[Clinical value of SCC-antigen, a subfraction of tumor antigen TA-4, in the management of cervical cancer].

The present study shows the clinical values of SCC-Antigen, a subfraction of TA-4, for the management of squamous cell carcinoma. Serum SCC-Antigen levels of 2.0 ng/ml was designated as the cut-off value. Serum antigen levels were positive in 53.5% of cervical squamous cell carcinoma, and the changes in serum antigen levels reflected the disease progress. The antigen was also positive in cancer of vagina, vulva, lung or esophagus. To detect the early cervical cancer, flow cytometric analysis of the exfoliative cells was conducted, using anti-TA-4 serum, which was found to be particularly useful for the detection of early disease. It was concluded that the use of SCC-Antigen would be promising in clinical care.

The use of a tumor antigen TA-4 for the management of squamous cell carcinoma.

A radioimmunoassay for a tumor antigen TA-4 of squamous cell carcinoma has been used for the management of cervical cancer. The present study shows the clinical value of the TA-4 assay for squamous cell carcinoma of various origins. Pretreatment serum TA-4 levels were detectable in 44.4% of cervical cancer, 47.1% of vaginal and vulvar cancer, 33.3% of lung cancer, 42.9% of esophageal cancer, and 10% of cancer of other origins. In 313 control cases, including other histologic types of malignancies or benign diseases, one case showed a positive TA-4 level. Serial determinations of serum TA-4 levels were performed in patients with squamous cell carcinoma, which revealed that changes in TA-4 levels were rapid and were helpful for monitoring the disease progress and for detecting the recurrent disease. These results indicate that the TA-4 assay would be a useful aid for the management of squamous cell carcinoma of various origins.

Tumor-antigen TA-4 in the detection of recurrence in cervical squamous cell carcinoma.

A radioimmunoassay for a tumor-antigen (TA-4) of squamous cell carcinoma was used to detect the recurrence in patients with squamous cell carcinoma of the uterine cervix. Of 17 patients who had recurrence, 15 cases showed reappearance of serum TA-4 levels. Reappearance of serum TA-4 was faster than other clinical signs of recurrence in 11 cases. These results indicated that TA-4 assay would be a useful aid for the detection of recurrence in cervical squamous cell carcinoma.

A quantitative relationship between conceptus number and ovarian steroid dehydrogenase activity in pregnant rats.

The effects of a conceptus-derived substance on the activity of 3 beta-hydroxysteroid dehydrogenase (3 beta-HSD) and 20 alpha-HSD in the ovary were studied in the rat. On day 7 of pregnancy (day 1 = insemination), rats were laparotomized and the desired number of conceptuses was aspirated from the uterus; thus, rats carrying one, two, three, four, five to seven or eight to ten conceptuses were prepared. They were autopsied on day 15 and 3 beta-HSD and 20 alpha-HSD activity in the corpus luteum (CL) or non-luteal ovarian tissue ( NLO ) was determined. Conceptus number was directly related to 3 beta-HSD and inversely related to 20 alpha-HSD activity in the CL. The serum progesterone level and CL weight were also directly related to conceptus number. Neither 3 beta-HSD nor 20 alpha-HSD activity in the NLO was affected by conceptus number. These results indicated that 3 beta-HSD and 20 alpha-HSD in the CL are probably regulated by placental hormone secreted in proportion to the number of conceptuses; in the NLO these enzymes may be controlled by a different mechanism.

Flow cytometric analysis of tumor antigen TA-4 in cervical squamous cells.

Flow cytometric (FCM) analysis of tumor antigen TA-4 was made on cervical cells from 24 patients with squamous cell carcinoma of the uterine cervix and from 22 patients without cancer by using an indirect immunofluorescence staining technique. Cellular TA-4 content was much greater in cancer cells than in non-malignant cells. In all cases of squamous cell carcinoma, FCM histograms showed a broad but conspicuous peak with higher fluorescence intensity, whereas such a fluorescence pattern as seen in malignant cases was not detected in 21 non-malignant subjects whose cells had low levels of the antigen. It would be possible to identify and sort cancer cells on the basis of cellular TA-4 content. Therefore, FCM analysis of TA-4 may provide the basis of a prescreening method for cervical cancer detection.

[Changes in ovarian 3 beta-hydroxysteroid dehydrogenase and 20 alpha-hydroxysteroid dehydrogenase activities in pregnant rats].

