We experienced two cases of the thoracoabdominal aortic aneurysms with Marfan's syndrome. Case 1; The 31 year-old-woman was operated upon by Crawford's procedure, which includes reconstruction of renal arteries, superior mesenteric artery, and celiac artery using temporary shunt. Although she had been well for three years after the operation before she developed acute aortic dissection, she could not tolerate the Bentall's procedure on it. Case 2; The 37 year-old woman was operated upon for dissecting aneurysm (DeBakey IIIb). The procedure included replacement of the descending and abdominal aorta by Dacron graft. Four years after the operation, enlargement of residual false lumen had been observed, which caused its dehiscence just proximal to the abdominal aortic anastomosis. The Crawford's procedure, which includes reconstruction of renal arteries, superior mesenteric artery, and celiac artery using temporary shunt was employed for repair of the remaining thoracoabdominal aortic aneurysm. She is well now 5 months after surgery. We conclude that cardiovascular changes in patients with Marfan's syndrome must be observed carefully because of its high recurrent rate after initial surgery. The operation at proper time and a careful observation of cardiovascular changes will improve the prognosis.
A 46-years-old man was admitted because of an abnormal shadow on X-ray. The operation underwent under the diagnosis of benign tumor in the antero-superior mediastinum. At the operation, the tumor arise from the right intrathoracic vagal nerve and it was resected with the transection of vagal nerve. The histological diagnosis was schwannoma. Postoperative course was uneventful except the slight hoarseness. Intrathoracic vagal tumor is rare and we have found 19 reported cases among Japanese literatures. This case is considered to be the 20th case originated from the intrathoracic vagal nerve in Japan.
Distal splenorenal shunt with splenopancreatic disconnection is a difficult surgical procedure; therefore its use is not widespread. We present a modified technique that facilitates the procedure and also ensures maintenance of portal flow by adding gastric devascularization and gastric disconnection. Our results compare favorably with the standard distal splenorenal shunt.

This 57-year-old female of MS had been treated by MVR with a 25 mm Omniscience valve on October 26, 1983. She was doing well until November 1984 when she developed general malaise and dyspnea. She was hospitalized again on June 3, 1985. Studies including echocardiogram and cardiac catheterization revealed mitral stenosis. The echocardiogram showed rapid blood stream toward the interventricular septum. The maximum opening angle of the disc was measured 30 degrees on the cine film. The cardiac catheterization showed increased PA and PA wedge pressures. The Omniscience valve was replaced with a 25 mm SJM valve on July 19, 1985. The postoperative course was uneventful. The postoperative echocardiogram and cardiac catheterization showed significant improvements in flow pattern and intracardiac pressure. She is doing well as an instructor of Japanese dancing after discharge from the hospital.
A 53-year-old male was admitted because of an abnormal chest roentgenogram showing a solitary tumor shadow in the right upper lung field. Laboratory data was normal. Although examination with the bronchoscope and sputum cytology showed no abnormal findings, a partial resection of the right upper lobe was performed. Microscopic examination of the lesion was cryptococcosis. As another focus was not found by systemic examination, the lesion was disclosed primary pulmonary cryptococcosis. Cryptococcosis is rare but we must think of it when we meet a solitary tumor shadow on the chest roentgenogram.
A case was presented of spontaneous rupture of the descending aorta through an atheromatous plaque without aneurysm or aortic dissection at the site of rupture. A 65 year-old-female with past history of well controlled hypertension developed chest pain along with hypovolemic shock. The patient underwent emergency CT examination which revealed left pleural effusion. There were no signs of a false lumen or aneurysm. At surgery a 5 mm of perforation in the mid portion of descending aorta was found to be surrounded by a large hematoma. The perforation was successfully plicated with predget-supported sutures. Spontaneous rupture of thoracic aorta is a rare, life-threatening condition for which emergency diagnostic and therapeutic measures are indicated. Only 7 cases could be found in the review of the literature, and this is the first reported case in this country.
