Laboratory and clinical studies on cefsulodin (CFS), a new antipseudomonal cephalosporin antibiotic, were carried out and the following results were obtained. 1. The MIC of CFS against P. aeruginosa showed the peak of susceptibility at 3.13 mcg/ml in the inoculum size of 10(8) cells/ml and at 1.56 mcg/ml in the inoculum size of 10(8) cells/ml. It has a higher superiority as compared with gentamicin or piperacillin and gentamicin-resistant strains of P. aeruginosa were sensitive to CFS. 2. CFS was given to 3 patients with acute pneumonia and 5 patients with chronic cystitis which were all due to P. aeruginosa. Clinical effects were good in 2 patients and poor in a patient with acute pneumonia, and those were good in 3 patients and poor in 2 patients with chronic cystitis. Side effects were not recognized at all.
Antitumor activity of bamboo leaf against various transplantable mouse tumor strains, such as Sarcoma 180 and Ehrlich ascites carcinoma has previously been demonstrated by Sakai et al.7) and Yammamoto et al.9). The present investigation was undertaken to determine the antitumor activities of BLE and BLL against the spontaneous tumor induced by benzopyrene (BP) and 4-nitroquinoline-1-oxide (4NQO) in mice and rats. The possible mechanism of antitumor action was also discussed as related to Kada's Rec-assay and Ames test in vitro. The in vivo antitumor test was performed using 16 groups of mice given the following solutions ad libitum for 120 days; Groups 1-4, 5-8, 9-12 and 13-16 were given water, 1% and 10% BLe, 0.1% BLL, and each respective group was treated with no injection (control), oil/s.c., BP 1 mg/s.c., and 4NQO 0.5 mg/s.c. Rat respective group was treated 30 days after in initiation of experiment with no injection (control), oil/s.c., BP 2.4 mg/s.c. Antitumor activities of the BLE and BLL were determined by the tumor incidence index and average weight of tumor. In vitro Rec-assay, cold incubation method, and Ames test were performed in the usual manner. Antitumor activity against BP induced tumor was the highest with 1.0% BLE (0.71 mg/ml), but no significant difference was found between the groups of the 10% BLE, 0.1% BL and control. A weak trend toward DNA damage was seen in the case of BLE, in the Rec-assay. His+ revertants with S-9 mix on Salmonella typhimurium TA98 were found in the case of BLL. It was concluded that antitumor activity against BP- and 4NQO-induced tumors was the highest with 1% BLE (0.71 mg/ml), and a direct action of BLE on tumor cells was indicated.

The intestinal absorption of labeled folic acid ([3H]pteroylmonoglutamate) was determined from urinary and fecal recoveries of tritium in pairs of monkeys fed control liquid diets or diets containing 50% of energy as ethanol for a 24-mo period. Weight gain, fecal fat excretion, nitrogen balance, D-xylose absorption, serum folate levels, jejunal histology, and intestinal enzyme activities were similar in each group. Liver biopsies obtained after 12 and 24 mo of feeding demonstrated steatosis and megamitochondria in the ethanol-fed group, with decreased hepatic levels of folate at 24 mo. Intestinal malabsorption of labeled folic acid in the ethanol-fed monkeys was indicated by decreased urinary recovery of tritium but increased fecal recovery of tritium after intragastric administration of [3H]pteroylmonoglutamate. These studies suggest that folic acid malabsorption follows the chronic administration of ethanol together with a nutritious diet.
To assess the role of altered hepatic folate metabolism in the pathogenesis of the folate deficiency of chronic alcoholism, the hepatic metabolism of a tracer dose of 3H-PteGlu was compared in monkeys given 50% of energy as ethanol for 2 years and in control monkeys. Long-term ethanol feeding resulted in mild hepatic injury, with a significant decrease in hepatic folate levels. Chromatographic studies of liver biopsies obtained after the tracer dose indicated that the processes of reduction, methylation, and formylation of reduced folate and the synthesis of polyglutamyl folates were not affected by long-term ethanol feeding. Hepatic tritium levels were significantly decreased in the ethanol-fed group. These studies suggest that the decrease in hepatic folate levels observed after long-term ethanol ingestion is due to a decrease in hepatic folate levels observed after long-term ethanol ingestion is due to a decreased ability to retain folates in the liver, whereas reduction and further metabolism of folates is not affected.

