[Clinical study of carumonam].

To evaluate the clinical efficacy of carumonam (CRMN, AMA-1080), the drug was used in the treatment of 10 patients including 4 with pneumonia and each with acute tonsillitis, chronic bronchitis, Mycoplasma pneumonia, primary atypical pneumonia (PAP), chronic pyelitis, and acute cystitis. Since beta-lactam antibiotics were not active against Mycoplasma pneumonia and PAP, these diseases were excluded from the clinical efficacy evaluation of CRMN. Responses were excellent in 1 patient and good in 7. Side effects were not observed. The laboratory test recognized slight elevations of GOT, GPT and eosinophilia in 1 patient and a slight leucopenia in another upon the administration of CRMN.

[Evaluation of tumor marker CA15-3 in breast cancer].

CA15-3, a new tumor marker for breast cancer, was determined in various malignant diseases including breast cancer and various benign diseases, and its clinical significance and usefulness were studied. In 18 normal individuals, the value of CA15-3 was 8.9 +/- 3.3 U/ml (mean +/- SD). In primary breast cancer, the positivity was 20% for Stage I, 0% for Stages II and III and 100% for Stage IV. Of 17 cases of recurrent breast cancer, 13 (77%) were shown to be positive. The therapeutic effect and the value of CA15-3 were well correlated with each other. As for other malignant tumors, positive cases were observed in 50% of recurrent cancer of the stomach and in 14% of malignant tumors of the biliary system. All of these cases were terminal-stage cancers. The CEA value determined simultaneously showed a good correlation, r = 0.87 (p less than 0.01) with CA15-3 in malignant tumors other than breast cancer. In breast cancer, however, the correlation between the two was low, r = 0.18. These results suggest that CA15-3 is not necessarily useful in the diagnosis of primary breast cancer, but is useful as an indicator of the effect of therapy for recurrent breast cancer and for the prediction of recurrence.

[Phase I-II study of recombinant interleukin-2].

A phase I-II study of recombinant interleukin 2 (rIL-2) (Shionogi Pharmaceutical Co.) was conducted against carcinoma of the lung (20 cases) and stomach (1 case) as well as metastatic pulmonary tumor (17 cases). The cytotoxicity of lymphocytes against K562, PC-9, PC-14 and Daudi cells was examined before and 1, 2, 3, 5, 7, 14, 21 and 28 days after rIL-2 administration. The subsets of lymphocytes and pharmacokinetics of rIL-2 were also analyzed. Four administration methods were used. 2-h drip infusion of 6.7 X 10(5) U/m2/day (A1: 6 cases) or 2.2 X 10(6) U/m2/day (A2: 8 cases) for 5 consecutive days, subcutaneous injection of 6.7 X 10(5) U/m2/day (B: 3 cases) for 28 consecutive days, 4-h continuous drip infusion of 3.3 X 10(5) U/m2/day (C1: B cases), 6.7 X 10(5) U/m2/day (C2: 7 cases) or 1.1 X 10(6) U/m2/day (C3: 5 cases) for 28 consecutive days, and (D) 24-h continuous drip infusion of 6.7 X 10(5) U/m2/day (D: 6 cases) for 5 consecutive days every week, repeating four cycles of the course. We discussed whether or not the problems of the phase I study with BRMs could be solved.

A case of adenocarcinoma of the lung which was successfully treated by chemotherapy with patient survival without recurrence for more than three years.

A 63-year-old man was admitted to the National Cancer Center Hospital on December 20, 1983 for a close examination of the abnormal shadows found in chest roentgenograms. Chest radiographs showed a massive right pleural effusion, a large tumor in the right hilar region and swelling of the left hilar nodes. Transcutaneous needle biopsy revealed the tumor's cytological type to be adenocarcinoma. The clinical stage was considered to be T3N2M1 (American joint committee (AJC) stage III M1), and his performance status was 4, so he was given etoposide (VP-16) alone as the initial chemotherapy treatment. He showed a partial response upon two courses of VP-16 therapy. Subsequently, one course of cisplatin (80 mg/m2) + vindesine (3.3 mg/m2), two courses of VP-16 (80 mg/m2 D1-4), five courses of cyclophosphamide (80 mg/m2) + adriamycin (50 mg/m2) + vincristine (1.4 mg/m2) and two courses of VP-16 (100 mg/m2 D1, 3, 5) were administered sequentially until May 31, 1985. No radiation therapy was given and, up to March 31, 1987, no sign of recurrence had been observed.

[Evaluation of CDDP administration into the body cavity].

