The uncoating of paramyxoviruses may not require a low pH mediated step.

The effect of lysosomotropic agents, chloroquine and NH4Cl, on the replication of paramyxoviruses was compared with that on the other enveloped RNA viruses whose uncoating is known to be blocked by the agents. Under the conditions where vesicular stomatitis or influenza virus infection was strongly inhibited at an early step, the agents exhibited little inhibitory effect on the progress of infection by Newcastle disease virus and Sendai virus, allowing viral primary transcription to occur and neither inhibiting the virus production significantly nor reducing the number of infected cells. These results are incompatible with the previous report showing that paramyxovirus and the other enveloped RNA viruses may have in common an intracellular step sensitive to lysosomotropic agents (Miller and Lenard, Proc. Nat. Acad. Sci. USA 78, 3605-3609, 1981) and suggest that paramyxovirus uncoating may not require a low pH mediated lysosomal step.

[Surgical management of the gastroesophageal varices--experience with 118 cases].

Between June 1968 and May 1982, 118 patients with esophagogastric varices due to portal hypertension were treated and one hundred thirteen of these underwent surgical interventions. Ninety-eight patients had liver cirrhosis, fourteen were diagnosed idiopathic portal hypertension and one was diagnosed extrahepatic portal obstruction. More progressed impairment of hepatic function and severe manifestation of clinical symptoms were observed in the cirrhotics, compared with those in non-cirrhotics. We preferred the one-staged combined procedure of transthoracic esophageal transection, transdiaphragmatic splenectomy and paraesophagogastric devascularization, because of its therapeutic effectiveness and technical feasibility. Splenectomy with or without proximal gastrectomy was chosen for the remaining small number of the patients. While the overall operative mortality was 12.4%, 14 patients (13.5%) succumbed within a month after esophageal transection. These procedures were contraindicated for the patient with uncontrollable ascites, jaundice and encephalopathy and should be carried out electively because of high mortality of emergent operation.

A case of lead intoxication: clinical and biochemical studies.

A 23-year-old Japanese male with severe lead intoxication accompanied by hemolytic anemia was studied. The patient had taken 12 g to lead in about a month. He had moderate hemolytic anemia (Hb 8.9 g/100 ml) with reticulocytosis ranging from 2.5 to 11.7%. Peripheral blood smear showed nucleated red cells (42/200 white blood cells) and marked basophilic stippling in the red cells. Activities of erythrocyte enzymes were either normal or increased except for pyrimidine 5'-nucleotidase (P5N) and delta-aminolevulinic acid dehydratase (ALA-D) both of which were found to be decreased, being 48.8% and 4.1% of the normal controls respectively. Erythrocyte reduced glutathione (GSH) was high (145.6 mg/100 ml RBC). Erythrocyte pyrimidine nucleotides were accumulated up to 10.2% of total nucleotides. The level of lead in peripheral blood was 112 microgram per 100 ml blood. Ca2+-Na2-EDTA was given to the patient as treatment. The level of lead in the blood decreased gradually and hemolytic anemia improved. Basophilic stippling in the red cells disappeared. These results confirmed the findings of Valentine et al [1] that lead-induced deficiency of P5N resulted in basophilic stippling and hemolytic anemia just like hereditary hemolytic anemia due to P5N deficiency.

Peptidoaminobenzophenones, a novel class of ring-opened derivatives of 1,4-benzoidazepines.

A series of noval peptidoaminobenzophenones has been prepared via several routes and was evaluated for CNS activity. The structure--activity relationships in the series are discussed. In general, dipeptido-N-methylaminobenzophenones showed higher activities than the corresponding NH derivatives. Some compounds had very high activities in antipentylenetetrazole and antifighting tests in mice when orally administered. Very weak toxicity was also found in these compounds. Water solubility of the peptidoaminobenzophenones and their salts were tested. Possible in these compounds. Water solubility of the peptidoaminobenzophenones and their salts were tested. Possible in vivo conversion of peptidoaminobenzophenone by enzymatic cleavage of the terminal amino acid, followed by chemical cyclization to 1,4-benzodiazepine, is also discussed. Such novel open-ring derivatives of 1,4-benzodiazepine may serve as useful CNS agents.

Korean haemorrhagic fever in staff in an animal laboratory.

In March, 1975, and January, 1978, there were outbreaks of Korean haemorrhagic fever (K.H.F.) among staff in the animal laboratory of the clinical research building of Tohoku University hospital. 13 doctors and 1 veterinarian had K.H.F., and all except 1 had a mild but typical clinical course. K.H.F. antibody titres were significantly raised in all 14 patients. 12 other doctors working in the same room had no symptoms and no K.H.F. antibody. K.H.F. antibody was also detected in 3 animal technicians who had not been ill, but who had worked in the same room. Antibody was not detected in any of the 4 technicians who worked in different animal rooms in another building. Epidemiological studies suggest that laboratory rats were the source of the infection. High titres of K.H.F. antibody were most common in rats reared in the animal laboratory associated with the outbreak.

[Syphilotherapy with amoxicillin, a synthetic penicillin (author's transl)].

Amoxicillin, a synthetic penicillin preparation, was given to 25 cases as an antiluetic. As for 13 cases of them, one year or more have passed since the treatment finished. 1. A daily dose of 1,000 approximately 1,500 mg of amoxicillin was continuously administered for 4 weeks, as one course. After each course finished, administrated was stoped for 2 approximately 4 weeks for examination of results of TPHA estimation, and then another course was started. 2. Effective rate in 13 cases reached about 40%. The remaining 12 cases are now under treatment. 3. Exanthema, a side effect, was observed in one of 25 cases.

Inhibitory effects of various drugs on dual asthmatic responses in wheat flour-sensitive subjects.

In order to investigate the mechanism of late asthmatic response (LAR), inhibitory effects of various drugs for LAR were examined in two wheat flour-sensitive asthmatic subjects who showed immediate and late responses in the allergen provocation test and skin test. Antihistamines did not inhibit the LAR but totally or partially inhibited the immediate response. By contrast, corticosteroids inhibited the LAR but not the immediate response. Disodium cromoglycate inhibited both responses. Diethyl carbamazine citrate, an inhibitor of release of SRS-A, seemed to shorten the duration of the LAR, although it has no effect on the immediate response and/or on the severity of the LAR. Indomethacin and acetyl salicylate, which inhibit prostaglandin synthesis, had no significant effects on either the immediate or the LAR.
