Effect of diabetic sera on the conversion of eicosapentaenoic acid (EPA) to prostaglandin I3 by cultured bovine aortic endothelial cells.

The effects of sera from diabetic patients and healthy donors on the synthesis of prostaglandin I2 (PGI2) and PGI3 in vitro were studied in confluent bovine aortic endothelial cells cultured with EPA. The products 6-keto-PGF1 alpha and delta 17-6-keto-PGF1 alpha were measured by GC/SIM as markers of PGI2 and PGI3 formation in growth medium after 60 min of incubation. PGI2 and PGI3 synthesis with 10% diabetic sera were less than with sera from healthy donors (p < 0.05). However, the total prostacyclin production (PGI2 and PGI3) in the cell cultures incubated with 10 microM EPA and 10% diabetic sera approximated that of the cultures incubated with the sera of healthy donors without EPA. These results suggest that the diabetic sera inhibits PGI2 and PGI3 synthesis in the cultured endothelial cells, and that EPA intake may reduce the complications of diabetes mellitus, such as microangiopathy and vaso-occlusive diseases, and enhances the production of PGI3 which seems to exert a strong anti-aggregatory effect.

Endovascular therapeutic occlusion following bilateral carotid artery bypass for radiation-induced carotid artery blowout: case report.

A patient with breast cancer received radiation therapy to the upper chest wall. Twenty-two years later, she presented with repeated severe bleeding through a left lower neck ulcer. She was taken to surgery for hemostasis, which was not successful because the carotid artery was surgically inaccessible. To manage for explosive carotid blowout, we performed common carotid artery ligation and endovascular coil embolization after contralateral-external-carotid to ipsilateral-common-carotid artery bypass with a polytetrafluoroethylene (PTFE) graft. The patient has experienced no ischemic events or bleeding since this treatment.

Mechanism of action of nicotine in isolated urinary bladder of guinea-pig: involvement of tachykinin(s) released by nicotine in the drug's sympathomimetic effect.

A sympathetic neurone blocking drug, guanethidine, and a tachykinin antagonist, [D-Arg1, D-Pro2, D-Trp7,9, Leu11]-substance P (rpwwL-SP), partially inhibited the contractile response to nicotine to the same degree in the isolated detrusor strips of guinea-pig urinary bladder. Application of rpwwL-SP completely abolished the inhibitory effect of guanethidine on the nicotine-induced contraction, suggesting that the tachykinin(s)-ergic transmission might be involved in the sympathomimetic effect of nicotine. Conversely, when the preparation was treated with guanethidine to block release of a mediator from the sympathetic nerve, the inhibitory effect of rpwwL-SP was diminished, suggesting an exclusive contribution of the sympathetic nerve communications to the action of the tachykinin(s). We previously suggested that nicotine may release acetylcholine and ATP to contract the detrusor strips, and that acetylcholine output may be increased by an unknown substance released from the sympathetic nerve by nicotine. In preparations treated with atropine, rpwwl-SP had no effect on the nicotine-induced contraction. The concentration-response curves for carbachol and ATP were not influenced by rpwwl-SP. After tachyphylaxis to capsaicin developed, the nicotine-induced contraction was not affected. It is suggested that in guinea-pig detrusor, tachykinin(s) from capsaicin-insensitive sites is (are) involved in the excitatory sympathomimetic effect of nicotine, and that the tachykinin(s) behave(s) as a modulator finally to increase the acetylcholine output from the parasympathetic cholinergic nerve.

Gastric leiomyoblastoma: report of a case.

The case of a 38-year-old man with an exogastric leiomyoblastoma is reported. CT and ultrasound examinations revealed a large mass in the left hypochondrium that had both solid and cystic components. These findings mimicked those of cystadenoma of the pancreas. Because of intraperitoneal hemorrhage in the preoperative course, emergency laparotomy was performed. A large tumor was found to arise from the greater curvature of the stomach. The diagnosis was confirmed histologically.

Astrocytomas arising in the tectal region of the mesencephalon: report of two cases.

Astrocytoma arising from the tectum of the mesencephalon is an unusual tumor that is different from other brain stem tumors in clinical features. We describe two patients with tectal low-grade astrocytoma. Each patient underwent CSF diversion and biopsy followed by radiation therapy. A review of the literature on the clinical, radiographic, and pathological findings and the management of tectal tumor is presented.

Significance of informed consent and truth-telling for quality of life in terminal cancer patients.

For 12 patients with terminal stage cancer who died within the period from June 1995 to the present, we retrospectively evaluated the correlation between the "information" concerning disclosure of the "diagnosis," "pathology," and "prognosis," with the length of the last admission before the death, "sedation" near death, and the choice of "do not resuscitate (DNR)." The average length of admission before death was markedly shorter for patients who had been told either the "diagnosis," "pathology," or "prognosis" than for patients who had not. A statistically significant difference was observed between those who had been told and those who had not been told the "pathology." Similarly, "sedation" tended to be done for those who had been provided with information on cancer. It was suggested that telling patients with terminal stage cancer the truth about "diagnosis," "pathology," and "prognosis" is important for them to spend a fulfilling terminal stage.