CDDP (100 mg bolus) was administered into the body cavity for cancerous pleural effusion and ascites, and its effectiveness and pharmacokinetics were studied. The cases treated consisted of 7 of gastric cancer, 1 of pancreas cancer, 2 of lung cancer and 2 of breast cancer. A decrease in body cavity fluid was observed in all cases. The cases in which treatment was effective were broken down into CR 2 cases, PR 8 cases and NC 2 cases, the efficacy rate being 83%. This procedure produced distinctly fewer side effects than intravenous administration. Disturbance of renal function and G.I. symptoms were negligible, but anemia was often found. The free-CDDP levels in the body cavity fluid remained of over 1.0 microgram/ml after 12 hours and those in plasma remained after 1 hour. Intracavitary administration thus seems an excellent method that enables a drug to act on cancer cells directly at high concentrations over many hours.

Chemical modification of arginine residues of porcine muscle acylphosphatase.

Acylphosphatase (acylphosphate phosphohydrolase, EC 3.6.1.7) from porcine skeletal muscle is inactivated by phenylglyoxal following pseudo-first-order kinetics. The dependence of the apparent first-order rate constant for inactivation on the phenylglyoxal concentration shows that the inactivation is also first order with respect to the reagent concentration. Among the competitive inhibitors for the enzyme examined, inorganic phosphate and ATP almost completely, and Cl- partially, protect the enzyme against the inactivation. The dissociation constants for inorganic phosphate and ATP determined from protection experiments by these inhibitors agree well with those from inhibition experiments by them. These results support the idea that the modification occurs at the phosphate-binding site. The amino-acid analysis reveals the lack of reaction at residues other than arginine. Circular dichroism spectra of the modified enzymes show that the inactivation seems not to be due to denaturation of the enzyme resulting from the modification of the non-essential arginine residues. The relationship between the loss of the enzyme activity and the number of arginine residues modified in the presence and absence of ATP shows that one arginine residue is possibly responsible for the inactivation of acylphosphatase.

Purification and characterization of the feline sarcoma virus tyrosine-specific kinase pp85gag-fes.

The transforming phosphoprotein pp85gag-fes of the Snyder-Theilen strain of feline sarcoma virus (ST-FeSV) was purified in a form which exhibits tyrosine-specific kinase activity. Cell lysates of ST-FeSV-transformed mink nonproducer cells were applied to a column of DEAE-Sephacel and eluted with a linear gradient of NaCl in phosphate buffer. Kinase activity was found uncomplexed (pp85gag-fes) in the flow-through and was eluted with 200 mM NaCl in a complex with pp50 and pp90. The flow-through material was further fractionated by chromatography on hydroxylapatite, followed by phosphocellulose and a final gel filtration step using Sephacryl S200. The final preparation was specifically enriched 1400-fold, and tyrosine-specific kinase activity was increased by about 300-fold as determined by autophosphorylation or phosphorylation of casein. Pp85gag-fes kinase activity was inhibited by nicotinamide adenosine dinucleotide (NAD) and slightly inhibited by NADH, while quercetin, a strong inhibitor for pp60src-associated tyrosine kinase activity, had no inhibiting effect.

Appropriate glycosylation of the fms gene product is a prerequisite for its transforming potency.

Processing inhibitors of N-linked glycans were used to determine whether correct glycosylation of the oncogene product gp140v-fms, encoded by the McDonough strain of feline sarcoma virus (SM-FeSV), is required to maintain the oncogenic properties of v-fms. SM-FeSV-transformed cells treated with the glucosidase-I inhibitors N-methyldeoxynojirimycin (MdN) or castanospermine synthesized predominantly a gp125v-fms species which had a normal half life. The molecule was transported to the plasma membrane and exhibited normal kinase activity as determined by autophosphorylation. However, although no significant change in cell morphology of the SM-FeSV-transformed cells was observed in the presence of castanospermine, growth of these cells became strictly serum-dependent. In addition, growth in soft agar was drastically retarded despite the presence of 10% calf serum, indicating that the transformed properties of the cells were altered. In contrast, swainsonine, an inhibitor of the processing alpha-mannosidase-II, had no effect. Cells transformed by the Snyder Theilen strain of FeSV were used to demonstrate that the altered proliferative properties were directly linked to the modified structure of the fms gene product. Our data suggest that the extracellular domain of gp140v-fms plays a role in regulating cell proliferation.

Development and use of a remote-controlled mitral valve.

A remote-controlled mitral valve was designed and constructed to occlude the mitral or tricuspid orifice at any time in the cardiac cycle. It is used to study ventricular properties in the anesthetized dog by controlling ventricular filling, atrial properties by controlling atrial emptying and interaction of the two chambers by uncoupling them. The device can produce transient or steady-state perturbations in filling volume which make possible studies of intrinsic control of cardiac output. When filling volume is transiently reduced, stroke volume and end-systolic volume are reduced. A compensatory increase in stroke volume of the next cycles occurs due to increased ventricular preload and reduced afterload. The compensation continues until the lost stroke volume is regained, at which time the system returns to its previous steady state.

Ampicillin concentrations in human serum, gingiva, mandibular bone, dental follicle, and dental pulp following a single oral administration of bacampicillin.

