[Ultrasonographic diagnosis of intracystic breast tumors].

Ultrasonography was performed on 1805 patients with breast diseases during the past 3 years and 20 (1.1%) intracystic tumors were detected. Among 20 intracystic tumors, 14 were finally diagnosed as benign and 6 as malignant. None of the 6 malignant lesions was judged as exclusively malignant and only 3 were considered as suspiciously malignant by ultrasonography. Ultrasonography is effective in making the location of intracystic tumors clear but not suitable for distinguishing malignant tumor from those that are benign. On the contrary, among the 6 malignant lesions, 5 were diagnosed as malignant and 1 as suspiciously malignant by aspiration cytology. Therefore, a combined examination of ultrasonography and aspiration cytology is a useful method for preoperative diagnosis of intracystic breast tumors.

[A phase II study of 590-S (1-phthalidyl-5-fluorouracil) in patients with breast cancer--multi-center cooperative study--Cooperative Study Group of 590-S in Breast Cancer].

A phase II clinical study involving 16 institutions was performed for the treatment of breast cancer with 590-S, a new masked fluoropyrimidine derivative. A dose of 900 mg/day was administered orally for more than 4 weeks. Out of 33 cases enrolled, 18 were completely recorded and evaluable. The response rate was 33.3%: 2 CR and 4 PR. It was noteworthy that, out of 12 cases previously treated with other 5-FU derivatives, 5 cases (41.7%) were judged as effective. Of 26 cases, side effects were observed in 8 (30.8%). All of these were gastrointestinal disorders such as nausea, vomiting, and loss of appetite. 590-S is considered to be a useful antitumor agent. However, further investigation will be required.

Fibrinolysis in patients with acute promyelocytic leukemia and disseminated intravascular coagulation during heparin therapy.

Two patients diagnosed as having acute promyelocytic leukemia (APL) and disseminated intravascular coagulation (DIC) were closely followed by serial fibrinolysis and coagulation studies from the day of admission until completion of the first course of chemotherapy. One patient was treated with intravenous heparin and Trasylol (Bayer AG, West Germany) and the other received heparin therapy without Trasylol. In Patient 1, hyperfibrinolytic activity, not observed during the administration of Trasylol, developed with its discontinuance. In Patient 2, hyperfibrinolysis was observed coincidentally with a decrease in APL cells due to chemotherapy. These results indicate that hyperfibrinolysis in APL is not associated with DIC.

The significance of the fibrin/fibrinogen degradation product in serum of carcinoma patients with hematogenous metastasis.

Fibrin/fibrinogen degradation product (FDP) was measured in 270 cases with various carcinomas. An elevation of FDP (5 micrograms/ml or more) was observed in 68 cases (25%). Of the 68 elevated FDP cases, 5 (82%) were inoperable or received nonradical resection. Forty of those 56 cases (71%) showed hematogenous metastasis. FDP in those cases which showed hematogenous metastasis was found to be due to fibrinolysis accompanied by consumption coagulopathy.

[A prospective randomized trial comparing epirubicin and doxorubicin in advanced or recurrent breast cancer].

A randomized clinical trial comparing epirubicin (EPI) and doxorubicin (DX) was conducted in patients with advanced or recurrent breast cancer. The dosage employed was 60 mg/m2 for EPI and 40 mg/m2 for DX at intervals of three weeks. There were 40 patients entered into the EPI group and 39 into the DX group. Background factors analysed were well balanced in both groups. The response rate was 56.3% in the EPI group (5 CR and 13 PR among 32 evaluable patients), and 35.5% in the DX group (1 CR and 10 PR among 31 evaluable patients), and the difference between both groups was statistically significant (P less than 0.05) by U-test. The response duration and time to response showed no significant difference between both groups. The incidences and grades of hematologic and non-hematologic toxicities were nearly identical. We conclude that EPI is a useful drug for the treatment of advanced breast cancer.

[Results of clinical trials with a CMitF (cyclophosphamide, mitoxantrone, and 5-fluorouracil) regimen versus a CAF (cyclophosphamide, adriamycin, and 5-fluorouracil) regimen in advanced/relapsed breast cancer].

Randomized clinical trials were conducted in patients with advanced/relapsed breast cancer, using CMitF (mitoxantrone, cyclophosphamide, and 5-fluorouracil) regimen in comparison with CAF (cyclophosphamide, adriamycin, and 5-fluorouracil) regimen. The response rate was 50% (13/26) for the CMitF group and 50% (10/20) for the CAF group. Classified by disease site, CAF group showed a tendency for a higher response rate in soft tissue, but in bone and viscera the rates were similar for both regimen groups. The median weeks to response (range) was 5 (3-21) weeks for the CMitF group, and 8 (3-22) weeks for CAF group, a tendency for slightly earlier response thus being shown in the CMitF group. The median duration of response (range) was 10 (4-47) weeks for the CMitF group, and 9 (4-50) weeks for CAF group, whereas by the method of Kaplan-Meier, at 40 weeks after the start of therapy, the proportion showing response still continued to be 60% for the CMitF group, which was a higher rate than the 27% for CAF group. The survival rate at 48 weeks was for both groups with no difference shown. Leukopenia, gastrointestinal symptoms and alopecia were major side effects in both groups, but the incidence and severity grade of gastrointestinal symptoms (nausea and vomiting) and alopecia were significantly less in the CMitF group than in CAF group. In both groups, the WBC nadir was 2,100/microliter and the time to nadir was 14 days, with no difference shown in hematological toxicity between the two groups. No severe cardiac, hepatic, or renal toxicity was noted.

