Albumin plays an important role in maintaining an adequate plasma colloid osmotic pressure (COP). On the basis of the Starling's law, we postulated that elevation of plasma COP produced by infusion of serum albumin would result in the withdrawal of cerebral interstitial fluid. To test the hypothesis that an oncotic gradient would produce cerebral dehydration and result in reduction of intracranial pressure (ICP), we infused intravenously 25% solution of salt-poor human serum albumin, 2g/kg over 60 minutes for 10 patients with increased ICP. In these 10 patients, the degree of brain edema was not severe but mild to moderate by judging from CT scan findings. In addition to ICP, various parameters such as mean arterial blood pressure (MABP), central venous pressure (CVP), pulse rate, PaCO2 and urine volume were measured before, during and after albumin infusion. Hematological studies including COP, total protein, albumin-globulin ratio, protein fractions, hematocrit (Ht), blood urea nitrogen (BUN), creatinine and electrolytes were also performed before, during and after administration of albumin. The raised ICP (27.4 +/- S.E. 1.6 mmHg) decreased significantly down to 19.1 (+/- 1.9) mmHg and remained stable for at least 240 minutes without any changes of MABP, pulse rate and PaCO2. CVP elevated significantly and remained at higher levels in comparison with control despite increase of urine output after administration of albumin. This results suggested that an adequate circulatory volume was maintained. A significant increase of total protein, serum albumin concentrations and albumin-globulin ratio was noted.(ABSTRACT TRUNCATED AT 250 WORDS)

There is a lot of controversy about potassium cardioplegic solution in its components and its method of perfusion. Although its effect emphasized in many reports, the optimal potassium level is still in question. In order to determine the optimal level of potassium, the isolated rat hearts were preserved in relatively bad conditions; such as 37 degrees C of infusates, non-oxygenation and continuous perfusion. The hypothermic effect on its optimal level was also studied by just reducing the temperature of the infusates to 4 degrees C. Thirty isolated rat hearts were divided into six groups of 5 hearts each. Each group received a different concentration (15, 25, 40, 60, 80, 100 mEq/L) in the cardioplegic solution. Using an isolated rat heart preparation, hemodynamic indices after 30 minutes preservation were compared with the previous control values of the same hearts. And the same protocol was performed on the profound hypothermia group using another 30 rat hearts. When Basic-Modified-Krebs-Solution was used as a base for the cardioplegic solutions, we concluded from the results that the solution' with 40 mEq/L of potassium chloride might offer the best myocardial protection of all concentrations tested; and that the safe range of the potassium was from 25 mEq/L to 60 mEq/l in the profound hypothermic preservation.

DNA sequences complementary to the Torpedo californica electroplax mRNA coding for the alpha-subunit precursor of the acetylcholine receptor were cloned. The nucleotide sequence of the cloned cDNA indicates that the precursor consists of 461 amino acids including a prepeptide of 24 amino acids. Possible sites for acetylcholine binding and antigenic determinants on the alpha-subunit molecule are discussed.
Ten patients who developed neurological deficits associated with angiographically proven cerebral vasospasm caused by ruptured aneurysm were treated with hyperdynamic therapy induced by administration of a large amount of human serum albumin. No vaso-active drugs were administered. Cardiopulmonary function and intracranial pressure were monitored during the treatment. Marked improvement of neurological function was observed in all cases. Nine patients recovered completely without any neurological residual following treatment. The degree of the improvement observed during treatment closely correlated with the decrease in total peripheral resistance. Infusion of albumin did not cause elevation of intracranial pressure. It was concluded that the hyperdynamic therapy induced by administration of albumin has a dramatic effect on the ischaemic cerebral insult caused by vasospasm. It is postulated that the main effect of this treatment is produced by cerebrovascular dilatation.
