[Evaluation of the effect of transcatheter arterial embolization as preoperative therapy of hepatocellular carcinoma].

From May 1982 till Nov. 1984, we performed 90 hepatic resection for primary or metastatic liver tumor. Preoperative transcatheter arterial embolization (TAE) was performed on 14 of them. All resected specimens of 14 cases were examined histologically. Effectiveness of TEA was estimated by the ratio of the necrotic area to the whole area of the tumor. Necrosis of the main tumor more than 90% was found in 10 of 14 cases, though necrosis of the daughter nodules was detected in only one of six cases. In hepatectomized patients, two-year-survival of the preoperative TAE cases had not been statistically improved compared with the non-TAE cases.

General pharmacological properties of cilostazol, a new antithrombotic drug. Part II: Effect on the peripheral organs.

The pharmacological effects of the new antithrombotic drug, cilostazol (6-(4-(1-cyclohexyl-1H-tetrazol-5-yl)butoxy]-3,4-dihydro-2(1H)-qui nolinone, OPC-13013) on the peripheral nervous system and miscellaneous organs were studied. Cilostazol produced a very slight increase in beating rate of the isolated atrium and a very slight increase in contraction of the papillary muscle of guinea pigs induced by cilostazol compared with that of isoproterenol (isoprenaline). The beating rate increasing effect was not antagonized by propranolol and it augmented isoproterenol's effect. When administered intravenously in anesthetized dogs, cilostazol increased blood flow in the coronary, internal carotid, vertebral and femoral arteries and transiently decreased blood flow in the renal and superior mesenteric arteries probably because of blood pressure fall. In anesthetized dogs, cilostazol decreased blood pressure by reducing the resistance in the peripheral blood vessels. An increase in heart rate, cardiac contractile force, myocardial oxygen consumption and respiration rate were also observed. In conscious rats, the drug increased heart rate. Cilostazol produced a slight relaxation of the smooth muscle of all organs except for blood vessels and slightly inhibited spontaneous motility of the isolated uterus of pregnant rats, the isolated ileum of rabbits and the ileum of rats in situ. It was considered that cilostazol had no specific effects against norepinephrine, serotonin, acetylcholine or histamine based on the results that the drug was only slightly antagonistic against the contraction of rabbit aorta induced by norepinephrine and serotonin, the contraction of isolated guinea-pig ileum induced by acetylcholine, histamine and barium chloride and the contraction of the isolated uterus of non-pregnant rats induced by oxytocin. The drug had little effect on the contraction of the nictitating membrane induced by stimulation of the preganglionic sympathetic nerve in the cat. These results suggest that cilostazol had little effect on the autonomic nervous system. Cilostazol slightly inhibited edema induced by carrageenin, but showed no diuretic effect and had little effect on neuromuscular transmission or the secretion of gastric juice, bile and pancreatic juice, and therefore it was considered to have no appreciable effect on the peripheral nervous system or organs except for its vasodilating and cardiac effects.

[A new strategy for malignancies by direct hemoperfusion using bacterial endotoxin bounded fiber].

Endotoxin derived from E.coli was chemically bounded to polystryen fiber (LPS immobilized fiber). Anti-cancer activities of LPS immobilized fiber (LPS-F) were evaluated by a single direct hemoperfusion (DHP) on the VX2 tumor-bearing rabbits. Examination I: Cancer-bearing rabbits were prepared by intradermal injection of 4 X 10(5) of VX2 tumor cells into the back. Five out of 11 (Group 1) were treated with DHP at 4-5th day, and 6 out of 10 (Group 2) at 7-9th day. Examination II: Seventeen out of 19 tumor-bearing rabbits were injected of 9.38 X 10(7) viable BCG intravenously. Next day they were implanted 1 X 10(6) of VX2 tumor into the thigh muscle. Twelve out of 17 were treated with DHP at 14 days after BCG infection (treated group). Other five of 17 belonged to the control group and two were not treated. Tumor growth were significantly suppressed in both groups (p less than 0.05). Survival rate was 2/5 (40%) in and 1/5 (20%) in group 2 and 0% in non-treated group. In examination II, survival rate were 8/12 (treated), 2/5 (BCG infected) and 0/2 (non-treated) respectively. In histological study, bleeding and tumor cell necrosis were found in tumor at 6 hr after DHP. DHP using LPS-F had tumoricidal activities on the VX2 tumor of rabbits. In BCG infected rabbits, this treatment could cause tumor necrosis and produce multiple humoral factors with tumoricidal activities.

Effect of cilostazol on platelet aggregation and experimental thrombosis.