Despite improvements of the results of aortic aneurysm repair, technical difficulty and a high incidence of perioperative morbidity still remains in the thoracoabdominal aortic aneurysm surgery. In this paper, we reported 19 cases of operation for thoracoabdominal aortic aneurysm and retrospectively studied surgical technique, early results and late results after operation. Graft inclusion technique with direct vessel reattachment was mainly employed for aortic reconstruction, and for 2 cases of inflammatory aneurysm. Hardy's operation and extraanatomic bypass were performed. Mortality was 16% with a 13% mortality for elective repair and paraplegia occurred in 2 cases (10%). Current results with thoracoabdominal aortic aneurysm repair established its safety, but to prevent postoperative paraplegia completely, the consideration for operative adjuncts and mechanisms of paraplegia needs to be continued.
Microinjection of an expression plasmid that carries complementary DNA encoding the receptor for dihydropyridine calcium channel blockers of skeletal muscle restores both excitation-contraction coupling and slow calcium current in cultured skeletal muscle cells from mice with muscular dysgenesis. This suggests that the dihydropyridine receptor in the transverse tubule membrane of skeletal muscle functions both as the voltage sensor for excitation-contraction coupling and as the slow calcium channel.
Utilizing RNA blot hybridization and immunoblotting techniques, the changes of the hepatic contents of acetyl-CoA carboxylase mRNA and of the enzyme protein in growing chicks have been investigated. In the post-hatching period, the hepatic mRNA level markedly increased at least 70-fold when compared to that before hatching. This increase was not observed in chicks receiving no diet. These changes were closely paralleled with the rise of the hepatic content of acetyl-CoA carboxylase protein in chicks up to 10 days old. Neither the acetyl-CoA carboxylase mRNA level nor the enzyme quantity significantly changed in heart. It is concluded from these results that the developmental regulation of acetyl-CoA carboxylase in the post-hatching period of chicks is tissue specific and occurs primarily at a pretranslational step. The content of acetyl-CoA carboxylase mRNA in adult chicken liver was low, which is comparable to those in embryos at 3 days before hatching and chicks at hatching day. Although acetyl-CoA carboxylase mRNA was detected in adult chicken brain, heart, lung, kidney, uropygial gland, spleen, testis, and chest muscle as well as liver, the mRNA level in these tissues was much lower than that in liver of growing chicks.
Susceptibilities of Gram-negative bacterial strains isolated at the University Hospital in 1987 to six beta-Lactam antibiotics (azthreonam cefmetazole, ceftizoxime, latamoxef, aminobenzyl-penicillin and piperacillin) and two aminoglycoside antibiotics (gentamicin and amikacin) were examined by agar dilution and the agar diffusion method. Results obtained from both methods correlated well in most of the strains. Most of the strains belonging to Escherichia coli, Klebsiella oxytoca, Klebsiella pneumoniae and Proteus mirabilis were sensitive to the beta-lactam antibiotics except aminobenzyl-penicillin and piperacillin. They were also sensitive to aminoglycoside antibiotics. Most strains of Citrobacter freundii, Enterobacter cloacae and Serratia marcescens were sensitive to azthreonam, latamoxef and aminoglycosides, but were less sensitive to other beta-lactams. Acinetobacter calcoaceticus was generally resistant to beta-lactams except ceftizoxime, but was sensitive to aminoglycosides. More than half of the strains of Pseudomonas aeruginosa were sensitive to azthreonam, piperacillin and aminoglycoside antibiotics and only latamoxef was active in Pseudomonas maltophilia.
The complete amino acid sequence of prostaglandin endoperoxide synthase from sheep vesicular gland has been deduced by cloning and sequence analysis of DNA complementary to its messenger RNA. The results were confirmed by digestion of the enzyme with carboxypeptidase Y and by automated Edman degradation of the intact enzyme polypeptide and peptide fragments obtained by limited proteolysis of the enzyme with Achromobacter proteinase I. Mature sheep prostaglandin endoperoxide synthase is shown to be composed of 576 amino acids with an Mr of 66,175. The precursor peptide is predicted to contain a 24-residue signal peptide. The serine residue susceptible to acetylation by aspirin is found to be located near the C-terminus of the enzyme polypeptide.
The ability of axonal outgrowth of serotonin neurons in the implanted brain tissue of subcommissural organ (SCO) was immunohistochemically studied. The serotonin neuron system of the experimental rats was completely destroyed by the intraventricular injection of 5,6-dihydroxytryptamine. The raphe region of normal fetal rats was implanted into the caudal part of the third ventricle of the neurotoxic drug pretreated rats. The host brain was examined 3 months after transplantation. The numerous serotonin fibers were distributed in the SCO and periventricular region of the third ventricle of the host brain. The outgrowing serotonin fibers from the raphe transplant seemed to innervate the SCO with the target specificity.
