause an elevated CSF Na concentration and an enhanced activity of brain RAS, which may increase activity of SNS and release of AVP, resulting in an enhanced development of hypertension.

Plasma renin activity (PRA) and plasma aldosterone concentration (PAC) were measured in 19 cases with primary aldosteronism (PA) and 72 with essential hypertension (EHT) to differentiate the two disorders in the following conditions: after overnight recumbency (basal state) and after oral administration of captopril. Screening criteria were determined in order to pick up all of PA patients as positive. After the captopril administration, the specificity of a criterion based on the combination of PAC and PAC/PRA ratio was 93% and positive predictive value 79%. This criterion was superior to other conventional screening methods. However, higher specificity (97%) and positive predictive value (90%) were obtained from a combination criterion based on the basal PAC and PAC/PRA ratio. The single oral administration of captopril may not bring an improvement in the screening of PA.
UNLABELLED: A retrospective analysis was performed comparing the survival in 483 patients with advanced breast cancer during the period from 1962 to 1986. RESULTS: Initial bone metastasis was found in 22%, and in 77% during course. The sites of bone metastasis were found more frequent in lumbar, thoracic, pelvis, rib, extremity and skull. Complications required with bone metastases were found in 19% of hypercalcemia which serum calcium level was more than 11 mg/dl, 14% of bone fracture, 10% sever spinal cord compression. Each complications caused by bone metastases had influence on the quality of life, but it had not effect on the patients survivals. Multidisciplinary treatment has been utilized as systemic therapy, such as combined chemoendocrine therapy. As local intensive therapy, the patients were treated with irradiation or surgical therapy. The patients with hypercalcemia were treated with calcitonin and mithramycin. Those treatments were useful for the prolongation of survival and the improvement of quality. As a result of those therapies, recently, the survival has been significantly prolonged. Especially, the survival in effective cases with those treatments was prolonged than the uncontrolled groups.
A retrospective analysis was performed comparing the survival in 483 patients with advanced breast cancer during the period from 1962 to 1986. Recently, the five-year survival rate at the start of therapy to the primary tumor was 63% and the five-year survival rate after the recurrence was 25%. The survival has been significantly prolonged as opposed to the figure of less than 5% among patients of 15 years ago. Multidisciplinary treatment has been utilized as adjuvant and recurrent therapies, such as combined chemotherapy, chemo-endocrine therapy and irradiation. Within these patients, 13 cases (3%) survived for more than ten-years from recurrence. The patients were recurrent cases with a small amount of metastatic sites and responsive to long-term endocrine therapy.
Plasma renin activity (PRA) and plasma aldosterone concentration (PAC) were measured in 19 cases with primary aldosteronism (PA) and in 72 with essential hypertension (EHT) to differentiate the two disorders during the following conditions on normal salt diet: after overnight recumbency (basal state) and 2 hours after oral administration of 25 mg of captopril. Screening criteria were determined so that all PA patients were diagnosed as positive, and their specificities were compared with those of other conventional screening methods for PA. After captopril administration, the specificity of a criterion based on a combination of PAC and PAC/PRA ratio was 93% and positive predictive value was 79%. This criterion was superior to blood pressure response to angiotension II analog infusion, PRA on salt depletion, and to PAC on salt loading. However, higher specificity (97%) and positive predictive value (90%) were obtained from a criterion based on a combination of basal PAC and PAC/PRA ratio. Therefore, the use of a combination criterion based on PAC and PAC/PRA ratio at basal state rather than after captopril administration may give a satisfactory result in the screening for PA.
We examined the central action of NaCl on blood pressure using intracerebroventricular (i.c.v.) and intracisternal (i.c.) injections of hypertonic NaCl solution in conscious, unrestrained spontaneously hypertensive rats (SHR) and normotensive Wistar-Kyoto (WKY) rats. The dose-dependent pressor response produced by i.c.v. hypertonic NaCl was greater in SHR than in WKY rats, while the dose-related pressor action produced by i.c. NaCl did not differ between the two strains. The hyperresponsiveness to i.c.v. NaCl in SHR was abolished by pretreatment with an i.c.v. injection of the angiotensin II (ANG II) analogue 1-Sar, 8-IIeu ANG II. Both ANG II and a combination of ANG II and NaCl given by i.c.v. injection had a greater pressor response in SHR than in WKY rats, although both ANG II and phenylephrine given intravenously elevated blood pressure to the same extent in both strains. Furthermore, i.c.v. ANG II both with and without hypertonic NaCl caused dipsogenic behaviour which lasted longer in SHR than in WKY rats. This response to i.c.v. hypertonic NaCl without ANG II was not substantially different between the two strains. Intracisternal hypertonic NaCl did not induce drinking behaviour. These observations suggest that in the SHR, the third ventricle rather than the brain stem is a more sensitive area to NaCl. The brain renin-angiotensin system in the SHR may play an important role in this accelerated pressor response and may be responsible, at least to some extent, for the enhanced reaction to chronic oral salt loading.

Tissues of 50 small cell lung carcinomas were examined for production of 17 peptide hormones. Only when the concentration of a peptide detected in the tumor was 10 pmol or more per g wet weight, was the peptide considered to be produced by the tumor. The frequency of production of at least one of these peptide hormones was 84%, and that of two or more hormones was 50%. These results indicate that peptide hormone production is a very common phenomenon in small cell lung carcinoma. Of the peptide hormones examined, gastrin-releasing peptide is produced with the highest frequency, suggesting that this peptide could play an important role in small cell lung carcinoma.
A sensitive radioimmunoassay for human c-myc protein was developed by using an amino-terminal fragment of c-myc protein, c-myc(11-24). Immunoreactive c-myc protein was found to be present in 3 human tumors (HL-60, N231, Lu-65), which are known to have c-myc gene amplification. In contrast, c-myc protein was undetectable in 1 human tumor (H69) without c-myc gene amplification as well as in human heart tissues.