[Assessment of response to therapy in advanced breast cancer].

In 1979, the protocol guidelines on clinical assessment of response to therapy in advanced breast cancer were proposed by the Chemotherapy Committee and the Endocrine Therapy Committee of the Japan Mammary Cancer Society, in accordance with guidelines presented by the UICC (1977) and by the Breast Cancer Task Force of the NCI (1978). After amending some clauses, these protocol guidelines were finally published in 1982 by the Japan Mammary Cancer Society. A discussion was presented on the consistency of the present guidelines with the criteria for the evaluation of cancer chemotherapy for solid tumors, that was proposed by Koyama-Saito's Committee and was later adopted by the Japan Society for Cancer Therapy. The newly organized committee of the Japan Mammary Cancer Society is now carrying out further work to clarify several questionable points which have arisen in the present protocol guidelines.

[Indications for surgical resection of lung metastasis from breast cancer--considerations according to the mechanism of extension of metastases].

The indications for surgical treatment of lung metastasis from breast cancer were analyzed according to the extension mode of the metastasis. Of lung lesions that seemed operable roentgenographically at the time of their first manifestation, about half developed new metastatic nodules within six months thereafter, while the other half did not do so for more than two years. No clinicopathologic findings at the time of mastectomy were helpful to differentiate the two groups, except that a cancer-free interval of less than a year fell exclusively into the former group. Thus, the only way to determine the indications is to watch the lesions for six months with chemo-endocrine therapy.

[Reconstruction methods of anterior chest wall resection--comparison between acrylic resins and myocutaneous flaps].

A total of 32 patients were treated by chest wall resection for primary and recurrent breast cancer or other malignant tumors of the chest wall. Reconstruction of the anterior chest wall was conducted using either acrylic resin plate or myocutaneous flap. Although acrylic resin plate proved to be satisfactory for maintaining stability of the chest wall and protecting important organs in the anterior mediastinum, some patients developed foreign body reactions including local infections, local retention of serous discharge and slight anterior chest pain during a prolonged observation. Reconstruction with myocutaneous flap achieved acceptable stability and caused no such foreign body reactions. We confirmed that myocutaneous flap was the best way to reconstruct the anterior chest wall after full thickness chest wall resection.

[Phase II study of THP (2''R)-4'-0-tetrahydropyranyladriamycin) in breast cancer].

In a phase II study of THP, fifty-six patients with advanced or recurrent breast cancer were evaluated, and a response rate of 23.2% including 1.8% with complete response and 21.4% with partial response, was obtained. Response rates of THP varied according to the location of metastasis and the highest rate was shown to be 38.5% in patients with soft tissue metastasis. The median dose for inducing response was 100 mg with a range from 60 mg to 160 mg. Toxicities such as leukopenia and gastrointestinal disorders were observed during THP treatment. The favorable response to THP was close to that of adriamycin, but it was noteworthy that a lower frequency of alopecia occurred.

Intra-arterial infusion chemotherapy as an induction therapy in multidisciplinary treatment for locally advanced breast cancer. A long-term follow-up study.

A multidisciplinary treatment including intra-arterial infusion chemotherapy as an induction therapy was administered to 55 patients with locally advanced breast cancer. Intra-arterial chemotherapy conducted preoperatively produced marked responses in primary and lymph node lesions with 78% complete + partial response (CR + PR), subsequently permitting extended radical mastectomy. Histologic examination of resected specimens also revealed that 33% of the patients had no viable cancer cells remaining in their lesions. Five-year and 10-year survival rates were 57% and 41%, respectively, compared with 24% and 18%, respectively for the 17 patients of historic control. Patients showing better local responses to intra-arterial chemotherapy had longer survival time with less frequent local recurrences. Intra-arterial chemotherapy is an effective modality for the treatment of locally advanced breast cancer.

Changes in distribution of hepatic blood flow induced by intra-arterial infusion of angiotensin II in human hepatic cancer.

Changes in the distribution of the hepatic blood flow induced by intra-arterial infusion of angiotensin II (AT-II) were studied in human hepatic cancers using extremely short-lived radioisotope (RI) (krypton 81 m [81mKr]; half-life, 13 seconds). After the start of continuous infusion of AT-II, the radioactivity of the tumor showed about a two-fold increase, whereas that of the nontumor region decreased to about one half as much as the level before the infusion. Consequently, the mean ratio of the arterial blood flow in the tumor region to that in the nontumor region (T/N ratio) increased to 3.30 (P less than 0.001). The T/N ratio showed a peak before the peripheral blood pressure reached the maximum, and thereafter tended to decrease. Intra-arterial infusion of AT-II raised the T/N ratio more obviously than did intravenous infusion of the drug, with less rise in the peripheral blood pressure. It is believed that intra-arterial infusion chemotherapy with local use of AT-II enables better accessibility of chemotherapeutic drugs to tumors.