Changes in ovarian 3 beta-hydroxysteroid dehydrogenase (3 beta-HSD) and 20 alpha-HSD activities were studied in pregnant rats. The activity of 3 beta-HSD was determined by measuring the rate of conversion of pregnenolone to progesterone. The activity of 20 alpha-HSD was determined by measuring the rate of conversion of 20 alpha-hydroxy-pregn-4-en-3-one to progesterone. 3 beta-HSD activity in corpora lutea (CL) was low between days 8 and 12, increased rapidly on day 15, and then gradually decreased until day 23. 20 alpha-HSD activity in CL markedly decreased from days 8 to 18, and then increased until day 23. The activities of 3 beta-HSD and 20 alpha-HSD in non-luteal ovarian compartment (NLO) showed no significant changes between days 8 and 23. These results indicated the important role of these enzymes in the secretion of progesterone from CL. It was also suggested that these enzyme activities in NLO might be controlled by a different mechanism.

Heterogeneity of a tumor antigen TA-4 of squamous cell carcinoma in relation to its appearance in the circulation.

The heterogeneity of a tumor antigen TA-4 of squamous cell carcinoma was studied by using isoelectric focusing (IEF). IEF of squamous cell carcinoma tissue of the uterine cervix showed 2 major peaks of TA-4 with pI values of 5.9 to 6.2 (acidic TA-4) and 6.3 to 6.6 (neutral TA-4). The non-malignant squamous epithelium of the uterine cervix showed a major TA-4 peak at pI 6.3 to 6.6 (neutral TA-4), while TA-4 in the blood of patients with squamous cell carcinoma was mainly eluted at pI 5.9 to 6.2 (acidic TA-4). These results indicated that acidic TA-4 is implicated in the appearance of this antigen in the circulation of patients with squamous cell carcinoma.

[Tumor antigen TA-4 of squamous cell carcinoma in patients with cervical cancer].

A tumor-marker TA-4 was purified from squamous cell carcinoma of the uterine cervix. TA-4 is a glycoprotein with a molecular weight of approximately 48,000. Although some amounts of TA-4 activity have been found in the non-malignant squamous epithelium, there is a significant difference in TA-4 release between the malignant and non-malignant tissues, resulted in the specific detection of TA-4 in the circulation of patients with squamous cell carcinoma. Clinical studies with the TA-4 RIA have demonstrated that TA-4 assay is useful to evaluate the extent or prognosis of disease and effectiveness of treatments, and to detect early recurrent disease.

Anti-mutagenic chalcones: antagonizing the mutagenicity of benzo(a)pyrene on Salmonella typhimurium.

Several chalcone derivatives; e.g. Ro 09-0204, Ro 09-0323 and Ro 09-0501 were found to reduce markedly the revertant increase of Salmonella typhimurium TA100 by benzo(a)pyrene during the incubation with S-9 Mix. The antimutagenic activity was 100 - 700 times stronger than that of L-ascorbic acid. Effect on other mutagens, the structure activity relationship and the possible mechanism of action are briefly discussed.

Prognostic significance of the tumor antigen TA-4 in squamous cell carcinoma of the uterine cervix.

Prognostic values of a tumor antigen (TA-4) of squamous cell carcinoma were studied in 135 patients with invasive squamous cell carcinoma of the uterine cervix. In order to evaluate the host defense responses against cancer, the percentage of lymphocytes (percentages of lymphocytes in total leukocyte counts in the peripheral blood) was also determined simultaneously in each case. All patients were followed up for 2 years. Sixty patients with Stage II disease underwent a radical operative procedure, and all other cases (75 cases) were treated primarily with radiation therapy. In both groups of patients, the survival rate or disease-free rate was significantly worse in those with TA-4 levels of greater than or equal to 15 microunits/ml compared to that of those with lower levels. The prognosis was particularly poor in those patients who had TA-4 levels of greater than or equal to 15 microunits/ml and lymphocyte percentages of less than 30. It was concluded that the simultaneous determinations of serum TA-4 and lymphocyte percentages would be useful in predicting the prognosis of cervical squamous cell carcinoma.

Value of tumor-antigen (TA-4) of squamous cell carcinoma in predicting the extent of cervical cancer.

