Cosegregation of the new region on chromosome 3 with salt-induced hypertension in female F2 progeny from stroke-prone spontaneously hypertensive and Wistar-Kyoto rats.

1. We investigated candidate loci for salt-sensitive high blood pressure (BP) in F2 progeny from crossing Wistar-Kyoto and stroke-prone spontaneously hypertensive rats. 2. In female F2 progeny, systolic and diastolic BP on the 12th day and the seventh month after salt loading was strongly linked with the D3Mgh12 and D3Mgh6 loci on chromosome 3, respectively. 3. These loci were linked with BP only in female F2 progeny, not in males. 4. These results indicate that hormonal factors may influence salt sensitivity, particularly with respect to gender differences.

The clinical efficacy of gamete intrafallopian transfer by minilaparotomy versus in vitro fertilization and embryo transfer.

OBJECTIVE: To evaluate the clinical performance of gamete intrafallopian transfer (GIFT) and in vitro fertilization and embryo transfer (IVF-ET). METHODS: Infertile women were divided into 2 groups: for GIFT, 239 patients (326 cycles) with at least 1 patent tube; and, for IVF-ET, 125 patients (210 cycles) with bilateral tubal occlusion. A specially designed retractor was developed to replace the gametes into an appropriate section of the Fallopian tube accurately and safely. Several parameters, including the pregnancy and delivery rates of each group, were compared. RESULTS: The success rate per trial in the GIFT group was approximately 1.5 times higher than that in the IVF group (pregnancy rate: 44.2% vs. 31.0%, p < 0.01; delivery rate: 33.4% vs. 22.9%, p < 0.01). The pregnancy and delivery rates of GIFT decreased steadily with the number of trials. These apparent decreases were not observed up to the 3rd trial in IVF-ET cases. CONCLUSIONS: GIFT with a minilaparotomy procedure yielded significantly higher success rates than IVF-ET. Accordingly, GIFT is considered to be the first treatment choice in infertility cases with at least 1 patent Fallopian tube.

Superoxide dismutase activity in cerebrospinal fluid and its relation to compression of the lumbosacral nerve root.

In the pathophysiology of lumbosacral radiculopathy, inflammation of the nerve root is of critical importance. Additionally, free radicals have been shown to be associated with some inflammatory process. This study was designed to investigate whether free radicals participate in the pathophysiology of nerve root involvement. We measured superoxide dismutase (SOD) activity in cerebrospinal fluid (CSF) of 31 patients with unilateral lumbosacral radiculopathy caused by a herniated disc using electron spin resonance (ESR) spectrometry. Then SOD activity was compared with the type of nerve root compression as seen on preoperative myelography. SOD activity in the normal control group was 7U/ml, while that in the hernia group remarkably decreased. The concentration gradient of SOD activity was different between central herniation and centrolateral herniation. Our findings indicate that free radicals are generated after nerve root compression. Under severe deficiency of SOD activity in CSF, serum SOD penetrates into CSF after further compression. In addition, SOD in CSF may play an important role in protecting against nerve root involvement.

Increased cerebrospinal fluid nitrite and nitrate levels in patients with lumbar spondylosis.

Nitric oxide is thought to mediate the cytotoxic effects of certain neurologic diseases. To investigate whether overproduction of nitric oxide may play a role in lumber spondylosis, we compared the levels of nitrite and nitrate in the cerebrospinal fluid (CSF) from patients with lumbar spondylosis with those from a control group by automated flow injection analysis. CSF levels of nitric oxide metabolites were significantly higher in patients with lumbar spondylosis compared with the control (p < 0.01). Nitric oxide may play a role in lumbar pain or nerve damage in sciatic and the CSF nitrite/nitrate may be used as a diagnostic parameter of spinal diseases.

When is the optimal time for delivery?--Purely from the fetuses' perspective.

To determine when the extrauterine environment becomes safer than the intrauterine environment with respect to fetal (neonatal) life, we analyzed all 4,896,505 livebirths, all 21,222 stillbirths, and all 7,513 early neonatal deaths after 26 weeks gestation that were recorded between 1989 and 1992 in Japan. Although the risk of early neonatal death ( < 1 week of age) and of neonatal death ( < 4 weeks of age) greatly exceed the risk of stillbirth at 26 weeks of gestation, those risks declined sharply by 39 weeks of gestation, then increased, while the risk of stillbirth within 1 and 4 weeks exceeded the risks of early neonatal death and of neonatal death at and beyond gestational weeks 40 and 38, respectively, for a singleton pregnancy. A similar reversal occurred at 37 and 35 weeks of gestation for multiple pregnancy. Neonates born at 39 and 37 weeks of gestation for singleton and multiple pregnancies, respectively, had the best outcome. It is concluded that the chance of survival for fetuses reaching 40 and 37 weeks of gestation for singleton and multiple pregnancies, respectively, were higher in the extrauterine than the intrauterine environment in Japan today.

Evaluation of latex agglutination test for alpha-fetoprotein in diagnosing rupture of fetal membranes.

To assess the utility of a latex agglutination test for alpha-fetoprotein in vaginal fluid for diagnosing rupture of the fetal membranes, we tested 91 pregnant women, including 32 with an established rupture, 19 with no evidence of rupture and 40 with a suspected rupture. Results were compared with those of the pH indicator and the fern tests. The latex agglutination test had better sensitivity, specificity, or both, than the pH indicator and the fern tests. We conclude that the latex agglutination test is more accurate than either the pH indicator or the fern test for diagnosing premature rupture of fetal membranes.

New thermoluminescent BaSO4:Eu sheet for in vivo measurement of spatial dose distribution in radiation therapy.