The complete amino acid sequence of acetyl-CoA carboxylase from chicken liver has been deduced by cloning and sequence analysis of DNA complementary to its messenger RNA. The results were confirmed by Edman degradation of peptide fragments obtained by digestion of the enzyme polypeptide with Achromobacter proteinase I or staphylococcal serine proteinase. Chicken liver acetyl-CoA carboxylase is predicted to be composed of 2,324 amino acid residues, having a calculated molecular weight of 262,706. The biotin carboxyl carrier protein domain is located in the middle region of the enzyme polypeptide. The amino-terminal portion of the acetyl-CoA carboxylase has been found to exhibit a homologous primary structure to that of carbamyl phosphate synthetase. Localization of possible functional domains including biotin carboxylase subsite in the acetyl-CoA carboxylase polypeptide is discussed.
The primary structures of the A and B subunits of Shiga toxin and of Shiga-like toxin I (VT1), isolated from the culture supernatants of Shigella dysenteriae 1 and Escherichia coli O157:H7, respectively, were analyzed by Edman degradation of intact proteins and peptides in their digests with trypsin or Achromobacter protease I and also by fast atom bombardment mass spectrometry of the digests. The results indicated that the A and B subunits of Shiga toxin and Shiga-like toxin I have the same primary structures. The identity of their primary structures was confirmed by determining the nucleotide sequence of the gene encoding Shiga-like toxin I cloned from a Shiga-like toxin I converting phage. This nucleotide sequence was different from that reported by Jackson et al. (Microbial Pathogenesis 1987; 2: 147-153), by Calderwood et al. (Proc Natl Acad Sci USA 1987; 84: 4364-8) and by Grandis et al. (J Bacteriol 1987; 169: 4313-9) in one base at position 231, which was found to be adenine instead of thymine, which they reported. The amino acid residue at position 45 from the N-terminus of the A subunit of Shiga-like toxin I deduced from the nucleotide sequence determined in this study is threonine, which corresponds with that found by amino acid sequencing, whereas from previous reports by other investigators it is serine. Edman degradation of the intact A subunit of Shiga toxin indicated that the A subunit was nicked between Ala253 and Ser254 to form A1 and A2 fragments linked by a disulfide bond.
Cathepsin B was purified, 400-fold, to homogeneity from chicken liver. The enzyme comprised a mixture of two kinetically indistinguishable forms (approximately 1:1), which were separated on concanavalin A (Con A)-Sepharose; one consisting of Mr 25,500 and 5,000 polypeptide chains bound to Con A-Sepharose but the other, composed of Mr 24,500 and 5,000 polypeptide chains, did not. N-terminal amino acid sequence analyses of a mixture of the Mr 25,500 and 24,500 polypeptide chains, and of the Mr 5,000 polypeptide chain revealed single amino acid sequences, respectively. These amino acid sequences were homologous to those of the heavy and light chains of mammalian enzymes, respectively. The chicken liver and mammalian cathepsin B were similar in structure and properties.
We have investigated the therapeutic effects of a combination of cyclophosphamide (CY, 150 mg/kg, iv) and human recombinant interleukin 2 (IL-2, 5 x 10(4) JU/day, ip for 5 days) on autochthonous tumors induced in mice by 3-methylcholanthrene. The initial treatment was carried out when the tumor had reached 8 to 10 mm in diameter. Twenty-eight out of 35 mice (80%) died of local recurrence and pulmonary metastasis of tumor cells within 53 +/- 40 days (mean survival time, MST +/- SD) after the surgical removal of the primary tumor. When these mice were treated with both CY and IL-2 following the operation (Op), only 10 out of 20 mice (50%) died of recurrence and metastasis. The survival rate, however, was not improved by CY chemotherapy alone or IL-2 immunotherapy alone, although each provided a prolongation of the MST. Natural killer cell and LAK precursor cell activities in the spleen cells from the treated mice were found to be restored by IL-2 alone or CY + IL-2, whereas they were suppressed by CY alone. These findings reveal that the restoration of the antitumor activity of spleen cells does not provide an improved therapeutic effect by itself and that IL-2 immunotherapy requires the associated effect of CY chemotherapy to achieve an improved therapeutic effect.
