Gene expression of insulin signal-transduction pathway intermediates is lower in rats fed a beef tallow diet than in rats fed a safflower oil diet.

To elucidate the effects of dietary fatty acid composition on the insulin signaling pathway, we measured the gene expression of the earliest steps in the insulin action pathway in skeletal muscle of rats fed a safflower oil diet or a beef tallow diet. Rats were meal-fed an isoenergetic diet based on either safflower oil or beef tallow for 8 weeks. Both diets provided 45%, 35%, and 20% of energy as fat, carbohydrate, and protein, respectively. Insulin resistance, assessed from the diurnal rhythm of plasma glucose and insulin and the oral glucose tolerance test (OGTT), developed in rats fed a beef tallow diet. Body fat content was greater in rats fed a beef tallow diet versus a safflower oil diet. The level of insulin receptor mRNA, relative expression of the insulin receptor mRNA isoforms, and receptor protein were not affected by the composition of dietary fatty acids. The abundance of insulin receptor substrate-1 (IRS-1) and phosphatidylinositol (PI) 3-kinase mRNA and protein was significantly lower in rats fed a beef tallow diet versus a safflower oil diet. We conclude that long-term feeding of a high-fat diet with saturated fatty acids induces decrease in IRS-1 and PI 3-kinase mRNA and protein levels, causing insulin resistance in skeletal muscle.

Triterpene alcohols from the flowers of compositae and their anti-inflammatory effects.

Eleven tabular and nine ligulate flowers from 15 species of Compositae plants were investigated for their triterpene alcohol constituents. This led to the isolation and identification of 11 triterpene alcohols as follows: heliaol, taraxasterol, psi-taraxasterol, alpha-amyrin, beta-amyrin, lupeol, taraxerol, cycloartenol, 24-methyl-enecycloartanol, tirucalla-7,24-dienol and dammaradienol. The tabular flowers of Calendula officinalis, Carthamus tinctorius, Cosmos bipinnatus, Chrysanthemum morifolium, Helianthus annuus and Matricaria matricarioides showed a characteristic feature by containing helianol as the most predominant component (29-86%) in the triterpene alcohol fractions. The triterpene alcohols from Compositae flowers were evaluated with respect to their anti-inflammatory activity against 12-O-tetradecanoylphorbol-13-acetate-induced inflammation (1 microgram per ear) in mice. All of these showed marked inhibitory activity, and their 50% inhibitory dose was 0.1-0.8 mg per ear.

Stimulation by neuropeptide Y of growth hormone secretion in prolactinoma in vivo.

We examined the effects of intravenous bolus injection of human neuropeptide Y (NPY, 100 micrograms) on the plasma growth hormone (GH) and prolactin (PRL) responses in 15 patients with prolactinoma (PRLoma). The GH and PRL responses to NPY were considered positive (a paradoxical increase) when an increase over baseline of at least 100% occurred. Although NPY did not affect PRL secretion in any of the patients examined, 60% (9 of 15) of the patients showed a significant rise in GH secretion after NPY. The presence or absence of the positive paradoxical increase in GH after NPY was not related to the age of the patients, basal PRL levels, the size of the pituitary adenoma (macro- or microadenoma), or the presence or absence of suprasellar extension of the adenoma. Although the underlying mechanism of the NPY stimulation of GH secretion and also its pathophysiological significance in PRLoma are open to question, the present observation may represent another example of paradoxical hormone responses which are occasionally found in functioning pituitary adenomas.

Mechanisms of transcriptional regulation and neural gene expression.

Gene transcription is governed by a set of basal transcription machineries and gene-specific factors. Eukaryotic RNA polymerases alone can not direct specific transcription, but need associated factors, namely general transcription factors (GTFs). The basal transcription machineries composed of RNA polymerase and GTFs bind to a promoter and govern efficient and correct transcription for constitutive gene expression. Protein-coding genes are transcribed by RNA polymerase (Pol) II whereas Pol I and Pol III synthesize ribosomal RNA and various small RNAs, respectively. Enhancer is another class of cis-element for Pol II to which transcription regulatory factors bind. Those factors are involved in inducible, repressive, and tissue-specific gene expressions via binding to their target sequences. Regulatory factors have multiple structural motifs and interact with basal machineries directly or indirectly (using mediators) in addition to DNA. Many transcription factors are known to regulate nervous system-specific gene expression, which include bHTH, bHLH, basic leucine zipper, and zinc finger factors and prorine-rich activators. These factors, some of which belong to a neural silencer factor, play roles in neural development, establishment of memory and learning, and expression of nervous system-specific proteins.