A new antithrombotic drug, cilostazol (6-[4-(1-cyclohexyl-1 H-tetrazol-5-yl)butoxy]-3,4-dihydro-2(1H)-quinolinone, OPC-13013) was studied for its inhibitory effect on platelet aggregation in vitro in various experimental animals and man and in dogs ex vivo, for its effect to disperse platelet aggregates in vitro in rabbits and man and for its antithrombotic effect in vivo using its effect to prevent death due to the formation of pulmonary thrombi in mice. Cilostazol produced a potent inhibition of platelet aggregation both in vitro and ex vivo and a dispersion of platelet aggregates in vitro. The mode of action of cilostazol was different from that of acetylsalicylic acid (ASA) in that cilostazol inhibits not only secondary platelet aggregation but also primary platelet aggregation induced by aggregating agents such as adenosine diphosphate (ADP). The drug potently prevented death due to pulmonary thrombosis by platelet aggregates in mice in vivo. Unlike ASA which prevented only death due to collagen-induced platelet aggregation, cilostazol prevented both collagen- and ADP-induced platelet aggregation. These results suggest that cilostazol is a promising antithrombotic drug.

Effect of cilostazol, a new antithrombotic drug, on an experimental model of peripheral circulation insufficiency.

Acute arterial obstruction in hind limb was experimentally induced by intra-arterial injection of sodium laurate saline solution in mongrel dogs, and the inhibitory action of cilostazol (6-[4-(1-cyclohexyl-1H-tetrazol-5-yl)butoxy]- 3,4-dihydro-2(1H)-quinolinone, OPC-13013) on ulcer formation was studied. Cilostazol at oral doses of 10 and 30 mg/kg inhibited ulcer development. Cilostazol inhibited decrease in skin temperature in obstructed hindlimbs in a dose-dependent manner. Cilostazol also inhibited increase of plasma aldolase, creatine phosphokinase and glutamic oxalacetic transaminase activities, presumably due to necrosis of the skeletal muscle.

Changes in acetoacetate/beta-hydroxybutyrate ratio in arterial blood following hepatic artery embolization in man.

Acetoacetate/beta-hydroxybutyrate ratio in the hepatic venous blood was compared to the ratios in arterial blood and peripheral venous blood in hypoxic state following right hepatic artery embolization in 5 patients with liver cancer. Ketone body ratios in right hepatic venous blood were positively correlated with those in arterial blood (r = 0.960, p less than 0.001), but not with those in peripheral venous blood. The free NAD+/NADH ratio of the liver mitochondria, which is reflected by the ketone body ration in hepatic venous blood, can be evaluated by the ketone body ratio in the arterial blood.

Application of fibrin glue to burns. Its haemostatic and skin transplant fixation effects in the excised wound.

Fibrin glue was clinically applied to the excised wound in 10 extensively burned patients, and its haemostatic and fixation effects on the isolated skin transplants were investigated. The following advantages resulted from the application of the fibrin glue: The haemostatic procedures and fixation of the isolated skin transplants were simplified. The quick and efficient haemostatic procedures considerably reduced the blood transfusion volume during and after the surgical procedures, by achieving a notable reduction in blood loss. Based on the above merits, the operation time was reduced and, eventually, the surgical stress might be alleviated. Satisfactory haemostatic effects were attained particularly in the limbs without supplemental haemostatic aids, leading us to conclude that the application of fibrin glue to the excised wound in burns is very useful.

Cholangiocellular carcinoma associated with multiple liver cysts.

A 46-year-old woman visited our outpatient clinic complaining of dull right hypochondrial pain. Ultrasonography revealed four cysts in the right lobe of the liver. Arteriography and CT could not determine whether these cysts were benign or malignant. Because one of these cysts contained chocolate-colored fluid, amebic abscess was suspected, and the cyst was drained. No amebae were found in the cultured exudate, and fresh bleeding continued; therefore, laparotomy was performed. A tumor was present on the surface of the liver. Right lobectomy was performed. The resected specimen contained one nonepithelialized cyst which was invaded by surrounding cholangiocellular carcinoma and three epithelialized cysts. Preoperative diagnosis of this kind of cholangiocellular carcinoma seems to be very difficult. Recently cholangiocellular carcinoma associated with developmental cysts has been classified as a new entity different from cystadenocarcinoma. Cholangiocarcinoma, which belongs to this group, seems to deserve special attention.

[Plasmapheresis in the patients with malignant tumor].

Therapeutic plasmapheresis for malignant tumor was first applied scientifically to transmissible tumors in dogs in 1908 by Crile et al. In the 1970s, immunosuppressive factors in the plasma of cancer-bearing patients were described. Plasmapheresis has been widely applied to many human malignant diseases for immunosuppressive factors being removed with new plasma separating methods, sachas continuous centrifugation or membrane plasma separation. Recently more selective removal method have been developed. The effects of plasmapheresis are recognized not only from an immunological point of view but also in performance status. The latter effect perhaps depends on the replacement of fresh frozen plasma rather than the removal mechanism itself. It is thought that plasmapheresis improves not only immunological status, but also homeostasis in patients with malignancies. There are no cancer treatments which improve the performance status of advanced cancer patients except plasmapheresis. In the future, plasmapheresis is likely to receive more attention and further development as an anti-cancer therapy for immunomodulation and fundamental treatment in combined immuno-chemotherapy.
