Effect of dietary quality of protein on liver microsomal mixed function oxidase system, plasma cholesterol and urinary ascorbic acid in rats fed PCB.

Exposure of rat to polychlorinated biphenyls (PCB) results in an increase in activity of liver microsomal mixed function oxidase system, plasma cholesterol and urinary ascorbic acid. Experiments were designed to study the effect of dietary quality of protein on these metabolic responses to dietary addition of PCB. The diets contained either whole egg protein, casein, fish protein, soy protein, soy protein plus 0.34% methionine, wheat-gluten, corn-gluten or gelatin fed at 10% of the diet. In general, the relative biological values of proteins as shown by the gains in body weight were positively correlated with activity of the microsomal enzyme system including aminopyrine N-demethylase, aniline hydroxylase, cytochrome P-450 and NADPH-cytochrome c reductase, plasma level of cholesterol and urinary ascorbic acid. Supplement of methionine to soy protein diet containing PCB caused a significant increase in liver and plasma cholesterol and in urinary ascorbic acid, although the influence on the microsomal enzyme system was slight. The gelatin diet containing PCB gave higher activity of the enzyme system than that expected from the biological value, although the same protein diet without PCB caused quite low activity as expected from the biological value.

Reconstruction of mandible with free osteocutaneous flap using deep circumflex iliac vessels as the stem.

One of the 2 patients introduced here had osteoradionecrosis, which has been difficult to reconstruct. The other had a massive adamantinoma that required replacement of the mandible from the midportion of the right ramus to the left angle with the iliac bone using the conventional method. The grafted iliac bone was severely absorbed, however. The mandible in both cases was successfully reconstructed with an osteocutaneous compound flap, using the deep circumflex iliac vessels as the stem. This method is considered a very good procedure for reconstruction of the mandible, which has been difficult by conventional procedures. Because the diameter of the stem is large, vascular anastomosis can be readily performed with an extremely high degree of safety. Pulsation of the stem can be palpated on follow-up even one year after surgery, and there are no signs of bone reabsorption.

Manifestations of carcinoma of the uncinate process by means of superior mesenteric arteriography.

Identification of carcinoma arising in the uncinate region of the pancreas is difficult because of the peculiar topographic relationship of this lesion with adjacent structures. Among the various diagnostic procedures performed upon ten patients with this malignant lesion, superior mesenteric arteriography provided the most helpful information, including signs of invasion in the main trunk of the superior mesenteric artery and vein and in the proximal portion of the jejunal arteries. A clearer visualization of these vascular involvements was attained in the right posterior oblique projection rather than in the anteroposterior projection. On the contrary, the arterial and portal vessels belonging to the celiac territory, such as superior pancreaticoduodenal arteries, dorsal pancreatic artery, gastroduodenal artery, splenic vein and portal vein, were almost free of involvement. At laparotomy, almost all of the patients had direct extension of the tumor to the superior mesenteric vessels, forming a cancerous core in the root of mesentery. All died of cachexia within six months after a palliative surgical procedure. Serial roentgenograms of superior mesenteric artery, especially taken in the oblique projection, are the best means of confirming carcinoma of the uncinate process, for which only a few available methods have been established to evaluate the clinical aspects.

[Relaxation of vascular smooth muscle induced by formaldehyde (author's transl)].

Using spiral strips of male rabbit thoracic aortae, we examined the effect of formaldehyde (HCHO) on vascular smooth muscle. HCHO (6.6 X 10(-4)M) did not inhibit the tonic contraction induced by 25 mM K+, while inhibition was apparent with a pretreatment of HCHO in the same dose. HCHO markedly inhibited Ca2+-induced contraction in the preparation depolarized by excess K+ in Ca2+-free media, but did not inhibit 50 mM K+-induced contraction in the preparation exposed to Ca2+-free media (EGTA deleted). In addition, HCHO did not inhibit the contraction induced by 2.2 mM Ba2+ in the preparation exposed to Ca2+-free media (added EGTA). These results suggest that HCHO inhibits the transmembrane influx of Ca2+. The contraction induced by norepinephrine (NE) was also inhibited by HCHO. High performance liquid chromatography revealed that NE was rapidly inactivated by HCHO in Krebs-bicarbonate solution at 37 degrees C. This finding suggests that the inhibitory effect of HCHO on the contraction induced by NE is mainly due to inactivation of NE. Therefore, the relaxation of the vascular smooth muscle induced by HCHO is probably mainly related to both the inactivation of catecholamines and the inhibitory effect on transmembrane influx of Ca2+.

Effect of dietary level of protein on liver microsomal drug-metabolizing enzymes, urinary ascorbic acid and lipid metabolism in rats fed PCB-containing diets.

Effects of dietary level of protein on liver microsomal drug-metabolizing enzyme system, lipids and urinary ascorbic acid metabolism were studied in the rats fed 0.1% polychlorinated biphenyl (PCB) containing diets. In the rats not receiving PCB, the activity of the enzyme system increased as the protein level was increased up to around 35%. In a short-term experiment (8-9 days), dietary PCB caused an increase in the activity of the enzyme system regardless of dietary level of protein. In a semi-long term experiment (29-30 days), however, 35-55% protein diets did not allow for induction to the same extent. As a result the highest activity was observed with 10-20% protein diets in this period. The increments in urinary ascorbic acid and in serum cholesterol were generally potentiated with high-protein diets. Maximum increments in urinary ascorbic acid and serum cholesterol were obtained with 20% protein and 35% protein, respectively, in the semi-long term experiment. In contrast to serum cholesterol, the accumulation of liver cholesterol and triglyceride due to PCB was markedly potentiated with low-protein diets.

Angiography and pancreatoductography in resectable carcinoma of the pancreas.

Angiography and pancreatoductography were performed in 18 patients with resectable carcinoma of the pancreas. The presence or absence of vascular and ductal tumor involvement was evaluated. Accurate diagnosis was obtained in 83 per cent at pancreatoductography alone, in 66 per cent at angiography alone, while combination of the methods resulted in 94 per cent accuracy. Tumor involvement of the capsule and secretion stagnation in the distal part of the pancreas are of importance for the surgical management and the survival after resection. An improved result after surgery may be obtained by combined evaluation of the vascular and ductal morphology as demonstrated at preoperative radiography.