Experimental infusion phlebitis: importance of titratable acidity on phlebitic potential of infusion solution.

The titratable acidity of infusion solutions has never been taken into account in infusion phlebitis. This study aims to clarify the importance of titratable acidity on the phlebitic potential of infusion solutions. Solutions with different titratable acidity (from 0.16 to 12 mEq/L) were infused into the ear veins of 6 rabbits for 6 h at 10 mL/kg/h, and the veins were later examined histopathologically. Even at pH 4.0, a commercial 10% glucose solution scarcely caused any changes because of its very low titratable acidity (0.16 mEq/L). A 10% glucose solution with its titratable acidity adjusted to 3 mEq/L (pH 4.3) with citrate and NaOH, however, caused phlebitic changes in all 6 rabbits, and adjusting the titratable acidity to 12 mEq/L (pH 4.4) increased the degree of phlebitis. On the other hand, a 10% glucose solution with a pH of 5.4 and a titratable acidity of 6 mEq/L caused slight phlebitic changes in half the rabbits. These results suggest that 1) the titratable acidity of infusion solutions is important to the phlebitic potential when the pH is low, 2) when the pHs are similar, the phlebitic potential of infusion solutions depend on the titratable acidity, and 3) the phlebitic potential of infusion solutions can not be estimated by pH or titratable acidity alone.

Effects of obesity on pregnant women: maternal hemodynamic change.

The purpose of this study is to clarify the effects of obesity on hemodynamic change in pregnant women. From 1322 primiparous women and 1027 multiparous women who delivered singleton babies with no life-threatening anomalies, 110 primiparous obese women, 1050 primiparous normal-weight women, 139 multiparous obese women, and 816 multiparous normal-weight women were selected according to body mass index at the first visit prior to the 10th gestational week. Women exhibiting chronic hypertension, over diabetes mellitus, and extreme skinniness were excluded. All of the women had been under the same management at Osaka City University Hospital. Their medical records were analyzed retrospectively. The study determined the arterial blood pressure of obese women to be significantly higher than that of normal-weight women throughout pregnancy in both primipara and multipara. The incidence of pregnancy-induced hypertension in obese women was significantly higher in primipara (22.7% versus 8.2%) and multipara (15.8% versus 5.0%). The incidence of intrapartum hypertension was also higher in the obese group. Hematocrit count at 3 gestational months (37.7 +/- 3.1 in primipara, and 37.6 +/- 2.7 in multipara) and 8 gestational months (34.4 +/- 2.7 in primipara, and 33.7 +/- 2.7 in multipara) were significantly higher than those in normal-weight women. Exercise tests using impedance cardiographs showed poor cardiac function in 5 of 17 obese women. These findings led us to conclude that obesity during pregnancy is clearly accompanied by hypertension, hemoconcentration, and poor cardiac function.

First trimester biological markers for the prediction of pregnancy-induced hypertension.