The HLA-A, B, DR and MB antigens were investigated in patients suffering from Takayasu disease (Aortitis syndrome). Out of twenty-one HLA-A and B antigens tested, only HLA-Bw52 was significantly deviated (30147, PF = 63.8%, RR = 7.8) from the controls (14/76, PF = 18.4%). Since in the Japanese, HLA-Bw52 is in positive linkage disequilibria with HLA-DR2 and MB1, the association of the DR2 and MB1 antigens with Takayasu disease was studied. The HLA-DR2 antigen was significantly increased (23/30, PF = 76.7%,, RR = 6.0) in patients compared with the control (18/51, PF = 35.3%). Moreover, an almost perfect association of MBI (29/30, PF = 96.7%, RR = 12.6) with Takayasu disease was demonstrated. This finding supports the hypothesis that the genes in the HLA-D region play a major role in determining the susceptibility to Takayasu disease.
Selective labeling with N-(1-oxyl-2,2,6,6-tetramethyl-4-piperidinyl)-maleimide of human serum LDL has been performed. The spin-labeled LDL exhibited an ESR spectrum containing signals of a strongly immobilized component only. The signals were completely reversible between 4 degrees C and 37 degrees C and fairly stable at each temperature. The spin-labeled LDL which was prepared by the usual method exhibited an ESR spectrum containing signals of both strongly immobilized and weakly immobilized components (5, 6). The latter was unstable above 25 degrees C and changed irreversibly. The strongly binding site showed higher affinity for the nitroxide radical than the weakly binding site, and two kinds of the strongly binding site were demonstrated kinetically. The rate of binding of the nitroxide radical to the two kinds of strongly binding site were estimated to be 4.7 x 10(4) M-1 . day-1 and 0.16 x 10(4) M-1 . day-1 at pH 7.4 and 4 degrees C, respectively. Both the strongly immobilized and weakly immobilized radicals were reduced with ascorbate at the same rate. It was also shown on gel filtration of the SDS-treated LDL derivatives that the strongly immobilized component was on the apoprotein B moiety, whereas either noncovalent binding to LDL or binding to some small molecular species other than protein was suggested for the weakly immobilized component.
The clinical efficacy of ticlopidine, a platelet aggregation inhibitor, was evaluated in a double-blind placebo-controlled study on 193 patients with ischemic ulcers due to chronic arterial occlusion. In the group treated with ticlopidine, significantly better efficacy was noted over the placebo group in terms of judgment on overall improvement by doctors in charge as well as by the committee assessing with color slide films. The drug was particularly effective for the treatment of patients with occlusion in the arteries on the distal side and for the ischemic ulcer with relatively small size. In the group with ticlopidine, granulation was significantly improved compared to the placebo group. Those patients who had pain due to arterial occlusion on the distal side were significantly improved by this agent. The mean diameters of ulcers were clearly reduced. Therefore, this agent could possibly be claimed as an effective remedy against the main symptoms of chronic arterial occlusion. The frequencies of side effects and abnormalities in laboratory tests were comparable in the group treated with ticlopidine to those of the group with placebo. It was, therefore, concluded that this agent has an outstanding clinical value as a conservative treatment in chronic arterial occlusion.
A series of 58 patients of aortitis syndrome were analyzed from the standpoint of the pathogenesis and surgical treatment. Investigations of HLA revealed a high incidence of HLA-A9, BW52, MT1 and DR2 antigens with statistically significant differences in the chi-square test. These data indicate that a haplotype composed of A9, BW52, MT1 and DR2 is common in patients with aortitis syndrome and suggest that a genetic factor plays an important role on the pathogenesis of the disease. Experiences with the surgical treatment of various types of aortitis syndrome, such as operation for cerebrovascular insufficiency (7 cases), for hypertension produced by coarctation or renovascular stenosis (14 cases), for aortic regurgitation (2 cases), and for aneurysm (7 cases) were analyzed. Reconstructive surgery is difficult in many of them because of the complicated lesion produced by inflammatory changes. Attention should be paid to the selection of the most suitable time for operation, application of special surgical technique, and prevention of the complications to obtain favorable results.
