Chemotherapy of human cystic echinococcosis: comparative efficacy of mebendazole and albendazole.

The relative efficacy of mebendazole (MEB) and albendazole (ALB) was tested by treating 51 patients with single, multiple and multi-organ hydatid cysts; 28 patients were treated with MEB, 50-70 mg kg-1 body weight daily for six to 24 months, and 23 with ALB, 10 mg kg-1 body weight daily in four courses of 30 days with 15-day intervals between courses. The therapeutic effect during the follow-up was assessed by ultrasonography, computerized tomography and chest X-ray. The criteria used when assessing the results were: 'success', evidence of effect on all cysts; 'partial success', evidence of effect on some of the cysts; and 'no success', no changes in cyst morphology. Treatment with MEB was successful in eight cases (28.6%), partially successful in eight (28.6%) and unsuccessful in 12 (42.8%). Treatment with ALB was successful in 10 cases (43.5%), partially successful in 10 (43.5%) and unsuccessful in three (13.0%).

Factors influencing the response to chemotherapy in human cystic echinococcosis.

As the effectiveness of mebendazole and albendazole in patients with echinococcosis has been found to vary, we investigated some of the factors likely to be responsible. A total of 79 patients who were treated with mebendazole (44 patients) or albendazole (35 patients) were included in the study. Evaluation of the treatment results was based on the changes in cyst morphology, as evidenced by the results of X-ray radiography, sonography, and computed tomography, and on analysis of the findings in relation to parasitic and drug factors. The response of cysts according to their site did not vary much, with the exception of the poor response of bone cysts. A more important factor seems to be cyst size, since the treatment was more efficacious against smaller and younger cysts. The presence of daughter cysts should be regarded as an unfavourable factor for treatment response. Cyst multiplicity did not present insurmountable difficulties, provided the cysts were small and a prolonged course of therapy was undergone. The choice of drug used for the therapy was important, with the results supporting the advantage of albendazole. In planning the chemotherapy of hydatid disease, factors such as cyst condition and drug used should therefore be taken into consideration.

[Cytologic control of protoplast formation and regeneration in Streptomyces erythraeus, strain BTCC-2].

Experiments on protoplast formation and regeneration in S. erythraeus, strain BTCC-2 (Saccharopolyspora erythrae) were performed under microscopic control at all the stages. It was shown that the highest protoplast titer was provided by the mycelium grown in one step in the absence of glycine. For characterizing the protoplasts formed by the mycelium grown under different conditions, their regeneration capacity was estimated by microscopic examination of the protoplasts after 15-20-hour growth in microchambers and evaluation of the regeneration efficiency 7-10 hours later. Of interest was the fact of spontaneous development of colonies consisting of the protoplast-like cells (L-cells) in 15-20 hours. Such colonies were formed only by the protoplasts grown from the mycelium incubated in one step in the absence of glycine or in the presence of 0.1 per cent of glycine. Such conditions provided also the maximum efficiency of the protoplast regeneration. The long-term storage of protoplasts led to a decrease in their viability.

Evaluation of response to chemotherapy of human cystic echinococcosis.

Fifty-five patients with echinococcosis were treated with mebendazole (30) and albendazole (25). To determine changes in cyst morphology and to evaluate the varying degree of response, progress during follow-up was shown by radiography, computerized tomography and ultrasonography. Persistence of cysts without any change in the morphology and cyst enlargement were taken as criteria for unsuccessful treatment. Development of echogenic foci and increase in density of cyst fluid were considered as a therapeutic effect. Changes in cyst wall such as thickening and deposition of calcium and detachment of the cyst membrane may also indicate response. An essential criterion, for a positive drug effect, was cyst size reduction as well as shape deformation. However, the most convincing and reliable criterion was the complete disappearance of hydatid cysts. Comparative assessment of the therapeutic effect of the two drugs favoured albendazole.

[A case of tertiary hyperparathyroidism following kidney transplantation complicated by lithiasis].

For the first time in Bulgaria the authors present a case of tertiary (autonomous) hyperparathyroidism, persisting in a patient who had successful kidney transplantation. Subtotal parathyroidectomy six months after the transplantation led to complete cure. Associated with the underlying disease, a rare complication was also observed in the patient--calculosis in the grafted kidney, spontaneously eliminated 40 days after the parathyroidectomy.

Avian bone-derived cell line infected with osteopetrosis virus pts-56.

Cell culture derived from embryonal turkey bone was infected with osteopetrosis virus pts-56. After 13 passages the morphology of the infected cells was changed and later a cell line was established. Some features of this cell line are described and compared with a non-infected cell culture.

[Oblique terminal-lateral gastroduodenoanastomosis in surgery of stomach ulcer].

The authors analyse the results of surgical treatment of 316 patients with peptic ulcer after gastric resection completed by the formation of terminolateral gastroduodenoanastomosis after the Haberer-Finney techniques. In 189 patients vagotomy was carried out in addition to the operation. The postoperative mortality rate was 0.3% (due to incompetence of the sutures of the anastomosis). The incidence of nonfatal complications was 9.4%. The late-term results were good (according to Visick's grading) in 98.65% of patients.

[Genetic analysis of Streptomyces erythreus heteroclones. II. Determination of the distances between genetic loci on the map].

As a result of recombination experiments between auxotrophic mutants of S. erythreus BTCC2 haploid recombinants and heteroclones were isolated. A genetic map of S. erythreus, including 15 auxotrophic loci was constructed by genetic analysis of the segregants of the heteroclones obtained. The genetic distances between 7 key loci on the map were determined and the entire length of the map of about 105 standard recombination units was calculated.

Albendazole treatment of human cystic echinococcosis.

The effect of albendazole was investigated in 20 patients with single or multiorgan hydatid cyst disease. Albendazole was used at a dose of 10 mg/kg daily in cycles of 28 d for four cycles with 15 d between cycles. Follow-up ranged from 12-18 months. Response varied according to the organ involved, the most successful results being with liver and peritoneal cysts. A positive response was seen in 47% of 34 cyst sites and a partial response was seen in a further 38% of sites. Overall response was classed as completely successful in 30% of patients, and partially successful in 60% at the end of follow-up. Adverse events were rare, although occasional abnormalities of liver function were seen.

Albendazole treatment of multiple cerebral hydatid cysts: case report.

A patient with primary multiple cerebral hydatid cysts was treated with albendazole. The daily dose was 10 mg/kg, taken 3 times a day with main meals, in 4 one-month courses separated by intervals of 15 days. Computerized tomography did not reveal any cerebral cysts at the end of therapy; complete calcification was observed at the sites of previous cysts 12 months later. Albendazole was very well tolerated. The results of therapy suggest that albendazole is a valuable contribution to the treatment of cerebral hydatid cysts.

[Treatment of hepatic and pulmonary echinococcosis with mebendazole (Vermox)].

The therapeutic efficacy of Mebendazole (Vermox) was studied in 13 patients: 8 with liver and 5 with pulmonary echinococcosis. The drug was given in a dose of 50-70 mg/kg body mass. Good therapeutic effect was found in 4 patients (2 with liver nad 2 with pulmonary echinococcosis), partial therapeutic efficiency was registered also in 4 patients (2 with liver and 2 with pulmonary echinococcosis) and failure in 5 patients. The drug was more effective in pulmonary echinococcosis, in younger and smaller cysts than in older cysts with thick walls and presence of filial cysts. The efficacy of the treatment is related to the drug plasma concentration but the results depend on the cysts condition, too. The careful examination of the patients--X-ray, ultrasonography and computer tomography--before, during and after the treatment ensures objective evaluation of the treatment results.