The purpose of this study is to predict pregnancy-induced hypertension (PIH) by standard methods at the first antenatal visit prior to the 12th gestational week (GW). This is a retrospective study of 1189 primipara and 957 multipara who delivered singleton babies having no major malformations after the 32nd GW. Multiple regression analysis for the development of PIH was performed using the mean arterial blood pressure (MBP), body mass index (BMI), hematocrit count at the first visit prior to the 12th GW, a family history of hypertension number of pregnancies, number of deliveries, past history of hypertension and evidence of threatened abortion as explanatory variables in primipara and multipara, respectively. We obtained a significant regression equation in both primipara and multipara. Multiple correlation coefficient for primipara was 0.40223 (p < 0.01) and for multipara was 0.50764 (p < 0.01). When MBP was over 80 mm Hg, BMI was over 23.6, a family history of hypertension was present, or a past history of hypertension was present, these variables were significantly correlated with the development of PIH both in primipara and multipara. By combining these variables, we obtained 32.7% positive predictive value, 93.5% negative predictive value, 31.7% sensitivity, and 93.7% specificity in primipara and 24.6%, 96.7%, 52.6%, and 89.8% in multipara, respectively. As the incidence of PIH was 8.7% in primipara and 5.9% in multipara, we could predict 3.7 and 4.1 times higher than the true incidence. PIH can therefore be predicted by the simple combination of MBP, BMI, a family history of hypertension, and a past history of hypertension prior to the 12th GW. It can also be predicted by the multiple-regression equation with the use of nine explanatory variables.

Effects of erythorbic acid on vitamin C metabolism in young women.

Erythorbic acid, an epimer of L-ascorbic acid, is used in the United States as a food additive. Studies were conducted to determine whether the ingestion of erythorbic acid in the diet had any beneficial or adverse effects on the human requirement for vitamin C. Young women were fed diets that contained controlled amounts of erythorbic acid and ascorbic acid. In pharmacokinetic evaluations, erythorbic acid and ascorbic acid were rapidly absorbed with little interaction. Erythorbic acid cleared from the body more rapidly than ascorbic acid. Some subjects received diets deficient in vitamin C for periods < or = 30 d. Increasing intakes of erythorbic acid or prolonged intakes of < or = 1 g erythorbic acid/d did not indicate any interactions with ascorbic acid. Consumption of erythorbic acid resulted in the presence of erythorbic acid in mononuclear leukocytes. Ascorbic acid concentrations in these cells were not affected by the presence of erythorbic acid. Erythorbic acid disappeared quickly from these cells with cessation of erythorbic acid supplements. Prolonged ingestion of erythrobic acid by young women neither antagonized nor spared their vitamin C status.

Retrospective analysis of the treatment of patients with small cell lung cancer showing poor performance status.

To assess the feasibility of treatments for patients with small cell lung cancer (SCLC) showing a poor performance status (PS, Eastern Cooperative Oncology Group; ECOG 3 or 4), we retrospectively reviewed the outcome for 13 SCLC patients showing poor PS treated at the National Cancer Center Hospital between January 1984 and May 1994. The main factors which contributed to poor prognosis were superior vena cava (SVC) syndrome, massive pleural effusion, tracheal stenosis due to lymph node swelling, pericardial effusion and pulmonary fibrosis (causing dyspnea in combination), brain metastasis resulting in neurological disturbance, cachexia, Eaton-Lambert syndrome causing muscle weakness, retroperitoneal lymph node metastasis causing abdominal pain, peritoneal effusion due to abdominal lymph node swelling, vertebral metastasis causing paraplegia, and dermatomyositis/polymyositis (DM/PM) causing muscle weakness. All of the patients received chemotherapy with or without radiotherapy. The PS of 8 patients improved with treatment, but no improvement was seen in 5. We analyzed these 13 patients and considered the treatments for those with poor PS. Chemo-radiotherapy was tolerable in SCLC patients showing PS 3, and improved their PS if severe conditions or combined disease did not arise concurrently. It was further suggested that PS 4 patients with severe conditions or combined disease should not be given the treatments.

A cb-type cytochrome-c oxidase terminates the respiratory chain in Helicobacter pylori.