Ninety-six patients who underwent the extraction of impacted mandibular third molars in a nonfasting state were given a single oral dose of bacampicillin preoperatively. Specimens of serum from venous blood (n = 107), gingiva (n = 57), mandibular bone (n = 68), dental follicle (n = 56), and dental pulp (n = 43) were obtained during the operation and assayed for ampicillin content. The mean peak concentrations in serum, gingiva, mandibular bone, dental follicle, and dental pulp all occurred approximately 90 minutes after administration. The concentration ratios of the tissues to the corresponding serum at the peak time were 0.50, 0.20, 0.34, and 0.61, respectively. Bacampicillin showed good absorption by the intestine, and sufficient concentrations of the resulting metabolite, ampicillin, were found in the oral tissues.

Negative chronotropic effects of thyroxine on the isolated bullfrog heart.

An acute application of L-thyroxine (T4) to everted sinus-atrium preparations of bullfrog showed negative chronotropic effects consisting of two processes: an initial peak at 2-5 min and a delayed plateau at 40-60 min. The initial effect was blocked by Cd2+ and tetraethyl-ammonium (TEA). The delayed effect was suppressed by Cd2+ and NaCN.

Ampicillin concentrations in human serum and periodontal membrane following a single oral administration of bacampicillin.

Ampicillin concentrations in human serum and periodontal membrane after a single oral administration of bacampicillin (500 mg) were assayed by the agar diffusion (paper disc) method. The peak times of serum and periodontal membrane were nearly identical, being approx. 90 min after administration. The peak concentrations of serum and periodontal membrane were 12.81 micrograms/ml and 6.97 micrograms/g, respectively. The concentration ratio of periodontal membrane to serum at the peak time was 0.56.

Reoperation after aortic valvuloplasty for aortic regurgitation associated with ventricular septal defect.

Five patients underwent reoperations because residual or recurrent aortic regurgitation occurred after aortic valvuloplasty for aortic regurgitation associated with ventricular septal defect. The mean age at reoperation was 22 years old, and the mean time interval between initial and second operation was 6 years, 10 months. The pathological findings of the aortic valves showed tears and perforation of repaired leaflets in four patients and a giant pseudoaneurysm of the Valsalva sinus in one. Aortic valvuloplasties were performed again in three patients, and aortic valves were replaced with prosthetic valves in two. Slight to moderate regurgitant murmurs are still audible in patients who underwent these valvuloplasties. Ventricular septal defects should be closed before aortic regurgitation develops. If it has developed, however, valvuloplasty should be considered as a first choice in young patients. For adult patients, aortic valve replacement is recommended.

Relation of patient age to premalignant alterations in choledochal cyst epithelium: histochemical and immunohistochemical studies.

Forty patients with choledochal cysts, 17 children and 23 adults, were treated by cyst excision in this institution (1972 to 1982). Histochemical and immunohistochemical studies were performed on tissue from the walls of these 40 cysts. The type and degree of histologic changes observed were found to correlate with patient age at time of cyst excision. Specimens from four patients less than two years of age at time of excision revealed epithelial desquamation, fibrosis, and minimal evidence of inflammation. Among specimens from 13 patients, 4 to 15 years of age, only two had an intact epithelial lining. Marked inflammation and intramural glandular structures were observed. Specimens from 23 patients, more than 15 years of age at the time of cyst excision, showed marked acute or chronic inflammation and destruction of the epithelial lining. In this age group, intramural glandular structures with goblet and argyrophil cells were regularly observed within the cyst walls, and gastrin and somatostatin-like immunoreactivity was detected in cells of these metaplastic glands. In association with these changes, in five of the 23 patients in the oldest age group, adenocarcinomas were observed in the cyst walls. These studies establish the increasing rate of epithelial metaplasia in the walls of choledochal cysts with advancing age, and suggest that these changes are a continuing process with malignancy one of the end results.

A case of small cell lung cancer successfully treated with VM26.

A 79-year-old man was admitted to our hospital for a close examination of an abnormal shadow on a routine chest roentgenogram. A large tumorous shadow in the right S1 segment, and hilar and mediastinal node swelling were observed. Transbronchial biopsy revealed that the histologic type of the tumor was oat cell carcinoma. The clinical stage was considered to be limited disease. Because of his age, 79 years, he was treated with VM26 as a single agent. VM26 was administered by intravenous drip infusion of 60 mg/m2 daily for five consecutive days every three to four weeks. After the third course of treatment with VM26, he achieved partial response, and then radiation therapy to the primary site and the hilar and mediastinal nodes was given at a total dose of 6,000 rad. There have been few phase II studies of VM26 in patients with small cell lung cancer (SCLC). Further phase II studies should be conducted for confirmation of the reproducibility of the phase II study of VM26 in SCLC, and the efficacy of combination chemotherapy including VM26 against SCLC should also be tested.