The value of a tumor-antigen (TA-4) of squamous cell carcinoma in evaluating the extent of disease was studied in patients who primarily had surgical treatments with a diagnosis of cervical squamous cell carcinoma. Pretreatment serum TA-4 levels were determined by a radioimmunoassay method. The extent of disease was determined by surgical evaluation and postoperative histologic examinations. Furthermore, all patients were followed for at least two years, and those patients who developed recurrence at the sites which were unresectable by radical hysterectomy were retrospectively regarded as the unresectable case. It was found that serum TA-4 levels reflected the extent of disease, and that high pretreatment TA-4 levels indicated the presence of the widespread tumor which was unresectable by radical hysterectomy.

Abortifacient effect and uterine cervix-dilating action of 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) in the form of a vaginal suppository (a randomized, double-blind, controlled study in the second trimester of pregnancy).

A newly synthesized 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) was clinically applied in the form of a vaginal suppository for therapeutic abortion of second trimester pregnancies. Its effects were studied in a double-blind test comparing it with an inactive placebo suppository. The study was conducted at 12 Japanese university hospitals. The number of patients was 125 in total, i.e. 63 receiving ONO 802 vaginal suppository (containing 1.0 mg ONO 802) and 62 receiving inactive placebo suppository. ONO 802 was more effective than placebo with a success rate of 87% (complete (71%) and incomplete (16%) abortions). The onset of uterine contractions was observed in 154.3+/-18.1 min. and the onset of uterine bleeding in 323.6+/-41.0 min. The expulsion of the fetus and placenta was observed in 955.4+/-97.0 and 961.6+/-97.0 min., respectively. The cervix dilating effect of ONO 802 was observed in 63.5% of the patients at 3 hours after the start of administration. Nausea, vomiting, abdominal cramps, diarrhea and pyrexia were noticed. However, all these side effects were transient and mild, requiring no treatment. No abnormality was observed in the puerperal course, duration of uterine bleeding or onset of subsequent menstruation following the therapeutic abortion. Therefore, the present study demonstrated that ONO 802 vaginal suppository was an effective and valuable drug for therapeutic abortion of second trimester pregnancies.

Role of the nongravid part of the uterus in the regulation of corpus luteum function in pregnant rats.

The effect of the nongravid part of the uterus on corpus luteum (CL) activity was studied in rats bearing a single conceptus during midpregnancy. On day 7 (day 1 = insemination), the number of conceptuses was reduced to one by aspirating all but one conceptus (the aspiration group), by removing all of of the uterus except that containing a single conceptus (the hysterectomy group), or by aspirating conceptuses, as in the aspiration group, plus ligating the uterus, as in the hysterectomy group, but without removing the uterus (the sham hysterectomy group). Rats with a full complement of conceptuses (the intact control group) served as additional controls. Blood samples were obtained on days, 10, 12, and 15, to determine the effects of these procedures on the increase in serum progesterone that normally occurs between days 12 and 15 and to observe any related changes in serum LH between days 10 and 15. On day 15, the mean serum prgesterone concentrations and weights of the CL in the aspiration and sham hysterectomy groups were significantly lower than those in the intact control group. In the hysterectomy group, however, serum progesterone was even higher than in the intact control group, although the CL were significantly smaller than in the intact control group and were not different from those in the aspiration or sham hysterectomy groups. Serum LH on day 10 was significantly lower in the hysterectomy group than in any of the other groups; the latter were not significantly different from each other. By day 15, serum LH in the intact group had fallen to less than 50% of the value on day 10, while in the aspiration group, it remained as high as on day 10. In the hysterectomy group, there was no significant difference in serum LH concentrations between days 10 and 15. These results suggest that the nongravid part of the uterus inhibits progesterone secretion, and that in its absence, even a single conceptus can increase progesterone secretion after day 12 as much as can a full complement of conceptuses, but without also increasing the size of the CL. The uterus may exert this effect either through the pituitary or in cooperation with a pituitary factor, possibly LH.

[The double contrast hysterography using CO2 insufflation (author's transl)].

The double contrast hysterography, which was performed by the continuous CO2 insufflation with the ordinary HSG, was applied to 162 infertile patients. Forty-nine of 162 patients showed the abnormal findings by this method. Hysteroscopic or histological examinations revealed that 26 of these patients had hyperplasia, 15 had polyp, 2 had myoma and 1 had intrauterine foreign body. When these results were compared with those obtained by ordinary HSG, it was found that the double contrast hysterography would be more useful to investigate the detailed intrauterine changes such as hyperplasia or polyp than the ordinary HSG, and, therefore, would be a useful aid in the clinical care.