A Helicobacter pylori membrane fraction oxidized yeast and equine cytochrome c, and N,N,N',N'-tetramethyl-p-phenylenediamine (TMPD). When ascorbate was used as reductant, the Vmax and apparent Km values were 612 nmol electron min-1 (mg protein)-1 and 14 microM for yeast, and 419 nmol electron min-1 (mg protein)-1 and 19 microM for equine cytochrome c, respectively. For TMPD oxidation, the Vmax and Km values were 640 nmol electron min-1 (mg protein)-1 and 182 microM, respectively. These oxidase activities showed a high affinity for oxygen. Inhibition of both cytochrome-c and TMPD oxidase activities by 50% was caused by about 4 microM cyanide and about 0.5 mM azide. Redox difference spectra of the membrane solubilized with Triton X-100 showed b- or c-type cytochromes but not aa3-type cytochromes. c-type and a part of some b-type cytochromes were reduced with ascorbate plus TMPD. A CO difference spectrum revealed that protohaem, but not an aa3-type cytochrome, may be interacting with CO/oxygen. Only protohaem was detected in the haem fraction extracted from the membrane. Three polypeptides (60, 38 and 29 kDa) were found to be bearing haem c after SDS-PAGE of the membrane. From these results, it was suggested that the cbb3-type cytochrome-c oxidase, having a haem-copper binuclear centre like the cytochrome aa3-type oxidase, but differing in a few other properties, functions as a terminal oxidase in the respiratory chain of H. pylori.

Measurement of skin elastic properties with a new suction device (II): systemic sclerosis.

We previously reported the skin elastic properties of normal subjects as measured by a newly developed suction device (6). In the present report, we examined the skin elastic properties of 62 patients with systemic sclerosis (SSc), clinically diagnosed as limited or diffuse type. We measured the skin elastic properties on the forearm and chest and investigated correlations with disease type and clinical skin score. Patients with the diffuse type SSc showed significantly lower values than those with the limited type SSc or normal subjects in distention and retraction ability at the chest and forearm. This suction device is clinically useful for the objective evaluation of skin elastic properties in diffuse systemic sclerosis.

Urinary 8-hydroxy-2'-deoxyguanosine (8-OHdG) levels in women with or without gynecologic cancer.

OBJECTIVE: To detect the level of 8-hydroxy-2'-deoxyguanosine (8-OHdG) which is an oxygen-radical-forming agent, in the urine of patients with (n = 18) or without (n = 10) carcinoma of the female genitalia. None of the patients had been receiving any treatment before their urinary 8-OHdG levels were measured. METHODS: Urinary 8-OHdG was extracted by a solid-phase technique, and its level was determined by high-performance liquid chromatography (HPLC) with an electric chemical detector (ECD). RESULTS: We determined that the urinary 8-OHdG level decreased with as the age of the patient increased, and was extremely high in advanced cancer and recurrent cancer in a considerable number of patients. The urinary 8-OHdG level (1,827 +/- 1,500 pmol/kg/day, mean +/- SEM) in 18 patients with carcinoma was significantly higher (p < or = 0.05) than that (747 +/- 425 pmol/kg/day) in 10 patients without carcinoma. CONCLUSION: These results suggest that it might be possible to determine the spread of cancer to some extent by determining a patient's urinary 8-OHdG level.

Prolonged luminal narrowing of internal mammary artery graft after percutaneous transluminal angioplasty.

The authors describe a case with an internal mammary artery (IMA) graft that demonstrated prolonged, but reversible, luminal narrowing persisting > twenty-four hours following percutaneous transluminal angioplasty. Luminal narrowing disappeared spontaneously, however, at the time of follow-up angiography one week after the angioplasty. Since haziness at the dilation site had been shown on preprocedural angiogram, suggestive of intraluminal thrombus, this phenomenon may be attributed to either prolonged vasospasm or distal embolization of the IMA graft.

Nitric oxide mediates interleukin-1-induced matrix degradation and basic fibroblast growth factor release in cultured rabbit articular chondrocytes: a possible mechanism of pathological neovascularization in arthritis.

Prolonged incubation with interleukin-1 beta (IL-1) induced the release of large amounts of NO and subsequently inhibited DNA synthesis and the biosynthesis and accumulation of proteoglycans in cultured rabbit articular chondrocytes (RAC). IL-1 also inhibited DNA synthesis in bovine aortic endothelial cells (BAE). On the other hand, DNA synthesis in BAE cocultured with RAC was not inhibited by prolonged incubation with IL-1. Moreover, conditioned media from RAC incubated for a long period with IL-1 stimulated DNA synthesis in BAE alone. This growth stimulatory activity was mainly due to the release of basic fibroblast growth factor, a heparin-binding growth factor, into RAC culture. Gelatin zymography of the RAC culture medium revealed that IL-1 increased the production of matrix metalloproteinase-2 (MMP-2) and MMP-9. NG-Monomethyl-L-arginine, an inhibitor of NO synthesis, inhibited all of these actions of IL-1. These results indicate that NO from RAC treated with IL-1 stimulates MMPs, which, in turn, degrade the extracellular matrix produced by RAC, resulting in the release of large amounts of basic fibroblast growth factor stored in the matrix, which then stimulates adjacent BAE proliferation. Thus, NO produced from RAC treated with IL-1 may modulate angiogenesis in the synovium of arthritic patients.