A new highly sensitive thermoluminescent (TL) sheet has been studied as a means of in vivo measurement of spatial dose distribution of radiation therapy. This TL sheet (40 cm x 50 cm x 0.2 mm), which is composed of teflon mixed with BaSO4:Eu doped powder, is very flexible and can be cut to the desired size. The TL sheet was found to have a linear response with a very wide dynamic range of at least 0.002 cGy to 5000 cGy absorbed dose. In addition, this sheet does not need be shielded, because of its insensitivity to room light, and therefore the sheet can detect low-energy electrons decreased by a few millimeter thick air for 3H(maximum beta-ray energy: 18 KeV). The spatial dose distribution was printed out with a newly developed digital readout system. In addition, another high-resolution dosimetry system, which exposes X-ray film with TL photons emitted from the irradiated TL sheet at constant room temperature, is reported. Clinically, the in vivo dose distribution on the surface of the rectal cancer for intracavitary radiation therapy was determined. The applicability of TL sheet for in vivo measurement of dose distribution is discussed.

[Age at natural menopause in Japanese women].

OBJECTIVE: To determine the age at natural menopause in Japanese women. DESIGN: Cross-sectional and retrospective study. SUBJECTS: Two thousand two hundred twenty one women attending gynecological clinics between the ages of 35 and 59 who were born between April, 1929 and March, 1953. MAIN OUTCOME MEASURES: Age of menopause estimated by the rate of the presence of menses at each age, calculated by probit transformation and by reference to the patient's memory. RESULTS: The median age of menopause based on a cross-sectional study was 50.54, and based on patient's memory is 49.47 +/- 3.53 (SD). The presence of fibroma uteri significantly delays the age of menopause (Odds ratio = 9.41).

Transcription of the MRP RNA gene in frog stage I oocytes requires a novel cis-element.

The RNA component of the mitochondrial RNA processing (MRP) enzyme is related to both replication of mitochondrial DNA and processing of 5.8S rRNA, which are accelerated in the frog earliest stage (stage I) of frog oocytes. Microinjection of the deleted genes into the stage I oocytes showed positive cis-elements in the upstream region of the gene. The specific binding of protein(s) to this region was detected in cell extracts from stage I oocytes and liver but not in extracts of stage II-IV oocytes and the concentration of this protein was 40 times higher in the extract of stage I oocytes than that in liver.

Macrophage colony-stimulating factor enhances platelet recovery following cisplatin/carboplatin chemotherapy in ovarian cancer.

To assess the effects of macrophage colony-stimulating factor (M-CSF) on platelet recovery in patients given chemotherapy for advanced and recurrent ovarian cancer, we selected 29 cases treated with carboplatin (280 mg/m2, Day 1) and cisplatin (70 mg/m2, Day 2). We evaluated the response and the platelet recovery effects of M-CSF, which was infused intravenously for 7 consecutive days after the second and fourth courses of chemotherapy. For the 54 courses of M-CSF infusion, the platelet nadir averaged 12.8 +/- 6.3 x 10(4)/microliters, with 9% of the courses designated Grade 3 or 4 on the toxicity scale, according to World Health Organization recommendations, both of which were significantly different from the M-CSF noninfused control courses (P < 0.05). The number of periods in which platelets were below 7.5 x 10(4)/microliters was significantly less in the courses of M-CSF infusion (P < 0.01). Response to treatment was pathologic complete in 23% and partial in 41%, for a total response of 64%. Cisplatin combined with carboplatin was effective in treating patients with advanced and recurrent ovarian cancer. M-CSF infusion may enhance the recovery from the thrombocytopenia that accompanies such chemotherapy.

HELLP syndrome: CT evaluation.

Patients with the HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count) require careful observation and expedient delivery. However, those who develop this syndrome peripartum do not always fulfill its diagnostic criteria before labor begins. We investigated the potential usefulness of liver and spleen CT evaluation in identifying such patients. Seven patients with this syndrome were studied by CT scanning on postpartum days 0-3 and again 3-4 weeks after delivery. Although 3 patients with severe thrombocytopenia had a reduced liver-spleen CT number ratio (< 1.1) and subsequent normalization, the remaining 4 with less severe thrombocytopenia demonstrated no CT changes. Thus, antepartum evaluation of liver and spleen by CT may not be sensitive enough to detect patients who develop HELLP syndrome during the peripartum period. Serial examination of platelet counts may be more useful than CT in detecting patients at risk for this peripartum syndrome.

Enzyme-cytochemistry of human chorion laeve at term: enzyme localization on the chorionic trophoblast.

Is the chorion laeve merely a remnant of the chorion frondosum in placental development? Or is it metabolically active, having something to do with maternofetal interactions? In order to answer these questions at least in part, we determined the ultracytochemical localizations of some important enzymes such as nonspecific phosphatase (alkaline phosphatase), specific phosphatase (Ca(++)-ATPase and 5'-nucleotidase) and adenylate cyclase in the human chorion laeve at term. Strong activities of these enzymes were localized by ultracytochemistry on the plasma membrane of the trophoblast in the chorion laeve. These enzyme activities were confirmed by a series of cytochemical-control experiments, i.e., substrate-free control, heat-stability control, and inhibition control by inhibitors of alkaline phosphatase. These observations indicate that the chorionic trophoblast is probably metabolically active and that it might play an important role in the physiology of the fetal membrane.

Active lupus and preeclampsia: a life threatening combination.

A patient with active systemic lupus erythematosus presented with generalized convulsions and acute myocardial infarction during the first trimester. Serial determinations of biochemical variables and liver histology indicated that preeclampsia might be responsible for the life threatening episodes.