A case with ruptured aneurysm of the sinus of Valsalva into the right atrium directly visualized by cross-sectional echocardiography was reported. A 31-year-old male was referred to our hospital for evaluation of recently developed cardiac murmur. A systolic ejection murmur and diastolic blowing murmur were audible maximally at 3-4L and the latter one was also well audible at the right lower sternal border. Cardiac catheterization data revealed a large left-to-right shunt at the atrial level. The cross-sectional echocardiogram, obtained with a transducer at 5R towards the upwards and the medial direction, revealed a mass echo showing a pendulous motion through cardiac cycle in the right atrial cavity. Ruptured aneurysm of the sinus of Valsalva originated from the non-coronary sinus was demonstrated by aortography, and it showed the same motion as in echocardiography. From this angiographic finding and its disappearance after operation, the mass visualized by echocardiography was considered to be ruptured aneurysm itself. Thus, direct visualization of ruptured aneurysm of the sinus of Valsalva into the right atrium, as well as other types, was possible by cross-sectional echocardiography. In addition, multiple coronary A-V fistulae with a small shunt were demonstrated by coronary angiography in this case. Clinical significance of the complication has not been clarified.
Phonocardiographic and echocardiographic investigation was performed in patients with progressive muscular dystrophy of Duchenne type (PMD). The clinical materials consisted of 90 patients with PMD (aged 8 to 21 yrs, a mean of 14.5), and 90 normal subjects (aged 6 to 19 yrs, a mean of 11.7). The patients with PMD were classified into 8 stages from the mildest, S(1), to the severest, S(8), according to Swinyard-Deaver' criteria. In the 90 normal subjects the diminished first heart sound was noted in 12 cases (13.3%), presystolic murmurs in 4 cases (4.4%), and diastolic rumbles in 9 cases (10%), whereas, in the patients with PMD the diminished first heart sound was noted in 47 cases (52.2%), presystolic murmurs in 41 cases (45.6%), and diastolic rumbles in 44 cases (48.9%). There was a significant difference in the incidence of the above-mentioned three phonocardiographic findings between the PMD patients and the normal subjects. But there was no significant difference in the incidence of a systolic click between these two groups. The incidence of the diminished first heart sound increased with the progress of Swinyard-Deaver' classification. A presystolic murmur was observed with the highest incidence in the stage of S(8). The incidence of a rumble was also augmented with increasing severity of the disorder from the stages of S(1) to S(7), but decreased in S(8). Another attenpt was made to relate the phonocardiographic findings to those of the echocardiogram. In the cases with anterior mitral leaflet fluttering, there were diastolic rumble in 69% whereas 16.7% of the patients without anterior mitral leaflet fluttering had diastolic rumbles. In two-dimensional echocardiography, the anterior and posterior mitral leaflets looked like pennants fluttering in the wind. All these observations positively indicate that anterior mitral leaflet fluttering was closely associated with the genesis of rumbles. Consequently, it can be concluded that the diminished first heart sound, presystolic murmurs and diastolic rumbles might be useful clinical signs in the assessment of the myocardial involvement in PMD.

A computer-based system which calculates stroke volume, heart rate and cardiac output automatically from the transthoracic impedance variation has been developed. A beat-by-beat determination was performed on several healthy subjects and patients with atrial fibrillation during spontaneous breathings at rest. The respiratory artifact appearing on the impedance cardiogram was canceled by (1) correcting the base line of dZ/dt for each cardiac cycle on the basis of assumption that the dZ/dt values at ECG-R waves would be invariable regardless of the respiratory phase when the respiratory movement ceases, and (2) averaging the signals for a number of cardiac cycles. Comparisons between the computed stroke volumes and those obtained from a manual calculation showed good agreement.