Inhibitory effect of Di- and trihydroxy triterpenes from the flowers of compositae on 12-O-tetradecanoylphorbol-13-acetate-induced inflammation in mice.

Ten dihydroxy- and trihydroxy triterpenes, viz., four taraxastanes: faradiol, heliantriol B0, heliantriol C and arnidiol; two lupanes: calenduladiol and heliantriol B2; two oleananes: maniladiol and longispinogenin; and two ursanes: brein and uvaol, isolated from the nonsaponifiable lipids of the flower extracts of Compositae plants were evaluated with respect to their anti-inflammatory activity against 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced inflammation in mice. All the triterpenes were found to possess marked inhibitory activity. The 50% inhibitory dose of these compounds with respect to TPA-inflammation (1 microgram) was 0.03-0.2 mg/ear.

Inhibitory effects of sterols isolated from Chlorella vulgaris on 12-0-tetradecanoylphorbol-13-acetate-induced inflammation and tumor promotion in mouse skin.

Inhibitory activity against 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced inflammation in mice was observed in the methanol extract of Chlorella vulgaris, a green alga. The hexane soluble fraction obtained from the methanol extract exhibited marked inhibitory activity from which were isolated two delta(5,7)-sterols (ergosterol and 7-dehydroporiferasterol), two delta(5,7,9(11))-sterols [7,9(11)-dehydroergosterol and 7,9(11)-bisdehydroporiferasterol], two 5 alpha, 8 alpha-epidioxy-delta(6)-sterols (ergosterol peroxide and 7-dehydroporiferasterol peroxide), and a 7-oxo-delta(5)-sterol (7-oxocholesterol), among others. The delta 5'7-sterols, 5 alpha, 8 alpha-epidioxy-delta(6)-sterols and 7-oxo-delta 5-sterol inhibited TPA-induced inflammation in mice. The 50% inhibitory dose of these compounds for TPA-induced inflammation was 0.2-0.7 mg/ear. Furthermore, ergosterol peroxide markedly inhibited the tumor-promoting effect of TPA in 7,12-dimethylbenz[a]anthracene-initiated mice.

Comparison of aortic valve replacement and percutaneous aortic balloon valvuloplasty for elderly patients with aortic stenosis.

The outcome of aortic balloon valvuloplasty (ABV) was compared with that of aortic valve replacement (AVR) in aortic stenosis (AS) patients more than 60 years old. The indications for ABV included low respiratory and renal function, cancer, the patient's refusal of surgery, and low daily activity. Twenty six patients underwent AVR and 13 underwent ABV. Initially, the AVR group was younger and more symptomatic than the ABV group. Two perioperative deaths occurred in the AVR group, while there were none in the ABV group. Twenty-four AVR patients and 12 ABV patients had a successful outcome, with remarkable pressure gradient reduction in both groups. In the follow-up, only 1 death and no cardiac events were detected in the AVR group (mean follow-up of 27 months), whereas 3 deaths, 6 heart failures, 2 repeated ABV, and 4 AVR were seen in the ABV group (mean follow-up of 10 months). The data showed that ABV was safer than AVR, but a higher rate of restenosis limited its efficacy. In the ABV group, a higher ratio of balloon size to aortic diameter correlated with longer event-free survival. We concluded that for elderly AS patients, ABV should be used only in those with high surgical risk as a palliative therapy or a bridge therapy to AVR, and AVR should be primarily recommended under rigid evaluation of the patient's physical status.
