Cardiovascular fiberoptic endoscopy: development and clinical application.

A slender, flexible cardiovascular fiberoptic endoscope has been developed. It is 4.0 mm in diameter and 120 cm in length. It is introduced through the peripheral vessels, and endoscopy is performed similarly to the procedure of the cardiac catheterization. It has been applied in 45 patients, for the preoperative and intraoperative diagnoses of various heart diseases in 25 and in 20 for the preoperative, intraoperative, and postoperative diagnoses of various vascular diseases. Most applications were aimed at the accurate preoperative diagnosis of an atrial septal defect. With flexible endoscope, the right sides of the heart and vascular lumens were clearly visualized. There were no infections, and no apparent injuries of the cardiovascular system were produced by the insertion and manipulation of the endoscope. It is concluded that the first step toward the use of cardiovascular endoscope as an adjunctive diagnostic procedure has been achieved.

Holter system electrocardiographic studies on 617 cases.

Holter system electrocardiograms were recorded for 617 patients who were treated at the Department of Cardiology, Tokai University Hospital. In cases of arrhythmia, ventricular premature contraction (VPC) was the most predominant, in 291 cases (69%) out of 423 with arrhythmia, followed by 59 (14%) with supraventricular premature contraction (SVPC), 23 (5.4%) with paroxysmal atrial tachycardia, 17 (4%) with second degree A-V block and 10 (2.3%) with transient atrial fibrillation (AF). In addition, nine (2.1%) cases of ventricular tachycardia (VT), one (0.2%) of transient ventricular fibrillation (VF) and one (0.2%) of third degree A-V block were found in particularly severe arrhythmia cases. Six out of nine cases of VT were cases of acute myocardial infarction (AMI) and all died suddenly while in the hospital or after discharge. Mild or moderate changes in ST-T were often observed even in normal subjects. Of the 617 cases, only 18 (2.9%) showed a significant elevation or depression of ST. Among these, three definitely had variant angina pectoris (Prinzmetal type). The above results indicate Holter EKGs are very useful for the diagnosis of arrhythmia and can also be used as a means of evaluating the prognosis in some cases, but there still are some problems in connection with its use for the diagnosis of ischemic heart disease except for the diagnosis of variant angina pectoris.

[Effect of ouabain on urinary output of potassium in rats loaded with NaCl (author's transl)].

Ouabain-induced kaliuresis was investigated in rats loaded with 1% NaCl (3 ml/100 g body weight, p.o.). The intraperitoneal administration of ouabain produced an increase in potassium excretion and a decrease in sodium excretion in doses over 1 mg/kg. The effect of ouabain in altering the electrolyte pattern of urine was more marked in NaCl-loaded rats than in water-loaded (3 ml/100 g body weight, p.o.) rats. At doses of 10 mg/kg, ouabain produced a significant decrease in excretions of sodium and H+ and equalized the output of sodium with that of potassium in terms of concentration and total output. Thus, factors other than the ion exchange mechanism in distal tubules appear to be responsible for the ouabain-induced kaliuresis, when the drug is given to rats in high doses.

[Results of surgical treatment of various vessel diseases. 1. Great vessel diseases (author's transl)].

Three hundred and three cases of great vessel diseases have been experienced during the last 18 years at the Second Department of Surgery, Hokkaido University Hospital. Those cases consisted of 36 congenital anomalies of the aorta, 41 aortitis syndromes, 145 aneurysms of the thoracic and abdominal aorta, 20 obstructions of the aorta, 10 arterio-venous fistulas, and 51 obstructions of the great vein. The results of surgical treatment of those cases were reviewed. Although the results have progressively improved, special problembs to be solved are remained in treatment of complexed aortic coarctation, aortitis syndrome, aneurysm of the thoracic aorta, dissecting aneurysm, and obstruction of the great vessels.

[Results of surgical treatment of various vessel diseases. 2. Peripheral vessel diseases (author's transl)].

Eight hundred and eighty-three patients of peripheral vessel diseases have been experienced during the last 18 years at the Second Department of Surgery, Hokkaido University Hospital. Those cases consisted of 52 arteriovenous fistulae, 26 arterial injuries, 41 peripheral aneurysms, 403 acute and chronic arterial occlusions, and 336 peripheral venous diseases. The results of surgical treatment of those cases were reviewed. Although the operative results of most diseases are quite satisfactory, careful evaluations or prompt surgical interventions are emphasized in treatment of arteriovenous fistula, arterial injuries, and arterial occlusions.

Subcellular membrane topology and turnover of a rat hepatic binding protein specific for asialoglycoproteins.

Examination of the topological distribution of the rat liver binding protein specific for desialylated serum glycoproteins has revealed a subcellular distribution wherein this receptor is located largely, if not entirely, on the luminal membrane surface of the Golgi complex and smooth microsomes. In contrast, the receptor protein has been identified on the external, or cytosolic, surface of the lysosomal membranes. Recovery of the purified binding protein from whole rat livers established an average half-life survival time of approximately 88 h. Subsequently, injection in vivo of the readily catabolized ligand, asialo-orosomucoid, was found to be without effect upon the turnover rate of the receptor purified from isolated plasma membranes. These observations are interpreted as suggesting the existence of a recycling mechanism wherein the receptor is spared destruction in the lysosomes and remains available for subsequent reinsertion into the hepatocyte plasma membrane as part of a continuing process for the clearance of galactose-terminated ligands from the circulation.

Results of stripping operation for varicose veins.

Approximately 150 stripping operations for primary varicose veins of the lower extremities were performed. Long-term evaluation (postoperative follow-up of more than five years) was made on 102 extremities and 83 per cent of these showed good or excellent results. Six patients were reoperated due to recurrence of varices. Accurate diagnosis and complete operation are most important in stripping operations.

Surgical treatment of aneurysms of the sinus of Valsalva.

From 1962 to 1976, 15 patients with aneurysms of the sinus of Valsalva underwent operative correction. All patients survived operation. Follow-up has been carried out for as long as fifteen years (median, six years) after operation. There have been no late deaths and no recurrences. Aortic insufficiency, present in 2 patients before and in 2 after operation, remained well compensated and required no further aortic valve procedure. A double approach, that is, through both the aorta and the involved cardiac chamber, is recommended for sufficient correction.

Partial amino acid sequences of two proteins in developing porcine enamel.

The enamel proteins prepared from porcine enamel samples at an early stage of development are separated at least 4 fractions by Sephadex G-100 gel filtration at pH 10.8. The main components of secondary and fourthly-eluted fraction were further purified by ion exchange chromatography. The results of sequence analyses of these proteins by sequential Edman degradation and by hydrolysis with carboxypeptidase Y showed that the partial amino acid sequences of these proteins were identical.

Proteolytic enzyme in porcine immature enamel.

A proteolytic enzyme cleaving the main component of enamel proteins obtained from immature enamel has been purified from a soluble extract of porcine immature enamel. It is optimally active around pH 6 against enamel protein. It is completely inhibited by phenylmethylsulfonyl fluoride and diisopropyl phosphofluoridate, and partially by benzamidine. EDTA does not affect its activity. The enzyme seems to sever initially its activity. The enzyme seems to sever initially enamel protein into two segments, one containing lysine, arginine and tyrosine and the other being free from these amino acids.

The surgical treatment of chronic lymphedema of the extremity.

Twenty-five patients with lymphedema of the extremity have been evaluated. Thirteen were treated surgically and 12 conservatively. The Thompson buried dermis flap operation is the most effective procedure and may be combined with omentaplasty.
